Cytokine Networks and Clinical Heterogeneity in Sjögren's Disease: From Glandular Inflammation to Therapeutic Stratification.

Sjögren's disease (SjD) is a chronic autoimmune disease characterized by lymphocytic infiltration and dysfunction of the exocrine glands, with manifestations extending beyond glandular sicca symptoms to multiple extraglandular systems. Although the pathogenesis of SjD remains incompletely understood, growing evidence indicates that a complex cytokine network involving both innate and adaptive immune pathways plays a central role in disease development. This narrative review summarizes recent updates on cytokine signaling in SjD across three clinically relevant domains. In glandular inflammatio

Long-Term Outcomes of Paediatric Patients With Unexplained Elevated Total Immunoglobulin E.

AIM: To clarify the aetiology and clinical relevance of markedly elevated total serum immunoglobulin E (IgE) levels (≥ 1000 IU/mL) in children and to characterise the longitudinal outcomes of patients with persistently elevated IgE levels in whom no underlying cause can be identified. METHODS: This retrospective cohort study included children aged 1-18 years evaluated between September 2019 and June 2025 with total serum IgE levels ≥ 1000 IU/mL. Patients with at least three outpatient visits and a minimum follow-up of 12 months were eligible for final analysis. Comprehensive clinical, allergi

Characteristic clinical and immunologic features of Sjögren's disease patients presenting with major salivary gland swelling at diagnosis.

INTRODUCTION/OBJECTIVES: Sjögren's disease (SjD) is a heterogeneous systemic autoimmune disorder with diverse clinical presentations. While sicca symptoms are considered hallmark features, a substantial proportion of patients present with non-sicca manifestations. Although major salivary gland swelling is a recognized feature, its potential implications for baseline evaluation and clinical characterization at diagnosis remain incompletely understood. We aimed to classify SjD patients according to presenting symptoms at diagnosis and to characterize the clinical features of patients presenting

An Uncommon Cause of Cystic Lung Disease.

A 59-year-old female with Sjögren's syndrome presented with a three-year history of dyspnea and cough beginning after COVID-19 infection. Serial chest computed tomography scans over a course of three years revealed multiple pulmonary cysts and nodules, including one that enlarged over time. Pulmonary function was largely preserved, aside from a mildly reduced diffusing capacity, while laboratory studies showed hypergammaglobulinemia and elevated kappa light chains. After a nondiagnostic bronchoscopic transbronchial biopsy, she underwent thoracoscopic wedge resection. Pathology revealed eosinop

Monoclonal Gammopathy of Thrombotic Significance.

Monoclonal gammopathy of undetermined significance (MGUS) is an asymptomatic premalignant precursor to multiple myeloma (MM). While development of end-organ damage in MM largely reflects increasing clonal burden, emerging evidence indicates that qualitative properties of the monoclonal immunoglobulins in MGUS can be directly pathogenic, leading to serious organ injury independent of disease burden. This has led to the recognition of a broader spectrum of disorders collectively termed monoclonal gammopathy of clinical significance (MGCS). MGCS can be further subclassified based on the organ sys

Association between diabetic control and progression of monoclonal gammopathy of undetermined significance to multiple myeloma.

Diabetes Mellitus (DM) is a risk factor for multiple myeloma (MM), but the relationship between diabetic control and risk of progression to MM in patients with monoclonal gammopathy of undetermined significant (MGUS) is unclear. We conducted a retrospective cohort study of US Veterans diagnosed with MGUS from 10/1/1999-12/31/2023 with prior DM. MGUS and MM diagnoses were confirmed using published natural language processing-assisted models. The exposure was time-varying glycated hemoglobin (HbA1c) > 7% during the follow-up (time of MGUS to MM, death, or being censored on 4/15/2025, whichever o

Early-onset parkinsonism as a presenting feature of suspected type 1 Gaucher disease with two pathogenic GBA1 variants: a case report.

Parkinson's disease (PD) is a common neurodegenerative disorder, and pathogenic variants in GBA1 are among its most frequent genetic risk factors. Biallelic pathogenic GBA1 variants cause Gaucher Disease (GD). In patients with parkinsonism and two detected GBA1 variants, the absence of biochemical confirmation and allele phasing may make it difficult to determine whether the neurological presentation occurs in the context of GD or represents GBA1-associated PD. We report a 35-year-old man with a 7-months history of progressive, asymmetric parkinsonism. Dopamine transporter PET/CT showed reduce

The Transformation of the Microenvironment: From Observer to Coordinator in Monoclonal Gammopathy of Undetermined Significance (MGUS) to Myeloma Development.

Monoclonal gammopathy of undetermined significance (MGUS) is a common asymptomatic plasma cell disorder and the major precursor state of multiple myeloma (MM). Although progression is partly shaped by intrinsic genetic and epigenetic changes within the plasma cell clone, increasing evidence indicates that the bone marrow microenvironment plays a decisive role in determining whether the clone remains clinically silent or evolves toward symptomatic disease. This review examines how the marrow niche changes across the MGUS-SMM-MM continuum. In early MGUS, the microenvironment may already be permi

Contribution of bone scintigraphy and PET/CT in the differential diagnosis of bone involvement in a patient with fibrous dysplasia and monoclonal gammopathy of undetermined significance and fibrous dysplasia treatment with denosumab.

We describe a patient who has both fibrous dysplasia (FD) and monoclonal gammopathy of undetermined significance (MGUS). The simultaneous occurrence of these two pathological conditions is rare, and it is necessary to distinguish whether the bones are being affected by FD or multiple myeloma. The patient's problems began with a non-healing tooth extraction, raising suspicion of osteomyelitis. Later, he was also examined for fever of unknown origin, as he had elevated inflammatory markers, low-grade fever, and headaches. According to skeletal scintigraphy, the patient had a craniofacial skeleto

Case Report: A rare co-occurrence of IgA pemphigus and pyoderma gangrenosum associated with IgA-κ type monoclonal gammopathy of undetermined significance: a 19-year diagnostic and therapeutic journey.

We report a complex case of neutrophilic dermatosis (ND) in a 61-year-old woman with a 19-year history of recurrent, pruritic, and painful vesiculopustular eruptions. Her clinical course was marked by evolving diagnoses, from pustular vasculitis to IgA pemphigus and pyoderma gangrenosum (PG). Concurrent investigations revealed an IgA-κ type monoclonal gammopathy of undetermined significance (MGUS). This case highlights the rare association between neutrophilic dermatoses (ND) and haematological disorders, and illustrates the therapeutic challenges posed by comorbidities such as osteoporosis an

Lymphoid interstitial pneumonia in Sjögren disease: clinical course and comparison with other ILD patterns.

OBJECTIVES: Lymphoid interstitial pneumonia (LIP) is a rare form of Interstitial Lung Disease (ILD), often associated with Sjögren Disease (SjD). However, the clinical-serologic characteristics of SjD-LIP remain poorly characterized. Our objective was to describe the clinical course and outcome of SjD-associated LIP and to compare this subgroup with other SjD-ILD patterns. METHODS: SjD patients with High-resolution Computed Tomography (HRCT)-confirmed ILD followed in Pisa Rheumatology Unit (January 2019-November 2024) were retrospectively enrolled. ILD patterns were classified through multidi

[Rosai-Dorfman Syndrome-a diverse clinical spectrum].

Rosai-Dorfman syndrome is a rare disease that belongs to the group of non-Langerhans cell histiocytosis. Affected patients can exhibit a wide variety of symptoms, which can be divided into nodal and extranodal manifestations. The syndrome primarily presents with fever, enlarged lymph nodes, and weight loss. Extranodal findings may include pancreatic cysts, hypergammaglobulinemia, and infiltration of testicular tissue. Skin involvement occurs in approximately one-third of cases. In addition to routine beside histopathology, immunohistochemistry with positivity for S100 and CD68 and negativity f

Sjögren's Disease-Associated Parotid MALT Lymphoma: Diagnostic Pitfall of Ultrasound-Guided Core Needle Biopsy.

Sjögren's disease (SD) is a chronic autoimmune disease associated with an increased risk of extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma), particularly in the parotid glands. However, distinguishing benign SD-associated sialadenitis from early lymphoma remains a diagnostic challenge, especially when biopsy samples are limited. We report an elderly woman presenting with progressive bilateral parotid swelling accompanied by xerostomia and xerophthalmia. Serological testing revealed positive anti-SSA and anti-SSB antibodies, hypergammaglobulinemia, and hyp

Mikulicz Disease Revealing IgG4-Related Tubulointerstitial Nephritis: A Case Report and Literature Review.

Background and Clinical Significance: IgG4-related disease (IgG4-RD) is a chronic fibroinflammatory, immune-mediated multisystem disorder that can mimic neoplastic, infectious, or autoimmune conditions. Among its head-and-neck manifestations, IgG4-related dacryoadenitis and sialadenitis, historically referred to as Mikulicz disease, should be distinguished from the classical Mikulicz syndrome, which describes secondary lacrimal and salivary gland enlargement due to other systemic disorders. Renal involvement, most commonly in the form of IgG4-related tubulointerstitial nephritis (IgG4-TIN), is

Laboratory Identification of Pseudohypercalcemia Induced by Hyperglobulinemia in Multiple Myeloma: A Case Report.

BackgroundPseudohypercalcemia is easily misdiagnosed in clinical practice, potentially leading to unnecessary interventions. Multiple myeloma (MM) presenting with concurrent hypergammaglobulinemia represents a rare etiology of pseudohypercalcemia, and there are few reports of systematic laboratory validation of this pathological entity.MethodsWe report a patient with MM who presented with markedly elevated serum total calcium on routine biochemical testing. The hypercalcemia persisted despite 1 month of calcitonin therapy administered at an external hospital. After admission, the clinical labo

Bildgebung bei monoklonalen Plasmazellerkrankungen

BackgroundImaging has a key diagnostic role in monoclonal plasma cell disorders. It can be used not only to detect osteolytic skeletal lesions but also for the detection of bone marrow involvement, extramedullary manifestations, impending complications, treatment response and the relapse assessment. Whole-body computed tomography (CT) has replaced the conventional skeletal survey, while whole-body magnetic resonance imaging (MRI) and 18F-fluorodeoxyglucose positron emission tomography-CT (FDG PET/CT) supplement the morphological skeletal assessment by depicting the bone marrow and functional-m

Teclistamab versus lenalidomide-dexamethasone in high-risk smoldering multiple myeloma: a randomized phase 2 trial.

Teclistamab, a B cell maturation antigen-targeting bispecific antibody, has demonstrated substantial activity in relapsed multiple myeloma (MM), particularly in earlier lines of therapy, and may have higher efficacy in high-risk smoldering MM (HR-SMM) with a more functional immune system. In the randomized phase 2 ImmunoPRISM trial, we compared fixed-duration teclistamab with lenalidomide-dexamethasone (Rd) in HR-SMM. After a six-patient safety run-in, patients were randomized 2:1 to teclistamab or Rd. The primary endpoint was complete response (CR) rate. As of 26 May 2026, 59 patients were tr
