BACKGROUND: Azithromycin remains an important oral treatment option for enteric infections in low- and middle-income countries (LMICs), where rising fluoroquinolone resistance, limited diagnostic capacity, and restricted access to parenteral therapy have encouraged widespread empirical use. Increasing azithromycin resistance among Shigella sp., typhoidal and non-typhoidal Salmonella, and Campylobacter sp., however, threatens its clinical utility.
OBJECTIVES: This Perspective examines azithromycin resistance as a health-system challenge rather than solely a microbiological phenomenon. Diagnost

Nontyphoidal Salmonella causes an estimated 1.3 million domestically-acquired foodborne infections in humans annually in the United States (Scallan Walter et al., 2025). From 1998 to 2023, pork was the fourth most common food category attributed to foodborne Salmonella illnesses (Interagency Food Safety Analytics Collaboration, 2024). We analyzed salmonellosis outbreaks associated with pork products between 2012-2021, using data from the Centers for Disease Control and Prevention's Foodborne Diseases Outbreak Surveillance System, System for Enteric Disease Response, Investigation, and Coordina

Cryptosporidium parvum is a highly pathogenic zoonotic parasite with limited therapies. Current study explored the anti-C. parvum effect of Qing Chang Hua Shi Formula (QHF) and allicin in vitro in immunosuppressed mice. In vitro, QHF and allicin inhibited over 90% of C. parvum. In vivo, the 8-day treatment reduced fecal oocyst shedding to 12.88% of the model group. Infection disrupted intestinal homeostasis, reduced tight-junction protein expression, and downregulated NLRP6, while inducing systemic inflammation and elevating IL-18/IFN-γ levels, which were further increased by treatment. Primary amoebic meningoencephalitis (PAM), caused by Naegleria fowleri, is a rapidly progressive and fatal infection, and inflammation of the olfactory bulb plays a key role in disease pathogenesis. Although inflammatory and oxidative mediators have been studied in vitro, their production during the progression of PAM remains poorly understood. This study evaluated the temporal production of TNF-α, IL-1β, IL-6, nitric oxide (NO), and reactive oxygen species (ROS) in the olfactory bulbs of BALB/c mice from 1 to 7 days after intranasal infection with N. fowleri trophozoites. RT-PCR detected incr

The intestinal tract is a key immune organ and microecological system, where colonization resistance (CR) mediated by gut microbiota prevents pathogen invasion. Salmonella disrupts CR via type VI and type III secretion systems (T6SS/T3SS), inducing dysbiosis, inflammasome-driven inflammation, and intestinal barrier damage. This creates a self-reinforcing cycle:dysbiosis alters microbial metabolites, sustains inflammation, and enhances pathogen fitness, favoring chronic infection. This review integrates molecular mechanisms of microbiota disruption, inflammasome activation, and metabolic imbala

BACKGROUND: Given emerging evidence on the waning of immunity from typhoid conjugate vaccines (TCV), the World Health Organization (WHO) commissioned a multi-model comparison to determine the optimal schedule in terms of health and economic impact to inform updated recommendations for TCV use across different settings.
METHODS AND FINDINGS: To identify optimal vaccination strategies across different incidence settings and vaccine waning assumptions, we compared two agent-based and two compartmental dynamic models of typhoid transmission. All models were fitted to harmonized age-specific incid

In immunocompetent hosts, Salmonella enterica serovar Typhimurium typically causes transient intestinal blooms with acute diarrhoea, followed by asymptomatic systemic carriage with intermittent reactivation. Inflammation elicited by bistable expression of type III secretion systems and pathogen-associated signals shapes Salmonella pathogenesis in both the intestine and systemic tissues. Inflammation is a two-sided coin that, over time, can either benefit or harm the host, microbiota and Salmonella. The host deploys the inflammasome, hypoxia and reactive oxygen and nitrogen species in both inte

STAT3 is a central mediator of pro-survival signaling within the STAT family, functioning through both canonical transcriptional activity and noncanonical mechanisms. As a pleiotropic regulator, STAT3 integrates cytokine, growth factor, and stress signals to coordinate diverse cellular processes, including epithelial architecture, T cell development, macrophage polarization, apoptosis, metabolism, and inflammation-associated wound healing. This integrative capacity positions STAT3 as a critical signaling hub at the interface of tissue homeostasis and immune regulation. Dysregulated STAT3 activ

Salmonella is a leading cause of foodborne illness in the United States, yet rural-urban differences in incidence and their underlying drivers remain poorly characterized. We conducted an ecological analysis of deidentified probable and confirmed Salmonella cases from the National Notifiable Diseases Surveillance System (NNDSS), 2011-2023, to evaluate temporal, regional, and population-level determinants of Salmonella incidence. Multivariable negative binomial models assessed rural-urban differences nationally and by U.S. Census region, and Blinder-Oaxaca decomposition quantified contributions

SUMMARYInvasive non-typhoidal Salmonella (iNTS) disease is a severe bloodstream and focal invasive infection caused by non-typhoidal serovars of Salmonella enterica, especially S. Typhimurium and S. Enteritidis. Its clinical importance is disproportionate to its routine visibility because disease often presents as undifferentiated fever, sepsis, meningitis, severe anaemia-associated deterioration, recurrent bacteraemia, or focal infection, while definitive recognition depends on blood or other sterile-site microbiology. Global estimates indicate substantial mortality, but the true incidence re

Vibrio cholerae is the causative agent of the human diarrheal disease cholera. Previous studies identified "hyperswarming" (HS) mutant V. cholerae with enhanced motility and decreased virulence factor expression, but the genetic basis of HS was unknown. Through whole-genome sequencing, we have identified point mutations within toxT, which encodes the master virulence regulator, as being responsible for the HS phenotype. These mutations are loss of function, and they eliminate transcription of cholera toxin and toxin-coregulated pilus genes. Using single-cell video tracking, these HS strains we

Balamuthia mandrillaris is a rare and lethal amoeba that causes granulomatous amoebic encephalitis (GAE), a severe brain infection with a high mortality rate. This manuscript presents a case study of a 5-year-old female patient with B. mandrillaris amoebic encephalitis. She presented from an outside hospital (OSH) with daily fevers, fatigue, and worsening mental status. She was without comorbidities, and sick contacts were denied. Diagnosis was challenging due to nonspecific encephalitis symptoms and nonspecific neurological imaging. Neuroimaging revealed symmetric diffuse restriction in the m

New therapeutic approaches are needed to treat infections due to multidrug resistant (MDR) Gram-negative bacteria. The use of antibody-recruiting molecules (ARMs) is a promising approach to address the MDR problem given their potential to engage both the adaptive and innate immune systems to treat bacterial infections. Here we present two new strategies to create ARMs. First, we show that a 9-mer antimicrobial peptide (K6) can be fused to a 11-mer mimotope peptide recognized by antibodies targeting the Vibrio cholerae O1 O-antigen to produce an ARM peptide (FZ5) that can render E. coli more se

Tetanus neurotoxin (TeNT) invades the central nervous system (CNS) via retrograde transport through motoneurons. Whether a host factor is needed to mediate TeNT trafficking has remained unknown. Here, using pull-down and mass spectrometry analysis, and by developing a luciferase-based in vivo transport assay, we identify receptor protein tyrosine phosphatase sigma (PTPRS; PTPσ) as the retrograde transport receptor for TeNT. Global or motoneuron-specific PTPRS knockout (KO) abolishes TeNT entry into the CNS in mouse models. PTPRS-TeNT interactions occur in endosomes, prevent toxin translocation

The characterization of pathogens affecting Antarctic wildlife has been historically limited and sporadic. With the introduction of H5N1 high pathogenicity avian influenza virus (HPAIV) in Antarctica in the austral summer 2023/2024, concerns about the health of Antarctic seabirds and mammals prompted immediate surveillance efforts. In March 2024, we visited the Danger Islands archipelago, that hosts two mega-colonies of Adélie penguins, and observed an unusual mortality estimated in thousands of Adélie penguins and other species. Swabs and tissues were collected for molecular detection of infe
