Impaired acid stress resistance in Salmonella Typhi Ty2.

UNLABELLED: Salmonella enterica encounters acid stress during gastrointestinal transit and within the phagosomal environment of macrophages. Acid stress resistance has been well characterized in Salmonella enterica serovar Typhimurium, but comparative studies in the human-adapted Salmonella enterica serovar Typhi are limited. We compared the growth of S. Typhimurium 14028s and S. Typhi Ty2 at pH values ranging from 3 to 8 and observed that Salmonella enterica serovar Typhimurium exhibits enhanced growth at pH 4.5 compared to S. Typhi. Comparative transcriptomic profiling of S. Typhimurium and

Infections After International Travel: Relevant Diagnoses in Children and Adolescents.

BACKGROUND: Families can acquire infections that are rare or nonexistent in Germany by international travel for business or private reasons and by migration between countries. Children and adolescents have special risk profiles, and their course of illness may be nonspecific and/or severe. A structured travel history is essential so that regionally specific infections will not be overlooked. METHODS: This narrative review is based on publications of the last 25 years that were retrieved by a PubMed search on infections after international travel, with an emphasis on retrospective and prospect

Global Mortality from Severe Infectious Diseases Among Adolescents Aged 10-19 Years, 1990-2023: Long-Term Trends and Cause Composition from the Global Burden of Disease 2023 Study.

BACKGROUND: Severe infectious diseases remain a preventable cause of adolescent mortality worldwide, yet global evidence focused on adolescence as a distinct life-course stage-and its vulnerability to health system disruption-remains limited. We examined long-term mortality rate trends, cause composition, and COVID-19-related changes among adolescents compared with late childhood. METHODS: We analyzed Global Burden of Disease 2023 mortality estimates from 1990 to 2023 for six acute severe infectious causes: lower respiratory infections, meningitis, encephalitis, diarrhoeal diseases, typhoid/p

Immunogenicity of a Bivalent Typhoid-Paratyphoid Conjugate Vaccine Against Diverse Salmonella Paratyphi A Isolates.

BACKGROUND: In a first-in-human phase-1, randomized, controlled trial, a bivalent conjugate vaccine (Vi-CRM197+O:2 CRM197) elicited robust IgG and bactericidal activity against vaccine-homologous Salmonella Paratyphi A strain. To further characterize the immune response, we assessed O:2 antigen-specific IgA and IgM responses as markers of mucosal immunity and early immune activation, and evaluated serum bactericidal activity against diverse S. Paratyphi A isolates. METHOD: The trial (NCT05613205) was conducted in three steps. For this exploratory analysis, 46 participants (mean age 27.2 years

A defined microbiota mouse model for Salmonella Paratyphi A oral infection.

INTRODUCTION: Salmonella Paratyphi A (SPtA) is an emerging pathogen that primarily infects humans and causes enteric fever, yet it has not received sufficient research attention. To facilitate further investigation, an appropriate animal model is necessary for testing SPtA infection. METHODS: In this study, we utilized gnotobiotic mice harboring the altered Schaedler flora (ASF) from either C3H/HeN or 129S6/SvEv genetic backgrounds. These mice, subjected to various inflammatory conditions, were orally challenged with SPtA. The colonization and persistence of the pathogen were assessed in both

Case Report: Acute acalculous cholecystitis due to Salmonella Typhi: a case of conservative treatment failure and review of the literature.

BACKGROUND: Acute acalculous cholecystitis (AAC) is an uncommon but potentially life-threatening inflammation of the gallbladder in the absence of gallstones, accounting for 5-10% of all cases of acute cholecystitis. While most cases occur in critically ill or postoperative patients, infectious etiologies-particularly Salmonella Typhi-are increasingly recognized. Typhoid-related AAC is well described in children from endemic regions but remains rare in adults, with limited evidence to guide management. CASE PRESENTATION: We report the case of a 67-year-old man presenting with fever, chills, a

Bile and capsid variation coregulate histo-blood group antigen recognition and serum neutralization potency in GII.12 human norovirus.

UNLABELLED: Human norovirus (HuNoV) is a leading cause of acute gastroenteritis, with young children and the elderly bearing the greatest disease burden. GII strains cause the majority of HuNoV outbreaks globally, with host susceptibility influenced by genetic polymorphisms that regulate mucosal expression of histo-blood group antigen (HBGA) binding ligands. HuNoV viral capsid (VP1) diversity in residues adjacent to the conserved HBGA binding pocket selectively stabilizes ligand interactions. Additionally, select HuNoV capsids demonstrate bile-influenced conformational plasticity, further enab

Corticosteroids for Haemophagocytic Lymphohistiocytosis Secondary to Typhoid Fever: A Case Report.

Unlike haemophagocytic lymphohistiocytosis (HLH) induced by tuberculosis or rickettsial disease, there is no consensus for the treatment of HLH secondary to typhoid fever (Salmonella Typhi). After a short illustrative example, we searched among the previously described cases of HLH secondary to typhoid fever to determine: (i) Is the specific treatment of HLH mandatory; and (ii) which option to choose between corticosteroids and intravenous immunoglobulins in association with antibiotherapy?

Unravelling Cholera's Footprints in India: A Data-Driven Investigation of its Burden, Risk, and Strain Variability.

INTRODUCTION: Binge Eating Disorder (BED) has high lifetime prevalence rates, low treatment success rates, and high rates of treatment dissatisfaction, early discontinuation of care, and recurrence. Complementary and integrative health (CIH) interventions (non-mainstream practices used with conventional approaches for whole-person treatment) hold potential to overcome many treatment barriers and improve BED treatment outcomes. Some CIH interventions have empirical support for use in eating disorders. However, little is known about the current state of CIH use in BED. METHODS: This mixed-metho

Liposomal hydrogel-based oral vaccine delivery for targeted induction of intestinal mucosal immunity.

Oral vaccines have attracted considerable attention due to their advantages of convenient administration and ability to induce mucosal immunity. However, unfavorable conditions such as the gastrointestinal barrier and acidic environment constrain their immunogenic efficacy. To address these issues, a novel oral vaccine delivery platform has been developed, in which mannose-decorated liposomes are complexed with antigen and retinoic acid, then enveloped by a thiolated alginate gel microsphere (MLip@Gel). Mannose-modified liposomes adsorb porcine epidemic diarrhea virus (PEDV) through electrosta

Large language models as versatile predictive engines for notifiable infectious diseases.

Accurate forecasting of infectious disease cases and deaths is crucial for public health decision-making. Traditional statistical and machine learning approaches may be challenged by complex temporal patterns and heterogeneous surveillance data. We evaluated large language models (LLMs) for infectious disease forecasting. We collected monthly reported cases and deaths data from China National Notifiable Diseases Surveillance System (NNDSS) from January 2009 to February 2025, and monthly reported cases from the United States NNDSS from 2016 to 2023, covering 79 notifiable infectious diseases an

The Performance of Large Language Models in Extracting Intestinal Symptoms From Electronic Health Records: Retrospective Observational Study.

BACKGROUND: Unstructured electronic health records (EHRs) hinder the monitoring of intestinal infections. Large language models (LLMs) enable automated symptom extraction. However, their clinical validation is limited by a lack of systematic multimodel comparisons, unclear prompting strategies, and the privacy risks of cloud-based models (eg, data leakage and cross-border data transfer). OBJECTIVE: This study aimed to systematically evaluate the performance of locally deployed open-source LLMs across 4 model families in extracting intestinal symptoms from unstructured EHR chief complaints und

Willingness to accept paediatric blood sample collection for clinical research purposes in Nepal: a qualitative study.

Blood sample collection is essential for clinical research but can be challenging due to cultural beliefs, emotional responses, and misconceptions. This can escalate in paediatric settings. Despite the growing landscape of clinical research, evidence from South Asia remains limited. This study explores the factors influencing willingness to accept paediatric blood sample collection for clinical research in Nepal, drawing on experiences from a clinical research cohort. A qualitative study grounded in a constructivist-interpretivist paradigm, using semi-structured interviews, was conducted with

Spontaneous splenic rupture in a patient with paratyphoid a fever: a case report and literature review.

BACKGROUND: Enteric fever, including both typhoid and paratyphoid fevers, is associated with a significant burden of morbidity and mortality in high-epidemic countries. However, as its incidence has declined in most countries, early diagnosis of its complications has become a clinical challenge. Spontaneous splenic rupture (SSR) is a rare but dangerous complication of typhoid fever, and its occurrence in patients with paratyphoid fever remains poorly understood. We report a case of SSR associated with paratyphoid A fever and briefly review the literature. CASE PRESENTATION: A 15-year-old male

Why did notifiable intestinal diseases persist while respiratory infections plummeted? Unraveling the divergent environmental drivers across the COVID-19 pandemic.

BACKGROUND: The COVID-19 pandemic drastically altered human behaviors alongside meteorological and environmental conditions, yet how these shifts differentially impacted respiratory versus intestinal infectious diseases remains unclear. This study investigates the evolving relationships between environmental exposures and disease transmission across pre-pandemic, pandemic, and post-pandemic phases. METHODS: We analyzed surveillance data for notifiable respiratory and intestinal diseases in Sichuan Province (2006-2023), integrating meteorological and environmental pollution data via principal

Research intensity for neglected tropical diseases and other infectious diseases before and after the COVID-19 pandemic.

BACKGROUND: Infectious diseases can cause tremendous health and economic damage, as shown by past pandemics. Although research tends to spotlight emerging infectious diseases, neglected tropical diseases (NTDs) require sustained research and development to advance global public health. However, there is concern that the recent COVID-19 pandemic may have interrupted research on NTDs, leading to further neglect of these diseases. RESULTS: We assessed the bias of research intensities for 67 infectious diseases, including 18 NTDs, from 2016 to 2023. We calculated the burden-adjusted research inte

Study of patients with acute undifferentiated fever identifies dengue as a growing threat to public health in Mali.

The circulation of arboviruses in sub-Saharan African countries remains poorly documented. The associated health burden may be underestimated and masked by the significance of malaria. Here, we have investigated acute undifferentiated fevers for arboviral infections in Mali (2016-2024). To estimate the proportion of patients with arboviral infection, and in particular dengue. A retrospective (2016-2022) and a prospective (2023-2024) studies were conducted in patients from health centers and hospitals of Mali (mainly in the Bamako region) selected by health professionals. Studies included patie

A Cardiomyopathy to Remember: A Case Report of Desmoplakin Cardiomyopathy.

We previously reported that Sildenafil enhances apoptosis and antitumor efficacy of doxorubicin (DOX) while attenuating its cardiotoxic effect in prostate cancer. In the present study, we investigated the mechanism by which sildenafil sensitizes DOX in killing of prostate cancer (PCa) cells, DU145. The death receptor Fas (APO-1 or CD95) induces apoptosis in many carcinoma cells, which is negatively regulated by anti-apoptotic molecules such as FLIP (Fas-associated death domain (FADD) interleukin-1-converting enzyme (FLICE)-like inhibitory protein). Co-treatment of PCa cells with sildenafil and
