Diagnosis of Paediatric Inborn Errors of Immunity in a MENA Cohort Referred for Recurrent Infections Using a Structured Clinical Algorithm: A Real-Life Cross-Sectional Study.

AIM: To evaluate the diagnostic yield and clinical triage performance of a structured, multistep algorithm in children referred for suspected inborn errors of immunity (IEI) due to recurrent infections. METHODS: This single-centre study included 705 children (0-18 years) referred for recurrent infections. All were screened using JMF and/or MENA criteria. Of these, 132 met at least one criterion and underwent stepwise immunologic evaluation, including advanced testing when indicated. RESULTS: Of 705 children referred with recurrent infections, 132 (18.7%) met screening criteria and underwent

Gene therapy via CRISPR/Cas9-mediated Cxcr4 disease allele inactivation reverses leukopenia in WHIM mice.

Warts, hypogammaglobulinemia, infections, and myelokathexis (WHIM) syndrome is an immunodeficiency caused by autosomal dominant hyperfunctional mutations in chemokine receptor CXCR4 that promote panleukopenia due to BM retention. We previously reported a preclinical gene therapy protocol involving allele-nonspecific Cxcr4 CRISPR/Cas9 inactivation, leveraging the known in vivo dominance of Cxcr4+/o (+, WT; o, inactivated) hematopoietic stem cells (HSCs) for autologous BM engraftment and leukocyte reconstitution over HSCs with other Cxcr4 genotypes. Here, we show that without BM conditioning, th

Building alliances for early detection of immunodeficiencies: from primary care to hematology.

Inborn errors of immunity (IEI), also known as primary immunodeficiencies, are a heterogeneous group of rare disorders characterized by increased susceptibility to infections, immune dysregulation, and malignancy. Early detection remains a major challenge due to the complexity of clinical presentations, limited awareness among non-specialists, and delayed diagnostic pathways. This review explores current strategies to enhance early detection of IEI, highlighting both technological innovations and clinical insights. Tools such as newborn screening, the Jeffrey Modell Foundation (JMF) warning si

Case Report: Transfusion-associated graft-versus-host disease in severe combined immunodeficiency.

Transfusion-associated graft-versus-host disease (TA-GVHD) is a rare but fatal blood transfusion complication, with a mortality rate of 90-100%. Severe combined immunodeficiency (SCID) is a life-threatening primary immunodeficiency with profound cellular and humoral defects. Patients with SCID are highly susceptible to TA-GVHD. Here, we report a 4-month-old male admitted for sepsis and severe pneumonia, with pustular rash and unhealed exudative Bacillus Calmette-Guérin vaccination site. Laboratory tests showed hypogammaglobulinemia and lymphopenia. Lymphocyte subset analysis confirmed the pres

FOXN1 immunodeficiency detected by TREC-based newborn screening - A challenge of management?

Incomplete genotype-phenotype correlations challenge the management of non-SCID FOXN1 immunodeficiency. We describe the detailed clinical course of three distinct newborns with four novel FOXN1 mutations identified by TRECNBS. For comprehensive immune characterization advanced flow cytometry-based immunophenotyping was employed alongside high-resolution single-cell RNA sequencing. In our cohort, we detected heterozygous FOXN1 mutations in P1 (c.1178delG; p.Gly393Alafs*157) and P2 (c.830+1G>T; p.?), and compound heterozygous FOXN1-mutations in P3 (c.1318C>T; p.Gln440* and c.668T>G; p.?). Despit

Neonatal erythroderma and immunodysplasia: Overlap of cartilage-hair hypoplasia and Omenn syndrome.

Cartilage hair hypoplasia (CHH) syndrome (OMIM #250250) is a rare autosomal recessive metaphyseal dysplasia, characterized by disproportionate short stature, hypotrichosis and variable extra-skeletal manifestations, including immunodeficiency, anemia, intestinal diseases, and predisposition to malignancies. CHH results from homozygous or compound heterozygous mutations in the RMRP gene on chromosome 9p13, which encodes an untranslated RNA component of mitochondrial RNA-processing endoribonuclease. RMRP pathogenic variants can also lead to Omenn Syndrome (OS) (OMIM #603554), a systemic inflamma

[XMEN disease diagnosed following persistent Epstein-Barr virus viremia and recurrent lymphadenopathy].

The patient was a 46-year-old man with a family history of malignant lymphoma. He presented with bilateral submandibular and cervical lymphadenopathy, which resolved spontaneously. However, approximately one year later, he was admitted to our hospital for treatment of systemic lymph node swelling and bacterial pneumonia. Inguinal lymph node biopsy showed residual lymph follicles with T-zone expansion and numerous Epstein-Barr virus encoding region in situ hybridization (EBER-ISH) positive B-cells. Epstein-Barr virus (EBV) was also detected in the plasma, and EBV encephalitis was diagnosed. Fol

Case Report: A successful case of allogeneic stem cell transplantation for pediatric XMEN characterized by neutropenia.

XMEN disease (X-linked immunodeficiency with magnesium defect, EBV infection, and neoplasia) is a rare Inborn Error of Immunity (IEI)characterized by impaired magnesium ion transport due to mutations in the MAGT1 gene, which subsequently affects immune cell function. Timely diagnosis and prompt intervention are essential for improving patient outcomes. Allogeneic hematopoietic stem cell transplantation (HSCT) offers a potential therapeutic approach to restore MAGT1 function. We report an infant with XMEN who acquired a novel mutation in the MAGT1 gene, presenting recurrent severe skin infectio

Genetic and epidemiological patterns of primary immunodeficiency diseases in Eastern Iranian patients.

Primary immunodeficiency diseases (PIDs) are monogenic inborn immune disorders. Of currently listed 555 genetic inborn errors of immunity (IEI) in 10 IUIS categories, PIDs range from life-threatening severe combined immunodeficiency (SCID) to milder antibody deficiencies. PIDs increase mortality risk and demand genetic/immunological testing. In Iran, high consanguinity rates may warrant influence of genetic factors. Diagnostic challenges due to heterogeneity necessitate application of advanced tools like whole exome sequencing (WES). This study was carried out on Eastern Iran’s PIDs and simila

Clinical Warning Signs in Detecting Inborn Errors of Immunity in Children: A Diagnostic Accuracy Systematic Review.

BACKGROUND: Early recognition of inborn errors of immunity (IEI), formerly primary immunodeficiencies, is crucial, yet the diagnostic accuracy of widely promoted clinical "warning signs" in children is uncertain. OBJECTIVE: To assess the diagnostic accuracy of clinical warning signs and established criteria for identifying pediatric IEI. METHODS: We searched MEDLINE and EMBASE (inception-May 2024) for studies reporting sensitivity and specificity of individual or combined warning signs in children (0-18 years) with suspected IEI. The reference standard was physician-confirmed IEI. Data were

Expanding Phenotype of GINS1 Deficiency: A Case Report and Review of the Literature.

Pathogenic variants in GINS1 are believed to cause a primary combined immunodeficiency and growth retardation syndrome with natural killer cell deficiency and chronic neutropenia. To date, however, very few cases have been reported. Thus, the role of GINS1 in disease, as well as the spectrum of variants and their associated phenotype, remains unclear. We present a 2-year-old female with growth retardation, chronic neutropenia, distinctive facial features, and glaucoma. Exome sequencing revealed two likely pathogenic variants in GINS1, c.-48C>G p.? and c.247C>T p.Arg83Cys, conferring a diagnosi

The Role of Immune Markers in Predicting Infectious Complications in Children with Congenital Heart Defects.

The literature review presents data from a limited number of available studies conducted over the last two decades on immunological deficiency in congenital heart defects (CHDs), which is the cause of frequent infectious complications before and after cardiac surgery. Several studies based on screenings at various levels indicate the presence of primary and secondary immunodeficiency in CHDs, in particular about 13 genetic syndromes in which CHD is combined with immunodeficiency. The available data suggests a greater severity of immunological disorders in patients with critical CHDs, cyanotic

Good's Syndrome Mirrors a Combined Immunodeficiency with Anti-Cytokine Antibodies in the Total Absence of B Cells.

Good’s Syndrome, an adult-onset immune deficiency, has been traditionally grouped with humoral immune deficiencies such as Common Variable Immune Deficiency (CVID) and X-linked agammaglobulinemia (XLA) despite the unique presence of the thymic neoplasm, late age of onset, frequent opportunistic infections and multiple cytopenias. Low prevalence and sporadic reporting have limited investigative efforts. We conducted a prospective, multimodal, study to establish the GS phenotype in the absence of severe infections. A previously characterized cohort of GS patients was compared to healthy individu

TFRC Germline Variants and Inborn Error of Immunity: Mechanistic Insights into Iron-Immune Crosstalk.

Mutations in TFRC, which encodes transferrin receptor 1 (TfR1/CD71), have emerged as a rare but significant cause of inborn error of immunity (IEI). TfR1 is a crucial membrane glycoprotein responsible for receptor-mediated iron uptake, playing fundamental roles in erythropoiesis and immune function, and linking impaired iron homeostasis to profound immune dysfunction. Notably, TFRC deficiency provides a biologically instructive human model demonstrating that iron uptake via TfR1 is a non-redundant metabolic and signaling checkpoint required for antigen receptor–driven immune activation, clonal
