Gastrointestinal stenosis: an underrecognized complication of CARMIL2 deficiency.

CARMIL2 deficiency is a rare autosomal recessive combined immunodeficiency classically associated with dermatitis, inflammatory bowel disease (IBD), recurrent infections, and Epstein-Barr virus-related tumors. Gastrointestinal (GI) stenosis remains an underrecognized but potentially life-threatening complication. We report a consanguineous Moroccan family in which all three siblings with CARMIL2 deficiency developed early-onset, severe, and progressive GI stenoses, including pyloric and esophageal involvement. The striking consistency and severity of this phenotype within a single family highl

A Novel Biallelic REL Frameshift Variant p.(Tyr9Ilefs*2) Causing Immunodeficiency-92 With Profound c-Rel Deficiency.

BACKGROUND AND AIMS: Inborn errors of immunity (IEI) refer to a heterogeneous group of monogenic disorders caused by germline variants that disrupt immune function. Among these conditions, immunodeficiency 92 (IMD92), an extremely rare autosomal recessive disorder due to c Rel deficiency that results from pathogenic variants of the REL gene. c Rel is a key actor of the NF-κB pathway with major implications in the immune response. Up untill now, only two patients with confirmed pathogenic REL variants have been reported. METHODS AND RESULTS: Here, a third case is described of a 5-year-old Moro

From PGT-M discovery to mechanism: functional validation of novel compound heterozygous RAG1 mutations in severe combined immunodeficiency.

PURPOSE: Severe combined immunodeficiency (SCID) is a life-threatening primary immunodeficiency disorder. This study aimed to identify novel recombination activating gene 1 (RAG1) variants in a Chinese pedigree and characterize their impact on protein structure and function, providing a genetic basis for preimplantation genetic testing for monogenic (PGT-M) cycle. METHODS: Potential RAG1 mutations of the probands were screened by whole-exome sequencing (WES) and confirmed by Sanger sequencing. Configuration predictions of the variants were achieved using SWISS-MODEL. PROVEAN, PolyPhen-2, and

Homozygous loss-of-function mutation in SIT1 leads to combined immunodeficiency due to dysregulated T-cell receptor signaling.

BACKGROUND: The transmembrane adaptor protein SIT1 (signaling threshold-regulating transmembrane adaptor 1) is a negative regulator of T-cell receptor (TCR) signaling, but its role in human immunity remains poorly characterized. OBJECTIVE: We sought to investigate the molecular consequences of a biallelic loss-of-function SIT1 variant in a patient with combined immune deficiency and recurrent lymphoma. METHODS: Exome sequencing identified the putative disease-causing variant. Immunophenotyping and functional assays were performed on patient-derived lymphocytes and CRISPR-Cas9-mediated SIT1 k

Inborn errors of immunity with DNA repair disorders: at the interface of immune deficiency, immune dysregulation, and malignancy.

DNA repair disorders are characterized by defective DNA damage response and DNA replication pathways due to pathogenic variants in multiple genes involved in DNA repair, such as base excision, nucleotide excision, mismatch, non-homologous end joining repair machineries, as well as the homologous recombination process. They comprise a group of inborn errors of immunity of variable severity from hypogammaglobulinemia to severe combined immunodeficiency, accompanied by immune dysregulation and increased risk of malignant transformation. Clinical conditions present with a spectrum of syndromic fea

From T-cell defect to tumorigenesis: Epstein-Barr virus-associated smooth muscle tumors in ITK deficiency.

Interleukin-2-inducible T-cell kinase (ITK) is critical for T-cell receptor signaling and antiviral immunity. ITK deficiency is a rare combined immunodeficiency, classically linked to Epstein-Barr virus (EBV)-driven lymphoproliferation, with only two previous reports of EBV-associated smooth muscle tumors (EBV-SMT). We report a 13-year-old boy with recurrent respiratory infections and bilateral adrenal masses. Immunophenotyping, targeted gene panel sequencing, histopathology, immunohistochemistry, and EBV in situ hybridization were performed to investigate underlying immunodeficiency and tumor

Literature Review on HIV-Mtb Coinfection and Stroke Risk.

Tuberculosis (TB) is a leading global cause of infectious disease. Growing evidence suggests that TB significantly increases the risk of comorbidity in HIV+ patients, particularly acute ischemic stroke (AIS). Prognosis is often poor, highlighting the need for early prevention and intervention. However, exact mechanisms linking HIV-Mtb coinfection and stroke remain poorly understood. A review of articles from 2016 to 2026 was conducted to study the risk of ischemic stroke and treatment strategies in HIV-Mtb coinfection. Both HIV and Mtb can increase stroke risk, but pathogenesis of the coinfect

[Pulmonary hemosiderosis revealing class II HLA deficiency in a 9-month-old infant: a case report].

Pulmonary hemosiderosis is a rare and severe respiratory condition in children corresponding to chronic alveolar haemorrhage. Combined immune deficiencies are considered rare diseases, characterised by a quantitative or qualitative defect of T lymphocytes, associated or not with a complete or partial deficiency of immunoglobulin synthesis. We report a rare association of pulmonary hemosiderosis and class II HLA deficiency. Infant T, aged 9 months, was admitted for acute respiratory distress and pallor. The clinical examination found a marked mucocutaneous pallor, arterial oxygen saturation at

CCR5 as a Therapeutic Target in HIV Disease: From CRISPR/Cas9 Gene Editing to Maraviroc-Mediated Inhibition.

The C-C chemokine receptor type 5 (CCR5) is the principal co-receptor for R5-tropic HIV-1 and remains one of the most promising therapeutic targets in the pursuit of an HIV cure. The discovery that individuals carrying the naturally occurring CCR5Δ32 mutation exhibit marked resistance to HIV infection established the foundation for both genetic and pharmacological approaches to CCR5 inhibition. This review summarizes recent advances in CCR5-targeted therapies with a focus on CRISPR/Cas9-mediated gene editing and maraviroc-mediated receptor blockade. We discuss the molecular mechanisms, preclin

Practice parameter for inborn errors of immunity: What is new in the 2025 update.

The Practice Parameter for Inborn Errors of Immunity was revised in 2025 and published recently. We reviewed the most significant changes of this update regarding the previous parameter update in 2015. In contrast to previous revisions focused on individual genetic entities, this update was written with a new structure of recommendations divided into 2 parts: diagnosis and management. The diagnosis part includes discussions of newborn screening for severe combined immunodeficiency, genetic testing, and surveillance of comorbidities, in addition to reviewing clinically available testing for imm

Clinical and genetic landscape of SCID in Yunnan, China: identification of two novel RAG2 variants.

Severe combined immune deficiency (SCID) is a rare inherited defect of lymphocytes causing life-threatening opportunistic infections in early infancy. Data on SCID from China are limited. This study explores the clinical, immunologic, and genetic features of a SCID cohort from Yunnan Province in China and reports novel variants. We collated clinical, laboratory, and molecular details from patients with a clinical profile suggestive of SCID. Trio-based whole-exome sequencing was performed to identify genetic variants. For the 9 previously reported variants identified in our cohort, we systemati

Clinical Cognition and Practice in Anti-Interferon-γ Autoantibody-Associated Immunodeficiency Syndrome.

Adult-onset immunodeficiency syndrome mediated by anti-interferon-gamma autoantibody (AIGA) is a rare disorder that has garnered increasing recognition in recent years. The condition is most prevalent among adults in Southeast Asia. It is characterized by recurrent disseminated opportunistic infections, frequently involving multiple organ systems such as the lungs, lymph nodes, bones, and skin. The pathogenesis is primarily attributed to the presence of neutralizing AIGAs, which impair the IFN-γ-mediated JAK-STAT1 signaling pathway, leading to compromised host defense against intracellular pat

[Immune deficiency/dysregulation-associated lymphoma with an immunophenotype resembling EBV-positive nodal T/NK-cell lymphoma during treatment of follicular lymphoma].

In the 5th edition of the WHO classification, Epstein-Barr virus (EBV)-positive nodal T/NK-cell lymphoma (EBV+ nTNKL) is a newly defined, poor-prognosis disease originating from cytotoxic T cells. Here, we report a rare case of lymphoma arising in immune deficiency/dysregulation with an immunophenotype of EBV+ nTNKL that emerged during treatment for follicular lymphoma (FL). A 71-year-old woman was diagnosed with FL grade 3A and received chemotherapy with rituximab. Despite treatment, the disease relapsed repeatedly. During follow-up, she presented with fever and lymphadenopathy. An axillary l

[Clinical profile of patients with secondary immunodeficiency on immunoglobulin replacement therapy].

BACKGROUND: Secondary immunodeficiency (SID) is a disorder in which the immune system is compromised by external or acquired factors, reducing the quantity or function of immunoglobulins. This condition increases the need for specialist consultation, follow-up, and replacement therapy, which in turn raises healthcare costs. To date, few studies have clinically characterized patients with SID; therefore, understanding their profile will enable improved management and treatment. OBJECTIVE: To describe the clinical profile of patients with SID receiving human immunoglobulin replacement therapy (

DNA-repair and transcriptional defects drive hematologic malignancies in xeroderma pigmentosum group C patients: A new hypothesis.

Xeroderma pigmentosum (XP) is a rare recessive autosomal genodermatosis caused by defects in nucleotide excision repair (NER). Patients with XP are extremely sensitive to ultraviolet-light, resulting in an increased incidence of skin cancers in sun-exposed areas. Thanks to improved photoprotection and therapeutic education, the life expectancy of patients with XP has increased substantially. Recently, however, it has been observed that some XP patients are also at very high risk of developing internal tumors. Notably, 50% of these tumors correspond to severe myelodysplastic syndrome (MDS) and/

ABO: A 3D stroma-supported culture platform enabling full human B-lymphopoiesis for disease modeling and gene therapy development.

Defective B cell development underlies a large proportion of inborn errors of immunity. Progress in understanding disease mechanisms and therapy development remains limited because current human in vitro models incompletely recapitulate B-lymphopoiesis. We present a three-dimensional aggregate culture platform composed of human hematopoietic stem and progenitor cells (HSPCs) and mouse bone marrow (BM) stromal cells as an assay for efficient B cell output (ABO). ABOs support progression through pro-B, pre-B, immature, and transitional B cell stages, with a transcriptional profile and B cell rec

Evaluation of three bulk tank milk enzyme-linked immunosorbent assays to estimate within-herd prevalence of bovine leukosis virus in dairy farms.

The aim of this study was to estimate the bovine leukosis virus (BLV) herd-level and within-herd prevalence (WHP) in dairy herds and to assess the ability of three bulk tank milk (BTM) ELISA-Ab tests (SVANOVIR, Bovichek, and IDEXX) to predict the BLV WHP. A cross-sectional study was performed on a convenience sample of 93 dairy herds from Québec, Canada, where individual milk samples from all lactating cows and a BTM sample were collected. Individual milk ELISA-Ab results (n = 7612 cows) from a previously validated kit were incorporated into a two-stage hierarchical Bayesian latent class model
