Cell heterogeneity contributes to the variable response of HIV-1 to latency-reversing agents.

Transcriptional noise contributes to gene expression variability, but its origin and impact in HIV-1 latency remain incompletely characterized. Here, we combine a dual-copy MS2-tagging system with dedicated mathematical analysis to investigate the variability of HIV-1 transcription in live cells. In the basal state, the transcriptional activity of proviruses located in the same cell was uncorrelated, indicating that expression variability primarily comes from the intrinsic stochasticity of promoter dynamics. Upon stimulation with diverse latency-reversing agents, transcription of viral copies

Characterization of Strategies to Enhance the Provision of Holistic Services Across Three Community-Based Organizations in King County, Washington, United States.

INTRODUCTION: Addressing social determinants of health within HIV services is essential to achieve HIV epidemic control in the United States. Community-based organizations (CBOs) are uniquely positioned to implement integrated services, particularly for under-resourced and marginalized communities. However, effective strategies to support integrated services are needed for these settings. In a community-academic partnership, we applied implementation science tools to identify, prioritize and characterize strategies supporting three CBOs' goals to enhance integrated services and reduce health i

Mapping the implementation process and outcomes of HIV counselling and testing programmes in low -middle-income countries: a scoping review protocol.

BACKGROUND: HIV counselling and testing (HCT) programmes remain a cornerstone of global HIV prevention, treatment, and care strategies, particularly in low- and middle-income countries (LMICs) where the burden of HIV is disproportionately high. Despite progress in expanding access to testing services, gaps persist in implementation, uptake, and effectiveness of HCT interventions. Variations in health system capacity, sociocultural factors, stigma, and resource constraints continue to influence programme delivery and experiences. Understanding implementation processes and outcomes is essential

Efficacy and safety of switching to once-daily oral doravirine plus raltegravir in virologically suppressed people with HIV-1: a randomized, open-label, multicentre phase 2 study (DORAL).

BACKGROUND: We evaluated the efficacy and safety of a once-daily oral two-drug regimen (2DR) combining doravirine and raltegravir in virologically suppressed people with HIV (PWH). METHODS: DORAL is a multicentre, open-label, randomized phase II trial conducted in France, Italy, and Spain. Virologically suppressed adults receiving stable antiretroviral therapy (ART), naïve to doravirine and without raltegravir resistance, were randomized (2:1) either to switch immediately to doravirine (100 mg) plus raltegravir (1200 mg) or to continue baseline ART until week 48 before switching to doravirine

Managing HIV in the extreme: Viral load and drug monitoring in a patient with BMI > 60 on long-acting cabotegravir/rilpivirine therapy.

This case describes the use of long-acting injectable (LA-I) cabotegravir/rilpirivine (CAB/RPV) therapy in a morbidly obese person (body mass index [BMI] = 62 kg m-2) living with HIV without sustained viral load (VL) suppression after historic poor adherence to daily oral antiretroviral therapy. Increased BMI > 30 kg m-2 is a known risk factor for VL rebound with CAB/RPV failures; thus, RPV therapeutic drug monitoring (TDM) was followed with regularly scheduled VL assessments. Based on TDM concentrations, monthly dose titration from standard CAB/RPV 400 mg/600 mg to 600 mg/900 mg increased tro

System-wide investments generate health and enhance HIV, TB and malaria control in Malawi.

Global health initiatives have expanded access to HIV, tuberculosis, and malaria services in low-income countries, yet these programmes often operate within fragile health systems where shortages of healthcare workers and essential medicines limit their effectiveness and sustainability. Evaluating investments in health system capacity remains challenging because their benefits are difficult to attribute. Here, using a dynamic microsimulation model of Malawi's health system, we show that health system strengthening investments alone could avert 11.4% of disability-adjusted life years between 20

Manipulation of the cell cycle by HIV-1.

Many viruses manipulate the host cell cycle to enhance viral replication and promote their evasion of host immune defenses. Human immunodeficiency virus type 1 (HIV-1) is no exception and triggers cell cycle arrest during its infection of replicating CD4+ T cells, acting predominantly through the activities of the viral proteins Vpr and Vif. Here, we summarize HIV-1's effects on cell cycle regulation, focusing on the distinct mechanisms that underpin Vpr and Vif's modulation of cell cycle checkpoints. We also discuss how these activities, when combined, may help the virus to enhance replicatio

Proton Radiotherapy Offers Immune-Sparing Benefits in Glioblastoma Treatment.

PURPOSE: Glioblastoma (GB) and x-ray-based radiation therapy are associated with lymphopenia, which worsens prognosis and impairs antitumor immunity. Proton therapy, with its dosimetric properties that spare healthy brain tissue, is an alternative to reduce radiotoxicity. This study examined the effects of brain irradiation with x-rays or protons on lymphopenia. METHODS AND MATERIALS: We used GL261-tumor-bearing and tumor-free mice models to evaluate various lymphoid and myeloid cell populations after brain irradiation, assessing volume of brain irradiation, beam orientation, and lymphopenia

Metabolic syndrome (MetS) is a significant factor for cardiometabolic comorbidities in people living with HIV (PLWH) and a barrier to healthy aging. The long-term consequences of HIV-infection and combination antiretroviral therapy (cART) in metabolic reprogramming are unknown. In this study, we investigated metabolic alterations in well-treated PLWH with MetS to identify potential mechanisms behind the MetS phenotype using advanced statistical and machine learning algorithms. We included 200 PLWH from the Copenhagen Comorbidity in HIV-infection (COCOMO) study. PLWH were grouped into PLWH with

HIV/AIDS remains a global health burden affecting 39 million people worldwide, including 466,978 cases in Indonesia. Beyond the medical challenge, people living with HIV/ AIDS (PLWHA) frequently experience anxiety due to stigma and uncertainty about the future. This study aims to analyse the effectiveness of integrated breathing exercises combining diaphragmatic and pursed-lip breathing in reducing anxiety among PLWHA. A quasi-experimental two-group pre-post design was conducted. A total of 46 PLWHA were allocated. Anxiety was measured using the Hamilton Anxiety Rating Scale (HARS). The Wilcox

Carotid inflammation on FDG-PET is associated with lower cognitive performance in people with treated HIV infection.

OBJECTIVES: Carotid atherosclerosis is an established risk factor for cognitive impairment. FDG-PET detects subtle inflammatory changes in the arterial wall that may precede plaque formation and rupture. We investigated whether carotid inflammation is associated with cognitive function and its relative importance as a predictor of concurrent cognitive function compared with other factors. DESIGN/METHODS: : In this cross-sectional study, we recruited individuals ≥40 years with undetectable viral load at moderate-to-high cardiovascular risk. Participants underwent FDG-PET, stress and cognitive

Recent insights from prophylactic passive immunization trials of HIV-1 broadly neutralizing antibodies: implications for vaccine design.

PURPOSE OF REVIEW: An HIV-1 vaccine remains an urgent need, underscored by funding cuts that threaten treatment programs. The antibody mediated prevention (AMP) trials confirmed that broadly neutralizing antibodies (bNAbs) can prevent HIV-1 acquisition, and provided paradigm-shifting insights into HIV-1 biology and protective thresholds. This review discusses the implications of these findings for HIV-1 vaccine design and identifies some of the key knowledge gaps that remain. RECENT FINDINGS: The AMP trials emphasized the necessity of potent, broad bNAbs to prevent HIV-1 acquisition. Subseque

The Rheumatic Manifestations in Patients with Combined Immunodeficiency.

Combined immunodeficiencies (CIDs) represent a rare group of inherited immune disorders in which defects in T- and B-lymphocyte function lead to recurrent infections, immune dysregulation, and an increased tendency toward autoimmune and rheumatologic complications. A retrospective cross-sectional analysis was performed on 150 patients with CID, diagnosed according to the European Society for Immunodeficiencies (ESID) criteria and followed at the Children's Medical Center and Mofid Children's Hospital (Tehran, Iran) between 2009 and 2020. Clinical records, immunologic evaluations, and rheumatol
