IMPACT (Immune Monitoring and Phenotype Assessment of Clinical Trajectory) for Patients With Inborn Errors of Immunity From the Primary Immune Deficiency Treatment Consortium (PIDTC).

BACKGROUND: Evaluating natural history and outcomes of rare inborn errors of immunity (IEIs) has been challenging due to the diverse disease manifestations and lack of standardized data. OBJECTIVE: To develop a set of standardized, quantitative, generalizable data collection modules to measure organ dysfunction in IEIs, modeled on the previously validated Common Terminology Criteria for Adverse Event system for grading treatment toxicity. METHODS: Disease conditions were organized by organ system. Experts from the Primary Immune Deficiency Treatment Consortium identified essential features o

Redo ascending aorta and total arch replacement in patient with Wiskott-Aldrich syndrome: A case report.

Wiskott-Aldrich syndrome (WAS) is an X-linked genetic disorder characterised by eczema, immunodeficiency, and thrombocytopenia. Cardiopulmonary bypass causes a reduction in platelet numbers and function leading to an increased risk of bleeding. We report a case of a 33 year old male with WAS who underwent a redo ascending aorta and total aortic arch replacement as part of a two stage surgery for a Type B aortic dissection. The report discusses the considerations given to cardiopulmonary bypass in order to mitigate platelet number reduction and the degree of systemic inflammatory response. Post

An asymptomatic WASF1 truncation reveals pathogenic mechanism and therapeutic strategy for neurodevelopmental disorders.

INTRODUCTION: Wiskott-Aldrich syndrome protein family member 1 (WASF1) truncating variants, such as c.1516C>T (p.Arg506Ter), are established causes of neurodevelopmental disorders (NDDs), but their underlying pathogenic mechanism remains debated. This study aimed to clarify the disease mechanism and identify potential therapeutic leads. METHODS: We characterized a novel, asymptomatic WASF1 truncating variant (c.873delA) and compared its clinical and molecular consequences with those of the known pathogenic c.1516C>T variant. To target the likely pathogenic mutant protein, we performed high‑th

Wiskott-Aldrich syndrome and X-linked thrombocytopenia: a review of the clinical and immunological spectrum with a case presentation highlighting glomerulonephritis.

BACKGROUND: Wiskott-Aldrich syndrome (WAS) is a rare X-linked primary immunodeficiency caused by mutations in the WAS gene, characterized by thrombocytopenia, eczema, recurrent infections, and immune dysregulation. Its milder form, X-linked thrombocytopenia (XLT), exhibits variable autoimmune manifestations. Renal involvement, particularly glomerulonephritis, is uncommon but clinically significant. CASE PRESENTATION: We report a 10-year-old boy presenting with severe thrombocytopenia, hyporegenerative anemia, nephritic-range proteinuria, and hematuria, alongside multiple autoantibodies includ

Targeting the WASF3 Regulatory Complex in Pancreatic Cancer Using Stapled Peptides.

Wiskott-Aldrich Syndrome Protein Family (WASF) members form a heteropentameric complex along with ABI1/2, NCKAP1, CYFIP1/2, and BRK1, termed the WASF Regulatory Complex (WRC), which regulates actin cytoskeletal remodeling. Upregulated WASF3 has been identified in pancreatic cancer and is associated with a poor prognosis, increased invasion, and metastasis. In this study, we sought to determine whether disruption of the WRC could suppress WASF3-mediated actin polymerization and subsequent cellular invasion and motility in pancreatic cancer cells. Here, we screened constrained peptides designed

Arf1 is involved in Neisseria meningitidis-induced cortical branched F-actin network reorganization.

Cells experience external forces that deform the plasma membrane to which they adapt by reorganizing their actin cytoskeleton. Here, using the extracellular bacterium Neisseria meningitidis as a model system, we explore how this bacterium reorganizes the cortical actin cytoskeleton subsequently to mechanical membrane deformations. Meningococci trigger the formation of tubular cellular plasma membrane protrusions by a previously described adhesion-driven process known as one-dimensional wetting. Cryo-electron tomography reveals that in epithelial cells such a deformation of the plasma membrane

Atypical Wiskott-Aldrich syndrome presenting with normal platelet volume and end-stage renal disease: from misdiagnosis as ITP to combined transplantation.

We report a 30-year-old male with a novel WAS mutation (c.252C > A, p.F84L) who was misdiagnosed with immune thrombocytopenia (ITP) for years. Despite persistent thrombocytopenia, his mean platelet volume remained normal, an atypical feature of Wiskott-Aldrich syndrome (WAS). His course was complicated by stage-5 chronic kidney disease, renal anemia, IgA nephropathy, and recurrent infections. Laboratory tests consistently showed low lymphocyte counts and reduced WAS protein (WASp) levels. After confirming WAS, the original plan of kidney transplantation alone was changed to combined hematopoie

The Wiskott-Aldrich Syndrome protein (WASp) contribution to microglial phagocytic function and neurodevelopmental support.

Myeloid progenitor cells colonize the brain during embryogenesis and differentiate in microglia. Microglia shape neuronal wiring during development and maintain brain homeostasis in adulthood, both actions requiring intact cytoskeletal functionality. The Wiskott-Aldrich syndrome protein (WASp) mediates cytoskeletal dynamics of peripheral myeloid cells, suggesting a similar role in microglia. To investigate WASp's role in microglia, we impaired WASp function in human induced pluripotent stem cells-derived microglia (iMicro) and zebrafish embryos.WASp colocalized with the actin cytoskeleton at m

Gene therapy approaches for inborn errors of immunity: from bench to bedside.

INTRODUCTION: Inborn errors of immunity (IEI) are rare genetic defects that disrupt immune function, often resulting in life-threatening infections, malignancies, and immune dysregulation. Allogeneic hematopoietic stem cell transplantation (HSCT), a curative option for some diagnoses, is limited by donor availability and the risks of graft-versus-host disease. This review explores the 30-year evolution of autologous gene therapy as a vital alternative to allogeneic HSCT for IEIs. AREAS COVERED: Literature search using PubMed for gene therapy for IEI in the last 20 years. We trace the transiti

The spectrum of bleeding in Wiskott-Aldrich syndrome: a systematic review and meta-analysis of incidence and mortality.

BACKGROUND: Bleeding is the predominant clinical manifestation and one of the leading causes of death of Wiskott-Aldrich syndrome (WAS). However, significant discrepancies in reported bleeding phenotypes persist. OBJECTIVE: This systematic review and meta-analysis aim to provide a comprehensive characterization of the bleeding phenotype and elucidate the resultant mortality burden among patients with WAS. METHODS: Observational studies reporting either the cumulative incidence of bleeding manifestations or the occurrence of fatal hemorrhagic events in patients with WAS were included. The Joa

Regulation and dysregulation at a critical hematopoietic junction: The human WASP-WIP complex.

The complex formed between Wiskott-Aldrich syndrome protein (WASP) and WASP-Interacting Protein (WIP) is a potent regulator of cytoskeletal changes in hematopoietic cells. Mutations in the WASP N-terminal domain cause the primary immunodeficiencies Wiskott-Aldrich syndrome (WAS) and X-linked thrombocytopenia (XLT). Using NMR we determine the structure of the WASP/WIP complex and provide a first molecular view of this key biochemical junction. The central feature of this complex is the extensive binding interface formed by four WIP epitopes that wrap around the canonical EVH1 domain. Phosphoreg

In-depth characterization of stem cell potency and genotoxicity for clinical-scale ex vivo CRISPR-Cas9 gene editing.

Translating CRISPR-Cas9-based homology-directed repair (HDR) strategies into clinical application remains a major challenge due to limited standardization, concerns over safety, and efficacy issues. Here, we present a comprehensive and clinically compliant preclinical framework for the ex vivo correction of Wiskott-Aldrich syndrome (WAS) using a CRISPR-Cas9-AAV6 platform targeting hematopoietic stem and progenitor cells (HSPCs). In this study, we established a clinical-compatible platform enabling large-scale manufacturing while preserving HSPC viability, stemness, and multilineage functionali

WAVE2 and REST/NRSF Regulate Clustered Gene Expression by Maintaining Heterochromatin Organization.

The actin polymerization machinery, comprising the ARP2/3 complex and its activators, the WASP family proteins, has been implicated in regulating a broad spectrum of nuclear processes, such as transcriptional regulation and nuclear organization. Here, using clustered protocadherin (cPcdh) and β-globin genes as model systems, we showed that WAVE2, a member of the WASP family, regulates chromatin organization by maintaining heterochromatin dynamics. Specifically, by CRISPR DNA-fragment editing, in conjunction with integrated analyses of ChIP-seq, MeDIP-seq, ATAC-seq, QHR-4C, and RNA-seq, we show

WHAMM deficiency alters splenic B-cell populations, BCR signaling, and actin remodeling.

Members of the Wiskott-Aldrich syndrome protein (WASP) family orchestrate cytoskeletal reorganization that modulates B-cell receptor (BCR) signaling and B-cell fate decisions. WHAMM, a WASP-family nucleation-promoting factor member that associates with actin, membranes and microtubules, has not been functionally characterized in B cells. To investigate the role of WHAMM in B-cell development and function, we analyzed a conditional mouse model in which Whamm was deleted in the B-cell lineage. WHAMM-deficient mice exhibited altered splenic B-cell composition, characterized by an accumulation of

Impaired Glycolysis Leads to Defective Efferocytosis and Impaired Plaque Resolution in Tet2 Clonal Hematopoiesis.

BACKGROUND: Clonal hematopoiesis (CH) arising from mutations in hematopoietic genes has been identified as an important risk factor for atherosclerotic cardiovascular disease. Despite the established role of some CH mutations in promoting atherosclerosis progression, their role in clinically relevant LDL (low-density lipoprotein) lowering-induced plaque remodeling or regression has not been extensively studied. METHODS: To assess the effects of TET2 (tet methylcytosine dioxygenase 2) CH on plaque resolution, we prepared control or chimeric Tet2+/- CH mice with conditional deletion of Tet2 in
