Effects of prior irradiation on the microenvironment of a radiation-associated breast cancer model increase sensitivity to immune checkpoint inhibition.

PURPOSE: Radiotherapy confers a later risk of secondary malignancies, including radiation-associated breast cancer. Distinctive molecular profiles emerge in tumors from pre-irradiated tumor beds, but effects on therapeutic options remain understudied. METHODS AND MATERIALS: We developed a murine model of radiation-associated breast cancer by transplanting p53-mutant mammary HC11 epithelial cells that had been previously exposed to radiation (HC11RT) into mammary fat pads (MFP) that had been previously irradiated. RESULTS: Tumor incidence from HC11RT cells increased two-fold when grown in irr

Markedly Delayed Onset ICANS in the Setting of CAR T-Cell-Mediated Aseptic Meningoencephalitis: Two Cases.

OBJECTIVE: Immune effector cell-associated neurotoxicity syndrome (ICANS) typically manifests within the first week following chimeric antigen receptor (CAR) T-cell therapy and has rarely been described to develop beyond 1 month after treatment. This report describes the occurrence of markedly delayed ICANS in the setting of non-infectious meningoencephalitis. METHODS: We report two cases of markedly delayed ICANS with cerebrospinal fluid (CSF) pleocytosis comprised primarily of CAR T-cells. RESULTS: Two patients, aged 54 and 67 years, developed ICANS at 71 and 46 days, respectively, followi

Repurposing antimalarial drug atovaquone in combination with radiation and PD-L1 checkpoint blockade to enhance antitumour response in head and neck squamous cell carcinoma (HNSCC).

Atovaquone is an approved antimalarial drug which could sensitise tumours to radiation and may modulate tumour immune microenvironment. Here we evaluated the triple combination of radiation with atovaquone plus anti-PD-L1 in HNSCC syngeneic mouse tumour models. We observed significant tumour growth delay induced by the triple combination treatment with differential responses in three different mouse models. There was complete tumour eradication in 6 of 12 mice in the MOCL1 model but not in the other two murine models. The triple combination was associated with increased populations of CD8+-T G

SPHK1 drives pyroptosis via ROS-dependent activation of the NLRP3 inflammasome in non-infectious uveitis.

INTRODUCTION: Non-infectious uveitis associated with systemic diseases is a sight-threatening inflammatory disorder often associated with dysregulated immune responses and oxidative stress. Although redox imbalance is thought to contribute to its pathogenesis, the precise molecular mechanisms remain poorly defined. METHODS: We performed transcriptomic analysis of human non-infectious uveitis datasets (GSE17114 and GSE18781) encompassing Behçet's uveitis (BU), sarcoidosis-associated uveitis, and spondyloarthropathy (SpA)-associated uveitis, combined with experimental validation to elucidate ox

Multi-omics analysis of GBP2 as a prognostic and immune-associated biomarker in papillary thyroid carcinoma.

BACKGROUND: Guanylate-binding protein 2 (GBP2) is dysregulated in several malignancies and participates in immune regulation. This study aimed to characterize its expression, prognostic relevance, cellular distribution, immune associations, and biological role in papillary thyroid carcinoma (PTC). METHODS: We integrated multi-omics data from The Cancer Genome Atlas thyroid carcinoma project (TCGA-THCA), Gene Expression Omnibus (GEO), and the Human Protein Atlas (HPA). The TCGA-THCA cohort primarily comprises differentiated thyroid carcinoma (DTC), with PTC as the predominant histologic subtyp

A Classic Cutaneous Presentation of Thymoma-Associated Multiorgan Autoimmunity and Review of the Literature.

Thymoma-associated multiorgan autoimmunity (TAMA) is a rare paraneoplastic syndrome characterized by graft-versus-host disease (GVHD)-like manifestations. The disorder is thought to arise from a disruption to central immune tolerance of T cells secondary to neoplastic growth of thymic epithelial cells. Thymic epithelial cells are crucial in T-cell education: distinguishing between self-antigens and foreign antigens. This process relies on a protein, the autoimmune regulator (AIRE), which negatively selects and eliminates T cells that are self-reactive. With neoplastic transformation of thymic

Ghost Melanoma: Endobronchial Melanoma of Unknown Primary Mimicking Primary Lung Cancer.

Melanoma of unknown primary (MUP) with endobronchial involvement is a rare diagnostic presentation that may clinically and radiologically mimic primary lung cancer. We report the case of a 46-year-old man, a chronic smoker with a 20-pack-year history and a previous excisional biopsy of an atypical scalp nevus, who presented with low-volume hemoptysis and constitutional decline evolving over three months. Chest imaging revealed a large necrotic mass involving the right middle and lower lobes, associated with bilateral pulmonary nodules in a cannonball pattern and mediastinal lymphadenopathy, in

Construction, expression, and immunogenicity analysis of a fusion gene based on the PE/PPE family in combination with adjuvants.

Tuberculosis (TB) remains the leading global cause of death from a single infectious agent and a major fatal infectious disease worldwide. However, the Bacillus Calmette-Guérin (BCG) vaccine-currently the only licensed TB vaccine in clinical practice-has critical limitations in preventing latent infection and disease reactivation, failing to deliver comprehensive protective immunity. Given these gaps, targeting Mycobacterium tuberculosis (M.tb) antigens with strong immunogenic potential is a key strategy for next-generation TB vaccine development. The pathogenic M.tb harbors the Pro-Glu (PE) a

Polyclonal breast milk-derived secretory IgA confers broad antiviral protection against multiple respiratory viruses.

Passive immunization is an important tool for protecting individuals with primary immune deficiencies from infectious diseases and offers a rapid response strategy for protecting vulnerable populations, particularly during the interval between pathogen emergence and vaccine deployment. Current approaches rely on intravenous delivery of IgG-enriched plasma or monoclonal antibodies, which limits accessibility, scalability, and effectiveness at mucosal surfaces, the primary entry point for respiratory viruses. Here, we tested LCTG-001, a polyclonal secretory IgA preparation derived from pooled hu

The ablation margin as a regenerative and immunosuppressive niche in hepatocellular carcinoma: multicellular remodeling and organoid-on-a-chip modeling.

Thermal ablation is an important local treatment for hepatocellular carcinoma (HCC), yet post-ablation local tumor progression remains a major obstacle to durable disease control. Traditionally, recurrence has been attributed mainly to incomplete thermal coverage, inadequate ablative margins, or heat-sink effects. However, increasing evidence suggests that the sublethal thermal injury (STI) zone at the ablation margin is not merely a passive physical boundary, but a biologically active injury-repair microenvironment. Within this region, gradients of thermal dose, hypoxia, damage-associated mol

Hybrid spheroids of urine-derived renal epithelial cells: Structural and functional characterization.

Urine-derived Renal Epithelial Cells (URECs) are voided in urine after acute kidney damage and in response to ischemia reperfusion events. We previously investigated the phenotype and immunomodulatory properties of URECs derived from kidney transplanted patients. Here, we establish a 3D model combining URECs with Wharton's Jelly Mesenchymal Stromal Cells (WJ-MSCs), to evaluate structure, cell distribution and viability of the spheroids. Immunomodulatory properties were analysed by coculturing spheroids with Peripheral Blood Mononuclear Cells (PBMCs); CD4+ and CD8+ T cells proliferation, and th

Scalable cultured gut microbiota-derived nanovesicles for oral augmentation of checkpoint blockade therapy.

Microbiota-targeted strategies, such as fecal microbiota transplantation (FMT), hold promise for enhancing immunotherapy but face clinical challenges including safety risks, batch-to-batch variability, and dependence on substantial quantities of donor fecal material. To overcome these limitations, we established a standardized in vitro expansion platform for responder patient-derived gut microbiota. Through optimization of culture conditions, we achieved stable viability and retention of key immunotherapy-enhancing strains over serial passages, enabling a scalable 6.4 × 107-fold expansion. Lev

Review Article: The Evolution and Future of Immunotherapy: The Promise of CAR T Cell-Derived Extracellular Vesicles.

CAR T cell therapy, pioneered by Prof. Zelig Eshhar, has transformed hematologic malignancies but faces significant barriers in solid tumors, including poor infiltration, toxicity, and cytokine release syndrome. This perspective explores CAR T cell-derived extracellular vesicles (CAR T EVs) as an emerging platform that extends Eshhar's vision by delivering CAR-mediated cytotoxicity with enhanced tumor penetration, physiological barrier, and reduced immunogenicity. EVs carry functional CARs as immunomodulatory cargo, enabling precise antigen targeting without cellular proliferation risks. Key a

Review Article: Does CAR-T Cell Therapy of Solid Cancer Need the Activation of an Innate Immune Response?

CAR-T cells have shown efficacious in the treatment of hematologic malignancies; however, their efficacy towards solid tumors is so far limited by multiple challenges such as poor T cell infiltration, limited persistence, and immune suppression within the tumor tissue. On the other hand, tumor regression, especially of metastases, can spontaneously occur, which has long intrigued the scientific community. Spontaneous regression is thought to be associated with a more global immune response, triggered through the activation of both innate and adaptive immune cells. The viewpoint discusses strat

Review Article: The Extracellular Matrix as a Barrier for CAR-T Cell Therapy in Solid Tumors.

Chimeric antigen receptor T (CAR-T) cell therapies, based on introducing a genetically modified T cell receptor to a patient's T cells, have transformed the treatment of hematological tumors. However, their efficacy in solid tumors is limited to date. Among the multiple barriers that shape CAR-T cell efficacy, including antigen heterogeneity, hypoxic tumor microenvironment (TME), and immunosuppressive cell populations, the altered extracellular matrix (ECM) has emerged as a dominant but understudied obstacle. In this review, we summarize the key ECM alterations characteristic of solid tumors a

A Flow Cytometry-Based Method for Profiling Immune Cells in Human Blood and Tumors.

Flow cytometry enables the simultaneous characterization of multiple immune-cell populations and their phenotypic subsets in clinical specimens. However, comprehensive immunophenotyping of limited blood and tumor samples can be challenging because of limited cell availability, sample-processing requirements, and the need for standardized staining and gating procedures. Here, we describe a 12-color full-spectrum flow cytometry method for profiling major immune-cell populations in human peripheral blood and tumor tissue. The panel comprises 11 antibody markers and DAPI and is designed to identif

Evasive encounters: Immunomodulatory CAFs interact with Tregs in the lung cancer microenvironment.

Ringham et al.1 identify CHL1+CXCL9+ cancer-associated fibroblasts at lung tumor margins that recruit suppressive CXCR3+ regulatory T cells, revealing how stromal chemokine gradients can promote immune evasion. Viewed alongside immune-stimulatory CCL19+ fibroblastic reticular cells, these findings suggest that immune-interacting fibroblasts are context-dependent organizers of tumor immunity.

Tumor-targeted IL-10 synergizes with IL-2 to enhance antitumor immunity while minimizing immune-related adverse events.

Tumor-targeting cytokines have emerged as a promising strategy for cancer immunotherapy, although their mechanisms of action often remain complex. In this study, we develop an approach by combining tumor-targeted IL-10 (CmAb-(IL10)2) with IL-2 (CmAb-IL2) to enhance antitumor immunity while minimizing immune-related adverse events (irAEs). We demonstrate that endogenous IL-10 plays a critical role in IL-2-mediated antitumor effects, and that the combination of CmAb-(IL10)2 and CmAb-IL2 preferentially expands less-exhausted CD8+ T cells within tumors. Mechanistically, we identify that the IL-10/

Integrative Multi-Omics and Experimental Validation Define RELB as an Immune-Related Prognostic Driver in Ovarian Cancer.

Ovarian cancer (OC) is one of the most aggressive gynecological malignancies worldwide. The tumor microenvironment and transcriptomic alterations play important roles in OC progression and therapeutic response. Integrative bulk and single-cell transcriptomic analyses may facilitate the identification of prognostic biomarkers and potential therapeutic targets. Multi-Omic datasets from TCGA and GEO were analyzed. Immune-related genes were first integrated with survival information and screened using a random survival forest model to identify five core genes. GO/KEGG analyses were performed on di

Review Article: Chimeric MHC Class I Receptors for Diverse Clinical Applications.

Chimeric antigen receptors (CARs) are synthetic, modular receptors engineered to redirect T cells and other immune cells to recognize pre-selected antigens in an MHC-independent manner. CARs consist of an extracellular antigen-binding domain-typically an antibody single-chain variable fragment (scFv), a hinge region, a transmembrane domain, and intracellular signaling components. The modular CAR design facilitates the engraftment of diverse non-scFv binding moieties for redirecting immune cells toward non-conventional antigens or replacement of signaling elements according to the immune cell t
