A protective hybrid protein vaccine informed by mapping major histocompatibility class II epitopes in Cryptococcus neoformans chitin deacetylases that stimulate CD4 + T cell responses.

UNLABELLED: Infections due to Cryptococcus species are estimated to cause over 100,000 deaths a year. No cryptococcal vaccines are approved for human use. We have shown that vaccine formulations consisting of C. neoformans chitin deacetylase (Cda)2 and Cda1 adjuvanted with Cationic Adjuvant Formulation 01 (CAF01) protect mice against experimental cryptococcosis by a mechanism dependent upon CD4 + T cells. Here, using overlapping peptide libraries, we mapped epitopes in Cda2 and Cda1 that stimulated interferon-gamma (IFNγ) release from splenocytes of C57BL/6 and DR4 mice immunized with these pr

From inflammatory initiation to fibrotic remodeling: mechanisms and precision therapeutic strategies in otorhinolaryngologic involvement of IgG4-related disease.

IgG4-related disease (IgG4-RD) is a systemic fibroinflammatory disorder characterized by complex immune-mediated mechanisms and broad organ involvement that remains substantially underdiagnosed in clinical practice. The head and neck region is one of the most commonly involved anatomical domains, encompassing the major salivary glands, lacrimal glands, nasal cavity and paranasal sinuses, larynx and subglottis, thyroid gland, middle ear, orbit, and cervical lymph nodes. This review integrates current understanding of the immunopathological mechanisms underlying IgG4-RD with organ-specific clini

[Development of universal off-the-shelf T cell therapies derived from ES/iPS cells for leukemia and COVID-19].

Cancer immunotherapy using patient-derived T cells genetically modified in vitro has been demonstrated to be effective. However, issues such as cost, time, and unstable quality must be resolved. To overcome these barriers, we developed the TCR-PS cell method, in which a specific TCR gene is introduced into pluripotent stem cells (PS cells), such as ES cells or iPS cells, and T cells are generated from those PS cells. We are currently preparing for a clinical trial in acute myeloid leukemia, targeting the WT1 antigen, with iPS cells provided by the CiRA Foundation as the starting material. In p

Anti-pituitary-specific transcription factor-1 hypophysitis developing after thymoma resection: a case report with review of literature.

Anti-pituitary-specific transcription factor-1 (PIT-1) hypophysitis is an acquired autoimmune disorder characterized by selective deficiencies of growth hormone (GH), prolactin (PRL), and thyrotropin (TSH). It is typically driven by ectopic PIT-1 expression in thymomas or other malignancies, and in previously reported cases, thymectomy has been shown to attenuate autoimmunity. In contrast, we report a novel case that developed after thymectomy. A 42-year-old woman with normal preoperative thyroid function underwent resection of a WHO type B2 thymoma. Eleven months later, she developed specific

CD19 CAR T-cell therapy is feasible for patients with pemphigus vulgaris treated without lymphodepletion in the RESET-PV trial.

Lymphodepleting preconditioning (LD) is essential for the efficacy of chimeric antigen receptor (CAR) T-cell therapy in hematologic malignancies. However, in the setting of autoimmune diseases (ADs), the contribution of LD for efficacy is unclear. Here, we report on the early safety, efficacy, and correlative data of the first 4 patients with pemphigus vulgaris (PV) who received resecabtagene autoleucel (rese-cel), a fully human CD19 CAR T-cell therapy, without LD in the RESET-PV trial, a substudy of the DesCAARTes trial. Following infusion, pemphigus disease area index scores improved signifi

Drug therapies and immune pathways in ocular manifestations of inflammatory skin diseases.

INTRODUCTION: Inflammatory skin diseases and ocular disorders frequently coexist due to shared embryological origins, overlapping immune pathways, and common barrier functions. The increasing use of targeted systemic therapies in dermatology has further highlighted the skin-eye interface, introducing both therapeutic benefits and iatrogenic ocular complications. AREAS COVERED: This review outlines the shared embryological and immunological mechanisms linking the skin and ocular surface, with emphasis on major inflammatory pathways, including TNF-α, Th17, and JAK-STAT signaling. We summarize t

Deep immunological resetting in refractory systemic lupus erythematosus post-kidney transplantation: A longitudinal immunophenotypic analysis.

BackgroundA 66-year-old female with a 15-year history of refractory lupus nephritis, complicated by severe hypersplenism requiring splenectomy and 8 years of hemodialysis, successfully underwent deceased-donor kidney transplantation. Post-transplantation, she achieved an unprecedented, durable serological remission with a distinctive biphasic platelet recovery. MethodsTo monitor her immune reconstitution, we performed longitudinal peripheral blood immunophenotyping, contextualized against an active systemic lupus erythematosus (SLE) cohort and an age-matched healthy control group to account fo

Concurrent hereditary angioedema type I and common variable immunodeficiency presenting as suspected antibiotic hypersensitivity: Case report and literature discussion.

Hereditary angioedema (HAE) and common variable immunodeficiency (CVID) are rare disorders with heterogeneous clinical presentations that pose significant diagnostic challenges. HAE is characterized by recurrent episodes of subcutaneous or submucosal edema affecting the skin, gastrointestinal tract, or airway, whereas CVID presents with variable immune dysfunction, recurrent infections, and impaired vaccine responses. The clinical manifestations of these conditions can mimic allergic reactions, drug hypersensitivity, mast cell disorders, autoimmune diseases, and other immunodeficiencies, makin

Broadly neutralizing antibodies in adult males living with HIV undergoing analytical treatment interruption: secondary and exploratory outcomes of the phase II randomized controlled RIO trial.

RIO is an ongoing phase 2 double-blind randomized placebo-controlled human trial. Sixty-eight adult men living with human immunodeficiency virus (HIV), who were predominantly white and initiated antiretroviral therapy (ART) during primary or early-stage infection, underwent treatment interruption and were randomly assigned to a group receiving one or two doses of two long-acting broadly neutralizing antibodies (bNAbs) 3BNC117-LS and 10-1074-LS (arm A, n = 34) or saline (arm B, n = 34). The primary clinical study met its primary endpoint and was published elsewhere, demonstrating significantly

Effects of transcutaneous auricular vagus nerve stimulation or combined vagal and trigeminal nerve stimulation on platelet function and laboratory hemostasis parameters in healthy human subjects.

BACKGROUND: Traumatic or surgical hemorrhage causes substantial morbidity and mortality. Electrical vagus nerve stimulation (VNS) reduces traumatic hemorrhage in animal models. VNS targets acetylcholine-producing T lymphocytes in the spleen to increase intracellular calcium within circulating platelets via α7 nicotinic acetylcholine receptors. Elevated calcium levels facilitate platelet activation (priming) after tissue injury to accelerate and increase clot formation that improves hemostasis. Trigeminal nerve stimulation (TNS) also decreases traumatic hemorrhage in mice, but the mechanism rem

T Cell Immunosenescence in Inflammatory Skin Diseases: Pathogenesis and Therapeutic Targets.

T cell immunosenescence refers to the progressive functional decline of T lymphocytes with aging, characterized by the phenotypic markers, mitochondrial dysfunction, and the senescence-associated secretory phenotype (SASP), representing a pivotal aspect of overall immune aging. This review systematically elucidates the critical role of T cell immunosenescence in the pathogenesis of common inflammatory skin diseases, including psoriasis, atopic dermatitis, rosacea, and seborrheic dermatitis. Senescent T cells drive the production of a disease-specific SASP via internally dysregulated signaling

Rituximab remodels the circulating immune landscape in pemphigus vulgaris revealed by high-dimensional immune profiling.

Pemphigus vulgaris (PV) is a severe autoimmune blistering disease characterized by pathogenic autoantibodies primarily targeting desmoglein-3. B-cell depletion by rituximab (anti-CD20) is an effective first-line treatment for PV; however, relapses are frequently observed. Evidence suggests that PV is associated with immune dysregulation beyond the B-cell compartment. So far, existing studies have largely focused on selected immune components or populations, or specific clinical states such as active disease, remission, or immediate post-rituximab, leaving it unclear how prior rituximab treatme

STING Drives CD4+T Cell Differentiation via JAK-STAT Signalling in Bullous Pemphigoid.

Bullous pemphigoid (BP) is an autoimmune blistering disease with an increasing incidence in recent years; however, the underlying immune regulatory mechanisms remain largely unclear. As a critical signalling hub linking innate and adaptive immunity, stimulator of interferon genes (STING) has recently been implicated in the pathogenesis of various autoimmune diseases and may regulate tissue inflammation and immune homeostasis through modulation of CD4+ T cell responses. In this study, we found that STING expression was significantly increased in lesional skin tissues from patients with BP compa

Induction of regulatory T cells by an aluminum-CpG combination adjuvant for therapeutic and preventive vaccination against pollen allergy.

Pollen allergy is a prevalent allergic disorder affecting a substantial proportion of the global population. Allergen-specific immunotherapy (ASIT) is the only disease-modifying treatment, acting by inducing regulatory T cells (Tregs) and Th1-based immunity. However, current ASIT approaches often elicit insufficient allergen-specific tolerance and fail to correct underlying immune dysregulation, thereby limiting clinical efficacy. Here, a combination adjuvant composed of aluminum oxyhydroxide and CpG oligodeoxynucleotides (Al-CpG) is developed and formulated with the T-cell epitope derived fro

Shared Mechanistic Pathways in Bullous Pemphigoid, Chronic Spontaneous Urticaria, Prurigo Nodularis, and Chronic Prurigo of Unknown Origin: Implications for Targeted Therapies.

Bullous pemphigoid, chronic spontaneous urticaria, prurigo nodularis, and chronic prurigo of unknown origin are distinct chronic skin diseases with a high disease burden and an ongoing need for safe and effective therapies. Advances in our understanding of disease mechanisms have highlighted convergent type 2-associated neuroimmune pathways that may contribute to chronic itch and skin lesions across these conditions. In bullous pemphigoid, autoantibody binding of two dermal-epidermal junction proteins followed by complement system activation shapes the local immune environment to favor T helpe

Identification of a shared antigen linking CD4+ T and B cell pathology in Sjögren's disease.

Sjögren's disease (SjD) is an autoimmune disorder characterized by lymphocytic infiltration of exocrine glands. Although B cells producing anti-Ro60 autoantibodies are frequently found in salivary gland lesions, the antigen specificity of CD4+ T cells has remained unclear. Given accumulating evidence for T cell involvement in local autoantibody production, we comprehensively investigated Ro60 reactivity among lesion-infiltrating CD4+ T cells. Over 200 T cell receptors (TCRs) enriched in salivary glands from Japanese and Caucasian patients with SjD were screened using a TCR reporter system, ide
