This study presents a refined within-host human immunodeficiency virus (HIV) dynamics model that integrates several biologically relevant mechanisms often treated in isolation. The model incorporates a Beddington-DeAngelis functional response to describe the infection incidence, accounting for saturation effects in both target cells and free virus particles. It further includes a cure rate for infected cells, representing the efficacy of antiretroviral therapy or intrinsic immune clearance, and logistic growth for CD4[Formula: see text] T-cell populations. A novel contribution is the explicit

BACKGROUND: The number of CD4 T cells is an important indicator of immune status in people living with HIV (PWH). In China, numerous CD4 count instruments have been approved; however, no studies have been conducted to systematically compare the performance of these instruments.
METHODS: We retrospectively analyzed data from the national proficiency testing program for CD4 T-cell counts in China from 2016 to 2023. We conducted comparative analysis to evaluate the performance of 10 mainstream CD4 count instruments. Passing-Bablok regression and mix-effects model analyses were used to analyze th

Naïve CD4 T cells harbour functional subsets that influence the quality of immune reconstitution and the persistent viral reservoirs in people with HIV (PWH) under antiretroviral therapy (ART). Here, we profiled the circulating naïve CD4 T cells from PWH under effective ART with distinct past histories of viral burden, namely starting treatment early in acute or late in chronic stage HIV-1 infection, and people with HIV-2 (PWH2), who feature low to undetectable viremia before ART and slow disease progression. Spectral flow cytometry enabled us to capture the heterogeneity of this overlooked T

Virally suppressed people with HIV (PWH) remain at risk for developing comorbidities due to chronic inflammation with one potential contributor being the HIV reservoirAssociations between the CD4 reservoir and inflammation have been extensively characterized, while the role the monocyte reservoir is poorly understood despite evidence that inflammatory monocytes play a role in HIV-associated comorbidities. Additionally, most studies focus on a single cellular reservoir, while it is highly likely that these reservoirs are interdependent. In a cohort of 164 PWH, we used the intact proviral DNA as

DLBCL is a common fatal disease in AIDS patients, especially involved in CNS, due to immunity dysfunction and unknown pathogenic mechanism. We analyzed scRNA-seq data from seven AIDS patients and two immunocompetent patients. Through subgroup analysis comparing the cellular composition characteristics of different CNS-DLBCL, thereby elucidating the pathogenic drivers in distinct patient populations. In AIDS related CNS-DLBCL samples, we identified a population of tumor cells with neural characteristics, designated as neuro-signature cells. These population demonstrated high-grade malignancy, c

Despite significant advances in antiretroviral therapy (ART), human immunodeficiency virus (HIV) persists in long-lived viral reservoirs, requiring lifelong treatment and highlighting the need for curative strategies. Viral persistence across anatomically distinct reservoirs, together with HIV-associated immune dysregulation, supports the development of combination immunotherapies capable of acting through multiple antiviral mechanisms. Here, we developed and evaluated a Hybrid chimeric antigen receptor (CAR) platform that combines the targeted cytotoxicity of CAR-T cells with the secretion of

The ANRS VRI06 trial investigated CD40.HIVRI.Env, an Antibody Mediated Vaccine composed by a CD40 monoclonal antibody fused with gp140 Env, designed to target antigen to dendritic cells. Initial results in 72 HIV-uninfected volunteers showed that administration of 0.3-3.0 mg of CD40.HIVRI.Env, given alone or with DNA-HIV-PT123, was safe and immunogenic up to 48 weeks. After week 48 in the trial, 45 volunteers were randomized to receive a late boost (LB) of 0.3 mg CD40.HIVRI.Env, either with or without adjuvant, to assess continued safety and immunogenicity at 2 and 24 weeks post-LB. During early HIV-1 infection, robust Cytotoxic T-lymphocyte (CTL) responses are mostly targeted at immunodominant Gag p24 epitopes to reduce HIV-1 viraemia to a set-point. The aim of this study was to review the current body of knowledge on HIV-1 Gag CTL epitopes in the southern African region where subtype C is prevalent. Peer-reviewed records were obtained from three databases: PubMed Central, Web of Science Core Collection, and Scopus, using the following search terms: HIV subtype C Gag epitopes, and HIV clade C Gag epitopes. The search results were restricted to countries within the southe

Cervical cancer remains a major global health challenge despite advances in human papillomavirus (HPV) vaccination, screening, and treatment. Persistent infection with high-risk HPV types, particularly HPV16 and HPV18, is a necessary cause of cervical cancer; however, only a small fraction of infections progress to malignancy, indicating the importance of additional cofactors. Increasing evidence identifies estrogen signaling as a critical modifier of HPV-driven carcinogenesis. Estrogen acts synergistically with HPV oncogenes E6 and E7 to promote genomic instability, immune evasion, and tumor

Worldwide, most HIV infections occur across a mucosal surface during sexual contact, with women accounting for nearly half of reported new HIV infections across the globe in 2024. Most new infections in women and girls are believed to involve transmission across mucosal surfaces of the lower reproductive tract, which includes the vagina and ectocervix. Nevertheless, HIV-susceptible target cells are present in both the lower and upper reproductive tract, as are antigen-specific T-cells. Advances in the field of T-cell biology over the past decades have elucidated the differentiation and traffic

OBJECTIVE: The aim of this study was to determine whether baseline immune activation and inflammation contribute to heterogeneity in SARS-CoV-2 vaccine-induced immunity in antiretroviral therapy (ART)-treated people with HIV (PWH) compared to controls.
METHODS: In this multicenter prospective study, 159 ART-treated PWH from two cohorts and 56 HIV-negative controls were enrolled. The two PWH cohorts differed in immunovirological control: one showed sustained viral suppression and CD4 + T-cell recovery, the other persistent CD4 + T-cell lymphopenia despite ART, sometimes with residual viremia.

BACKGROUND: Failure of immune reconstitution (IR), characterized by suboptimal CD4+ T-cell recovery despite virologic suppression on antiretroviral therapy (ART), remains a critical challenge for people living with human immunodeficiency virus (PLWHIV). The contribution of immune senescence to this failure is incompletely understood.
OBJECTIVE: This study aimed to determine the role of pre-ART immune senescence characterized by lymphocyte profile and lymphocyte cellular senescence markers in nonimmune reconstitution (NIR) after 48 weeks of ART initiation.
METHODS: We first performed a SenMay

Juvenile-onset recurrent respiratory papillomatosis (JORRP) is a rare, human papillomavirus (HPV)-driven pediatric disease characterized by recurring papillomas in the respiratory tract. The contribution of dysregulated T-cell immunity to disease severity remains poorly understood. This study included 97 JORRP patients and 124 controls. Due to sample limitations, downstream analyses, including flow cytometry, TCR, and RNA sequencing, were performed on distinct patient subsets (detailed in Methods). We identified a selective expansion of TCR Vβ3+ T cells, including expanded TRBV28 clonotypes, i

Human papillomavirus type 16 (HPV16) causes more cancer than any other virus. However, most HPV16 infections are controlled by the host's immune system and it remains unclear how viral and host genetic variation contribute to infection outcomes. Here, we analyze 4704 HPV16 whole genomes to identify 56 viral codons putatively under positive natural selection to change their amino acids, with evidence including dN/dS > 1, evolutionary convergence, and structural importance in the protein. We find that codons under positive selection disproportionately overlap known HPV16 immune epitopes recogniz

As the primary receptor for HIV-1 entry, human CD4 is widely recognized as the hallmark of HIV-1 target cells. However, viral negative factor (Nef) and viral protein U (Vpu) down-regulate surface CD4 on infected cells, raising an intriguing question: If an infected CD4+ T cell survives and establishes latency, can CD4 expression recover? By tracking the fate of HIV-1-infected CD4+ T cells, we unexpectedly found that a subset of these cells directly convert to CD8+ T cells. T cell receptor (TCR) sequencing and single-cell RNA sequencing data revealed that these induced CD8+ T cells primarily or

Persistent reservoirs and chronic immune activation are hallmarks of HIV, despite the effectiveness of antiretroviral therapy (ART) in suppressing viral replication. Here, we use rhesus macaques and primary and induced pluripotent stem cell (iPSC)-derived human immune cells to evaluate the virologic and immunologic consequences of cannabidiol (CBD) exposure during HIV/SIV infection. We show that CBD, in the absence of ART, suppresses viral replication and establishment of the viral reservoir to levels comparable with first-line therapies during acute SIV infection of rhesus macaques. This anti

BACKGROUND: Human T-cell leukemia virus type 1 (HTLV-1) and simian T-cell leukemia virus type 1 (STLV-1) primarily infect CD4+ T-cells and spread via cell-to-cell contact in vivo. However, the mechanism underlying viral establishment and persistence in local tissues remain poorly understood. From the perspective of sexually transmitted infections, this study aimed to visualize the infection status of the genital epithelium in relation to sexual STLV-1 infection before and after menopause.
METHODS: In this study, we evaluated viral localization in the genital epithelium of naturally infected f

BACKGROUND: Antiretroviral therapy (ART) reduces cancer risk in people with HIV (PWH) but the long-term impact of immediate ART initiation on infection-related and infection-unrelated cancers remains unclear.
METHODS: In the Strategic Timing of Antiretroviral Treatment (START) trial, 4684 ART-naïve adult PWH with CD4 + T-cell count above 500 cells/mm3 were randomized to immediate or deferred ART initiation arms (i.e., until CD4 + T-cells below 350 cells/mm3). In May 2015, unblinding revealed a 74% reduction in infection-related cancer risk in the immediate arm. Post-2016, participants in the
