Polyclonal breast milk-derived secretory IgA confers broad antiviral protection against multiple respiratory viruses.

Passive immunization is an important tool for protecting individuals with primary immune deficiencies from infectious diseases and offers a rapid response strategy for protecting vulnerable populations, particularly during the interval between pathogen emergence and vaccine deployment. Current approaches rely on intravenous delivery of IgG-enriched plasma or monoclonal antibodies, which limits accessibility, scalability, and effectiveness at mucosal surfaces, the primary entry point for respiratory viruses. Here, we tested LCTG-001, a polyclonal secretory IgA preparation derived from pooled hu

Malignancies in inborn errors of immunity: a retrospective-prospective study on a large Italian cohort of pediatric and adult patients.

BACKGROUND: Inborn Errors of Immunity (IEI) are associated with an increased risk of malignancy due to impaired immune surveillance, genetic instability and chronic inflammation, However, comprehensive epidemiological data from large national cohorts remain limited. METHODS: We conducted a multicenter, retrospective-prospective observational study involving 4,540 pediatric and adult patients with IEI, followed across 30 Italian referral centers, with the aim of evaluating those who developed cancer. Clinical, immunological, genetic, and oncological data were prospectively collected from Octob

PIDgeon: An Explainable AI Model for Improved Flow Cytometry-Based Screening of Lymphoid Primary Immunodeficiencies.

BACKGROUND: Primary immunodeficiencies (PIDs) are rare disorders caused by immune system defects that are commonly screened using multi-parameter flow cytometry (FCM). To counter the subjective and time-consuming manual data analysis of FCM data, we present PIDgeon, a fully automated computational pipeline based on artificial intelligence (AI) techniques. PIDgeon is designed to characterize PID immune profiles, suggest PID subtypes based on altered immune profiles, age, and immunoglobulin levels, and generate interpretable reports. METHODS: The PIDgeon pipeline, including FlowSOM and extreme

IgA defects in CVID lead to bacterial translocation, increased serum γ-interferon, and BAFF.

Common variable immunodeficiency (CVID) is a primary antibody defect that leads to frequent infections, but inflammatory complications appear in 30-50%, leading to increased morbidity and mortality. We have previously shown that circulating bacterial 16S ribosomal DNA (rDNA), originating from gut commensals, is significantly increased in the serum of patients with CVID with inflammatory conditions (P = 0.0007). Here we examined the relationships between serum 16S ribosomal DNA (rDNA), isotype-switched memory (SM) B cells, serum IgA, IgA+ SM B cells, serum IFN-γ, and serum B cell-activating fac

Sarcoidosis: Disease mechanisms, diagnostic pathway and treatment.

Sarcoidosis is an inflammatory granulomatous disease that affects people worldwide and can involve virtually any organ but most commonly the lungs and thoracic lymph nodes. The cause of sarcoidosis remains unknown, but occupational and environmental exposures, genetic background, and ethnicity are likely contributors to disease development. Recent immunological studies, including single-cell RNA sequencing and spatial transcriptomics, have increased our understanding of disease pathogenesis. Diagnosing sarcoidosis is often challenging due to the lack of a diagnostic gold standard and the remar

Managing Primary Immunodeficiency Immunoglobulin Replacement Therapy-Related Adverse Events With Recombinant Human C1 Esterase Inhibitor Prophylaxis: A Case Report.

Immunoglobulin replacement therapy (IRT) for primary immunodeficiency reduces infection risk and subsequent complications and can be lifesaving. However, IRT can cause severe systemic adverse events (AEs) that may limit adequate dosing. These AEs may be caused, in part, by activation and/or consumption of complement proteins, thereby lowering C1 esterase inhibitor (C1-INH) levels. Data suggest that C1-INH administration prior to intravenous immunoglobulin (IVIG) may reduce IVIG-related AEs. This case describes an adult with common variable immunodeficiency unable to tolerate IRT therapy (subcu

Immune-related enteropathy.

PURPOSE OF REVIEW: Immune-related enteropathies (IREs) represent a heterogeneous group of disorders characterised by immune-mediated small bowel injury. While coeliac disease is the most common cause, widespread use of small bowel endoscopy has expanded the recognition of rarer causes. This review aims to summarise recent advances in diagnosis, classification and management of IRE. RECENT FINDINGS: Significant overlap exists between coeliac disease, autoimmune enteropathy, collagenous sprue, drug-induced enteropathies, common variable immunodeficiency (CVID) associated enteropathy and other I

Investigating serum free light chains in patients with common variable immunodeficiency disorder in compare with other immunodeficiency diseases.

Serum free light chains (sFLCs) have recently been introduced as a diagnostic biomarker in common variable immunodeficiency (CVID) patients. Most patients with CVID have undetectable or lower levels of sFLCs compared to people with other types of immune system deficiencies with hypogammaglobulinemia, except for patients with agammaglobulinemia (AGG). Decreased production of kappa (κ) and lambda (λ) light chains over immunoglobulins (Ig) suggests a malfunction in early B cell development or plasma cell dysfunction may cause CVID. In this cross-sectional study, immunoturbidimetry was used to mea

Autosomal dominant CDC45 deficiency with allelic expression bias causes a novel genetic disease of the immune system.

Here we describe a damaging heterozygous variant in CDC45 in an individual with common variable immunodeficiency (CVID) and recurrent viral infections. This individual has variably decreased number of circulating NK cells, disruption in the ratio of CD56bright to CD56dim cells, and consistently decreased NK cell function. Interestingly, the inherited CDC45 variant is also present in a sibling with less severe clinical manifestations; we determined that allelic bias of the damaging allele accounts for this differential expressivity. As previously reported for other helicase variants that cause

Case Series and Literature Narrative Review of Immune-Mediated Thrombotic Thrombocytopenic Purpura in Children.

Background/Objectives: Immune-mediated thrombotic thrombocytopenic purpura (iTTP) is a rare but life-threatening thrombotic microangiopathy in children. Secondary forms, occurring in association with immune dysregulation, autoimmune disease, or other triggers, are particularly challenging to diagnose and manage, and pediatric-specific data remain limited. This study aimed to describe the clinical characteristics, diagnostic pathways, and management of pediatric iTTP and to contextualize these findings within the recent literature. Methods: We conducted a retrospective case series of pediatric

Antimicrobial use on Campylobacter revealed by next-generation sequencing in patients with common variable immunodeficiency.

Because few studies have focused on recurrent Campylobacter bacteremia, we investigated two clinical cases of patients with common variable immunodeficiency and repeated Campylobacter bacteremia over a period of 6-10 years. We analyzed and compared genomes from isolates obtained from both patients during follow-up. For patient #1, 18 isolates of Campylobacter coli and 17 isolates of Campylobacter jejuni were obtained from 2014 to 2024. For patient #2, 10 isolates of C. coli were obtained from 2019 to 2024. Next-generation sequencing was used to identify species, characterize antimicrobial resi

Primary immunodeficiencies: Clinical spectrum and follow-up challenges.

Primary immunodeficiencies (PIDs) are a rare group of diseases with a broad clinical spectrum. Patients usually present with infections; they may also present with autoimmunity, lymphoproliferative disorders, and/or cancers or these comorbidities may occur during the disease. We aimed to analyze the demographic and clinical characteristics of adult patients with PID and evaluate the relationship between laboratory values and comorbidities with disease and each other. This study included 55 adult PID patients receiving immunoglobulin replacement therapy. Demographic characteristics, laboratory

Genetic Analysis of Patients With Common Variable Immunodeficiency Followed in the Pulmonology Department: A Report of Five Patients.

Common variable immunodeficiency (CVID) is a primary immunodeficiency characterized by hypogammaglobulinemia, impaired vaccine response, and increased susceptibility to infections. Advances in molecular genetics have enabled the identification of monogenic forms in an increasing proportion of patients. In this study, a targeted genetic panel based on the 2022 International Union of Immunological Societies classification was performed in five patients presenting with a phenotype compatible with CVID. A genetic abnormality was identified in two (40%) patients, including a variant in the NFKB1 ge

Extraintestinal enterobiasis presenting as a cervical mass in an immunocompromised adult after colorectal cancer surgery: A case report.

Enterobius vermicularis is a common intestinal helminth, whereas extraintestinal disease is rare and may present as a tumor-like lesion. We report a 50-year-old man with common variable immunodeficiency (CVID) and prior right hemicolectomy with adjuvant chemotherapy for cecal adenocarcinoma (pT3N1bM0) who developed a slowly enlarging left-sided cervical mass during oncologic follow-up. The lesion was painless and showed no overlying skin changes. Neck ultrasonography demonstrated a hypoechoic, round mass over the sternothyroid muscle with an elongated hyperechoic compartment and no Doppler flo

Multiniche mycobiome profiling identifies distinctive fungal dysbiosis in common variable immunodeficiency.

BACKGROUND: Common variable immunodeficiency (CVID) is associated with bacterial dysbiosis, particularly in patients with immune dysregulation, but the contribution of the fungal microbiome (mycobiome) remains poorly understood. METHODS: We conducted a cross-sectional, multi-compartment study in 41 adults with CVID (24 with immune dysregulation, dCVID; 17 with infectious-only manifestations, iCVID) and 15 matched healthy controls. Saliva, sputum and stool samples were analyzed using ITS1 amplicon sequencing with amplicon sequence variant-based taxonomic assignment, followed by α/β-diversity a

Sponge-like sonographic pattern of the spleen in immune dysregulation disorders.

BACKGROUND: Although splenic ultrasound (US) assessment is periodically performed in most patients with inborn error of immunity (IEI), splenic microtexture has not been specifically investigated in this setting. We describe a newly described "sponge-like" splenic microtexture pattern in patients with different IEIs and hematologic immune dysregulation, and discuss its potential implications. METHODS: We present an index case (P1) followed by a cohort analysis including 26 patients with IEIs and 20 controls with chronic immune thrombocytopenia, for whom splenic microtexture data and iconograp

Does CVID exist in children? A genetic architecture and manifestation map derived from 7,525 patients.

Diagnosing common variable immunodeficiency (CVID) in childhood remains contentious, as monogenic inborn errors of immunity (IEIs) are increasingly recognized in CVID-like phenotypes. We analyzed 7,525 ESID Registry patients with a clinical diagnosis of CVID to investigate age-dependent genetic architecture and associated phenotypes. Among living CVID patients, monogenic defects were identified in 82 of 251 children younger than 18 years (32.7%) versus 447 of 5,225 adults (8.6%; OR: 5.18, 95% CI: 3.86-6.92). Pediatric-onset disease (<18 years) likewise demonstrated increased monogenic underpin

How I treat: Diagnostic clues and treatment for activated phosphoinositide 3-kinase delta syndrome.

Activated phosphoinositide 3-kinase delta syndrome (APDS) is a form of common variable immunodeficiency (CVID) caused by gain-of-function variants in PIK3CD (APDS1) or loss-of-function variants in PIK3R1 (APDS2), resulting in hyperactivation of the PI3Kδ pathway. Clinically, it is characterized by recurrent respiratory infections, lymphadenopathy, splenomegaly, and progressive airway destruction, often leading to bronchiectasis. Enteropathy, cytopenia, and immune dysregulation have also been observed in several patients. Some patients develop malignant lymphomas, which can be fatal. Prophylact
