Large scale causal modeling to identify adults at risk for combined and common variable immunodeficiencies.

Combined immunodeficiencies (CID) and common variable immunodeficiencies (CVID), prevalent yet substantially underdiagnosed primary immunodeficiencies, necessitate improved early detection. Leveraging large-scale electronic health records (EHR) from four nationwide US cohorts, we developed a novel causal Bayesian Network (BN) model to identify antecedent clinical phenotypes associated with CID/CVID. Consensus directed acyclic graphs (DAGs) demonstrated robust predictive performance within each cohort (ROC AUC: 0.61-0.77) and generalizability across unseen cohorts (ROC AUC: 0.56-0.72) in identi

Diagnostic Challenges in Enteropathies: A Histopathological Review.

Various enteropathies, including immune-mediated (IME) and infection-related conditions, can lead to small intestinal mucosal injury and malabsorption. While immune dysregulation plays a central role in diseases like celiac disease and autoimmune enteropathy, other conditions such as small intestinal bacterial overgrowth (SIBO) and tropical sprue (TS) involve infectious or microbial pathogenesis. Common clinical manifestations include weight loss, chronic diarrhea, and nutritional deficiencies. While celiac disease (CD) remains the most prevalent IME in adults, an expanding spectrum of non-cel

X-linked Lymphoproliferative Disease Type 1 Presenting as Lymphoma in a Male Patient With Atypical Common Variable Immunodeficiency Features: A Case of Delayed Diagnosis.

X-linked lymphoproliferative disease type 1 (XLP1) is a rare, often fatal primary immunodeficiency (PID) caused by mutations in the SH2D1A gene that result in an uncontrolled immune response to Epstein-Barr virus (EBV) infection. However, a small number of patients do not encounter EBV exposure and exhibit clinical and immunological features similar to those of common variable immunodeficiency (CVID), leading to delayed diagnosis. Herein, we report the case of a 13-year-old boy diagnosed at eight years of age with CVID after he experienced recurrent pneumonias, hypogammaglobulinemia, and chron

Infection prevention in patients with inborn errors of immunity: Insights from Japan's nationwide study.

BACKGROUND: Differences in infection prevention methods and their corresponding effects on patients with inborn errors of immunity (IEI) between departments that treat patients with IEI and other departments, such as local hospitals, are not known. METHODS: We compared the differences in infection prevention methods between low-volume departments (treating fewer than 20 patients with IEI) and high-volume departments (treating 20 or more patients with IEI) using recent nationwide survey data from Japan. RESULTS: There was an uneven distribution of IEI categories between low- and high-volume d

Common Variable Immunodeficiency Disorder: A Decade of Insights from a Cohort of 150 Patients in India and the Use of Machine Learning Algorithms to Predict Severity.

Common Variable Immunodeficiency (CVID) is a heterogeneous disorder characterized by impaired antibody production and recurrent infections. In this study we investigated the clinical and immunological features of CVID in Indian patients and develops a machine learning model for predicting disease severity. We retrospectively analyzed 150 patients diagnosed with CVID over a decade at a tertiary care center in India. The median age of diagnosis was 18 years, with a male predominance (62%). The majority of patients (66.6%) had a severe phenotype, with recurrent respiratory tract infections being

Beyond Post-tuberculosis Sequelae: Uncovering Common Variable Immunodeficiency in an Adult.

Common variable immunodeficiency (CVID) is a rare immunodeficiency syndrome presenting with wide manifestations and leading to a delayed diagnosis. A 40-year-old male, a case of old treated tuberculosis, presented with a productive cough and hemoptysis. He had a history of recurrent respiratory symptoms previously attributed to post-tuberculosis sequelae with bilateral bronchiectasis, which can also occur as a manifestation of underlying common variable immunodeficiency (CVID). After a detailed evaluation, serum immunoglobulin levels were markedly reduced, confirming CVID. The patient was star

Thrombocytopenia in patients with inborn errors of immunity.

BACKGROUND: Inborn errors of immunity (IEI) are inherited defects of innate or adaptive immune system. Thrombocytopenia is a significant multifactorial complication in IEI patients leading to severe clinical consequences including coagulative disorders and vasculopathies. METHODS: In the present study, we assessed frequency of thrombocytopenia in the most common IEI including combined immunodeficiency (CID), common variable immunodeficiency (CVID), selective immunoglobulin A deficiency (SIgAD), agammaglobulinemia (AGA), hyper immunoglobulin M (HIGM) syndrome, chronic granulomatous disease (CG

Common variable immunodeficiency.

Common variable immunodeficiency (CVID) is characterized by defective differentiation of B lymphocytes, hypogammaglobulinemia, and infections, although its clinical presentation is much more heterogeneous, with autoimmunity, interstitial lung disease, enteropathy, lymphoproliferation, malignancy, among others. Patients with CVID should undergo extensive immunological evaluations and regular assessment of the multiple organs that may be affected, ideally by multidisciplinary teams specialized in immunodeficiencies. Infections usually improve significantly after the initiation of immunoglobulin

A mobile health-based alcohol reduction intervention and its usability among persons with Human Immunodeficiency Virus in Uganda.

BACKGROUND: Human Immunodeficiency Virus (HIV) and alcohol use are widespread in sub-Saharan Africa (sSA), negatively impacting the continuum of HIV care, from prevention and testing to treatment outcomes. Reducing alcohol consumption among people with HIV (PWH) is therefore a public health priority. Most alcohol reduction strategies require in-person interactions and can be challenging during pandemics and social unrest, which are common in sSA. Mobile Health (mHealth) interventions may offer a more accessible reduction counselling, an accessible alternative to, or may augment, in-person inte

Current concepts: Common variable immunodeficiency.

Common variable immunodeficiency (CVID) is the most frequent symptomatic primary immunodeficiency disorder found in clinical medicine. The immune defect occurs in males and females equally and is characterized by reduced serum levels of IgG, deficient IgA and/or IgM levels, and poor to absent specific antibody responses to infection or vaccinations. Although CVID is considered a primary immune defect, most subjects are diagnosed between the ages of 20 and 40 years old. However, due to the heterogeneous clinical appearance, a diagnostic delay of 5 to 8 years after the first cardinal symptom is

CXCL13 as a Biomarker of Complex Common Variable Immunodeficiency.

BACKGROUND: Common Variable Immunodeficiency (CVID) is a group of heterogeneous disorders with common denominators of impaired antibody production and function, and recurrent infections. Currently, prognostic biomarkers for CVID are limited. CXCL13 is a critical regulator of germinal centre responses and antibody production, with T follicular helper (Tfh) cells as a major source, and acts as a potent B cell chemoattractant. Serum levels of CXCL13 are increased in chronic inflammatory conditions and malignancy. OBJECTIVES: We aimed to explore whether serum CXCL13 levels are altered in CVID and

Standing Still: A Case of Stiff Person Syndrome and Common Variable Immunodeficiency.

Stiff-person syndrome (SPS) is a rare autoimmune neurologic disorder characterized by progressive rigidity and spasms, while common variable immunodeficiency (CVID) features hypogammaglobulinemia and recurrent infections. Their coexistence complicates management by requiring autoimmune suppression without further compromising host defense. A 54-year-old man with CVID, diagnosed based on hypogammaglobulinemia (IgG 386 mg/dL) and recurrent sinopulmonary infections, subsequently developed SPS with progressive truncal and limb stiffness, causing gait impairment and dyspnea. He experienced a severe

Primary Humoral Immunodeficiencies and Bronchiectasis in Adults.

Primary humoral immunodeficiencies are a heterogeneous group of disorders defined by quantitative and/or functional defects in one or more immunoglobulin classes, often with associated cellular immune abnormalities. Their link with bronchiectasis, whose prevalence varies across specific defects, is largely driven by recurrent respiratory infections. Selective Immunoglobulin-(Ig)A deficiency and IgG2 subclass deficiency are the most frequent forms, but common variable immunodeficiency (CVID) is the condition most often associated with bronchiectasis and is usually diagnosed earlier because of i
