The immunogenic cross-talk in pediatric MASLD: does subclinical immune dysfunction accelerate early hepatic destruction?

The pathogenesis of pediatric Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD) is increasingly recognized as an immunometabolic process driven by chronic low-grade inflammation (metainflammation). While nutritional surplus and visceral adiposity are established primary drivers, the contribution of underlying subclinical host immune variations remains largely unexplored. This mini-review investigates the intersection between childhood obesity and subclinical humoral immune dysfunctions, specifically variants of Common Variable Immunodeficiency (CVID). We propose that subtle defe

Autosomal dominant CDC45 deficiency with allelic expression bias causes a novel genetic disease of the immune system.

Here we describe a damaging heterozygous variant in CDC45 in an individual with common variable immunodeficiency (CVID) and recurrent viral infections. This individual has variably decreased number of circulating NK cells, disruption in the ratio of CD56bright to CD56dim cells, and consistently decreased NK cell function. Interestingly, the inherited CDC45 variant is also present in a sibling with less severe clinical manifestations; we determined that allelic bias of the damaging allele accounts for this differential expressivity. As previously reported for other helicase variants that cause

Are Selenium Status and Inflammation Linked to Cardiometabolic Risk Factors in Inborn Errors of Immunity?

BACKGROUND/OBJECTIVE: Patients with Inborn Errors of Immunity (IEI) are at increased cardiometabolic risk due to inflammation, immune dysfunction, and oxidative stress. Given selenium's essential role in antioxidant defense, this study evaluated plasma selenium concentration in individuals with IEI and its associations with inflammatory and metabolic biomarkers and body mass index (BMI). METHODS: A controlled cross-sectional study involved 85 patients, 22 with ataxia-telangiectasia (A-T), 32 with common variable immunodeficiency (CVI) and 31 with juvenile systemic lupus erythematosus (jSLE).

Case Series and Literature Narrative Review of Immune-Mediated Thrombotic Thrombocytopenic Purpura in Children.

Background/Objectives: Immune-mediated thrombotic thrombocytopenic purpura (iTTP) is a rare but life-threatening thrombotic microangiopathy in children. Secondary forms, occurring in association with immune dysregulation, autoimmune disease, or other triggers, are particularly challenging to diagnose and manage, and pediatric-specific data remain limited. This study aimed to describe the clinical characteristics, diagnostic pathways, and management of pediatric iTTP and to contextualize these findings within the recent literature. Methods: We conducted a retrospective case series of pediatric

Antimicrobial use on Campylobacter revealed by next-generation sequencing in patients with common variable immunodeficiency.

Because few studies have focused on recurrent Campylobacter bacteremia, we investigated two clinical cases of patients with common variable immunodeficiency and repeated Campylobacter bacteremia over a period of 6-10 years. We analyzed and compared genomes from isolates obtained from both patients during follow-up. For patient #1, 18 isolates of Campylobacter coli and 17 isolates of Campylobacter jejuni were obtained from 2014 to 2024. For patient #2, 10 isolates of C. coli were obtained from 2019 to 2024. Next-generation sequencing was used to identify species, characterize antimicrobial resi

Primary immunodeficiencies: Clinical spectrum and follow-up challenges.

Primary immunodeficiencies (PIDs) are a rare group of diseases with a broad clinical spectrum. Patients usually present with infections; they may also present with autoimmunity, lymphoproliferative disorders, and/or cancers or these comorbidities may occur during the disease. We aimed to analyze the demographic and clinical characteristics of adult patients with PID and evaluate the relationship between laboratory values and comorbidities with disease and each other. This study included 55 adult PID patients receiving immunoglobulin replacement therapy. Demographic characteristics, laboratory

Malignancies in the context of Inborn errors of immunity: an immunologist's view.

INTRODUCTION: Inborn errors of immunity (IEIs), also known as primary immunodeficiency diseases (PIDs) since 2017 Inborn Errors of Immunity Committee classification, comprise a heterogeneous group of genetic disorders resulting in impaired immune development and function. Malignancy is a major challenge in IEIs, particularly in those with defects in DNA repair, tumor suppression, immune surveillance, or chronic inflammatory control, highlighting the close interplay between immune dysfunction and oncogenesis. AREAS COVERED: Hematologic malignancies, especially non-Hodgkin lymphomas, predominat

Genetic Analysis of Patients With Common Variable Immunodeficiency Followed in the Pulmonology Department: A Report of Five Patients.

Common variable immunodeficiency (CVID) is a primary immunodeficiency characterized by hypogammaglobulinemia, impaired vaccine response, and increased susceptibility to infections. Advances in molecular genetics have enabled the identification of monogenic forms in an increasing proportion of patients. In this study, a targeted genetic panel based on the 2022 International Union of Immunological Societies classification was performed in five patients presenting with a phenotype compatible with CVID. A genetic abnormality was identified in two (40%) patients, including a variant in the NFKB1 ge

Extraintestinal enterobiasis presenting as a cervical mass in an immunocompromised adult after colorectal cancer surgery: A case report.

Enterobius vermicularis is a common intestinal helminth, whereas extraintestinal disease is rare and may present as a tumor-like lesion. We report a 50-year-old man with common variable immunodeficiency (CVID) and prior right hemicolectomy with adjuvant chemotherapy for cecal adenocarcinoma (pT3N1bM0) who developed a slowly enlarging left-sided cervical mass during oncologic follow-up. The lesion was painless and showed no overlying skin changes. Neck ultrasonography demonstrated a hypoechoic, round mass over the sternothyroid muscle with an elongated hyperechoic compartment and no Doppler flo

Multiniche mycobiome profiling identifies distinctive fungal dysbiosis in common variable immunodeficiency.

BACKGROUND: Common variable immunodeficiency (CVID) is associated with bacterial dysbiosis, particularly in patients with immune dysregulation, but the contribution of the fungal microbiome (mycobiome) remains poorly understood. METHODS: We conducted a cross-sectional, multi-compartment study in 41 adults with CVID (24 with immune dysregulation, dCVID; 17 with infectious-only manifestations, iCVID) and 15 matched healthy controls. Saliva, sputum and stool samples were analyzed using ITS1 amplicon sequencing with amplicon sequence variant-based taxonomic assignment, followed by α/β-diversity a

Sponge-like sonographic pattern of the spleen in immune dysregulation disorders.

BACKGROUND: Although splenic ultrasound (US) assessment is periodically performed in most patients with inborn error of immunity (IEI), splenic microtexture has not been specifically investigated in this setting. We describe a newly described "sponge-like" splenic microtexture pattern in patients with different IEIs and hematologic immune dysregulation, and discuss its potential implications. METHODS: We present an index case (P1) followed by a cohort analysis including 26 patients with IEIs and 20 controls with chronic immune thrombocytopenia, for whom splenic microtexture data and iconograp

Does CVID exist in children? A genetic architecture and manifestation map derived from 7,525 patients.

Diagnosing common variable immunodeficiency (CVID) in childhood remains contentious, as monogenic inborn errors of immunity (IEIs) are increasingly recognized in CVID-like phenotypes. We analyzed 7,525 ESID Registry patients with a clinical diagnosis of CVID to investigate age-dependent genetic architecture and associated phenotypes. Among living CVID patients, monogenic defects were identified in 82 of 251 children younger than 18 years (32.7%) versus 447 of 5,225 adults (8.6%; OR: 5.18, 95% CI: 3.86-6.92). Pediatric-onset disease (<18 years) likewise demonstrated increased monogenic underpin

How I treat: Diagnostic clues and treatment for activated phosphoinositide 3-kinase delta syndrome.

Activated phosphoinositide 3-kinase delta syndrome (APDS) is a form of common variable immunodeficiency (CVID) caused by gain-of-function variants in PIK3CD (APDS1) or loss-of-function variants in PIK3R1 (APDS2), resulting in hyperactivation of the PI3Kδ pathway. Clinically, it is characterized by recurrent respiratory infections, lymphadenopathy, splenomegaly, and progressive airway destruction, often leading to bronchiectasis. Enteropathy, cytopenia, and immune dysregulation have also been observed in several patients. Some patients develop malignant lymphomas, which can be fatal. Prophylact

Common Variable Immunodeficiency Associated With Connective Tissue Diseases: A Report of Two Cases.

Common variable immunodeficiency (CVID) is the most frequent symptomatic primary immunodeficiency in adults and is increasingly recognized as a disorder of immune dysregulation. Autoimmune diseases, including connective tissue diseases, may precede the diagnosis of CVID and contribute significantly to morbidity, particularly through pulmonary involvement. We report two adult women patients with connective tissue diseases (rheumatoid arthritis and primary Sjögren's syndrome) complicated by recurrent respiratory infections, interstitial lung disease, and severe hypogammaglobulinemia. In both cas

A Short Case Series of Common Variable Immunodeficiency From a Tertiary Care Hospital in South India.

Common variable immunodeficiency (CVID) is the most prevalent clinically significant primary antibody deficiency, characterised by markedly reduced serum immunoglobulin levels and impaired vaccine responses. It typically presents with recurrent sinopulmonary infections and may be associated with autoimmune phenomena, granulomatous disease, and malignancy. Despite its clinical impact, CVID remains substantially underdiagnosed, with a considerable delay between symptom onset and diagnosis. We present three cases managed at a tertiary care centre in Kerala, India, illustrating the diverse clinica

Antimicrobial toxic epidermal necrolysis in an ICU patient with multisystem organ failure and sepsis.

Toxic epidermal necrolysis (TEN) is a rare, life-threatening dermatological emergency characterised by blistering and desquamation, often triggered by medications. Early identification and discontinuation of offending agents are critical for improving outcomes. Here, we describe a woman in her 30s with a complex medical history, including systemic lupus erythematosus and common variable immunodeficiency, who developed TEN following antifungal and antibacterial therapy. She initially presented with respiratory distress and was treated for pneumonia. Over the ensuing days, her clinical course rap

Gastric Adenocarcinoma Associated With CTLA-4 Haploinsufficiency in the Background of Common Variable Immunodeficiency: A Rare Case.

Common variable immunodeficiency (CVID) is associated with immune dysregulation and an increased risk of gastric malignancy. The coexistence of CVID with autoimmune gastritis, pernicious anemia, and genetic defects such as cytotoxic T-lymphocyte antigen-4 (CTLA-4) haploinsufficiency may suggest a possible association with altered gastric adenocarcinoma risk. However, the clinical interplay remains incompletely understood and not well established. We present a case of a 54-year-old Caucasian woman with a high-grade intestinal tubulovillous adenoma in the stomach body, who developed gastric aden

Clinical and biochemical footprints of inherited disorders of autophagy.

Autophagy is an evolutionarily conserved lysosomal recycling system that integrates nutrient sensing, organelle quality control, proteostasis, cellular stress responses and metabolic adaptation. Autophagy is particularly relevant for post-mitotic tissue such as neurons, skin, and immune cells. Monogenic disorders disrupting autophagy or closely coupled endolysosomal trafficking pathways have recently emerged as a recognizable group of inherited metabolic diseases. These conditions are individually rare inborn errors of metabolism and collectively important because they bridge neurodevelopmenta

25-Hydroxyvitamin D Sufficiency Is Associated With Reduced Atelectasis in Common Variable Immunodeficiency: A Cross-Sectional Study.

BACKGROUND Common variable immunodeficiency (CVID) is an inborn error of immunity characterized by impaired antibody production, poor vaccine responses, and reduced serum immunoglobulin levels, resulting in respiratory manifestations that can progress to pulmonary complications. Although vitamin D has been associated with pulmonary outcomes in other populations, the association between serum 25-hydroxyvitamin D [25(OH)D] status and pulmonary manifestations, particularly atelectasis, in CVID remains unclear. MATERIAL AND METHODS We conducted a retrospective cross-sectional study of 48 adult pat
