Specification and Causal Pathway Diagramming of Implementation Strategies to Support Development of Access-Enhancing Injectable HIV Treatment Programmes at Eight Diverse Health Clinics Across the United States.

INTRODUCTION: Long-acting injectable antiretroviral therapy (LAI ART) represents a transformative development in HIV care because it may mitigate longstanding barriers that contribute to gaps in health outcomes. Providing LAI ART to people not consistently on HIV treatment or lacking treatment options requires intentional programme planning to overcome implementation barriers, without which only well-resourced patients benefit, worsening health outcome gaps. The ALAI UP Project delivered clinic-level implementation strategies to support eight clinics across Ending the HIV Epidemic jurisdiction

Newborn screening for severe combined immunodeficiency in the NHS newborn blood spot screening programme: a systematic review.

BACKGROUND: Severe combined immunodeficiency is an inherited condition arising from mutations in at least 19 known genes. Severe combined immunodeficiency can be identified through screening, family history or clinical presentation. Severe combined immunodeficiency is usually asymptomatic at birth and presents, in infancy, as recurrent and frequently severe infections. Without treatment, severe combined immunodeficiency is usually fatal in the first year of life. OBJECTIVES: To summarise the available evidence relevant to newborn screening for severe combined immunodeficiency in the UK NHS ne

Adult T-Cell Leukemia/Lymphoma and Targeted Maternal Screening.

IMPORTANCE: Adult T-cell leukemia/lymphoma (ATLL) is a rare but highly aggressive malignant disease caused by lifelong infection with human T-cell leukemia virus type 1 (HTLV-1), typically acquired through mother-to-child transmission. In southwestern Japan, the world's highly documented HTLV-1 endemic region, maternal HTLV-1 screening and breastfeeding counseling have reduced vertical transmission. Because of the long latency between infection and disease, reductions in ATLL burden are expected to occur over time. In the US, HTLV-1 screening is limited to blood donors, while mother-to-child t

A multi-phase project to develop a patient-reported measure of barriers to antiretroviral therapy adherence for use in HIV care: The 7-Item I-Score.

Adherence to antiretroviral therapy (ART) is a priority in HIV care. Our goal was to develop and validate a new, short patient-reported outcome measure to screen for barriers to daily oral ART adherence in HIV care in Canada and France. The overarching design was that of a multi-phase, multi-site instrument development project. It involved a previously published qualitative research synthesis (to develop a conceptual framework from which to generate items). Here, we present the results of cognitive interviews (for content validity testing of the preliminary items) and of a longitudinal survey-

Switch to fixed-dose doravirine (100 mg) and islatravir (0·25 mg) once daily in virologically suppressed adults with HIV-1 on oral antiretroviral therapy: 48-week results of a phase 3, multicentre, randomised, open-label, non-inferiority trial.

BACKGROUND: Doravirine and islatravir is an investigational, once-daily, single-tablet regimen containing two potent antiretrovirals with complementary mechanisms of action and resistance profiles. We aimed to evaluate the efficacy and safety of switching from stable, oral antiretroviral therapy (ART) to the fixed combination of doravirine (100 mg) and islatravir (0·25 mg) in virologically suppressed adults living with HIV-1. METHODS: This phase 3, randomised, active-controlled, open-label, non-inferiority trial was conducted at 53 research, community, and hospital-based clinics in eight coun

Point-of-care early infant HIV diagnosis at birth in a pragmatic cluster-randomized trial in Mozambique and Tanzania: A comparative cost and cost-effectiveness study.

BACKGROUND: Timely access to early infant diagnosis (EID) is crucial for newborns with HIV, as late diagnosis can delay lifesaving antiretroviral treatment (ART). We assessed the comparative cost and cost-effectiveness of integrating point-of-care EID at birth into routine care in primary healthcare settings. METHODS AND FINDINGS: This pre-specified secondary analysis was nested in the cluster-randomized LIFE study conducted at 28 primary healthcare facilities in Mozambique and Tanzania from October 2019 to September 2021. We estimated the health system cost of point-of-care birth plus 4-8-we

Semaglutide slows epigenetic aging in a randomized trial of HIV-associated lipohypertrophy.

Glucagon-like peptide-1 (GLP-1) receptor agonists have attracted interest as gerotherapeutics, yet clinical-trial evidence for their effects on biological aging is lacking. We report a post hoc exploratory epigenetic age analysis of a 32-week, randomized, double-blind, placebo-controlled phase 2b trial (NCT04019197) of semaglutide in adults with human immunodeficiency virus (HIV)-associated lipohypertrophy (semaglutide n = 45; placebo n = 39). A flexible rank-based framework for individualized treatment selection with mixed-type multivariate outcomes.

Personalized treatment selection is often based on a single primary endpoint, even though complex diseases are typically monitored using multiple, heterogeneous outcomes. We develop a rank-based framework for individualized treatment selection that accommodates mixed collections of continuous, ordinal, binary, and right-censored survival outcomes. For each outcome and treatment, we summarize the conditional outcome distribution given patient covariates using clinically meaningful parameters, including conditional quantiles, survival probabilities, and tail probabilities at ordered thresholds,

Towards ending tuberculosis in South Africa: an uncertainty analysis of the programmes and factors most critical to future declines.

BACKGROUND: The WHO End Tuberculosis (TB) strategy targets 80% and 90% reductions in TB incidence and mortality, respectively, between 2015 and 2030. OBJECTIVE: To assess which policy-changeable and external factors are most critical to reducing future TB in South Africa. METHODS: We adapted an existing mathematical model of TB and HIV in South Africa. Prior distributions were specified to represent uncertainty ranges for 27 model parameters that are highly uncertain and potentially important in driving future TB dynamics. Latin Hypercube Sampling was used to sample 1000 parameter combinatio

Guidelines for the transfer of people living with HIV attending primary healthcare facilities in South Africa: a scoping review.

People living with HIV may move between health facilities: this is called 'transfer', and includes up- and down-referral based on clinical condition and lateral transfer (e.g. between primary healthcare [PHC] facilities for reasons such as geographic mobility or stigma). Transfers involving PHC facilities occur frequently and are associated with viraemia and disengagement. We reviewed the South African National Department of Health (NDOH) and Southern African HIV Clinicians Society websites and contacted NDOH officials to identify national guidelines applicable to HIV care in South Africa for

Sexual Health Advocacy for Guys (SHAG): a randomized trial of the impact of a text-messaging program on HIV incidence and STI testing among a national sample of sexual minority cisgender adolescent and young adult men.

BACKGROUND: Disparities in sexually transmitted infections (STI) including human immunodeficiency virus (HIV) among sexual minority boys and young men are substantial. Effective HIV and STI prevention programs that include access to pre-exposure prophylaxis (PrEP) medication do not consistently include younger sexual minority men. Text-messaging programs for HIV prevention have been associated with increases in HIV testing among sexual minority adolescent boys, but these programs have not incorporated a focus on PrEP or STIs beyond HIV. METHODS: We will conduct a two-arm randomized trial with

Threshold sensitivity analysis for HIV-1 transmission cluster detection using different genomic regions and subtypes.

HIV-1 cluster analysis has been widely used in characterizing HIV-1 transmission and some countries have implemented such molecular epidemiology as part of their prevention strategy. However, HIV-1 sequences derive from varying genome regions, which affects phylogenetic clustering outputs. Here, we apply different tools to run a sensitivity analysis for assessing which threshold give the most cohesive clustering outputs for different data sources. We used a dataset of 174 full-length sequences of subtype B from the Swiss HIV Cohort Study and publicly available subtype C from South Africa. Each

Analytical and clinical performance evaluation of the Wantai chemiluminescence microparticle immunoassay on the Wan200 + analyzer for diagnosing HIV.

On September 1, 2019, China implemented the "Blood Station Technical Operation Procedures (2019 Edition)," specifying Enzyme-Linked Immunosorbent Assay (ELISA) and Chemiluminescent Immunoassay (CLIA) for serological testing. However, no CLIA HIV reagent has been approved for blood donor screening in China. This study evaluates the WT CMIA reagents for HIV screening in blood stations, providing evidence for their approval as a national screening tool. A controlled trial compared WT CMIA and ELISA reagents, as well as CLIA reagents from other manufacturers, through clinical experiments and refer

Implementation strategies to integrate HIV and hypertension care in Kampala and Wakiso districts, Uganda: study protocol for a stepped wedge cluster randomized trial (PULESA-Uganda).

BACKGROUND: Hypertension (HTN) is the leading cause of cardiovascular morbidity and mortality among people living with HIV (PLWH) in Africa, yet integration of hypertension treatment in HIV clinics is sub-optimal. Cost-effective strategies to implement evidence-based hypertension care for this population are urgently needed to preserve the life-expectancy gains of antiretroviral therapy. METHODS: Building on insights gained from a comprehensive mixed-methods formative assessment, we used a human-centered design approach to develop a multi-component HIV-HTN integration (HTN-PLUS) strategy.
