Novel cutaneous adverse effects of the increasingly prevalent glucagon-like peptide 1 agonists.

Prescribing of the glucagon-like peptide 1 receptor agonists (GLP-1RA) is rapidly increasing as their efficacy for multiple indications becomes apparent. A better understanding of their cutaneous adverse effect profile is crucial, particularly as they are suggested for use in inflammatory dermatoses. We describe two patients who developed novel cutaneous reactions associated with initiation of a GLP-1RA. Patient 1 developed multiple tender subcutaneous nodules on the trunk and limbs 4 weeks after the initiation of semaglutide. Histology demonstrated a noncaseating granulomatous dermatitis, and

Risk and characteristics of interstitial lung disease in patients with inflammatory bowel disease: A retrospective cohort study.

BACKGROUND: Inflammatory bowel disease (IBD) is a disorder with extraintestinal manifestations that may involve multiple organ systems. Interstitial lung disease (ILD) is an extraintestinal manifestation that remains underrecognized, and its prevalence, clinical characteristics, and risk factors in IBD are poorly defined. METHODS: We conducted a retrospective study of adults (≥18 years) with IBD who had at least 1 clinical encounter at our hospital between January 1, 2013, and December 31, 2023. IBD and ILD diagnoses were identified by using International Classification of Diseases, Ninth [or

Efficacy and safety of extended-release oral opioid for cough in idiopathic pulmonary fibrosis: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND AND AIM: Patients with idiopathic pulmonary fibrosis (IPF) experience a high burden of cough. Currently, no established effective therapy exists for cough in IPF. The overall evidence base of oral extended-release (ER) opioid therapies in this population remains limited. This study aimed to compare the efficacy and safety of ER oral opioids versus placebo for cough in patients with IPF. METHODS: PubMed, Embase, Cochrane, and ClinicalTrial.gov databases were reviewed for randomized controlled trials (RCTs). The primary outcome was 24-hour objective cough frequency. Secondary outcome

Fibroblast proliferative capacity predicts prognosis in idiopathic pulmonary fibrosis and identifies PITX1 as a growth-associated gene.

BACKGROUND: Idiopathic pulmonary fibrosis (IPF) is a progressive fibrotic lung disease with poor prognosis. Lung fibroblasts isolated from patients with IPF exhibit enhanced proliferative capacity; however, the molecular determinants linking fibroblast proliferation to clinical outcomes remain incompletely understood. METHODS: Fibroblast growth slopes were measured in primary lung fibroblast derived from patients with IPF and control subjects. Transcriptome microarray data were integrated with fibroblast growth kinetics to identify growth slope-associated genes. Differential expression analys

Latest updates in quality of life and psychological impact of sarcoidosis.

PURPOSE OF REVIEW: Health-related quality of life (HRQoL) is now recognized as a major dimension to take into account for a holistic care of sarcoid patients. Accordingly, it has to be precisely and reliably evaluated all along disease evolution in order to prompt appropriate measure when needed. When significantly impaired, HRQoL can now be managed through different approaches, whether or not pharmacological. The purpose of this review is to report on the factors identified as having a negative impact on HRQoL in sarcoid patients and on the innovative techniques to measure the latter and to m

[Chinese expert consensus on the clinical application of lung biopsy in interstitial lung disease].

Interstitial lung disease (ILD) comprises a highly heterogeneous group of pulmonary disorders, whose diagnosis often requires the integration of clinical, radiologic, and pathologic evidence. Lung biopsy is a key method for obtaining a histopathologic diagnosis, but unified standards are lacking for determining indications, selecting biopsy techniques, managing patients perioperatively, and integrating pathologic and clinical information. To address these gaps, the Respiratory Physicians Branch of the Chinese Medical Doctor Association and the ILD Group of the Chinese Thoracic Society spearhea

Derivation of a peripheral blood-based transcriptomic risk score for extrapulmonary sarcoidosis.

BACKGROUND: Extrapulmonary sarcoidosis can be identified in many tissues but as an unpredictable display in an already rare disease, has not been methodically examined. We sought to identify gene-expression differences in blood and derive a transcriptomic risk score (TRS) to predict extrapulmonary manifestations of sarcoidosis. METHODS: We used RNA sequencing data from the Genomic Research in Alpha-1 Antitrypsin Deficiency and Sarcoidosis (GRADS) study and a cohort from our institution. We performed differential expression mega-analysis as well as pathway enrichment and cell-type deconvolutio

Cardiac Sarcoidosis: A Practical Guide for Cardiologists.

PURPOSE OF REVIEW: Cardiac sarcoidosis (CS) is an inflammatory granulomatous cardiomyopathy associated with atrioventricular block, ventricular arrhythmias, heart failure (HF), and sudden cardiac death (SCD). Diagnosis and management remain challenging because myocardial involvement is patchy and evidence from randomized trials is limited. This review summarizes current knowledge on CS, focusing on advances in diagnosis, immunosuppression, and prevention of arrhythmic events. RECENT FINDINGS: Cardiac involvement is clinically recognized in 5-10% of patients with sarcoidosis but is more freque

Optic Perineuritis Mimicking Optic Nerve Sheath Meningioma: A Case Series.

Nanomaterials are increasingly applied in biomedicine and have significant potential in reproductive medicine. However, because they can impact germ cells, use of nanomaterials could represent a "double-edged sword", whose benefits must be balanced with the possible risks. Because of their excellent physicochemical properties, nanoparticles (NPs) can serve as multi-functional platforms for targeted drug delivery, enhancing outcomes in assisted reproductive technologies, refining in vitro culture systems, and improving the precision of diagnostic imaging. Nevertheless, accumulating evidence ind

Vertebral Sarcoidosis Mimicking Metastatic Disease in a Patient With Prior Melanoma: A Diagnostic Challenge.

Sarcoidosis is a multisystemic granulomatous disease that most commonly affects the lungs and intrathoracic lymph nodes. Osseous involvement is uncommon, and vertebral disease is particularly rare, often mimicking metastatic malignancy and posing a significant diagnostic challenge. We report the case of a 51-year-old woman with a history of resected melanoma who was incidentally diagnosed with sarcoidosis following abnormal findings on screening mammography. Subsequent evaluation revealed pulmonary involvement. Months later, she developed acute back pain following minor trauma, and magnetic re

Effect of home-based pulmonary rehabilitation in patients with interstitial lung disease in a tertiary care centre - A randomized controlled trial.

BACKGROUND AND AIM: Interstitial lung disease causes functional limitation and exercise intolerance. Although pulmonary rehabilitation in ILD can improve physical function and quality of life, multiple factors hinder its utilisation including lack of facilities, mobility, caregivers and cost. Therefore, alternative models such as home-based pulmonary rehabilitation are required to bridge the gap between guidelines and practice. This study explored whether a 12-week home-based pulmonary rehabilitation program could improve the functional capacity of ILD patients as measured by 6MWD when compare

The onset of sarcoidosis in relation to tattoos: Focus on the treatment landscape. A critical literature review and case report.

Sarcoidosis is a complex multisystem granulomatous disease characterized by the formation of non-caseating granulomas in affected tissues, primarily involving the lungs and the lymphatic system. While its precise etiology remains elusive, a combination of genetic predisposition, environmental factors, and immune dysregulation is believed to contribute to disease development. In recent years, an intriguing association has emerged between cutaneous sarcoidosis and tattooing. Several case reports and observational studies have documented instances where sarcoid granulomas develop within tattooed
