BACKGROUND: Sarcoidosis is a multisystem granulomatous disease that most commonly affects the lungs. 18F-2-fluoro-2-deoxyglucose (18F-FDG) PET/CT is a sensitive imaging modality for assessing of sarcoidosis. However, fasting before 18F-FDG PET/CT is generally required to improve image quality and diagnostic accuracy. In this context, our study aims to explore the utility of 68Ga-fibroblast activation protein inhibitors (68Ga-FAPI) PET/CT in sarcoidosis, with particular emphasis on lesion uptake patterns and intensity, as well as the correlation with disease activity.

UNLABELLED: A 61-year-old woman presented to our hospital with palpitations and dyspnea persisting for several months. Electrocardiography revealed atrial tachycardia, and X-ray revealed bilateral hilar enlargement. Computed tomography (CT) was performed for suspected sarcoidosis of the respiratory system, which revealed hilar lymph node enlargement in the bilateral lungs. 18F-Fluorodeoxyglucose (18F-FDG)-positron emission tomography-CT revealed abnormal 18F-FDG uptake in the hilar lymph nodes, liver, and left atrium (LA), but not in the left ventricle. Contrast-enhanced cardiac magnetic reson

Sarcoidosis is a multisystem granulomatous disorder of unknown aetiology that predominantly affects the lungs and intrathoracic lymph nodes. Pleural involvement is uncommon, and sarcoidosis-associated pleural effusion (SAPE) represents a rare clinical manifestation, reported in approximately 1% of cases. We describe a 24-year-old male presenting with bilateral pleural effusion and mediastinal lymphadenopathy. Comprehensive diagnostic evaluation revealed a lymphocyte-predominant exudative pleural effusion with negative microbiological and cytological findings. Endobronchial ultrasound-guided tr

BACKGROUND AND OBJECTIVES: The diagnosis of mediastinal lymphoproliferative disorders (LPDs) is often challenging and requires histopathological assessment from large tissue samples. As a result, new diagnostic techniques have been introduced alongside the commonly used endobronchial ultrasound-guided trans-bronchial needle aspiration (EBUS-TBNA). Among them, EBUS-guided transbronchial mediastinal cryobiopsy (EBUS-TBMC) has shown promising potential, although high-quality evidence remains limited. The aim of the study was to assess the diagnostic yield of EBUS-TBMC compared with EBUS-TBNA in p

Background/Objectives: Differentiating mediastinal lymphadenopathy associated with lung cancer from sarcoidosis remains a clinical challenge because of overlapping imaging findings. This study evaluated whether CT-derived radiomic features, alone and in combination with clinical variables, could support the non-invasive differentiation of these two entities. Methods: In this retrospective single-center study, 204 histopathologically confirmed mediastinal lymph nodes were analyzed. A total of 107 radiomic features were extracted from manually segmented contrast-enhanced thoracic CT images. Mult

Sarcoidosis is a multisystem granulomatous disorder that most commonly involves the lungs and intrathoracic lymph nodes. Hypercalcemia is a recognized but uncommon manifestation of sarcoidosis and is typically mild; however, severe hypercalcemia as the initial presentation is rare and may pose a diagnostic challenge. We report a 66-year-old female patient with multiple comorbidities, including stage 4 chronic kidney disease, who presented with fatigue, decreased appetite, vomiting, and significant weight loss. Laboratory findings demonstrated severe hypercalcemia (15.7 mg/dL) with acute worsen

Sarcoidosis is a multisystem granulomatous disease that most commonly involves the lungs and intrathoracic lymph nodes. Osseous involvement is uncommon, reported in approximately 1%-14% of cases, and is frequently subclinical or discovered incidentally on imaging. Symptomatic bone involvement as the presenting manifestation of sarcoidosis is rare. In the largest single-center series, only 1 of 20 patients (5%) had osseous disease as the sole manifestation. We describe a 48-year-old man who presented with progressive atraumatic right shoulder pain and was initially suspected to have rotator cuf

Sarcoidosis is a systemic granulomatous disorder of unclear origin, most commonly impacting the lungs and mediastinal lymph nodes. Respiratory manifestations occur in most patients and are the main contributors to disease-related morbidity and mortality. Despite decades of clinical experience, the evidence base for treatment has historically been limited, with corticosteroids serving as first-line therapy largely on the basis of observational data and expert consensus rather than robust randomized controlled trials. This comprehensive review examines the current landscape of clinical trials fo

BACKGROUND: Sarcoidosis is a systemic granulomatous disease that typically presents with bilateral hilar or mediastinal lymphadenopathy. Unilateral hilar involvement is uncommon and may closely mimic lung cancer, lymphoma, or tuberculosis. Inflammatory activation in sarcoidosis may also disrupt iron metabolism and thyroid hormone homeostasis, but these extra-radiological clues are often overlooked in routine practice.
CASE PRESENTATION: A 53-year-old woman was admitted with fever, chest tightness, and a left hilar mass with mediastinal lymphadenopathy. Laboratory tests showed pancytopenia and

Cardiac sarcoidosis is a potentially fatal inflammatory disorder with the tendency to mostly affect the myocardial tissues. It is commonly associated with extra-cardiac structures such as the lungs and the thoracic lymph nodes; however, when it manifests as an isolated disease, it can be insidious. The clinical presentation is variable depending on the location and extent of the cardiac tissues involved, including asymptomatic cases, conduction abnormalities, arrhythmia, heart failure and sudden cardiac death (SCD). It is an enigmatic disease to diagnose owing to the variable clinical presenta

Background/Objectives: Accurate diagnosis of lymph node pathology is essential for the management of both malignant and benign diseases. This study compares the diagnostic performance of endobronchial ultrasound-guided transbronchial mediastinal cryobiopsy and endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in a real-world cohort. Methods: A single-center retrospective observational study was conducted, including 91 patients who underwent both cryobiopsy and EBUS-TBNA for lymph node evaluation. EBUS-TBNA was performed with a 19-G needle and cryobiopsy with a 1.1 mm

Sarcoidosis is a chronic, idiopathic granulomatous disease primarily affecting the lungs and intrathoracic lymph nodes, although it can involve nearly any organ system, including bones. Osseous involvement of sarcoidosis is uncommon and rarely the initial manifestation of the disease. We present a rare case of toe dactylitis as the sole presenting symptom of systemic sarcoidosis, with further evaluation revealing osseous, pulmonary, lymphatic, and subcutaneous involvement. This case highlights an unusual musculoskeletal presentation of sarcoidosis, as well as the imaging findings that can aid

We present the case of a young woman in whom the association between sarcoidosis, extensor digitorum longus tendon tenosynovitis, and the presence of numerous rice bodies within the tendon sheath revealed a new clinical manifestation of sarcoidosis, in the absence of pulmonary or lymph nodes involvement. The most important aspect of this case is the assessment of tenosynovitis using multimodal ultrasound (grey scale, color/power Doppler, strain elastography, contrast enhanced ultrasound) which demonstrated specific findings that may facilitate further recognition of this clinical condition.

INTRODUCTION: Intra-thoracic lymph nodes (ILNs) are often visible on CT scans of patients with interstitial lung diseases (ILDs). Potential reasons for this include non-specific inflammatory processes, exposure to dust, infection, and neoplasia. In some cases, invasive or minimally invasive diagnostic procedures are necessary to categorise them. The aim of this study was to report on the prevalence of different cytological diagnoses obtained by endobronchial ultrasound (EBUS) from ILNs associated with ILD and to describe ILN morphological features on CT scans and 18-fluorodeoxyglucose PET-CT s

Differentiating sarcoidosis lesions from metastatic lymph nodes is a well-known clinical challenge, particularly in patients with malignancy in whom sarcoid lesions may mimic nodal metastases on 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT). We report a 75-year-old woman with concurrent sarcoidosis and vulvar squamous cell carcinoma in whom 18F-FDG PET/CT demonstrated avid uptake in enlarged lymph nodes throughout the mediastinum, pulmonary hilum, upper abdomen, and right groin. 18F-fluorothymidine (18F-FLT) PET/CT performed 7 days later showed FLT up

Suspicious thyroid lesions are often evaluated using fine-needle aspiration (FNA) with the Bethesda System for Reporting Thyroid Cytopathology (TBSRCT) to estimate malignancy risk and guide management. Indeterminate findings create a diagnostic gray zone where neither molecular testing nor imaging alone can provide certainty. We report the case of a 57-year-old female with a progressively enlarging right-sided thyroid nodule, classified as Bethesda IV on FNA cytology, with a positive neuroblastoma rat sarcoma viral oncogene homolog (NRAS) mutation on ThyroSeq v3, suggestive of high-risk malign

Sarcoidosis is a systemic granulomatous disease characterized by non-caseating granulomas, most commonly affecting the lungs and intrathoracic lymph nodes. Central nervous system involvement (neurosarcoidosis) is rare but associated with significant morbidity and mortality, particularly in advanced systemic disease. We report a fatal case of a 43-year-old man with long-standing pulmonary sarcoidosis who developed progressive neurosarcoidosis complicated by obstructive hydrocephalus and subsequent ventriculoperitoneal shunt infection. High-resolution computed tomography demonstrated advanced pe

Sarcoidosis is an inflammatory granulomatous disease that affects people worldwide and can involve virtually any organ but most commonly the lungs and thoracic lymph nodes. The cause of sarcoidosis remains unknown, but occupational and environmental exposures, genetic background, and ethnicity are likely contributors to disease development. Recent immunological studies, including single-cell RNA sequencing and spatial transcriptomics, have increased our understanding of disease pathogenesis. Diagnosing sarcoidosis is often challenging due to the lack of a diagnostic gold standard and the remar

Pulmonary sarcoidosis is an idiopathic granulomatous disorder primarily affecting the lungs and mediastinal lymph nodes. Pulmonary cryptococcosis, an opportunistic mycosis caused by Cryptococcus species, may occur concurrently with sarcoidosis, presenting substantial diagnostic challenges, particularly in treatment-naïve patients. A 63-years-old previously healthy female presented with insidious-onset dyspnea and low-grade fever. Chest computed tomography (CT) showed mediastinal and hilar lymphadenopathy, accompanied by small nodules in the right lower lobe. She was diagnosed with pulmonary sa

INTRODUCTION: Sacoidosis is a multisystem granulomatous disorder characterized by the formation of non-caseating granulomas in affected organs, predominantly the lungs and lymph nodes. Despite extensive research, its etiology remains elusive. Recent evidence suggests fungi may play a role in disease pathogenesis through immune modulation and chronic inflammation.
METHODS: We conducted a comprehensive fungal profiling study using samples from thirteen patients with confirmed sarcoidosis and five controls with other pulmonary diseases. Multiple specimen types were analyzed, including bronchoalv
