BACKGROUND: Rapid on-site cytologic evaluation (ROSE) during endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) has the potential to influence specimen acquisition, but ROSE's ability to inform decision-making during bronchoscopy for granulomatous disease has not been assessed.
RESEARCH QUESTION: Does ROSE detect granulomas accurately enough to guide lymph node sampling reliably during EBUS-TBNA for granulomatous disease?
STUDY DESIGN AND METHODS: We retrospectively studied individuals who underwent EBUS-TBNA with ROSE performed by cytotechnologists from 2013 throug

In sarcoidosis, a T helper 1 (Th1) response is an essential event and the up-regulation of interleukin-12 (IL-12) has been detected in affected disease sites. In order to investigate the clinical usefulness of circulating IL-12, we measured the serum concentrations of IL-12 by ELISA and performed immunohistochemistry using specific MoAbs for IL-12 in the lungs and scalene lymph nodes of patients with sarcoidosis. The serum concentration of IL-12 p40 was detectable in all 45 patients with pulmonary sarcoidosis and 18 normal controls, whereas that of IL-12 p70 was undetectable. The serum concent

Sarcoidosis is a multisystem granulomatous disease that commonly involves the lungs, with rare involvement of the sinonasal region. We present the case of a 64-year-old Asian female with a five-month history of painless nasal swelling and deformity. Imaging revealed an aggressive, fluorodeoxyglucose (FDG)-avid lesion involving the frontal sinuses, nasal septum, hard palate, and adjacent soft tissues, raising concern for malignancy such as lymphoma or squamous cell carcinoma. However, multiple biopsies from the nasal lesion, mediastinal lymph nodes, and a subcutaneous elbow nodule revealed non-

Positron emission tomography/computed tomography (PET/CT) is widely used to differentiate benign and malignant lesions. However, increased fluorodeoxyglucose (FDG) uptake may occur under certain benign conditions, leading to potential false positives in malignancy assessments. Tuberculosis and sarcoidosis are 2 conditions that can exhibit FDG uptake, presenting with both lymph node and extrathoracic involvement, alongside pulmonary manifestations. This study aimed to explore the utility of PET/CT in distinguishing between thoracic and extrathoracic involvement in patients with sarcoidosis and

AIM: To differentiate between various diagnoses of TB, sarcoidosis, and malignancy by looking at echoic features of mediastinal lymph nodes during endobronchial ultrasound (EBUS).
METHOD: 126 patients underwent EBUS, and 208 mediastinal lymph nodes were sampled. EBUS images were reviewed by 3 different individuals blinded to the final diagnosis. Features such as size, shape, margins, echogenicity (heterogeneous or homogeneous), coagulation necrosis sign, and other notable features were observed and compared.
RESULT: Out of 208 lymph nodes, 60/208 (28.8 %) nodes had a diagnosis of TB, 62/208

Sarcoidosis is a chronic, idiopathic, multisystemic inflammatory disease characterized by non-caseating granulomas, most commonly affecting the lungs and mediastinal lymph nodes. Radiological imaging plays a fundamental role in the diagnosis, assessment of disease extent, and differentiation from other pulmonary conditions. This narrative review offers a comprehensive overview of the imaging features of pulmonary sarcoidosis, focusing on both typical patterns-such as bilateral hilar lymphadenopathy, perilymphatic nodules, and upper lobe-predominant infiltrates-and atypical manifestations-inclu

Sarcoidosis is a chronic granulomatous disease of unknown etiology that can affect multiple organs, most frequently the lungs, lymph nodes, skin, and eyes. Osseous involvement is a rare manifestation, typically affecting small bones, although axial skeleton lesions may also occur. We report the case of a 42-year-old woman with no relevant past medical history who presented with fatigue, night sweats, diffuse joint pain, and low back pain. Fluorine-18 fluorodeoxyglucose positron emission tomography/computed tomography demonstrated multiple hypermetabolic pulmonary nodules, predominantly in the

INTRODUCTION AND IMPORTANCE: Sarcoidosis is a multisystemic, non-caseating granulomatous disease of unknown etiology, predominantly affecting the lungs and lymph nodes. Gastrointestinal involvement, especially in the gallbladder, is rare. Here, we presented a unique case of granulomatous lymphadenitis in a gallbladder-associated lymph node, revealing systemic sarcoidosis.
CASE PRESENTATION: A 70-year-old woman with a history of atrial fibrillation, hypertension, and autoimmune gastritis presented with right upper quadrant pain and vomiting. Ultrasound showed multiple gallstones, and laborator

BACKGROUND: Mediastinal and hilar lesions may be benign or malignant. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is increasingly used for the diagnosis of these lesions as it is both minimally invasive and safe.
OBJECTIVE: To investigate the clinical efficacy of EBUS-TBNA in the diagnosis and differential diagnosis of mediastinal and hilar lesions.
METHODS: A retrospective observational study was undertaken to investigate patients diagnosed with mediastinal and hilar lymphadenopathy based on imaging at our hospital from 2020 to 2021. After evaluation, EBUS T

Sarcoidosis is a granulomatous inflammatory disease of uncertain cause. It occurs most commonly in young and middle-aged adults and less frequently in children; therefore, few data on pediatric sarcoidosis exist in the literature. The diagnosis and management of sarcoidosis remain challenging because of diverse and often nonspecific clinical and imaging findings. In addition, the clinical picture varies widely by age. Prepubertal and adolescent patients often present with adult-like pulmonary disease; however, early-onset sarcoidosis is typically characterized by the triad of arthritis, uveiti

In patients with sarcoidosis, the development of pulmonary hypertension is associated with significant morbidity and mortality. The global prevalence of sarcoidosis-associated pulmonary hypertension (SAPH) reportedly ranges between 2.9% and 20% of sarcoidosis patients. Multiple factors may contribute to the development of SAPH, including advanced parenchymal lung disease, severe systolic and/or diastolic left ventricular dysfunction, veno-occlusive or thromboembolic disease, as well as extrinsic factors such as pulmonary vascular compression from enlarged lymph nodes, anemia, and liver disease

Sarcoidosis is a chronic inflammatory granulomatous disease of unknown aetiology. Although the lungs and thoracic lymph nodes are the most frequently affected structures, every tissue can be affected. Ophthalmological involvement occurs in 25 % of cases. We report a clinical case in which the diagnosis of sarcoidosis was revealed by isolated unilateral optic disc edema.

Tuberculosis (TB) and sarcoidosis are both granulomatous diseases. Here, we compared the immunological microenvironments of granulomas from TB and sarcoidosis patients using in situ sequencing (ISS) transcriptomic analysis and multiplexed immunolabeling of tissue sections. TB lesions consisted of large necrotic and cellular granulomas, whereas "multifocal" granulomas with macrophages or epitheloid cell core and a T-cell rim were observed in sarcoidosis samples. The necrotic core in TB lesions was surrounded by macrophages and encircled by a dense T-cell layer. Within the T-cell layer, compact

BACKGROUND: The role of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in staging mediastinal and hilar lymph nodes in non-small cell lung cancer (NSCLC) is well established. However, evidence of its diagnostic utility in other pathologies-such as lymphoma-remains inadequate. This retrospective observational study aims to determine the diagnostic yield of EBUS-guided miniforceps biopsy (EBUS-MFB) compared to EBUS-TBNA in both malignant and nonmalignant conditions.
METHODS: We conducted a retrospective cross-sectional chart review of all adult patients referred fo

BACKGROUND: Pulmonary tuberculosis (PTB) is an important infectious disease that threatens the health and life of human beings. In the diagnosis of PTB, imaging plays a dominant role, but due to the increasing drug resistance of Mycobacterium tuberculosis, atypical clinical manifestations, "different images with the same disease" or "different diseases with the same image" in chest imaging, and the low positivity rate of routine sputum bacteriology, which leads to a high rate of misdiagnosis of PTB. We report a case of pulmonary tuberculosis that was misdiagnosed on imaging. We report a case o
