The prevalence of cryoglobulinaemia in a series of patients with chronic liver disease from the Campania area has been studied. The series included: 14 cases of chronic persistent hepatitis (CPH), 70 cases of chronic active hepatitis (CAH) and 113 cases of liver cirrhosis. Liver function tests were carried out on the serum of each patient and cryoglobulines were studied. Liver biopsy was carried out when indicated. About a third of the patients were under the effect of previous treatment with anti-inflammatory steroids and-or azathioprine. Cryoglobulines were found in 4 of the 197 patients (2%

Two measurements of serum immune complexes, cryoglobulinaemia and 125I-C1q binding, have been performed in patients with severe rheumatoid arthritis (RA) and compared with normal levels. Cryoglobulinaemia was present in 20 out of 28 patients (71%) with extra-articular disease (mean level 17 micrograms/ml) including nodules, digital vasculitis, cutaneous ulcers, rash, neuropathy, lung disease and scleritis, but in none of 32 patients with joint disease alone (uncomplicated RA) (mean level 3 micrograms/ml). Cryoglobulinaemia correlates with, but probably does not antedate, extra-articular diseas

The polyethylenglycol (PEG) precipitation technique has been employed for the measurement of immune complexes in the circulation of 100 normal subjects and in 14 patients suffering from a variety of diseases (systemic lupus erythematodes, nephrosic syndrome, cryoglobulinaemia, Buckley's syndrome). Values higher than 0.80 UA on the absorption scale were considered pathological, namely 2 standard deviations above the mean (0.32 UA) in the subjects examined; in the patients, values between a minimum of 0.98 UA and a maximum of 2.38 UA were observed. Longitudinal study of these cases also pointed

Large amounts of cryoglobulins and soluble immune complexes were detected in the sera of thirteen patients with tropical spenomegaly syndrome (TSS). Complexes were detected by three different methods: radiobioassay, a modified rheumatoid factor-binding activity method and a modified Clq-binding assay. Protein precipitable by 4% polyethylene glycol (PEG) was also measured. The cryoglobulins contained IgM, IgG and in some cases, C3. It is likely that in TSS, marked immune complex formation is associated with hypergammaglobulinaemia and that continuous engulfment of these complexes by cells of th

In twenty-six patients affected by essential cryoglobulinaemia, 188 determinations of serum complement components (SCC) were made. A peculiar pattern was observed which was characterized by: (a) low levels of early components (Clq, Cls and C4), (b) normal levels of C3 and high concentrations of late components (C5, C9) and (c) CH50 values significantly lower than normal. No relationship could be observed between early SCC and C3 levels. Thirty-three crossed immunoelectrophoreses were performed in thirteen patient's blood samples. The C3c peaks were not different from normal. Follow-up data (15

Cryoglobulins cause an increase in plasma viscosity and this has been attributed to increased protein-protein interaction. The purpose of the present study was to analyse the interaction between red cells and these proteins by measuring the viscosity of whold blood from 21 patients with cryoglobulinaemia not only at high (73 sec-1) and low (0.18 sec-1) shear rates, but also at different temperatures. At 35 degrees blood hyperviscosity was found in 62% of the patients at the high shear rate, and in 76% of patients at the low shear rate. Cooling of the blood to 25 degrees caused an increase in t

Waldenström's or primary macroglobulinaemia has rarely been documented in Black Africans. Six South African Negro patients with Waldenströms macroglobulinaemia were assessed and followed during a 10-year period from 1966 to 1976. The hyperviscosity syndrome was the presenting feature in four patients and one patient each presented with the nephrotic syndrome and a lacrimal mass respectively. On initial evaluation of the six patients, the serum IgM varied from 1.3 to 4.8 g% and in two patients cryoglobulinaemia was found. The patients received supportive treatment, chemotherapy, and when necess

An unsuspected cryoglobulinaemia may be responsible for incorrect serum protein results and thus cause dysglobulinaemia to remain undetected. When cryoglobulins precipitate above laboratory temperature, they will be removed with the clot during centrifugation of the blood sample if they have not previously been suspected clinically or biologically. In the case of massive cryoglobulinaemia, one might find normal protein results but such results are incorrect and if only total protein estimation has been requested, dysglobulinaemia can remain undetected. However, if electrophoresis can also been

In about 5 years of clinical and immunological research, 39 cases of cryoplasmopathies (5 with cryoglobulinaemia and 34 with cryohaemoagglutinaemia) were identified on the basis of immunological typing studies (immunoelectrophoresis, immunodiffusion, identification and isolation of cryoantibodies, cryocrit, titration, identificcation of antibody specificity). Discussion and physiopathological interpretation of possibly associated cases and diseases are conducted on the basis of personal immunological data and the most recent literature on matters of clinical immunohaematology.

We describe a 30-year-old woman with systemic lupus erythematosus (SLE), in whom the only evidence of the disease, during the 10 years before diagnosis was established, was an accelerated erythrocyte sedimentation rate and a positive Wassermann test. Her disease was characterized by the presence of an anticoagulant without indication of bleeding, mixed cryoglobulinaemia, steroid-resistant renal damage, and persistent false-positive tests for syphilis. The relationship between these results and the presence of anticoagulant, mixed cryoglobulinaemia and renal damage in patients with SLE are disc

A Greek child aged 2 with diaphyseal dysplasia developed an anaemia which required transfusion. The liver, spleen and lymph nodes were enlarged, there was pancytopenia and polyclonal hypergammaglobulinaemia. Biopsy provided evidence for myelofibrosis with extramedullary erythropoiesis. He was treated with small doses of corticosteroids and during his third year there was complete regression of the disease without recurrence when treatment was stopped. Disturbances of haematopoiesis in diaphyseal dysplasia has not been reported previously. These unusual findings are of clinical interest and als

A 5-year-old girl had a chronic disease characterised by fever, lymphoid hyperplasia, interstitial pneumonitis, thrombocytopenia, and polyclonal hypergammaglobulinaemia. Evidence for severe, persistent Epstein-Barr virus (E.B.V.) infection was found: titres of antibody to E.B.V. viral capsid antigen (IgM and IgG) and early antigen were extremely high, cells containing E.B.V.-associated nuclear antigen (E.B.N.A.) were found in lymph nodes and blood, and spontaneous permanent lymphoblastoid cell lines were established from both sources over a period of a year. After exacerbation of the polyclona

Three weeks after electrocoagulation of an enormous epithelioma of the floor of the mouth, a 62-year-old man presented after a shower-bath with symmetrical cyanosis of the toes and fingers with painful oedema coloured by livedo reticularis of the lower third of the legs and forearms. In the hands this developed into a dry necrosis and necessitated their amputation. In the absence of biological tests and a posteriori the authors discuss the diagnosis of cryoglobulinaemia and intravascular disseminated coagulopathy; but in fact, these two syndromes produce very similar cutaneous symptoms, notabl

Cryoglobulins were removed from the plasma of two patients with mixed cryoglobulinaemia (IgM(k)-IgG type) associated with severe membrano-proliferative glomerulonephritis and angiitis. The removal of about half the circulating cryo-Igs ('cryo-Igpheresis') was followed by a brisk rebound in cryo-Igs due to the rapid synthesis rate of monoclonal IgM(k) with anti-IgG activity. 'Cryo-Igpheresis' + cyclophosphamide treatment suppressed the rebound of cryo-IgGs, but not that of the cryo-IgMs. This differential effect was due to decrease of the antibody activity of cryo-IgMs. The clinical implication

Seventeen consecutive patients subjected to jejunoileostomy for obesity have been investigated for complement abnormalities and cryoglobulinaemia. The study took place 1-9 1/2 years after the operation. A concomitant clinical examination revealed recurrent arthritis in 6 (30%) of the patients. In 6 of the patients complement abnormalities were found, as activation of the classical pathway in 3 and activation of the alternative pathway in another 3 could be suspected from immunochemical data. One patient showed activation of both the classical and the alternative pathway. Two of the patients wi

Essential cryoglobulinaemia was detected in 44 out of 150 patients (29%) screened on the basis of histological confirmation of chronic inflammatory liver disease (chronic persistent or aggressive hepatitis, or cirrhosis). Cryoglobulinemia prevailed in the patients whose hepatic tissue showed more features of active inflammation; also, a female prevalence was observed. There were no correlations between cryoglobulinaemia and either HBsAg positivity or alcoholic liver disease. Mixed cryoglobulins made of heterogeneous immunoglobulins without monotypic components were mostly associated with estab
