Sex-Specific Cytokine Responses and Metabolic Adaptation to Weight Loss in Obesity with Insulin Resistance.

Background/Objectives: Obesity-related insulin resistance is accompanied by chronic low-grade inflammation, but the extent to which weight loss modifies circulating cytokines in a sex-specific manner remains insufficiently understood. The aim of this study was to assess sex-specific cytokine responses and metabolic adaptation in adults with obesity and insulin resistance following a six-month weight-reduction program (WRP). Methods: Thirty-six participants (24 women and 12 men) with a value of Homeostatic Model Assessment of Insulin Resistance (HOMA-IR) ≥ 2 underwent an individualized low-calo

Stevia Rebaudiosides Usage as a Sugar Reduction Tool: A Narrative Review of Their Metabolic, Gut Microbiome and Weight Management Effects in Human Clinical Studies.

Background/Objectives: Stevia rebaudiosides represent a class of compounds extracted from the Stevia rebaudiana Bertoni plant or produced via yeast fermentation, which provide a sweet taste with little to no calories. These compounds are commercially referred to as stevia and are used in the food industry to reduce sugar in foods and beverages. Stevia is a non-nutritive sweetener (NNS), which is a class of ingredients which represent both artificial and plant-based sweeteners. NNSs are widely used and have been well studied. However, their effects on efficacy for weight management as a sugar r

Ketogenic Diet in Obesity and Diabetes: A Narrative Review.

A ketogenic diet (KD) is a low-carbohydrate, high-fat dietary approach. Beyond treating neurologic disorders, KDs have attracted significant media attention for their potential to improve obesity and diabetes. The diet induces a metabolic shift from glucose toward fatty acid oxidation and ketone body production. This shift leads to ketosis, which may reduce hunger, partly through the anorexigenic effects of ketone bodies, thereby contributing to weight loss and improved metabolic parameters, including glycaemic control and insulin sensitivity. In particular, the positive effects of KDs lower i

Exercise-based interventions for sarcopenic obesity in middle-aged and older adults: an umbrella review of systematic reviews with pairwise meta-analyses and network meta-analyses.

INTRODUCTION: Sarcopenic obesity (SO) is an increasingly important nutrition- and exercise-relevant phenotype in middle-aged and older adults, but the secondary evidence base is complicated by heterogeneous SO definitions, mixed exercise and nutrition interventions, and repeated use of the same primary trials across reviews. METHODS: We conducted an umbrella review of systematic reviews with pairwise meta-analysis or network meta-analysis. PubMed, Web of Science, SPORTDiscus, Cochrane Library, Embase and Scopus were searched from inception to 1 March 2026. Eligible reviews focused on middle-a

Cardiovascular disease risk and associated factors among people with hypertension in the West Bank, Palestine.

BACKGROUND: Hypertension is a major contributor to cardiovascular disease (CVD) morbidity and mortality worldwide, particularly in low- and middle-income countries where comprehensive risk-based prevention remains limited. Estimation of absolute cardiovascular risk offers a more informative approach to prevention than blood pressure control alone. However, evidence on predicted cardiovascular risk and its determinants among People with hypertension in Palestine remains scarce. METHODS: An institution-based cross-sectional study was conducted between August and October 2025 among People with h

Post-transplant MASLD: a risk-stratified approach to assessment and personalized management.

Metabolic dysfunction-associated steatotic liver disease (MASLD) has become a major indication for liver transplantation (LT) and is expected to account for an increasing proportion of transplant candidates worldwide. As long-term post-transplant survival continues to improve, post-LT MASLD has increasingly emerged as a major metabolic complication affecting long-term prognosis. Unlike MASLD in the non-transplant setting, post-LT MASLD is strongly influenced by transplant-specific factors, including recipient metabolic dysfunction, immunosuppressant use, graft-related characteristics, and meta

From Engel's Bio-Psycho-Social model to the personalized health determinants model: a comprehensive framework and illustrative operationalization for precision health.

Engel's Bio-Psycho-Social (BPS) model (1977) reframed healthcare by integrating biological, psychological, and social perspectives. Despite its influence, the model has been criticized for insufficient specificity in domains critical to precision health, including nutrition, lifestyle, socioeconomic, environmental, and structural factors. To address these limitations, we propose the Personalized Health Determinants Model (PHDm), a comprehensive nine-dimension framework, Biological, Psychological, Social, Cultural, Environmental, Economic, Political, Spiritual, and Lifestyle, synthesized from E

Seeing Beyond GLP-1: A Systems View of Sustainable Obesity Care.

INTRODUCTION: Glucagon-like peptide-1 (GLP-1) receptor agonists represent a major therapeutic advancement, producing substantial weight loss and rapidly reshaping clinical practice, commercial markets, and policy discussions. Their accelerating adoption, however, is unfolding alongside decades of public health progress to address the complex drivers of obesity. Without integration into a broader systems-based approach, growing reliance on GLP-1 therapies risks reinforcing a reactive, treatment-driven model of obesity management that is costly, difficult to sustain, insufficient for addressing
