Types 1 diabetes mellitus and Graves' disease in Sanjad-Sakati syndrome: a case report in Saudi Arabia.

Sanjad-Sakati syndrome (SSS) is a rare autosomal recessive disorder caused by TBCE gene variants and characterized by congenital hypoparathyroidism, severe growth retardation, developmental delay, and dysmorphic features. We report a nine-year-old Saudi boy with genetically confirmed SSS due to a homozygous 12-bp deletion in exon 3 of the TBCE gene (c.155_166del12bp; p.S52_G55del), who presented with hypocalcemic convulsions at three weeks of age. At three years, he developed type 1 diabetes mellitus (T1DM) with profound insulin deficiency and positive anti-islet cell and anti-GAD-65 antibodie

Myxoedema coma in a patient with alcohol dependence: a diagnostic pitfall.

Myxoedema coma is a rare but life-threatening manifestation of hypothyroidism with high associated mortality. We present the case of a man in his sixties with known hypothyroidism and alcohol dependence who collapsed at home with profound hypothermia, refractory hypoglycaemia, hypoxia and reduced consciousness. Initial findings were easily attributable to excess alcohol; however, clinical features including periorbital oedema and bedside echocardiogram revealed moderate pericardial effusion, which prompted further investigation. Thyroid function tests confirmed severe hypothyroidism with eleva

Co-Designing a Provincial Newborn Screening Implementation Framework in Pakistan Using a Sequential Blended Workshop Model.

Newborn screening (NBS) in Pakistan remains limited by fragmented service delivery, uneven provincial capacity, and the absence of an implementation pathway that links policy intent to operational planning. This study aimed to co-design a provincial NBS implementation framework for Punjab. To do so, we used a sequential blended workshop model supported by visual planning methods. We conducted a qualitative, three-phase co-design process with 19 participants from laboratory medicine, pediatrics, obstetrics, public health, academia, and policy. Phase I consisted of pre-work and an online worksho

Feasibility of short imaging protocols for [18F]fluordeprenyl-D2 PET in autoimmune encephalitis and multiple system atrophy.

INTRODUCTION: [18F]fluorodeprenyl-D2 ([18F]F-DED) positron emission tomography (PET) imaging detects reactive astrogliosis in patients with autoimmune encephalitis (AIE) and multiple system atrophy (MSA). Although dynamic 60-min acquisitions are established, shorter static imaging protocols are desirable for severely impaired patients. This study investigated the feasibility of short static time windows for [18F]F-DED PET imaging in AIE and MSA. METHODS: Dynamic 60-min [18F]F-DED PET scans were analyzed in 20 patients with AIE, 20 patients with MSA (MSA-P/MSA-C), and 16 controls (CTRL). Disea

Thyroid tests in kidney failure: diagnostic stewardship before levothyroxine therapy.

Advanced chronic kidney disease and kidney failure frequently produce thyroid-test patterns that resemble primary hypothyroidism. The clinical challenge is not to dismiss abnormal results, but to distinguish persistent, treatment-requiring thyroid failure from uraemic, non-thyroidal, volume-related, medication-related, and analytical mimics before committing a vulnerable patient to long-term levothyroxine. This structured, evidence-informed narrative review critically appraises epidemiology, dialysis-specific sampling studies, prognostic cohorts, treatment evidence, and major thyroid guidance.

Iodine and Hypothyroidism.

Hypothyroidism is a thyroid disorder disease caused by a decrease in the synthesis or secretion of thyroid hormones and is one of the most common thyroid disorders in clinical practice. Elemental iodine is consumed daily in salt and food, such as seaweed. Iodine deficiency is the most common cause of thyroid disorders. Iodine is oxidized in the body to produce iodine-containing thyroid hormones. Iodine deficiency can lead to goitre deficiency and can also affect children and infants, leading to mental retardation and developmental delays. The causes of hypothyroidism are complex and can be div

Air pollution and risk of autoimmune thyroid diseases in children and adolescents: Findings from a nationwide cohort study in Korea.

Epidemiological evidence on the association between long-term air pollution exposure and the incidence of autoimmune thyroid diseases (AITD) in children and adolescents remains limited. This study aimed to examine the relationship between long-term exposure to air pollution and the onset of AITD in children and adolescents in South Korea. This nationwide cohort study included 1,400,433 children aged 6 years at baseline (2008-2010) from the National Health Information Database, with follow-up until 19 years of age. The onset of AITD was defined based on the 10th revision of the International Cl

Finnish-Enriched SLC26A7 Variant in Congenital Hypothyroidism: Clinical Spectrum, Thyroid Histopathology, and Expression Analysis.

BACKGROUND: Defects in thyroid hormone synthesis at birth lead to congenital hypothyroidism (CH). Recently, pathogenic variants in the SLC26A7 gene have been linked to dyshormonogenetic goitrous CH. This anion transporter is highly expressed in the thyroid and is involved in thyroid hormone synthesis; however, its exact function and cellular localization remain unclear. In this study, we investigated SLC26A7 variants in Finnish patients with CH, characterized the phenotypes, and analyzed thyroid-specific gene expression. METHODS: SLC26A7 variants were identified from a clinical CH cohort (n =

Substrate for Thyroid Hormone Synthesis: Biochemistry, Evolution, and Physiology.

The use of iodotyrosines to control development and other biological processes predates the evolution of the thyroid gland. Endogenous thyroid hormone synthesis evolved with the ability to enzymatically iodinate protein secreted and directed for gastrointestinal proteolysis-and evolved further to specialize for iodination of protein entrapped extracellularly within closed follicles. The evolutionary appearance of the vertebrate thyroid gland with its follicular architecture correlates closely with the first appearance of thyroglobulin, which acts as the primary protein scaffold for thyroid hor

Right Thyroid Lobe Agenesis With Contralateral Compensatory Hypertrophy: A Case Report.

Thyroid hemiagenesis is a rare congenital anomaly with a marked predominance of left lobe involvement. Right lobe agenesis is an exceptionally uncommon presentation that can pose significant diagnostic challenges. We report an unusual case of right thyroid lobe agenesis with contralateral compensatory hypertrophy in a 52-year-old female patient who presented for consultation after detecting anterior cervical asymmetry and increased volume via self-palpation. The biochemical profile revealed a state of clinical euthyroidism. Cervical ultrasonography demonstrated the absence of thyroid tissue in

Thyroid dyshormonogenesis caused by iodotyrosine deiodinase pathogenic variant: three cases presenting in adolescence.

We describe three patients from three unrelated consanguineous families, all homozygous for the c.301C>T (p.Arg101Trp) variant in the iodotyrosine deiodinase (IYD) gene. These patients presented in late childhood around puberty with diffuse goiter, primary hypothyroidism, and negative thyroid autoantibodies, consistent with thyroid dyshormonogenesis (DH). Initiation of levothyroxine therapy led to a significant reduction in goiter size and clinical improvement. Notably, all patients had normal thyroid function at neonatal screening, suggesting a delayed clinical manifestation of congenital hyp

DUOXA2 Variants as an Underrecognized Cause of Diffuse Goiter: A Euthyroid Adult Case Responsive to Levothyroxine.

Hydrogen peroxide (H2O2) is an essential substrate for thyroid peroxidase (TPO) and plays a key role in iodine oxidation, organification, and coupling reactions that generate thyroid hormones. Dual oxidase 2 (DUOX2) and its maturation factor dual oxidase maturation factor 2 (DUOXA2) are critical for H2O2 production in thyroid follicular cells. Although DUOXA2 variants typically present with congenital hypothyroidism, phenotypic variability has been reported. We present an adult woman with a diffuse thyroid goiter and normal thyroid function who was found to have compound heterozygous DUOXA2 va

[Dyshormonogenetic goiter in a patient with congenital hypothyroidism: clinical, morphological and genetic features].

Dyshormonogenetic goiter (DG) is a rare genetic disorder of thyroid gland development resulting from deficiencies in enzymes involved in thyroid hormone synthesis. Clinical manifestations include congenital hypothyroidism (CH), thyroid enlargement, and nodule formation. Morphologically, the thyroid gland exhibits histoarchitectural heterogeneity and cellular polymorphism, which can mimic malignant neoplasms and complicate differential diagnosis. Morphological examination revealed pronounced polymorphism of thyroid nodules without signs of malignancy. The pathogenic heterozygous variant was det

Functional Impact of a Novel Intronic TG Variant in Congenital Goiter and Thyroid Dyshormonogenesis.

INTRODUCTION: Thyroglobulin (TG) is essential for the biosynthesis of thyroid hormones. Biallelic pathogenic variants in the TG gene cause primary congenital hypothyroidism due to dyshormonogenesis, which often presents with congenital goiter. Here, we report a novel homozygous intronic TG variant, identified in a family with multiple cases of congenital goiter, and demonstrate its pathogenicity using cDNA sequencing. CASE PRESENTATION: The proband, a 15.5-year-old male, presented with a thyroid nodule and had a history of congenital goiter. He had been treated with levothyroxine since birth.

Life-threatening fetal goiter: diagnosis and outcomes in four consecutive cases with critical appraisal-why this diagnosis is not obviously trivial.

BACKGROUND: The diagnosis of a fetal goiter constitutes a potentially life-threatening in utero condition for the fetus. Even with early diagnosis and close therapy with highly specialized interdisciplinary management, the prognosis remains poor. We discuss diagnostic and therapeutic challenges regarding the pre- and postnatal outcome based on four consecutive cases with a critical appraisal of latest heterogeneous recommendations in literature. METHODS: A retrospective review of four cases of fetal goiter is presented that were diagnosed at a tertiary university centre during targeted ultras

Multidisciplinary diagnosis and treatment of lingual thyroid presenting as hematemesis: A case report.

RATIONALE: Hematemesis is a common symptom of gastrointestinal disorders and is often associated with upper gastrointestinal issues. However, lingual thyroid, a rare ectopic thyroid condition, can also present with hematemesis, complicating diagnosis and increasing the risk of misdiagnosis. This paper reports a case of lingual thyroid in which the primary symptom was hematemesis. PATIENT CONCERNS: A 59-year-old male patient with dwarfism and congenital hypothyroidism underwent irregular thyroid hormone therapy. He was admitted to the hospital after experiencing hemoptysis and a sensation of a

Management of endocrine emergencies: joint consensus statement for management of myxoedema coma.

Myxoedema coma is a rare, life-threatening endocrine emergency resulting from severe hypothyroidism, most commonly affecting older adults. It is characterised by altered mental status, hypothermia, and multiorgan dysfunction, with a high mortality rate if not promptly recognised and treated. This consensus statement provides recommendations for the diagnosis, initial resuscitation, and definitive management of myxoedema coma based on clinician expertise and experience. Key elements include early recognition of precipitating factors, the use of early empiric stress dose glucocorticoids, support

A novel homozygous thyroglobulin gene variant presenting with massive congenital goiter and neonatal airway obstruction.

Congenital hypothyroidism due to thyroid dyshormonogenesis is a rare inherited disorder that may present with goiter of variable severity. Massive congenital goiter causing life-threatening respiratory distress at birth is exceptionally uncommon. We report a late-preterm male neonate who presented immediately after delivery with severe respiratory failure requiring endotracheal intubation due to critical tracheal compression caused by a massive congenital goiter. Postnatal imaging revealed marked thyroid hyperplasia with airway narrowing. Thyroid function tests demonstrated isolated low free t
