A novel homozygous thyroglobulin gene variant presenting with massive congenital goiter and neonatal airway obstruction.

Congenital hypothyroidism due to thyroid dyshormonogenesis is a rare inherited disorder that may present with goiter of variable severity. Massive congenital goiter causing life-threatening respiratory distress at birth is exceptionally uncommon. We report a late-preterm male neonate who presented immediately after delivery with severe respiratory failure requiring endotracheal intubation due to critical tracheal compression caused by a massive congenital goiter. Postnatal imaging revealed marked thyroid hyperplasia with airway narrowing. Thyroid function tests demonstrated isolated low free t

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Revisiting the Neuropsychological and Clinical Profile of Mosaic Turner Syndrome With a Ring X Chromosome.

Determining karyotype-phenotype correlations for individuals with Turner syndrome ("TS individuals") is a longstanding research endeavor. The limited literature on Turner syndrome (TS) with a ring X chromosome hinders counseling about the neuropsychological and clinical features. To further characterize these phenotypes, we compared 27 TS individuals with 46,X,r(X)/45,X ("ring X") to 50 non-mosaic 45,X, and 27 mosaic 45,X/46,XX ("mosaic 45,X") individuals. This retrospective cohort study of 104 individuals reported on cardiac and renal malformations, endocrine and neuropsychological conditions

Hashimoto's thyroiditis- What's in a name?

Hashimoto's thyroiditis (HT) is the most common autoimmune endocrine disease worldwide with an annual incidence of 0.3-1.5 per 1000 people and a prevalence of 8% of the general population. At least nine terms appear in the literature denoting HT, which are used as synonyms or are terms describing disorders closely related to HT. Moreover, the definitions of HT vary, and the role of several parameters in making a diagnosis remains unclear. Furthermore, the term "thyroiditis" is often used among experts to describe the triphasic evolution in thyroid status (thyrotoxicosis, hypothyroidism, and eu

Prevalence and predictors of obesity hypoventilation syndrome in an Indian cohort with obstructive sleep apnea.

Obesity hypoventilation syndrome (OHS) is a comorbidity in patients with Obstructive sleep apnea (OSA). It has a significant impact on the treatment and outcomes. There is paucity of data on the prevalence of OHS and its associated factors. Our objective was to estimate this for our Indian cohort. This is a retrospective study on consecutive patients who underwent a sleep study for suspicion of OSA from January 2022 to December 2022. Details of these patients were extracted from the electronic medical records, and the sleep studies were analysed. The prevalence of OHS was calculated. Multiple

Using Medication Dispensation Data to Identify Clusters with Similar Prescribing Patterns in Older Adults Living with Dementia.

BACKGROUND AND OBJECTIVES: Older adults living with dementia are a heterogeneous group, which can make studying optimal medication management challenging. Unsupervised machine learning is a group of computing methods that rely on unlabeled data-that is, where the algorithm itself is discovering patterns without the need for researchers to label the data with a known outcome. These methods may help us to better understand complex prescribing patterns in this population. The objective of our study was to use clustering methods to determine whether common prescribing clusters exist in older adult

A rare case of unilateral parotid gland agenesis.

Parotid gland agenesis is a rare congenital anomaly with fewer than 30 cases reported in the literature. It can present with xerostomia, facial asymmetry, recurrent infections, or may be entirely asymptomatic. Due to its rarity and potential to mimic other pathological conditions, especially in cases of facial asymmetry, awareness is crucial for accurate diagnosis. We report the case of a 39-year-old man referred to our clinic for routine follow-up of a thyroid nodule. The patient had no history of systemic illness or symptoms related to salivary gland dysfunction. During neck ultrasonography,

Myxoedema Coma in a Medically Compliant Young Patient Presenting With Cardiogenic Shock and Requiring Cardiopulmonary Resuscitation.

Myxedema coma, a rare and life-threatening complication of severe hypothyroidism, manifests with altered mental status, hypothermia, bradycardia, hypotension, hyponatremia, and respiratory failure, potentially leading to multisystem organ failure. Cardiogenic shock associated with myxoedema coma, resulting from impaired myocardial contractility and reduced cardiac output, is a severe complication associated with increased mortality risk. This endocrine emergency requires prompt recognition and aggressive management. We report a case of a 30-year-old female with a history of hypothyroidism and

Hashimoto's thyroiditis and congenital long QT syndrome: a dangerous addition causing torsades-case report.

BACKGROUND: Hypothyroidism leads to a prolonged QT interval due to modulation of the potassium channel by low circulating T3, leading to a disproportionate lengthening of the action potential. Patients with long QT syndromes (LQTS) have an increased risk of symptomatic ventricular arrhythmias and sudden cardiac death (SCD). CASE SUMMARY: A 40-year-old female with a previous history of congenital LQTS, Type 1 diabetes mellitus, anaemia, and untreated hypothyroidism was brought to the hospital with an abrupt onset of palpitations and disorientation following multiple (>30) automated implantable

Recovery from Atrophic Autoimmune Thyroiditis in a Child: Thyroid Stimulation-Blocking Antibody as a Prognostic Marker.

INTRODUCTION: Atrophic autoimmune thyroiditis (AAT) is a form of autoimmune hypothyroidism characterized by the absence of a goiter. Thyroid stimulation-blocking antibody (TSBAb) has been detected in a subset of pediatric AAT cases. Although the disappearance of TSBAb has been related with the recovery of thyroid function in adult AAT cases, similar outcomes have not been documented in pediatric cases. CASE PRESENTATION: A 2-year-old Japanese boy presented for evaluation of stunted growth from 1 year 10 months of age. Tests for congenital hypothyroidism were negative on newborn screening, and

Permanent Congenital Hypothyroidism due to Rare Thyroglobulin Gene Variant (p.Cys1476Arg): A Delayed Diagnosis of Thyroid Dyshormonogenesis.

Thyroid dyshormonogenesis is an inherited hypothyroidism caused by a monogenic defect, in the vast majority of cases, in thyroid hormone biosynthesis. It is commonly associated with thyroid enlargement which is vulnerable to nodule formation. We present a Qatari patient with an overlooked diagnosis of thyroid dyshormonogenesis due to thyroglobulin gene mutation. A 10.5-year-old boy has been following up for congenital hypothyroidism since the age of 4 years. He was diagnosed by newborn screening that was confirmed by laboratory thyroid function testing; however, no further workup was done to u

Functional Noncoding Variants in a TTTG Microsatellite on Chromosome 15q26.1 Are a Common Genetic Etiology of Congenital Hypothyroidism with Thyroid Gland in situ.

INTRODUCTION: Variants affecting a microsatellite on the noncoding region of chromosome 15q26.1 are associated with familial non-autoimmune thyroid abnormalities characterized by mild congenital hypothyroidism (CH) with elevated thyroglobulin (Tg) levels. Some individuals who carry these variants may develop multinodular goiter if left untreated. Although these variants have been recognized as genetic etiologies of CH, nongoitrous, 3 (CHNG3), the associated severity of CH remains unclear. METHOD: A cohort of 63 participants diagnosed with CH at Tohoku University underwent screening for geneti

Association between Human Leukocyte Antigen Alleles and Endocrine Disorders in a Cohort of 895 Patients from Russian Clinical Population.

Endocrine disorders represent a serious public health problem and frequently can be caused by genetic factors or their combination with environmental and lifestyle factors. Assessment of relevant genetic factors is important to estimate the risk of endocrine pathologies in an individual before their manifestation. Identification of genetic variations in proteins of the major histocompatibility complex is important with regard to the autoimmune nature of many endocrine pathologies, including type 1 diabetes. In this study, we investigated the relationship between human leukocyte antigen (HLA) g

Airway compression in a neonate with thyroid dyshormonogenesis due to SLC26A7 defect.

SLC26A7 encodes an apical iodide transporter in thyroid follicular cells and causes, when mutated, congenital hypothyroidism (CH). We report a patient diagnosed with goitrous CH, who initially presented with neck enlargement and inspiratory stridor on day 3 of life. Levothyroxine therapy resulted in prompt improvement in upper airway compression, and surgical interventions were avoided. The levothyroxine dosage per body weight was gradually reduced to 0.5 μg/kg/day by age 11 months. Genomic sequencing of CH-related genes identified compound heterozygous protein-truncating SLC26A7 variants: c.6
