Sanjad-Sakati syndrome (SSS) is a rare autosomal recessive disorder caused by TBCE gene variants and characterized by congenital hypoparathyroidism, severe growth retardation, developmental delay, and dysmorphic features. We report a nine-year-old Saudi boy with genetically confirmed SSS due to a homozygous 12-bp deletion in exon 3 of the TBCE gene (c.155_166del12bp; p.S52_G55del), who presented with hypocalcemic convulsions at three weeks of age. At three years, he developed type 1 diabetes mellitus (T1DM) with profound insulin deficiency and positive anti-islet cell and anti-GAD-65 antibodie

Hypothyroidism is a thyroid disorder disease caused by a decrease in the synthesis or secretion of thyroid hormones and is one of the most common thyroid disorders in clinical practice. Elemental iodine is consumed daily in salt and food, such as seaweed. Iodine deficiency is the most common cause of thyroid disorders. Iodine is oxidized in the body to produce iodine-containing thyroid hormones. Iodine deficiency can lead to goitre deficiency and can also affect children and infants, leading to mental retardation and developmental delays. The causes of hypothyroidism are complex and can be div

Epidemiological evidence on the association between long-term air pollution exposure and the incidence of autoimmune thyroid diseases (AITD) in children and adolescents remains limited. This study aimed to examine the relationship between long-term exposure to air pollution and the onset of AITD in children and adolescents in South Korea. This nationwide cohort study included 1,400,433 children aged 6 years at baseline (2008-2010) from the National Health Information Database, with follow-up until 19 years of age. The onset of AITD was defined based on the 10th revision of the International Cl

BACKGROUND: Defects in thyroid hormone synthesis at birth lead to congenital hypothyroidism (CH). Recently, pathogenic variants in the SLC26A7 gene have been linked to dyshormonogenetic goitrous CH. This anion transporter is highly expressed in the thyroid and is involved in thyroid hormone synthesis; however, its exact function and cellular localization remain unclear. In this study, we investigated SLC26A7 variants in Finnish patients with CH, characterized the phenotypes, and analyzed thyroid-specific gene expression.
METHODS: SLC26A7 variants were identified from a clinical CH cohort (n =

The use of iodotyrosines to control development and other biological processes predates the evolution of the thyroid gland. Endogenous thyroid hormone synthesis evolved with the ability to enzymatically iodinate protein secreted and directed for gastrointestinal proteolysis-and evolved further to specialize for iodination of protein entrapped extracellularly within closed follicles. The evolutionary appearance of the vertebrate thyroid gland with its follicular architecture correlates closely with the first appearance of thyroglobulin, which acts as the primary protein scaffold for thyroid hor

Thyroid hemiagenesis is a rare congenital anomaly with a marked predominance of left lobe involvement. Right lobe agenesis is an exceptionally uncommon presentation that can pose significant diagnostic challenges. We report an unusual case of right thyroid lobe agenesis with contralateral compensatory hypertrophy in a 52-year-old female patient who presented for consultation after detecting anterior cervical asymmetry and increased volume via self-palpation. The biochemical profile revealed a state of clinical euthyroidism. Cervical ultrasonography demonstrated the absence of thyroid tissue in

We describe three patients from three unrelated consanguineous families, all homozygous for the c.301C>T (p.Arg101Trp) variant in the iodotyrosine deiodinase (IYD) gene. These patients presented in late childhood around puberty with diffuse goiter, primary hypothyroidism, and negative thyroid autoantibodies, consistent with thyroid dyshormonogenesis (DH). Initiation of levothyroxine therapy led to a significant reduction in goiter size and clinical improvement. Notably, all patients had normal thyroid function at neonatal screening, suggesting a delayed clinical manifestation of congenital hyp

Hydrogen peroxide (H2O2) is an essential substrate for thyroid peroxidase (TPO) and plays a key role in iodine oxidation, organification, and coupling reactions that generate thyroid hormones. Dual oxidase 2 (DUOX2) and its maturation factor dual oxidase maturation factor 2 (DUOXA2) are critical for H2O2 production in thyroid follicular cells. Although DUOXA2 variants typically present with congenital hypothyroidism, phenotypic variability has been reported. We present an adult woman with a diffuse thyroid goiter and normal thyroid function who was found to have compound heterozygous DUOXA2 va

Dyshormonogenetic goiter (DG) is a rare genetic disorder of thyroid gland development resulting from deficiencies in enzymes involved in thyroid hormone synthesis. Clinical manifestations include congenital hypothyroidism (CH), thyroid enlargement, and nodule formation. Morphologically, the thyroid gland exhibits histoarchitectural heterogeneity and cellular polymorphism, which can mimic malignant neoplasms and complicate differential diagnosis. Morphological examination revealed pronounced polymorphism of thyroid nodules without signs of malignancy. The pathogenic heterozygous variant was det

INTRODUCTION: Thyroglobulin (TG) is essential for the biosynthesis of thyroid hormones. Biallelic pathogenic variants in the TG gene cause primary congenital hypothyroidism due to dyshormonogenesis, which often presents with congenital goiter. Here, we report a novel homozygous intronic TG variant, identified in a family with multiple cases of congenital goiter, and demonstrate its pathogenicity using cDNA sequencing.
CASE PRESENTATION: The proband, a 15.5-year-old male, presented with a thyroid nodule and had a history of congenital goiter. He had been treated with levothyroxine since birth.

BACKGROUND: The diagnosis of a fetal goiter constitutes a potentially life-threatening in utero condition for the fetus. Even with early diagnosis and close therapy with highly specialized interdisciplinary management, the prognosis remains poor. We discuss diagnostic and therapeutic challenges regarding the pre- and postnatal outcome based on four consecutive cases with a critical appraisal of latest heterogeneous recommendations in literature.
METHODS: A retrospective review of four cases of fetal goiter is presented that were diagnosed at a tertiary university centre during targeted ultras

RATIONALE: Hematemesis is a common symptom of gastrointestinal disorders and is often associated with upper gastrointestinal issues. However, lingual thyroid, a rare ectopic thyroid condition, can also present with hematemesis, complicating diagnosis and increasing the risk of misdiagnosis. This paper reports a case of lingual thyroid in which the primary symptom was hematemesis.
PATIENT CONCERNS: A 59-year-old male patient with dwarfism and congenital hypothyroidism underwent irregular thyroid hormone therapy. He was admitted to the hospital after experiencing hemoptysis and a sensation of a

Congenital hypothyroidism due to thyroid dyshormonogenesis is a rare inherited disorder that may present with goiter of variable severity. Massive congenital goiter causing life-threatening respiratory distress at birth is exceptionally uncommon. We report a late-preterm male neonate who presented immediately after delivery with severe respiratory failure requiring endotracheal intubation due to critical tracheal compression caused by a massive congenital goiter. Postnatal imaging revealed marked thyroid hyperplasia with airway narrowing. Thyroid function tests demonstrated isolated low free t

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

INTRODUCTION: Atrophic autoimmune thyroiditis (AAT) is a form of autoimmune hypothyroidism characterized by the absence of a goiter. Thyroid stimulation-blocking antibody (TSBAb) has been detected in a subset of pediatric AAT cases. Although the disappearance of TSBAb has been related with the recovery of thyroid function in adult AAT cases, similar outcomes have not been documented in pediatric cases.
CASE PRESENTATION: A 2-year-old Japanese boy presented for evaluation of stunted growth from 1 year 10 months of age. Tests for congenital hypothyroidism were negative on newborn screening, and

Thyroid dyshormonogenesis is an inherited hypothyroidism caused by a monogenic defect, in the vast majority of cases, in thyroid hormone biosynthesis. It is commonly associated with thyroid enlargement which is vulnerable to nodule formation. We present a Qatari patient with an overlooked diagnosis of thyroid dyshormonogenesis due to thyroglobulin gene mutation. A 10.5-year-old boy has been following up for congenital hypothyroidism since the age of 4 years. He was diagnosed by newborn screening that was confirmed by laboratory thyroid function testing; however, no further workup was done to u

INTRODUCTION: Variants affecting a microsatellite on the noncoding region of chromosome 15q26.1 are associated with familial non-autoimmune thyroid abnormalities characterized by mild congenital hypothyroidism (CH) with elevated thyroglobulin (Tg) levels. Some individuals who carry these variants may develop multinodular goiter if left untreated. Although these variants have been recognized as genetic etiologies of CH, nongoitrous, 3 (CHNG3), the associated severity of CH remains unclear.
METHOD: A cohort of 63 participants diagnosed with CH at Tohoku University underwent screening for geneti
