Metabolic Dyshomeostasis, neuroinflammation, and glymphatic clearance deficits as convergent drivers of Alzheimer's disease: an exposome-based conceptual framework for prevention.

The repeated failures of anti-amyloid therapies in Alzheimer's disease (AD) have exposed critical limitations of the linear amyloid cascade hypothesis and underscored the need for a broader etiological framework. The objective of this review is to synthesise mechanistic evidence for an exposome‑based model of sporadic AD that integrates metabolic, inflammatory, and sleep/circadian contributions to brain aging. Epidemiological evidence indicates that up to 40% of AD risk is attributable to modifiable environmental and lifestyle factors. These factors, collectively termed the aging exposome, rep

[Mechanism of electroacupuncture combined with antibiotics targeting dorsal motor nucleus of vagus nerve to activate vagus-adrenal axis, alleviate inflammatory response and reduce mortality in septic mice].

OBJECTIVES: To explore the mechanism by which electroacupuncture (EA) combined with antibiotics regulates the vagus-adrenal axis via the dorsal motor nucleus of vagus (DMV), promotes catecholamine secretion, attenuates inflammatory storm, and improves survival rate in septic mice. METHODS: Experiment 1:fifty C57BL/6 mice were randomly divided into sham operation group, model group, antibiotic group, EA group, and EA + antibiotic group, with 10 mice in each group. Cecal ligation and puncture (CLP) was performed to establish the sepsis model. Mice in the EA group received 15 min EA (10 Hz, 0.5

High-Frequency Electrograms Abolition as a Valuable Target for Remap During Ventricular Tachycardia Ablation. The HF REMAP Study.

BACKGROUND: peak frequency (PF) analysis has recently emerged as a novel tool to assess electrogram properties through their frequency spectrum. PF is an indirect marker of local myocardial activity, and its reduction after radiofrequency ablation reflects effective tissue modification. OBJECTIVE: our study aims to evaluate PF analysis as an objective method to validate lesion efficacy during ventricular tachycardia (VT) ablation. METHODS: we retrospectively analyzed 43 (out of 247) consecutive patients who underwent VT ablation between January 2023 and December 2024, using the EnSite X mapp

Eco-geomorphic response of the Funafuti Atoll reef flat, central Pacific, to sea-level rise and tropical storms over 120 years (1898-2019).

This study examines eco-geomorphic changes on the exposed reef flat of Funafuti Atoll over the past 120 years to assess the relative influence of sea-level rise and storm events on reef flat and adjacent environments. Despite a documented sea-level rise of approximately 0.45 m during this period, we find no evidence of long-term structural or ecological alteration on the reef flat itself. We attribute this stability to emergent properties that may buffer against sea-level-driven impacts observed elsewhere. By contrast, significant geomorphic adjustments occurred at the reef flat margins: spur

Elevated local field potential power characterizes choreic storms identified by deep brain stimulator event capture.

Some patients with hyperkinetic movement disorders may present with a severe and prolonged state of uncontrolled chorea and/or ballism, termed 'choreic storm', causing distress and prompting intervention. These events may be more frequent and severe in patients with childhood-onset movement disorders. Deep brain stimulation (DBS) can be effective for medically refractory dyskinesias including chorea, and the availability of event-based local field potential (LFP) sensing in clinically available devices present an opportunity to better understand the underlying neurophysiology of these episodic

The Landmark Series: Perioperative Management of Hepatocellular Carcinoma.

BACKGROUND: Hepatocellular carcinoma (HCC) remains a leading cause of cancer-related mortality worldwide, with an increasing burden of metabolic dysfunction-associated steatotic liver disease-associated HCC in Western populations. In treating HCC, recurrence after curative-intent therapy remains common, creating a compelling rationale for perioperative systemic therapy. METHODS: This Landmark Series review examines the pivotal studies that have shaped the surgical and perioperative management of HCC, tracing the evolution from principles of resectability to contemporary adjuvant and neoadjuva

Mitochondrial DNA mediated Kupffer cell activation in the pathogenesis and progression of liver diseases.

The liver serves as a pivotal metabolic and immunological hub, yet its homeostatic equilibrium is highly susceptible to mitochondrial dysfunction. Recent studies have implicated mitochondrial DNA (mtDNA) as an important damage-associated molecular pattern (DAMP) that links mitochondrial injury to sterile hepatic inflammation and KCs (KCs) activation. This review provides a comprehensive synthesis of the mtDNA mediated KCs across four major hepatic pathologies: Alcohol-associated Liver Disease (ALD), Metabolic Dysfunction-associated Steatotic Liver Disease (MASLD), Hepatic Ischemia-Reperfusion

Angry Purkinje Syndrome: A Review of Short-Coupled Polymorphic Ventricular Tachycardia with Quinidine Treatment.

BACKGROUND: Polymorphic ventricular tachycardia (VT) mediated by short-coupled premature ventricular complexes in patients with recent myocardial infarction is termed 'Angry Purkinje Syndrome'. These arrhythmias are typically refractory to conventional antiarrhythmic therapy but may respond to quinidine. METHODS & RESULTS: We present a case series of patients with short-coupled polymorphic VT occurring 2-8 days after acute myocardial infarction and coronary revascularisation. All three patients had recurrent polymorphic VT episodes that were consistently initiated by ventricular ectopy with a

Early parathyroid hormone elevation after denosumab therapy following parathyroidectomy: a diagnostic pitfall.

Persistently elevated parathyroid hormone (PTH) after parathyroidectomy is often attributed to residual disease but may reflect altered calcium (Ca) homeostasis. The impact of denosumab on perioperative PTH interpretation remains unclear. A 77-year-old woman presented with a hypercalcemic crisis and was initially managed at another hospital. During treatment with hydration and elcatonin (40 units twice daily), denosumab (60 mg) was administered, resulting in a rapid decline in corrected Ca (c-Ca). Eleven days later, at presentation to our institution, her c-Ca was 9.9 mg/dL (SI: 2.48 mmol/L; r

Cardiac arrest as the first sign of thyrotoxicosis: a case report of angiographically proven coronary vasospasm.

BACKGROUND: Cardiovascular manifestations of thyrotoxicosis are common, typically presenting as tachyarrhythmias or high-output physiology. Coronary artery spasm (CAS) is an uncommon but increasingly recognized complication, usually causing transient ischaemia rather than malignant ventricular arrhythmias. Cardiac arrest due to thyrotoxicosis-induced CAS is exceedingly rare. Early recognition is vital because management differs from that of typical acute coronary syndromes (ACS). CASE SUMMARY: A 63-year-old man suffered an out-of-hospital cardiac arrest with an initial shockable rhythm, achie

Investigation of the correlation between serum levels of interleukin-6 and clinical and paraclinical findings in COVID-19 patients.

BACKGROUND: COVID-19 frequently presents with pneumonia. Impaired immune function, particularly in older adults and immunocompromised individuals, increases susceptibility to infection and contributes to higher morbidity and mortality. Among inflammatory cytokines, Interleukin-6 (IL-6) plays a pivotal role in immune dysregulation and cytokine storm. Therefore, this study aimed to investigate the association between serum IL-6 levels and clinical, laboratory, radiological, and outcome-related findings among hospitalized patients with COVID-19. METHODS: This descriptive-analytical cross-section

Gal-3 N-terminal prolines within residues 69 to 117 regulate CRD glycan binding, phase separation and Gal-3-mediated cellular function.

Galectin-3 (Gal-3), the only chimeric galectin, is a central driver and potential biomarker for cancer progression, fibrotic disorders, and heart failure. As a key proinflammatory factor, Gal-3 aggravates various disease states by promoting immune cell activation, cytokine storm amplification, and cell migration. Previously, we reported on the structural and functional role of prolines within Gal-3 NT residues 1-68. Here, we report on how the 14 NT prolines within the 69-117 segment affect Gal-3 structure and function. Circular dichroism (CD) and fluorescence spectroscopies showed that none of

Decoding Pyroptosis in Pulmonary Fibrosis: Molecular Mechanisms and Therapeutic Horizons.

BACKGROUND: Pulmonary fibrosis (PF) is a progressive and fatal interstitial lung disease characterized by the depletion of alveolar epithelial cells (AECs) and the pathological accumulation of extracellular matrix (ECM). Despite its high mortality rate, the molecular drivers of PF progression remain partially understood. Recent evidence suggests that pyroptosis, a pro-inflammatory programmed cell death, plays a pivotal role in orchestrating the inflammatory cascade and fibrotic remodeling in the lung. METHODS: This study employed bibliometric and visual analysis via CiteSpace 6.2.4 to systema

Excess IL-18 drives fatal mousepox cytokine storm via NK cell immunodeficiency and nonspecific T-cell hyperactivation.

Hemophagocytic Lymphohistiocytosis (HLH) is a hyperinflammatory syndrome with multiple contributors, often including an infectious trigger. The Macrophage Activation Syndrome (MAS) subtype is linked to marked IL-18 excess, cytotoxic T-lymphocyte (CTL) activation, and natural killer (NK) cell abnormalities. We asked how excess IL-18 shapes NK and CTL responses to viral challenge. Mice with chronic IL-18 overproduction (Il18tg), showed baseline NK-cytopenia and CTL activation resembling MAS-susceptible patients. NK transcriptomes reflected IL-18/IFNg-axis activation and cell cycle upregulation.

Thyroid and Parathyroid Dysfunction Following Larynx-Trachea-Thyroid-Parathyroid Allotransplantation.

Larynx-trachea-thyroid-parathyroid allotransplantation can restore essential laryngeal functions, but postoperative endocrine changes remain poorly understood. This study assessed thyroid and parathyroid function in four recipients. All developed early thyroid hormone alterations consistent with euthyroid sick syndrome, followed by variable endocrine outcomes. Two experienced thyrotoxicosis associated temporally with graft rejection, while another developed hypothyroidism and elevated parathyroid hormone during late rejection. Transient parathyroid hormone suppression was also observed. In con
