Angiosarcoma of the thyroid and concurrent hyperthyroidism.

We describe the exceptional association of an angiosarcoma of the thyroid and hyperthyroidism in a 74-year-old woman who presented with a toxic multinodular goiter of recent growth. This association reported in two other cases may not be fortuitous but due a vascular effect of the angiosarcoma contributing to the development of thyrotoxicosis. This case also illustrates the diagnostic difficulties and the poor prognosis of this tumor.

Is it hyperthyroidism? You can't always tell from the clinical picture.

Hyperthyroidism usually causes a typical clinical syndrome in younger patients, but in some cases it presents with only minimal or atypical symptoms. This is especially true in older patients, who may have none of the typical signs, such as goiter. Differentiation between a euthyroid hyperthyroxinemic state and hyperthyroidism is crucial to avoid unnecessary treatment of persons with a normally functioning thyroid gland. In addition, distinguishing between causes of hyperthyroidism is important, because the treatments may be completely different. For example, in subacute thyroiditis, supportiv

[Thyrotoxicosis at the onset of diabetes mellitus. Report of five cases].

Graves' disease and nodular toxic goitre are known to occur with increased frequency in patients with diabetes mellitus. It may be difficult to distinguish the symptoms of the two conditions from each other at the onset of diabetes. Thyrotoxic crisis is an uncommon, but extreme accentuation of thyrotoxicosis with a high mortality even in patients undergoing correct treatment. Reports on "major adverse life events" precipitating either diabetes or thyrotoxicosis are known. The symptoms of thyrotoxic disease in the elderly are often vague with few symptomatic manifestations. Five cases with simu

Role of colestipol in the treatment of hyperthyroidism.

The enterohepatic circulation of thyroxine (T4) and triiodothyronine (T3) is higher in thyrotoxicosis. Bile-salt sequestrants bind iodothyronines and thereby increase their fecal excretion. We, therefore, evaluated the effect of colestipol-hydrochloride administration on clinical and biochemical indices of patients with hyperthyroidism. In a prospective, controlled trial, ninety-two adult volunteers with Graves' disease, toxic autonomous nodule or toxic multinodular goiter were randomly assigned into the following treatment protocols: Group 1, 30 mg of methimazole (MMI) and 20 g of colestipol-

[Thyrotoxicosis induced by amiodarone].

The Authors report a case of toxic multinodular goiter induced by longstanding administration of Amiodarone, in which the option was near total thyroidectomy for control of toxic symptoms without withdrawal of the antiarhuthmic drug. In this case, the post-operative period was complicated by compressive cervical hematoma, which was managed by performing an emergent tracheostomy.

Sporadic medullary thyroid carcinoma associated with toxic multinodular goitre.

First described in 1959, medullary thyroid carcinoma (MTC) arises from parafollicular C cells distributed throughout the thyroid, and may occur in one of two clinical forms, sporadic (80%) or familial. Familial MTC may present as a palpable thyroid nodule, but may also be diagnosed by screening relatives of an index case for a raised basal serum calcitonin or a rise in calcitonin following a stimulated pentagastric test. Both tests are highly sensitive for C-cell hyperplasia and MTC. Sporadic cases of MTC most commonly present as asymptomatic thyroid nodules with normal thyroid function tests.

Autoimmune hyperthyroidism occurring late after radioiodine treatment for volume reduction of large multinodular goiters.

131I treatment is an effective alternative to surgery in patients with a large, (non-)toxic, compressive goiter. Late development of hyperthyroidism after 131I therapy for nontoxic nodular goiter is considered rare. We have seen this complication in 3 of approximately 80 patients treated with radioiodine for volume reduction of a large, multinodular goiter. Three women, aged 60 to 71 years, had large, multinodular goiters causing tracheal compression. They were clinically euthyroid before 131I therapy and had normal free thyroxine (FT4) levels. Serum thyroid-stimulating hormone (TSH) levels we

Thyrotoxicosis.

Thyrotoxicosis is a clinical syndrome caused by circulation of excess thyroid hormones. Classic hyperkinetic thyrotoxicosis is readily recognizable. Atypical presentations, however, can lead to diagnostic dilemmas. The sensitive thyroid-stimulating hormone assay has become an invaluable tool in the diagnosis of thyrotoxicosis. Causes of thyrotoxicosis include Graves' disease, toxic multinodular goiter, toxic adenoma, thyroiditis, inappropriate secretion of thyroid-stimulating hormone, trophoblastic tumor, exposure to iodine, and use of drugs. Increased uptake of radioidine by the thyroid gland

Role of high-resolution pinhole tomography in the evaluation of thyroid abnormalities.

The present study was undertaken to evaluate the contribution of pinhole-single photon emission computed tomography (P-SPECT) to scintigraphy of the thyroid gland. Thirty-six patients, referred for thyroid scintigraphy because of palpatory or ultrasonographic findings, were studied after intravenous administration of technetium-99m pertechnetate (222 MBq) using a rectilinear scanner and a single-headed rotating gamma camera equipped with a pinhole collimator. P-SPECT study was acquired on a 128 x 128 matrix, in 30 projections over 180 degrees and an acquisition time of 40 seconds per step. Aft

Hypercalcemia due to subacute thyroiditis.

OBJECTIVE: To describe a patient who had hypercalcemia attributable to subacute thyroiditis. METHODS: A case history, including detailed results of laboratory tests, is presented, and the findings are discussed. RESULTS: A young woman with neck pain and fever was found to have increased free thyroxine and triiodothyronine levels but a decreased thyrotropin value. Concurrently, the serum calcium concentration was increased. Subacute thyroiditis was diagnosed. Within 1 month, the free thyroxine and serum calcium levels were normal. CONCLUSION: Thyrotoxicosis is a known cause of hypercalcemia.

Serum immunoglobulin E and soluble CD23 in patients with Graves' disease.

It is not clear whether the IgE system plays a role in the pathogenesis of Graves' disease which is an autoimmune disorder. The low-affinity receptor of IgE, which is known as CD23, is not simply a receptor, it has various functions related to immune cells. There is no information about its levels in Graves' disease. For these reasons, serum IgE and soluble CD23 levels were determined in 40 patients with thyroid disease, free from allergic disorders and/or parasitic infestations. Patients were divided into three groups: Group 1: Patients with Graves' disease (n = 15, age: 33.4 +/- 9.3, 9 women

Evaluation and management of multinodular goiter.

Nodular goiters are encountered commonly in clinical practice by primary care physicians, endocrinologists, surgeons, and otolaryngologists. Epidemiologic data suggest that in the United States, the incidence of such goiters is approximately 0.1% to 1.5% per year, translating into 250,000 new nodules annually. Nodular goiters are more common in women than in men, with advancing age, and after exposure to external irradiation. These goiters may be asymptomatic, with normal TSH levels (nontoxic), or may be associated with systemic thyrotoxic symptoms (toxic MNG or Plummer's disease). Diagnostic

Treatment options for hyperthyroidism.

Antithyroid drugs induce sustained remission in only 55% of cases. Radioiodine, the overwhelming choice of specialists, may pose risks, including aggravation of ophthalmopathy. Surgical ablation should be considered only if a highly skilled surgeon is available. Thyroid storm requires aggressive management, and definitive treatment is needed for toxic adenoma or toxic multinodular goiter.

Serum levels of interleukin 6 and tumor necrosis factor-alpha in hyperthyroid patients before and after propylthiouracil treatment.

Contrary to the usual inhibitory role of tumor necrosis factor-alpha (TNF-alpha) in thyroid metabolism, it also has specific stimulatory effects in autoimmune thyroid disorders, including induction of HLA class II antigen-presenting cell-T cell interaction. Despite high intrathyroidal concentrations, various studies were not able to demonstrate high serum levels of TNF-alpha in patients with Graves' disease. To investigate this discrepancy we determined TNF-alpha and interleukin 6 (IL-6) levels in 25 hyperthyroid patients who responded to propylthiouracil treatment (16 with Graves' disease and

Congestive heart failure due to reversible cardiomyopathy in patients with hyperthyroidism.

The authors describe the clinical characteristics and response to therapy of seven patients with hyperthyroidism, dilated cardiomyopathy, and low-output cardiac failure. All patients (4 women and 3 men, age 47 +/- 4 years, mean +/- standard error of the mean) were admitted with the primary diagnosis of congestive heart failure. The cause of hyperthyroidism was Graves' disease in six patients, and toxic multinodular goiter in one. On admission, the mean serum T4 was 21 +/- 1 microgram/dL and mean serum T3:411 +/- 77 ng/mL, and serum thyroid-stimulating hormone was suppressed ( < 0.03 microU/mL)

Ultrastructural evidence of thyroid damage in amiodarone-induced thyrotoxicosis.

Amiodarone-induced thyrotoxicosis occurs in 2-12.1% of patients on chronic amiodarone treatment. In most cases its pathogenesis is related to iodine overload in the presence of preexisting thyroid abnormalities, such as multinodular or diffuse goiter or autonomous nodule. A minority of patients show apparently normal glands or pictures of non-autoimmune thyroiditis. However, there is recent evidence of a direct toxic effect of amiodarone, with consequent release of iodothyronines into the circulation. We report a patient with amiodarone-induced thyrotoxicosis with toxic thyroid effects demonst

[Status of 3 years of hyperthyroidism treatment with iodine 131].

We analysed retrospectively the results obtained during 3 years in the treatment of thyrotoxicosis with 131-iodine. A group of 32 patients, including 21 Graves' diseases, 10 multinodular toxic goitres and 1 toxic adenoma has been collected. The therapeutic efficacy of one dose of radioiodine, assessed after 6 months was 100% in toxic multinodular goitres and 57% in Graves' diseases. No acute complications were observed. The best radiosensitivity of toxic multinodular goitres compared with diffuse goitres is probably due to the mechanisms themselves of thyrotoxicosis in these diseases. Results

Scintigraphic diagnosis of thyroid cancer. Correlation of thyroid scintigraphy and histopathology.

Scintigraphy is part of the routine examination of thyroid disease, yet there are few published reports on its utility and pitfalls. Out of 2025 consecutive thyroid examinations after excluding 230 cases of proven thyroid malignancies, 403 patients who underwent surgery were studied for histopathologic correlation. There were 232 with cold nodules, 143 with multinodular goiters (MNG) and 28 with uniformly increased uptake suggestive of toxic goiter. Malignancy was reported in 72 of the 232 cold nodules (31%). In solitary nodules, the examination was useful in selecting cases for surgery and, i

The clinical characteristics of nodular goitres: relatively frequent (4-7%), diagnostic difficulties deriving from heterogeneity as well as the marked difference of treatment and prognosis. The pathogenesis of multinodular goitres has been discussed and the different endogen and exogen factors which are important in the development of autonomies.
