A case of thyroid storm due to thyrotoxicosis factitia.

We describe a case of thyroid storm due to thyrotoxicosis factitia, which was caused by the ingestion of excessive quantities of exogenous thyroid hormone for the purpose of reducing weight. An 18-year-old female was admitted to the hospital 24 hours after taking up to 50 tablets of synthyroid (1 tablet of synthyroid : levothyroxine 100 microg). Because of her stuporous mental state and acute respiratory failure, she was intubated and treated in the intensive care unit. After reviewing her history carefully and examining plasma thyroid hormone levels, we diagnosed this case as a thyroid storm

Recurrent hamburger thyrotoxicosis.

Recurrent episodes of spontaneously resolving hyperthyroidism may be caused by release of preformed hormone from the thyroid gland after it has been damaged by inflammation (recurrent silent thyroiditis) or by exogenous administration of thyroid hormone, which might be intentional or surreptitious (thyrotoxicosis factitia). Community-wide outbreaks of "hamburger thyrotoxicosis" resulting from inadvertent consumption of beef contaminated with bovine thyroid gland have been previously reported. Here we describe a single patient who experienced recurrent episodes of this phenomenon over an 11-yea

Thyroglobulin autoantibody levels below the cut-off for positivity can interfere with thyroglobulin measurement.

Antithyroglobulin antibodies can interfere with the measurement of thyroglobulin yielding spuriously high or low levels depending on the method used. Interference is unrelated to the antibody concentration and can occur at very low concentrations. We report a patient in whom antithyroglobulin antibodies below the cut-off for positivity nearly led to an incorrect diagnosis of thyrotoxicosis factitia.

[The management of thyrotoxicosis: a schematic approach].

Thyrotoxicosis is a well defined clinical entity, determined by an increase of plasma levels of thyroid hormones (T3 and T4). A number of causes of thyrotoxicosis are known, and it is therefore very important for the treatment to establish its etiology. In fact, metimazole or propylthiouracil are indicated for the thyrotoxic states caused by thyroid gland's hyperfunction (hyperthyroidism), but are not effective when thyrotoxicosis is determined by a follicular damage and disruption with leakage of preformed thyroid hormones, or in case of thyrotoxicosis factitia. Besides medical therapy, other

Iatrogenic thyrotoxicosis. Causal circumstances, pathophysiology and principles of treatment. Review of the literature.

Thyrotoxicosis is the clinical syndrome that results when tissues are exposed to high levels of circulating thyroid hormones. In most instances, thyrotoxicosis is due to hyperthyroidism, a term reserved for disorders characterized by overproduction of thyroid hormones by the thyroid gland. Nevertheless, thyrotoxicosis may also result from a variety of conditions other than thyroid hyperfunction. The present report focuses on the etiologies, pathophysiology and treatment of iatrogenic thyrotoxicosis. Iatrogenic thyrotoxicosis may be caused by 1) subacute thyroiditis (a result of lymphocytic inf

The age of patients with thyrotoxicosis factitia in Italy from 1973 to 1996.

Thyrotoxicosis factitia, a syndrome due to the surreptitious ingestion of excess thyroid hormones, has generally been diagnosed in young or middle-aged women with psychopathological disturbances. We reviewed all the cases seen at our Institution over a 24-yr period, from 1973 to 1996. All 25 patients were women. Analysis was restricted to 17 patients who were born and lived in Tuscany (our region), since only these patients were distributed during the whole observation period. Diagnosis of thyrotoxicosis factitia was based on the following parameters: elevated serum total and/or free thyroid h

Thyroid vascularity and blood flow are not dependent on serum thyroid hormone levels: studies in vivo by color flow doppler sonography.

OBJECTIVE: Thyroid blood flow is greatly enhanced in untreated Graves' disease, but it is not known whether it is due to thyroid hormone excess or to thyroid hyperstimulation by TSH-receptor antibody. To address this issue in vivo patients with different thyroid disorders were submitted to color flow doppler sonography (CFDS). SUBJECTS AND METHODS: We investigated 24 normal subjects, and 78 patients with untreated hyperthyroidism (49 with Graves' hyperthyroidism, 24 with toxic adenoma, and 5 patients with TSH-secreting pituitary adenoma (TSHoma)), 19 patients with thyrotoxicosis (7 with thyro

Serum and tissue thyroglobulin measurement: clinical applications in thyroid disease.

Since the first demonstration by Van Herle et al. that thyroglobuling (Tg), the main iodo-protein of the thyroid gland, was detectable in the circulation of normal subjects by using specific radioimmunoassay, an impressive amount of papers has been produced, describing several clinical applications of Tg measurements. Now, measurement of Tg is the mainstay in the post-surgical follow-up of differentiated thyroid cancer and an integral part in the diagnostic evaluation of patients with benign thyroid diseases. In this article we will review the most relevant clinical application of Tg measureme

Color flow Doppler sonography in thyrotoxicosis factitia.

Color flow doppler sonography (CFDS) is a powerful technique which displays tissue blood flow and vascularity. Hyperthyroidism due to Graves' disease is characterized by variable degrees of increased blood flow at CFDS. The purpose of this study was to evaluate CFDS patterns in five women with thyrotoxicosis factitia, a condition due to surreptitious ingestion of excess thyroid hormone. Diagnosis was supported by the finding of elevated free thyroxine (FT4), ranging 24.2-67.6 pmol/L (normal values: 8.3-20.5), elevated free triiodothyronine (FT3), ranging 9.9-26.7 pmol/L (normal values: 3.8-8.4

An outbreak (159 cases) of transient thyrotoxicosis without hyperthyroidism in Japan.

We report an outbreak of transient thyrotoxicosis that occurred between June and September 1993, in Matsuyama City, Ehime Prefecture. One hundred fifty-nine cases of thyrotoxicosis were identified, all in individuals without goiter. Thyroid autoantibodies were absent, and the serum thyroglobulin level was depressed. Ultrasonography of the thyroid disclosed no abnormal findings, while scintigraphy of the thyroid with 99mTc revealed poor uptake. About ten days after the onset, the serum thyroid hormone levels were normalized. No significant elevation of serum viral antibodies was found. These ob

Factitious hyperthyroidism causing acute myocardial infarction.

Myocardial ischemia is a rare but severe and possibly life threatening manifestation of hyperthyroidism, but does not usually result in persistent ischemia. We report on a 71-year-old woman who had undergone total thyroidectomy with subsequent irradiation because of follicular carcinoma 3 years ago. Since then, she had been maintained on oral levothyroxine replacement therapy at a dose of 0.15 mg alternating with 0.2 mg daily. When latent hypothyroidism became evident despite replacement therapy, the dose of levothyroxine was increased to 0.3 mg a day. Three weeks later, the patient suffered f

Thyrotoxicosis factitia in a post-aortocoronary bypass patient.

A 52-yr-old man presented with an evolving myocardial infarction and unstable angina. Previously, he had undergone aortocoronary bypass surgery for triple vessel disease and at that time was diagnosed as hypothyroid. He had been refractory to thyroxine treatment and now required 0.3 mg thyroxine daily. On admission, he was hypertensive, tachycardic and found to be thyrotoxic secondary to excess thyroid hormone ingestion. Treatment with iopanoic acid was started. Despite medical therapy he continued to have unstable angina. Coronary angiography confirmed further triple vessel disease with block

The measurement of fecal thyroxine in the diagnosis of thyrotoxicosis factitia.

Two patients with prolonged thyrotoxicosis are presented with low radioiodide uptake, low-normal serum thyroglobulin concentrations, and low iodide content of the neck on x-ray fluorescence studies. The surreptitious intake of thyroid hormone was directly proven by a high fecal concentration of T4 (12.38 and 23.99 nmol/g) compared with low concentrations in healthy subjects (1.03 +/- 0.64 nmol/g; mean +/- SD, n = 6) and in patients with hyperthyroidism due to Graves' disease (1.93 +/- 1.86; n = 8). Fecal thyroid hormone measurement may help to provide formal proof of factitious thyrotoxicosis.

Occupational thyroid disease.

A case of thyrotoxicosis due to the occupational exposure to cosmetics in a 35-year-old beautician is reported. The hormonal pattern was consistent with exogenous thyroid hormone administration, but not with iodine hyperthyroidism. The patient denied she was using thyroid hormones; also, she lacked the typical features of patients with thyrotoxicosis factitia. Her occupational history was carefully reviewed: A heavy exposure of the unprotected skin to cosmetic creams containing iodine, thyroid hormones, and thyroid extracts had occurred in the previous months. The patient was advised to refrai

[Elevated thyroid hormone levels in the absence of hyperactivity].

Elevated blood levels of thyroid hormones may be due to causes other than glandular hyperactivity. We have seen transient increases in total thyroxine (TT4), free thyroxine index (FTI), free thyroxine (FT4), and total triiodothyronine (TT3) in 12 women and 3 men with subacute thyroiditis and 2 women with painless (silent) postpartum thyroiditis. Elevated TT4, FTI, and FT4 were found in 11 of 85 patients treated with amiodarone. High TT4, but not FTI or FT4, was seen in 4 women using contraceptives, in 2 pregnant women and in 2 men with liver dysfunction. All hormones, except TSH, were elevated
