'When you go to bed, and you cannot sleep … ': smallholder farmers' experiences and perceptions of climate change events and their impacts on health in northern Ghana.

Despite the critical role of smallholder farmers in promoting food and nutrition security, research suggests that their health is severely threatened by experiences of climate change events. Our study assesses smallholder farmers' experiences and perceptions of rainfall, drought, flood, storm, and wildfire; and explores how such experiences impact food security and health. We employ the convergent mixed-method design, combining binomial regression analysis of survey data from a random-purposive sample of 1042 smallholder farmers with qualitative insights from inductive thematic analysis of dat

A rule-based multi-objective optimization framework for flood control in coastal urban river networks under extreme rainfall and tidal backflow.

Coastal urban river networks are increasingly exposed to severe flood risk from extreme rainfall and tidal backflow. To mitigate this risk, this study proposes a rule-based multi-objective optimization framework to derive operating rules for hydraulic infrastructure. Within this framework, Storm Water Management Model (SWMM) is coupled with the NSGA-II algorithm to form a multi-objective model to minimize overflow risk and operational cost. Performance metrics evaluate convergence, adaptability under diverse hydrological conditions, and identify optimal water-level rule-based control (WL-RBC)

Rationalisation of a thrombophilia panel using laboratory medicine Delphi-like consensus evaluation and secondary artificial intelligence-based simulation assessment.

OBJECTIVES: It is important to review laboratory test panels regularly and omit unnecessary tests. This avoids overdiagnosis and makes laboratory work more targeted. Artificial intelligence is increasingly being discussed as a possible aid in such decisions. The aim of the study was to revise an existing thrombophilia panel with the help of a modified Delphi consensus of laboratory physicians and to examine whether large language models (LLMs) can mimic such decision-making processes and serve as a support tool. METHODS: The study was conducted in two steps. First, six experts evaluated vario

Exploring the role of extreme climate events (ECEs) in the incidence of respiratory disease in South Africa.

In recent decades, escalating extreme climate events (ECEs) have raised significant concerns regarding their effects on public health in South Africa, particularly respiratory illness. This study examined the relationship between ECEs and respiratory health outcomes over a 12-year period (2008-2019). A total of 48 ECEs were analyzed, of which 28 occurred in regions reporting more than 100 medical insurance claims for respiratory diseases. These events included storms, heatwaves, cold waves, floods, and tornadoes. Using a two-week lag period, we assessed their short-term association with respir

Ellobiid snail loss in seawall-fronted mangroves.

Seawalls are widely constructed to protect coastal communities and infrastructure against storm surges and tidal flooding; however, they often exert significant ecological pressure on adjacent wetland ecosystems. Understanding these impacts is critical for sustainable coastal management, particularly in the context of intensifying coastal squeeze. In this study, we investigated the effects of seawall construction on benthic mollusk communities within subtropical mangrove wetlands by comparing assemblages in mangroves fronting seawalls with those in adjacent, undisturbed natural mangroves. The

Therapeutic responses in patients with advanced NSCLC enrolled in early-phase clinical trials at MD Anderson.

We evaluated clinical outcomes and safety profiles in patients with non-small cell lung cancer (NSCLC) treated in early-phase trials. A retrospective review of 546 NSCLC cases treated from January 2016 to December 2024 at The University of Texas MD Anderson Cancer Center was performed using the MD Anderson CHIMERA database. Patients were categorized into seven groups based on treatment regimen. The overall objective response rate (ORR) was 19.9%, and the highest ORRs were in the targeted combination (30.8%) and targeted monotherapy (29.7%) groups. Gene alteration-matched therapy, serum albumin

AI-powered segmentation and prognosis with missing MRI in pediatric brain tumors.

Brain MRI is the primary imaging modality for pediatric brain tumors, yet incomplete acquisitions are common, hindering the clinical utility of existing deep learning models for tumor segmentation and prognosis. These models are typically trained on complete MRI sequences and exhibit performance degradation when MRI sequences are missing at test time. In this retrospective study of 715 patients from the Children's Brain Tumor Network and BraTS-PEDs, and 43 patients with 157 longitudinal MRIs from PNOC003/007 clinical trials, we developed strategies for handling missing sequences. Methods inclu

Defining the user needs for an assistive pediatric hand exoskeleton: An Italian explorative survey.

Children with neuromotor disorders often experience hand impairments that limit independence in daily tasks as well as the acquisition of new skills. Robotic aids like hand exoskeletons can improve function, but many designs fail to meet user needs, reducing adoption. This study collected parental feedback to guide the development of user-centered hand exoskeletons. An anonymous online survey was distributed to the parents of children with neurological disorders and previously treated at IRCCS E. Medea - Associazione "La Nostra Famiglia" in Bosisio Parini, Italy. Only parents of children with

Microfibers Accumulation within a Mediterranean Submesoscale Cyclone.

Cyclonic eddies are widespread upper ocean features known to enhance primary productivity via nutrient upwelling; yet, their role in the transport and retention of anthropogenic contaminants remains poorly understood. Here, we present high-resolution oceanographic measurements from a submesoscale cyclone in the Western Mediterranean Sea, revealing a pronounced subsurface accumulation of textile microfibers (MFs) within the eddy core (0.34 MF l-1) relative to surrounding waters (0.09 MF l-1). This enrichment persisted in a secondary, smaller cyclone that detaches following the main cyclone's fr

Surface water microbiome response to pyrogenic carbon after a wildland-urban interface fire.

Fires in the wildland-urban interface (WUI) introduce pyrogenic organic contaminants to surface waters, but their impacts on microbial dynamics have not been evaluated. We studied the interactions between microbial communities and pyrogenic carbon during post-fire storms in a WUI fire-impacted creek in Orange County, CA. The first storms following the fire (low intensity) brought about the highest discharges of polycyclic aromatic hydrocarbons (PAHs), e.g. benzo[a]pyrene, benz[a]anthracene. Dissolved organic carbon (DOC) loads reached up to 11.2 g-C s-1 during the more severe storms. PAHs corr

Hyperactive BMP and Mechanosignaling Remodel Chromatin to Drive Aberrant Osteogenesis in FOP.

UNLABELLED: Fibrodysplasia ossificans progressiva (FOP) is a rare genetic disorder in which a recurrent ACVR1 (R206H) mutation drives progressive heterotopic ossification (HO). While aberrant BMP hypersensitivity has been studied, how this mutation enforces a persistent pro-osteogenic state remains unclear. Here, we combined super-resolution stochastic optical reconstruction microscopy (STORM), transposase-accessible chromatin with sequencing (ATAC-Seq), and RNA sequencing (RNA-Seq) to investigate how Acvr1 R206H remodels chromatin to promote osteogenic transcriptional programs. Mutant mouse e

Taller Trees Experienced Less Crown Damage During a Severe Hurricane in a Tropical Forest.

Hurricanes strongly regulate vegetation structure and dynamics in coastal tropical forests by causing tree mortality and damaging the crowns of surviving trees. Despite the importance of this damage for long-term forest demography and carbon dynamics, the patterns and drivers of variation in hurricane-induced damage across heterogeneous landscapes remain poorly quantified. Here, we integrate field inventory data with repeated airborne LiDAR measurements to quantify tree crown damage following Hurricane María (Category 4) in the Luquillo Experimental Forest, Puerto Rico. We applied two approach

The Weight of the Storm: Using Weighting Methods to Determine the Effect of Hurricane Harvey on Mental Health Outcomes in Harris County, Texas.

OBJECTIVES: Nonprobability sampling, commonly used in disaster research, can lead to incorrect estimates or limit the generalizability of results. We collected data through the Texas Flood Registry (TFR) and used raking and propensity score weighting to provide insight into the effect of Hurricane Harvey (hereinafter, Harvey) on Harris County, Texas. METHODS: From April 2018 through October 2020, residents of areas affected by Harvey enrolled in the TFR completed a survey on their storm-related experiences (n = 20 653). Using logistic regression, we assessed the relationship between Harvey-re

Enhanced submarine groundwater discharge and nutrient fluxes in response to typhoon events in Zhanjiang Bay, China.

Typhoons significantly influence the spatiotemporal dynamics of submarine groundwater discharge (SGD) and associated nutrient fluxes in coastal systems. This study investigates the impact of multiple typhoon events of Typhoons Lionrock and Kompasu on the distribution of 222Rn and SGD in Zhanjiang Bay, South China, and evaluates the consequent variations in nutrient (DIP, NO2- + NO3-, DSi) inputs via SGD. Results show that typhoons induce a substantial increase in SGD fluxes, with peak post-typhoon values reaching up to 8 times pre-typhoon levels, primarily driven by intense rainfall that enhan

SERS-enabled immune monitoring: Decoding the tumor microenvironment for precision Cancer immunotherapy.

The tumor immune microenvironment (TME) is pivotal in regulating cancer initiation, progression, and therapeutic response. Despite major advances in immunotherapy, clinical outcomes remain limited by low response rates and unpredictable immune-related adverse events (irAEs). Surface-enhanced Raman scattering (SERS) technology, with its single-molecule sensitivity and multiplexing capabilities, offers a transformative approach for dynamic TME profiling. This review highlights recent progress in SERS-enabled immune monitoring, including quantitative detection of immune checkpoint molecules (e.g.
