Thyroid Hormone Excess in Sudden Death: A Clinicopathological Spectrum in an Autopsy Study.

Thyrotoxicosis is associated with heightened adrenergic activity, arrhythmogenic susceptibility, and procoagulant alterations; yet, its contribution to sudden death outside overt thyroid storm remains incompletely understood. We retrospectively screened 975 consecutive autopsies and identified 7 cases with biochemical evidence of thyroid hormone excess defined by elevated free T3 and/or free T4 levels. Clinical background, medication exposure, thyroid-related biomarkers, including thyroglobulin, thyroid autoantibodies, and interleukin-6, together with detailed histopathological examination of

Drug-induced liver injury due to potassium iodide during thyroid storm.

Thyroid storm is a life-threatening condition causing multiorgan failure, including liver dysfunction. Potassium iodide (KI) is one of the key drugs used to control thyroid hormone levels. While thionamides sometimes cause drug-induced liver injury (DILI), KI has not been reported to cause DILI. Herein, we report a case of likely KI-induced DILI during treatment for thyroid storm due to Graves disease. A 30-year-old man presented with thyrotoxicosis due to Graves disease, with hyperbilirubinemia and elevated liver transaminases. The patient was diagnosed with thyroid storm, and treatment was i

Case Report: Bilateral coronary ostial spasm triggered by iatrogenic thyrotoxicosis mimicking left main coronary artery disease.

BACKGROUND: Thyrotoxicosis is associated with enhanced sympathetic activity and increased coronary vasomotor reactivity. However, bilateral coronary ostial spasm mimicking critical left main coronary artery disease is rare. CASE SUMMARY: A 61-year-old woman presented with a 3-day history of exertional chest pain. She had undergone total thyroidectomy 18 years earlier and was receiving levothyroxine 75 μg daily. One month prior, carvedilol 6.25 mg twice daily had been initiated for palpitations. Treadmill testing demonstrated ST-segment elevation in aVR and anterior leads with reciprocal ST de

Initial disease severity and methimazole titration for Graves' disease: a retrospective longitudinal cohort study.

CONTEXT: Graves' disease (GD) is the most common cause of thyrotoxicosis. Medical therapy with methimazole is the most common way to treat GD, but it has a narrow and dynamic therapeutic index, requiring frequent monitoring for titration. It is unclear how much initial disease severity (IDS) affects titration patterns. OBJECTIVE: Identify associations between IDS with methimazole dose and thyrotoxicosis control after initiation of therapy. DESIGN: Retrospective longitudinal cohort study (2018-2024). SETTING: A single tertiary medical center. PATIENTS OR OTHER PARTICIPANTS: The study includ

Outpatient presentation of ventricular fibrillation in paediatric left ventricular assist device patients: a case series with mechanistic insights.

BACKGROUND: Ventricular fibrillation in paediatric recipients of left ventricular assist devices is rare but potentially fatal; however, because systemic perfusion may be temporarily maintained by the mechanical support, haemodynamic deterioration may be delayed, leading to underrecognition and presentation in the outpatient setting rather than as an immediate in-hospital emergency. METHODS: We report three paediatric left ventricular assist device recipients who developed outpatient malignant ventricular arrhythmias, each representing a distinct mechanistic category: structural, mechanical,

Measuring thyroid-stimulating hormone receptor antibodies using the IMMULITE® 2000 TSI assay is better than using the BRAHMS TRAK assay in an unselected clinical population, but both perform worse than expected: a real-world retrospective observational study.

BACKGROUND: Graves’ hyperthyroidism is caused by stimulatory autoantibodies. Its diagnosis and monitoring are commonly based on measurement of thyrotropin receptor antibodies (TRAb) with unspecific immunoassays, that also detect neutral and blocking antibodies. TRAb analyzed using the Siemens IMMULITE® 2000 TSI immunoassay (TRAb-IM), designed to target stimulatory immunoglobulins, represents a promising alternative. This study aimed to determine the clinical performance of TRAb-IM and the Thermo Fisher BRAHMS TRAK KRYPTOR immunoassay (TRAb-KR) in a real-world setting. METHODS: Over 3 months, T

Atrial fibrillation and associated cardiovascular disorders in adults with hyperthyroidism: a retrospective cohort study from two teaching hospitals.

INTRODUCTION: Atrial fibrillation (AF) is a major cardiovascular complication of thyrotoxicosis but its burden and predictors are poorly described in sub-Saharan Africa. We assessed the prevalence, clinical course, and predictors of AF and related cardiovascular complications in adults with hyperthyroidism at two tertiary hospitals in Addis Ababa, Ethiopia. METHODS: Retrospective cohort study of adults (≥ 18 years) with biochemically confirmed hyperthyroidism managed between April 2019 and September 2024. Clinical, ECG and echocardiographic data were abstracted from records; primary outcomes w

Impact of Autoimmune Disease on Outcomes After Total Thyroidectomy for Thyrotoxicosis.

INTRODUCTION: Thyrotoxicosis with diffuse goiter is most commonly caused by Graves disease, which frequently coexists with other autoimmune diseases (ADs). Although total thyroidectomy is a definitive treatment in select cases, there is a lack of comprehensive data on how ADs influence surgical outcomes. This study evaluates complication rates within 90 d of surgery among patients undergoing total thyroidectomy for thyrotoxicosis with diffuse goiter, comparing those with and without coexisting ADs. METHODS: A retrospective cohort study was conducted using a large multi-institutional database.

Ethnicity, TSH-Receptor Antibody (TRAb) and Thyrotoxicosis Severity Are Determinants of Neutrophil Count in Newly Diagnosed Hyperthyroidism.

OBJECTIVE: A retrospective cross-sectional analysis with an ethnicity-matched control cohort examining the relationship between free thyroxine (FT4), thyroid-stimulating hormone receptor antibody (TRAb) and absolute neutrophil count (ANC) in newly diagnosed hyperthyroidism, and assessing the influence of ethnicity on ANC. METHODS: Adults with newly diagnosed hyperthyroidism between August 2020 to July 2025 and concurrently available FT4, TRAb and ANC were included. Graves' disease (GD) was defined by Roche Elecsys TRAb ≥ 1.75 IU/L. An ANCOVA tested main effects of TRAb and FT4 on ANC and thei

Double filtration plasmapheresis in antithyroid drug-refractory Graves disease with a massive goiter.

We report a case of double filtration plasmapheresis (DFPP) performed in medication-resistant Graves disease (GD) with a massive goiter. A 19-year-old female was diagnosed with GD and initially demonstrated poor medication compliance. She presented with loss of consciousness from hypoxemia due to goiter-related tracheal narrowing. Despite inpatient drug therapy, the thyroid hormone levels remained high. We performed DFPP to control thyrotoxicosis before proceeding with thyroidectomy. After 4 sessions, thyroid hormone levels improved, enabling total thyroidectomy. GD with a large goiter may be

Determinants of diagnostic delay in thyroid storm: a 12-year single-center cohort study.

OBJECTIVE: Thyroid storm is a life-threatening endocrine emergency. Although predictors of mortality have been described, determinants of diagnostic delay remain unclear. We aimed to identify factors associated with diagnostic delay in thyroid storm and its relationship with severe clinical outcomes. METHODS: We conducted a retrospective cohort study of adults admitted with thyroid storm at a tertiary-care hospital over 12 years. The primary endpoint was time from symptom onset to diagnosis (days). Factors associated with time to diagnosis were evaluated using multivariable linear regression

Comparative Analysis of Elevated Serum Total and Free Triiodothyronine Results Obtained by Immunoassays.

BACKGROUND: Measurement of triiodothyronine (T3) levels is important for the laboratory diagnosis and management of certain cases of thyrotoxicosis and for identifying conditions involving altered peripheral conversion of thyroxine (T4) to T3. However, whether total T3 (TT3) or free T3 (FT3) should be measured remains unclear. METHODS: In this study, we performed a retrospective analysis of the results from 605 blood samples from 465 patients, in which thyroid-stimulating hormone (TSH), free T4 (FT4), TT3, and FT3 levels were measured in parallel. All the parameters were quantified using a wi

Thyroid Dysfunction Following Tirzepatide Use in a Post-thyroidectomy Patient on Stable Levothyroxine Therapy: A Case Study.

Tirzepatide, a dual glucagon-like peptide-1 (GLP-1)/glucose-dependent insulinotropic polypeptide (GIP) receptor agonist, is increasingly prescribed for obesity and glycemic control. Its effects on gastric emptying, appetite regulation, and rapid weight loss may influence thyroid hormone homeostasis and the pharmacokinetics of narrow-therapeutic-index medications such as levothyroxine. Emerging clinical experience suggests that GLP-1-based therapies can suppress thyroid-stimulating hormone (TSH), alter thyroid hormone dynamics, and precipitate symptoms of thyrotoxicosis in patients receiving th

Estimation of radiation dose to commuters using patient transport from thyrotoxicosis patients treated with I-131 at universitas academic hospital.

The prevailing economic disparity causes patients from various parts of the Free State and Northern Cape Provinces in South Africa and Lesotho to be treated with radioactive I-131 for thyrotoxicosis at Universitas Academic Hospital. Some patients are obliged to travel long distances (4-9 h) using patient transport services after discharge, causing concern for public exposure of nearby passengers due to a public dose limit of 1 mSv year-1. The study aimed to estimate the commuters' radiation exposure from 62 thyrotoxicosis patients post-treatment with I-131. An electronic personal dosimeter and

A case series and literature review of thyroid cancer presenting with thyroid hot nodule.

Thyroid nodules are commonly found in clinical practice, with a prevalence of 5%-7% on physical examination and up to 20%-67% when detected by high-resolution ultrasound in adults. The annual incidence of thyroid cancer varies significantly in different countries, ranging from 2.0 to 3.8/100,000 in women and from 1.2 to 2.6/100,000 individuals in men, with papillary thyroid cancer being the most frequently diagnosed type. While most of these nodules are nonfunctioning and benign, some may be hyperfunctioning ("hot" nodules), potentially causing thyrotoxicosis. Differentiated thyroid cancers, s

Neonatal Hyperthyroidism: Case Report and Literature Review.

Fetal and neonatal hyperthyroidism are rare, but severe, conditions caused by transplacental passage of thyrotropin receptor antibodies from a pregnant patient with current or past Graves disease. We present the case of a late preterm infant who developed thyrotoxicosis secondary to maternal Graves disease treated with radioactive iodine, followed by a review of the literature, clinical implications, and best practices on the diagnosis and treatment of neonatal hyperthyroidism. We highlight the importance of finding the underlying cause for hypothyroidism during pregnancy, ensuring those with
