The coexistence of preeclampsia and uncontrolled thyrotoxicosis leading to acute respiratory failure is rare and poses significant diagnostic and therapeutic challenges, particularly with respect to safe fluid management in the obstetric intensive care setting. We report the case of a 35-year-old woman at 37-week gestation who presented with progressive dyspnea, acute pulmonary edema, and fetal distress, necessitating emergency cesarean section and subsequent intensive care admission. Point-of-care ultrasound (POCUS) demonstrated impaired left ventricular systolic function (ejection fraction 4

INTRODUCTION: Thyrotoxicosis is characterized by excessive action of thyroid hormones in tissues, leading to significant morbidity and mortality. Anti-thyroid drugs are the primary treatment for thyrotoxicosis; however, the time required to achieve euthyroid status varies among patients. The aim of this study was to determine the time to euthyroidism and its predictors among thyrotoxicosis patients on anti-thyroid drugs at tertiary hospitals in Nekemte city.
METHODS: A five-year hospital-based retrospective cohort study (from January 1, 2020, to December 31, 2024) was conducted among patients

Thyroid storm represents a critical exacerbation of thyrotoxicosis, often associated with Graves' disease, characterized by fever, gastrointestinal disturbances, neuropsychiatric manifestations, and tachyarrhythmias, closely resembling sepsis in its clinical presentation. The elevation of inflammatory biomarkers can be significant, frequently leading to delayed or missed diagnosis. Conventional treatment (beta-blockade, thionamides, iodine after thionamide, and corticosteroids) involves rapid management of adrenergic symptoms alongside inhibition of thyroid hormone synthesis and release; howev

SUMMARY: For half a century, antithyroid drugs (ATDs) have been the first-line treatment for Graves' disease. ATDs, e.g. carbimazole, are associated with minor and serious adverse effects. These include rash, urticaria, arthralgia, hepatotoxicity, vasculitis, and agranulocytosis. Agranulocytosis is a potentially serious adverse effect characterised by a neutrophil count <0.5 × 109 cells/L, which may deteriorate further into sepsis and death. This is a case report of a 47-year-old female who had a 17-year history of Graves' disease. She received carbimazole for 2 years, which was stopped after

Thyroid storm is a rare, severe, and life-threatening exacerbation of thyrotoxicosis, characterized by dysfunction of the thermoregulatory, central nervous, gastrointestinal, hepatic, and cardiovascular systems. We present a 31-year-old female with newly diagnosed Graves' disease complicated by carbimazole-induced agranulocytosis. The delay in initiating adjunctive therapy to control the thyrotoxicosis due to suspected sepsis was followed by rapid development of thyroid storm requiring therapeutic plasma exchange (TPE) and urgent thyroidectomy. This case highlights the importance of prompt rec

Antithyroid drug-induced agranulocytosis is a rare but potentially life-threatening complication, particularly in patients with limited therapeutic options. This case illustrates the complexity of managing uncontrolled thyrotoxicosis in the context of a patient with decompensated heart failure, chronic obstructive pulmonary disease (COPD), and adverse reactions to both methimazole and propylthiouracil. A 54-year-old male patient with a 2-year history of Graves' disease presented with worsening dyspnea and tachycardia in the setting of thyrotoxicosis with a TSH of 0.0007 mIU/L. He had previousl

Thyroid storm, also known as thyrotoxic crisis, is an uncommon and potentially life-threatening condition that can cause metabolic, cardiovascular and neurological complications. Thyrotoxic crisis often develops in the context of untreated or poorly controlled hyperthyroidism and is typically precipitated by acute stressors such as infection, surgery, or trauma. While the progression from hypothyroidism to hyperthyroidism is itself an uncommon clinical occurrence, the emergence of thyroid storm in this setting is sporadic. Such a case highlights the importance of being aware of atypical presen

While it is presumed that thyrotoxicosis is a contributing factor for cardioembolic stroke secondary to atrial fibrillation (AF), cerebrovascular events in patients without cardiac arrhythmias remain exceptionally rare. We present the case of a 41-year-old African female patient with no traditional stroke risk factors who presented with sudden-onset left hemiplegia and dizziness (National Institutes of Health Stroke Scale (NIHSS) 9). Imaging revealed a right middle cerebral artery (MCA) territory infarct with hemorrhagic transformation and malignant edema (midline shift 3 mm), necessitating em

Magnetic milli/microrobots hold strong potential for biomedical applications, but their deployment as multi-robot systems is limited by short-range magnetic coupling, which prevents independent control in confined environments. Here, we present an expanded field-free region (FFR)-based control framework for spatially selective actuation of multiple magnetic torque-driven robots in confined environments. A generalized selective-control criterion incorporating the minimum rotating magnetic field required for robot actuation enables the framework to accommodate robots with different magnetic resp

BACKGROUND: Recurrent, gestation-limited severe abdominal colic without a conventional diagnosis is rarely described. Endocrine-disrupting chemicals (EDCs) have been associated with adverse reproductive outcomes, but their relevance to recurrent pregnancy-specific gastrointestinal syndromes remains uncertain.
CASE PRESENTATION: A 35-year-old Chinese woman had five prior pregnancies; four were terminated because of recurrent severe lower abdominal colic, constipation, inability to maintain oral intake, and marked weight loss. Symptoms reproducibly began at 11-12 gestational weeks and resolved

BACKGROUND: Amiodarone is an effective antiarrhythmic agent but causes thyroid dysfunction in 15-20% of patients, resulting in amiodarone-induced hypothyroidism (AIH) and thyrotoxicosis (AIT).
METHODS: We performed a narrative review of randomized trials, prospective cohort studies, systematic reviews, and major guideline documents. PubMed and Embase were searched from January 2000 to December 2024 using terms related to amiodarone, thyroid dysfunction, thyrotoxicosis, and hypothyroidism. Priority was given to higher-quality evidence and contemporary society guidelines, including those from t

OBJECTIVE: Distinguishing Graves' disease (GD) from the other causes of thyrotoxicosis is essential for appropriate management. The clinical phenotype of GD has evolved over time and is currently more heterogenous and milder than in the past. Consequently, the differential diagnosis of thyrotoxicosis is often challenging. We aim to provide clinicians with a practical guide to achieve a more reliable etiological diagnosis of thyrotoxicosis.
METHODS: We reviewed international guidelines and the main papers indexed in PubMed from inception to March 1, 2026, on the diagnosis of thyrotoxicosis.

RATIONALE: Graves' disease (GD) may be associated with ischemic stroke through nonatherosclerotic intracranial arteriopathy during thyrotoxicosis. However, the optimal antithrombotic regimen and treatment duration for this condition remain uncertain.
PATIENT CONCERNS: A 38-year-old man presented approximately 6.5 hours after the last known well with sudden dysarthria, aphasia, and right-sided weakness. His National Institutes of Health Stroke Scale (NIHSS) score was 9 on admission.
DIAGNOSES: Brain magnetic resonance imaging demonstrated acute ischemic infarcts, and magnetic resonance angiog

Peripheral edema in thyrotoxicosis is uncommon and is typically associated with cardiac dysfunction. Unilateral edema without overt heart failure is rarely reported. We present the case of a 40-year-old female patient who presented with edema involving the right lower limb and suprapubic and genital regions. Investigations revealed thyrotoxicosis associated with atrial fibrillation (AF). The patient demonstrated significant clinical improvement following treatment with antithyroid therapy, beta-blocker, glucocorticoids, and supportive management. This case highlights the importance of consider

INTRODUCTION AND IMPORTANCE: Periodic paralysis is an uncommon presentation of thyrotoxicosis characterized by acute flaccid paralysis due to hypokalemia. Thyrotoxicosis is often missed as the cause of periodic paralysis due to a lack of overt symptoms. Early recognition and appropriate management can prevent recurrent paralysis and fatal complications, such as cardiac arrhythmias.
CASE PRESENTATION: We report a case of a 28 years old male who presented with acute-onset flaccid quadriparesis following a carbohydrate-rich meal. Investigations revealed severe hypokalemia (2.1 mmol/L) and thyrot

Thyrotoxic periodic paralysis (TPP) is a rare complication of hyperthyroidism characterized by acute weakness and hypokalemia due to intracellular potassium shift. Clinical management of TPP consists of cautious potassium replacement, non-selective β-blockade, and prompt treatment of the underlying thyrotoxicosis. Definitive control of hyperthyroidism, combined with avoidance of known precipitants, is essential to prevent recurrent attacks. Failure to recognize the syndrome can result in repeated admissions, unnecessary evaluations, and life-threatening complications. We report the case of a 3

Acute mesenteric ischemia remains one of the most lethal abdominal emergencies, particularly when bowel necrosis has already occurred at the time of diagnosis. Embolic occlusion of the superior mesenteric artery is most commonly associated with atrial fibrillation. Although hyperthyroidism is a recognized trigger for atrial fibrillation, widespread systemic arterial embolization in this setting is rarely reported. We describe a 77-year-old woman with biochemical thyrotoxicosis and new-onset atrial fibrillation who developed simultaneous superior mesenteric and peripheral arterial occlusions, r

Thyrotoxic hypokalemic paralysis is a rare complication of hyperthyroidism, characterized by acute, reversible episodes of muscle weakness associated with severe hypokalemia, secondary to intracellular potassium redistribution mediated by excess thyroid hormones and activation of the Na+/K+ - ATPase pump. Its incidence is approximately 0.2% and it occurs predominantly in males. In this report, we describe four cases of hypokalemic paralysis associated with thyrotoxicosis. All patients were male, aged between 21 and 37 years. The predominant clinical presentation was acute-onset muscle weakness

Thyrotoxic periodic paralysis (TPP) is a rare but life-threatening cause of acute flaccid paralysis associated with hypokalemia. Although more common in males and individuals of Asian descent, it may present atypically in young females and be easily misdiagnosed. We report a case of a 30-year-old female who presented with acute quadriparesis. Extensive neuromuscular evaluation initially pointed toward Guillain-Barré syndrome (GBS), but subsequent investigations revealed profound hypokalemia. Further workup confirmed thyrotoxicosis as the underlying cause. Prompt potassium replacement and initi
