SUMMARY: Amiodarone-induced thyrotoxicosis (AIT) is a known complication of amiodarone usage in adults that occurs secondary to unregulated overproduction of thyroid hormone due to excess iodine exposure in an abnormal thyroid gland (type one), the release of stored thyroid hormone due to a direct cytotoxic effect on a normal thyroid gland (type two), or both (mixed type). Therapy for AIT depends on the subtype present and typically consists of discontinuation of amiodarone and the addition of thyrostatic drugs or corticosteroids. AIT is a very uncommon adverse side effect of amiodarone use in

BACKGROUND: Tirzepatide is a dual agonist of the glucose-dependent insulinotropic polypeptide (GIP) receptor and glucagon-like peptide-1 (GLP-1) receptor, demonstrates significant efficacy in lowering blood glucose levels and reducing body weight. Consequently, its use has increased among patients with type 2 diabetes and obesity. Gastrointestinal symptoms are commonly recognized as adverse effects of tirzepatide, while other adverse effects, such as thyroid abnormalities, are rare. This report describes a rare case of unilateral Graves' disease that developed during treatment with tirzepatide

Amiodarone-induced thyrotoxicosis (AIT) is an uncommon but clinically significant complication of amiodarone therapy that may be associated with substantial morbidity, particularly in patients with underlying cardiovascular disease. We report the case of a 61-year-old gentleman with a history of atrial fibrillation, ischaemic heart disease, heart failure, and obesity, who developed severe thyrotoxicosis after approximately 3.5 years of amiodarone treatment. Routine thyroid function monitoring showed stable thyroid levels for three years; however, despite an abnormality identified at a later st

Hypercalcaemia is a recognised complication of thyrotoxicosis usually related to increased bone resorption. However, coexistent hyperphosphataemia is uncommon and may create diagnostic uncertainty. It is postulated that serum calcium and phosphate levels rise due to high bone turnover and increased renal reabsorption of phosphate in response to excess thyroxine. Hypercalcaemia may also alter the clinical presentation of thyrotoxicosis, making both diagnosis and management challenging. We report two patients with this unusual biochemical presentation. The first patient was a 69-year-old woman w

Graves disease is the most common cause of hyperthyroidism in children. Although neutropenia and thrombocytopenia have been reported in adults with untreated Graves disease, these complications are rare in children. Here, we report a pediatric case of Graves disease complicated by agranulocytosis and thrombocytopenia at diagnosis. A 12-year-old girl presented with increased appetite, irritability, fever, and petechiae. Laboratory examination revealed thyrotoxicosis, and this was associated with agranulocytosis and thrombocytopenia. There was no history of antithyroid drug use. The diagnosis of

Graves' disease can be managed with antithyroid drugs (ATDs), radioactive iodine (RAI), or surgery. When ATDs are ineffective or poorly tolerated, RAI or surgery may be indicated. Methimazole (MMI) and propylthiouracil (PTU) are commonly used, sometimes with adjunctive agents such as potassium iodide (KI), β-blockers, or corticosteroids. Although ATDs are essential in management, rare but severe allergic reactions may occur, necessitating careful monitoring. We report a case of refractory Graves' disease with a history of ATD-induced hemophagocytic syndrome and multiple drug allergies. A 43-ye

Thyrotoxic periodic paralysis (TPP) is a reversible but potentially severe complication of thyrotoxicosis characterized by acute muscle weakness and hypokalemia resulting predominantly from intracellular potassium redistribution. Although classically associated with Asian populations, TPP is increasingly recognized in Latin America. We report a 38-year-old Colombian man with recurrent nocturnal proximal quadriparesis, preserved deep tendon reflexes, serum potassium of 2.3 mEq/L, and biochemical thyrotoxicosis secondary to Graves disease. In parallel, we conducted a structured review of publish

RATIONALE: A bicornuate uterus is an uncommon mullerian duct malformation strongly associated with recurrent early pregnancy loss and adverse obstetric outcomes. When combined with endocrine disturbances such as hyperprolactinemia and postpartum thyroid dysfunction, fertility becomes even more challenging. This case highlights how coordinated reproductive-endocrine management can overcome multiple intersecting barriers to conception and successful pregnancy.
PATIENT CONCERNS: A 24-year-old woman presented with a history of 3 first-trimester miscarriages, difficulty conceiving, and prior breas

Amiodarone is known to cause thyroid dysfunction, due to both direct cytotoxicity and high iodine content. Due to its lipophilic properties, amiodarone gets stored in the adipose tissue and may induce delayed amiodarone-induced thyrotoxicosis (AIT), months after discontinuation. We present the case of a 60-year-old man admitted with new-onset atrial fibrillation. He had a history of ventricular tachycardia, treated with amiodarone that was discontinued 14 months earlier. On admission, thyrotropin was undetectable, with markedly elevated free thyroxine and total triiodothyronine. He had no prev

Previous studies on the relationship between thyroid diseases (TDs) and autoimmune hepatitis (AIH) are inconclusive. As a result, the aim of this study is to elucidate the potential causality between TDs and AIH. A bidirectional 2-sample Mendelian randomization (MR) analysis was performed to explore the causal associations between TDs and AIH. The primary approach utilized was the inverse variance weighted method, with sensitivity analyses conducted using the weighted median, MR-Egger, Cochran Q, and MR-PRESSO methods. The MR analysis showed that the odds ratio (OR) of hypothyroidism or myxede

BACKGROUND: Hypokalemic periodic paralysis (HPP) presenting as acute quadriparesis is a neuromuscular emergency. While its etiology is described in general wards, its severe "critical care phenotype" in the intensive care unit (ICU) remains poorly characterized. We aimed to define this phenotype by analyzing the clinical profile, etiological spectrum, and predictors of life-threatening severity.
METHODS: A retrospective study was conducted of 12 patients (nine male, three female; median age: 31.5 years) admitted to a tertiary ICU (2015-2021) with acute quadriparesis and hypokalemia (median po

BACKGROUND: Thyroid storm is a life-threatening endocrine emergency with high mortality. Existing diagnostic score primarily focuses on diagnosis rather than predicting outcomes. This study aimed to identify prognostic factors and develop a practical clinical prognostic score for 30-day all-cause mortality in patients with thyroid storm.
METHODS: A retrospective cohort study was conducted at a tertiary care center in Thailand. Patients diagnosed with thyroid storm were included. To identify prognostic factors of 30-day all-cause mortality, clinical and biochemical variables were analyzed usin

Thyrotoxic crisis is a rare, life-threatening condition requiring rapid recognition and close multidisciplinary management. In this case report, we present a 59-year-old woman who developed cardiogenic shock and cardiac arrest despite intensive therapy. Emergency thyroidectomy was performed with extracorporal membrane oxygenation prepared as rescue support, but postoperative collapse persisted, and she died shortly after. The case underscores the critical importance of early identification and coordinated interdisciplinary care.

Thyroid storm (TS) is an extreme hypermetabolic state due to thyrotoxicosis. TS may manifest as cardiovascular symptoms, including atrial fibrillation (AF) (9 - 23%) and heart failure (HF). Dilated cardiomyopathy (DCM) and HF develop at a later stage in approximately 1% of thyrotoxicosis patients. A 46-year-old man presented to the emergency department with a chief complaint of severe dyspnea and rapid AF. Blood test examination showed overt thyrotoxicosis. The Burch-Wartofsky point scale is 60 (highly suggestive of TS). Echocardiography showed severely reduced left ventricular (LV) systolic f

BACKGROUND: Thyroid pneumatosis is an extremely rare condition, previously reported only once as a complication of an ultrasound-guided thyroid fine needle aspiration (FNA).
CASE REPORT: A 70-year-old male sought emergency care after experiencing a foreign body sensation in his throat. He was diagnosed with a retropharyngeal abscess caused by upper digestive tract perforation from fishbone ingestion, which was surgically extracted. Postoperative day 3, a neck computed tomography (CT) revealed gas bubbles within the thyroid parenchyma, indicative of thyroid pneumatosis, likely leading to destr

Thyrotoxicosis-induced myopericarditis is an uncommon but increasingly recognized cardiovascular manifestation of Graves' disease that may closely mimic acute coronary syndrome (ACS). We present a 42-year-old woman with obesity and tobacco use who presented with severe chest pain, ST-segment elevations, and elevated troponins concerning for acute coronary syndrome. Coronary angiography demonstrated non-obstructive coronary arteries, while further evaluation revealed severe thyrotoxicosis with impending thyroid storm and inflammatory findings consistent with myopericarditis. The patient was tre

Graves' disease (GD) is the most common cause of thyrotoxicosis in the pediatric population. Methimazole (MMI) is the first-line therapy; however, it may rarely cause agranulocytosis, a potentially life-threatening adverse effect. Recurrent or delayed-onset agranulocytosis presents diagnostic and therapeutic challenges due to the scarcity of reported cases. This report aims to describe a pediatric case with recurrent MMI-induced agranulocytosis and to discuss potential mechanisms and management strategies. We describe a 16-year-old female with GD who developed recurrent MMI-induced agranulocyt

BACKGROUND: Spontaneous coronary artery dissection (SCAD) is an increasingly recognized cause of myocardial infarction with non-obstructive coronary arteries (MINOCA). Although Graves' disease is a well-established cause of coronary vasospasm, its potential association with structural coronary arterial injury remains incompletely understood. We report a case of hyperthyroidism-associated SCAD that provides clinical insight into a possible link between severe thyrotoxicosis and coronary arterial vulnerability.
CASE PRESENTATION: We report a 56-year-old female with uncontrolled Graves' disease
