[Branchial cleft anomalies of the neck : Embryology, clinical presentation, and management].

Branchial cleft anomalies are the second most common congenital neck lesion. They present as cysts, sinuses, or fistulas arising from remnants of the pharyngeal apparatus; second arch anomalies predominate. The embryologic origin determines their anatomical relationships to nerves, vessels, and the pharynx and guides diagnosis and surgical planning. Typical findings are cervical swelling, cutaneous discharge, or recurrent infection. Third and fourth branchial anomalies may cause recurrent neck abscesses or acute suppurative thyroiditis, particularly in children. Ultrasonography is the primary

Anesthesia Considerations for Transfemoral Osseointegration in a Patient With CRPS, Propofol, and Ketamine Allergies: A Case Report.

A 20-year-old female with asthma, Ehlers-Danlos syndrome, Hashimoto's thyroiditis, mast cell activation syndrome, and complex regional pain syndrome Type 1 (CRPS1) presented for left transfemoral amputation with osseointegration and nerve reconstruction. To limit CRPS1 exacerbation, neuraxial anesthesia with femoral and sciatic blocks was administered for analgesia. Given the reported history of convulsions following ketamine and propofol, sedation with midazolam, dexmedetomidine, and fentanyl was used. On POD 1, fascia iliaca and sciatic blocks were repeated to mitigate postoperative pain. Th

Association of GLP-1 Receptor Agonist Use with Thyroid Outcomes in Adults with Type 2 Diabetes.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are widely used to treat type 2 diabetes mellitus, but their associations with thyroid disorders remain uncertain. We evaluated whether initiating a GLP-1 RA, compared with initiating a dipeptidyl peptidase-4 inhibitor (DPP-4i), was associated with newly recorded autoimmune thyroiditis and other thyroid outcomes. METHODS: This new-user, active-comparator cohort study emulated a target trial using the TriNetX Collaborative Network. Adults with type 2 diabetes who initiated a GLP-1 RA or DPP-4i between January 1, 2018, and Decemb

Efficacy and safety of acute-phase immunotherapy for adult autoimmune encephalitis: a systematic review and network meta-analysis.

BACKGROUND: Autoimmune encephalitis (AE) is an immune-mediated inflammatory disorder of the central nervous system associated with substantial morbidity. Acute-phase immunotherapy is crucial for improving outcomes, yet the comparative efficacy and safety of available regimens remain unclear. We therefore performed a Bayesian network meta-analysis to compare acute-phase immunotherapeutic strategies for AE. METHODS: PubMed, Embase, Web of Science, and the Cochrane Library were searched from inception to October 2025 for studies evaluating acute-phase immunotherapy in AE. A Bayesian network meta

Autoimmune hepatitis following long-term denosumab therapy in a patient with Hashimoto thyroiditis.

Denosumab, a receptor activator of nuclear factor-κB ligand (RANKL) inhibitor, is widely used to treat osteoporosis. Its role as a trigger for autoimmune hepatitis (AIH) remains uncertain. We report a 68-year-old woman with osteoporosis and Hashimoto thyroiditis who developed jaundice and severe hepatocellular liver injury after 3 years of denosumab therapy. Alternative viral, obstructive, and metabolic causes were excluded. Liver biopsy demonstrated interface hepatitis with a lymphoplasmacytic portal infiltrate consistent with AIH, and autoimmune serologies were positive. Hemophagocytic lymph

New-Onset Graves' Disease in the Postpartum Period Initially Suspected to Represent Postpartum Thyroiditis.

Postpartum thyroid dysfunction is usually attributed to postpartum thyroiditis (PPT), a self-limited autoimmune condition. Graves' disease (GD) can also develop during the postpartum period, and the two conditions often present with overlapping features. We describe the case of a 37-year-old woman who developed severe anterior neck pain and abnormal thyroid function nine months postpartum, after she stopped breastfeeding. She was initially thought to have PPT, but serial testing showed steadily declining thyroid-stimulating immunoglobulin (TSI) and thyroid-stimulating hormone (TSH) receptor an

Radiosynthesis and quality control testing of two 18F-labeled radiopharmaceuticals for α-synuclein imaging in clinical investigations.

BACKGROUND: 18F-Labeled (E)-1-fluoro-3-((2-(4-(5-(methylamino)pyrazin-2-yl)but-1-en-3-yn-1-yl)benzo[d]thiazol-6-yl)oxy)propan-2-ol ([18F]SPAL-T-06) and (E)-1-fluoro-3-((2-(4-(6-(methylamino)pyridine-3-yl)but-1-en-3-yn-1-yl)benzo[d]thiazol-6-yl)oxy)propan-2-ol ([18F]C05-05) are positron emission tomography (PET) imaging agents for α-synuclein pathologies. Recently, we demonstrated the utility of PET imaging with [18F]SPAL-T-06 for α-synuclein pathologies in patients with multiple system atrophy and with [18F]C05-05 for α-synuclein pathologies in Parkinson's disease and related disorders. Clinical and genomic features of Chryseobacterium species isolated from clinical cultures.

Chryseobacterium species are environmental bacteria occasionally implicated in human infections; however, their clinical and genomic features are not fully characterized. This study investigated the clinical, genotypic, and phenotypic characteristics of Chryseobacterium strains collected through the MultiDrug-Resistant Organisms Clinical Research Network, a multicenter prospective study of patients with carbapenem-resistant gram-negative bacilli isolated at tertiary care hospitals in Japan between April 2019 and March 2024. Among 18 patients with Chryseobacterium spp. confirmed by whole-genome

Cardiovascular risk factors in autoimmune thyroiditis: role of adiponectin, resistin, inflammation and auto-immunity - a cross-sectional study.

OBJECTIVE: Autoimmune thyroiditis (AIT) has been associated with an increased cardiovascular risk. Whether adipokines, inflammation and auto-immunity contribute to this increased risk is uncertain. Our aim was to evaluate the interrelationships of adipokines, inflammation, auto-immunity, and thyroid function with cardiovascular risk in autoimmune thyroiditis. METHODS: Sixty-two patients with AIT, with normal free T4 (FT4) and free T3 (FT3), without other comorbidities were studied. Patients were divided into two groups: AIT with thyroid-stimulating hormone (TSH) ≤2.5 mU/L and AIT with TSH >2.

Efficacy of nutraceuticals as adjunctive therapies in autoimmune diseases: A systematic review of clinical trials.

INTRODUCTION: Autoimmune diseases are characterized by a dysregulation of the immune system, resulting in chronic inflammation and tissue damage. Although immunosuppressive therapies have advanced, their limited efficacy and the adverse effects associated with long-term use restrict their clinical utility, highlighting the need for alternative treatments. This study aimed to synthesize current evidence on the efficacy of nutraceutical interventions for these disorders. METHODS: A comprehensive search was conducted on PubMed, Embase, and ClinicalTrials.gov to identify studies published between

How I Do It: An Imaging Approach and Review of Autoimmune Encephalitis.

Encephalitis is a medical emergency characterized by inflammation of the brain parenchyma due to an infective cause or an immune-mediated process. Autoimmune encephalitis (AE) is characterized by a misdirected immune attack against neuronal structures, resulting in an inflammatory response. Triggers for the development of antibodies are often paraneoplastic or parainfectious. AE may occur in children or adults with symptoms including subacute onset of altered mentation, cognitive issues, drug-resistant epilepsy, movement disorders, and/or autonomic dysfunction. Limbic encephalitis is a common
