Riedel thyroiditis mimicking malignancy: a fatal surgical outcome.

PURPOSE: Riedel thyroiditis (RT) is a rare fibroinflammatory disorder that can mimic malignancy clinically and radiologically. We report an unusually extensive, ultimately fatal case and highlight its diagnostic and therapeutic lessons. METHODS: Case report of a 40-year-old man, with review of the clinical, imaging, histopathological and follow-up data, reported in accordance with the CARE guidelines. RESULTS: The patient presented with a right cervicothoracic mass (64 × 45 × 115 mm) circumferentially encasing the common carotid, subclavian and innominate vessels with mediastinal extension.

Psychosocial stress and autoimmune thyroid disease: a systematic review of clinical phenotypes and vulnerability factors.

Psychological stress has been proposed as a potential trigger or modulator of autoimmune thyroid diseases, but evidence remains fragmented. This systematic review examined the relationship between stress and thyroid autoimmunity, with particular attention to Hashimoto's thyroiditis and Graves' disease, highlighting the critical methodological distinction between stressor exposure (objective events) and the subjective stress response (psychological burden). PubMed, Embase, Scopus, and PsycINFO were searched for controlled human studies; 25 studies involving a total of N = 117,034 participants m

Mucosal Small Cell Neuroendocrine Carcinoma of the Gallbladder With Extraregional Lymph Node Metastasis: A Case Report.

Gallbladder small cell neuroendocrine carcinoma (SCNEC) is a rare and highly aggressive malignancy, and an optimal treatment strategy has not been established. We report a case of mucosal gallbladder SCNEC with extraregional lymph node metastasis that responded well to chemotherapy. A 69-year-old woman presented with severe epigastric pain and inflammatory findings. Ultrasonography, endoscopic ultrasonography, and computed tomography revealed a 40 mm gallbladder mass and enlarged lymph nodes in the hepatoduodenal ligament. Because inflammation and pain could not be controlled by conservative t

Resistance to Thyroid Hormone Beta or Thyroid-Stimulating Hormone (TSH)-Secreting Pituitary Adenoma: A Diagnostic Dilemma With a THRB Variant of Uncertain Significance.

Elevated thyroid hormone concentrations with nonsuppressed thyroid-stimulating hormone (TSH) present a challenging diagnostic problem. Resistance to thyroid hormone beta (RTHβ), analytical interference, and a TSH-secreting pituitary adenoma (TSHoma) are important considerations. We describe a woman with persistent elevation of free thyroxine (FT4) and nonsuppressed or elevated TSH for more than five years, autoimmune thyroiditis, a heterozygous THRB c.284G>C (p.Gly95Ala) variant classified as a variant of uncertain significance (VUS), and a 4.9 × 4.4 mm pituitary microadenoma. Human anti-mouse

Healthy sleep pattern is associated with reduced risk of incident thyroid disease: a prospective cohort study from the UK Biobank.

INTRODUCTION: The longitudinal association between sleep behaviors and the incidence of various thyroid diseases remains unclear. This study aimed to prospectively investigate the association between a healthy sleep score and the risk of incident thyroid diseases using data from the UK Biobank. METHODS: This prospective cohort study included 499, 084 adults aged 37-73 years from the UK Biobank. A healthy sleep score ranging from 0 to 5 was constructed based on five self-reported sleep behaviors: chronotype, sleep duration, insomnia, snoring, and daytime excessive sleepiness. Participants with

Hymenoptera Envenomation as a Potential Trigger for Acute Diffuse Alopecia Areata: A Case Report.

INTRODUCTION: Alopecia areata (AA) is an autoimmune hair loss disorder often triggered by genetic and environmental factors. Acute, massive shedding is unusual and may indicate a potent immune trigger. CASE PRESENTATION: We report a 34-year-old woman with Hashimoto thyroiditis who developed rapid, diffuse AA 10 days after bee stings. Clinical examination showed marked hair loss (SALT score 30→70) affecting scalp, eyebrows, and eyelashes. Laboratory workup revealed elevated IgE and venom-specific hypersensitivity. Treatment with corticosteroid pulses followed by oral methylprednisolone and met

Iopanoic Acid in Severe Thyrotoxicosis: Rediscovering an Effective Therapy.

BACKGROUND: Severe thyrotoxicosis remains a high-risk endocrine emergency, particularly in thyroid storm and amiodarone-induced thyrotoxicosis (AIT), where morbidity and mortality remain substantial despite modern intensive care. Rapid reduction of circulating triiodothyronine (T3) is central to stabilization, yet conventional therapies are often slow, unpredictable, or ineffective, particularly in iodine-replete states such as AIT. SUMMARY: We reviewed mechanistic, pharmacokinetic and clinical evidence for iopanoic acid, a potent inhibitor of type I and type II 5'-deiodinase that produces ra

Hashimoto Thyroiditis Modifies the Association Between Nuclear Atypia and Malignancy in Bethesda III Thyroid Nodules.

Background: Nuclear atypia is associated with an increased risk of malignancy in Bethesda III thyroid nodules; however, reactive nuclear changes related to Hashimoto thyroiditis (HT) may complicate its interpretation. We investigated whether HT modifies the association between nuclear atypia and malignancy in Bethesda III nodules. Methods: This retrospective single-center study included 426 surgically selected patients with Bethesda III nodules who underwent thyroidectomy between January 2015 and August 2026. HT was defined by postoperative histopathology. The association between nuclear atypi

Preoperative Systemic Inflammatory Indices as Adjunctive Biomarkers for Malignancy Prediction in Bethesda III (AUS) Thyroid Nodules.

Background/Objectives: Atypia of undetermined significance (AUS; Bethesda category III) remains a challenging thyroid cytological diagnosis because of its heterogeneous malignancy risk and limited preoperative certainty. Molecular testing can improve risk stratification but is not universally available. This study evaluated the diagnostic and incremental predictive value of preoperative complete blood count-derived systemic inflammatory indices in surgically treated Bethesda III thyroid nodules. Methods: This retrospective cohort included 352 patients who underwent thyroidectomy for Bethesda I

Artificial Intelligence for Thyroid Nodule Diagnosis: From Multimodal Integration to Clinical Utility.

Artificial intelligence (AI) for thyroid nodules should be evaluated against the decisions it changes, not diagnostic accuracy alone. This critical narrative review examines ultrasound, cytology, histology, and molecular models and distinguishes modality-specific prediction from implemented multimodal learning. The original PubMed/MEDLINE search through 10 August 2026 was supplemented on 25 September 2026 by targeted OpenAlex searches and checks of primary publications, reporting standards, and regulatory sources. Selected studies are compared by sample size, reference standard, acquisition se

Pathological characteristics rather than thyroid size predict incidental parathyroidectomy during thyroid surgery.

BACKGROUND: Incidental parathyroidectomy (IP) is a recognized event during thyroid surgery and may contribute to postoperative hypocalcemia and impaired parathyroid function. Although several clinical and operative factors associated with IP have been described, the contribution of thyroid size-related characteristics remains uncertain. This study aimed to evaluate the association between thyroid size-related parameters, clinicopathological characteristics, and the occurrence of IP. METHODS: This retrospective cohort study included consecutive patients who underwent thyroidectomy between Janu
