Acute suppurative thyroiditis is rarely seen during childhood. The classic clinical features of this illness (fever, neck pain, and a swollen, tender mass over the thyroid gland) can differentiate acute thyroiditis from the more common subacute thyroiditis. Twenty-four cases of anaplastic thyroid carcinoma seen in the National Taiwan University Hospital from 1980 to 1988 were reviewed. The median age was 61.5 years. The ratio of men to women was 1:1.4. A preceding history of long-standing goiter could be obtained in 14 patients (58.3%) with a median duration of 20 years. A case of suppurative thyroiditis is reported. The initial course was insidious and mimicked De Quervain subacute thyroiditis. The abscess was surgically drained. It contained numerous Eikenella corrodens bacilli. Fibroscopy of pharyngo-laryngeal region showed that the left pyriform sinus was abnormal. Surgical removal of a fistula of the fourth branchial pouch was performed in order to prevent recurrence of the thyroiditis.

Exposure of the thyroid gland to ionizing radiation has been associated with a variety of abnormalities. Among these are tardive hypothyroidism and an increased risk of developing thyroid nodules and cancer. Although acute thyroiditis has been known to complicate radioactive iodine 131 therapy, it has rarely been associated with external beam irradiation. The authors report a case of suppurative thyroiditis revealing cancer of the piriform sinus. The course ran two distinct phases. Initially, symptoms resolved under methylprednisolone and ampicillin given with a diagnosis of acute thyroiditis. After corticosteroids were discontinued, a tumefaction in the thyroid area recurred, with clinical features indicating centesis that removed 30 cc of pus ORL examination connected this suppurative thyroiditis to an epithelioma of a piriform sinus.

Infection is extremely rare in the pathology of the thyroid gland. One case is reported, and a review of the literature has helped to emphasize some specific points. Acute thyroiditis can be misdiagnosed in its initial phase because a fever by itself may not lead to a through examination of the neck, even if a goitre is present; the latter being the site of infection in the case reported. A viral aetiology for sub-acute (de Quervain's) thyroiditis has long been postulated although proof has been elusive. Infectious mononucleosis has been associated with sub-acute thyroiditis in only two papers, and histological evidence of sub-acute thyroiditis was lacking in both. A case of sub-acute thyroiditis, confirmed by histology, occurring in a patient with infectious mononucleosis is now reported.

A young woman developed the characteristic clinical and fluorescein angiographic findings of acute posterior multifocal placoid retinal pigment epitheliopathy while being treated for acute thyroiditis. Widespread involvement of the cornea, uveal tract, as well as the optic nerve and retina indicate a generalized ocular inflammation in the clinical course of this patient. The relationship between the subacute thyroiditis and acute posterior multifocal placoid retinal pigment epitheliopathy may indicate an underlying viral cause of both disorders.

Because of increasing concern over continuing medical and potential nonmedical exposure of the thyroid to radiation, risk estimates have been developed for acute thyroiditis, hypothyroidism, and both benign and malignant thyroid nodules following exposure of the human thyroid to external and internal sources of ionizing radiation.
