Increased Risk of Thyrotoxicosis After COVID-19 in Adolescents and Young Adults During the Omicron-Predominant Period: A Nationwide Matched Study.

OBJECTIVE: Thyroid dysfunction has been increasingly reported after the coronavirus disease 2019 (COVID-19) pandemic. To date, no population-based study has examined this association using more than 10 million matched pairs. We compared the risk of documented thyroid outcomes after COVID-19 with that after influenza during the Omicron-predominant period. METHODS: We conducted a nationwide retrospective cohort study using the Japanese National Database of Health Insurance Claims, including patients with COVID-19 between January 2022 and March 2023, and those with influenza between September 20

Sweet syndrome and thyroid diseases: a case-based review of a possible association.

Sweet syndrome, also known as acute febrile neutrophilic dermatosis, is a rare dermatological condition characterized by the abrupt onset of tender, erythematous skin lesions, often presenting with fever and systemic inflammation. It is commonly associated with infection, malignancy, and medication exposure. Isolated case reports have described the co-occurrence of Sweet syndrome and thyroid disease, but these reports have not previously been collected or appraised together. A case-based narrative review was conducted using published case reports and case series describing patients with both c

Case Report: Intracranial hypertension occurring in association with subacute thyroiditis.

Subacute thyroiditis (SAT) is a self-limited inflammatory thyroid disorder that typically produces a transient thyrotoxic phase. Intracranial hypertension (IH) is a rare but potentially sight-threatening complication of thyrotoxicosis, reported almost exclusively in hyperthyroidism, with only a single previous report linking it to SAT. We report the case of a 29-year-old woman who developed IH during the thyrotoxic phase of subacute thyroiditis, temporally related to dental manipulation. She presented with headache, nausea, vomiting, and fever, followed by a syncopal episode; clinical, laborat

A Multisystem Presentation of West Nile Neuroinvasive Disease: Lessons in Diagnostic Complexity.

West Nile virus (WNV) is an uncommon but serious case of encephalitis and meningitis, often presenting with nonspecific symptoms that can mimic a wide range of infectious, metabolic, and endocrine disorders. We present the case of a 61-year-old male with multiple comorbidities who arrived with 5 days of fever, chills, profuse diarrhea, black stools, and worsening confusion. Initial evaluation in the emergency department revealed hyponatremia, transaminitis, and concern for tick-borne illness, prompting empiric doxycycline. However, the patient denied any tick bites. Given his known thyroid nod

Remission of Persistent Hypothyroidism Following Subacute Thyroiditis After Discontinuation of Thyroxine: A 9-Year Retrospective Study.

OBJECTIVE: The long-term outcomes of discontinuing thyroxine replacement therapy in patients with persistent hypothyroidism following subacute thyroiditis are unknown. This study involved an extended follow-up of a cohort of patients who participated in a clinical trial of prednisone for the treatment of subacute thyroiditis. METHODS: This retrospective cohort study included 52 patients with moderate to severe scores who were hospitalized between August 2013 and December 2014. Patients previously received prednisone for 1 week, followed by nonsteroidal anti-inflammatory drugs for 1 week, and

Atypical Subacute Thyroiditis without Thyroid Dysfunction: A Report of Two Cases.

Subacute thyroiditis (SAT) is a painful inflammation of the thyroid that causes transient thyrotoxicosis, followed by hypothyroidism and an eventual recovery. We report two atypical cases of SAT that presented with anterior neck pain, tender thyroid nodules, elevated inflammatory markers, and characteristic ultrasound findings, all suggestive of SAT, but who remained euthyroid throughout their courses. The serum thyroglobulin level increased in one case. These cases demonstrate rare euthyroid presentations of SAT despite not meeting the current diagnostic criteria for SAT established by the Ja

Subacute Thyroiditis Without Neck Pain: An Unusual Cause of Persistent Fever in the Elderly Patient.

Subacute thyroiditis can present as fever of unknown origin, particularly in elderly patients. Absence of neck pain or thyroid tenderness does not exclude the diagnosis. Thyroid function tests and inflammatory markers are key to diagnosis. Corticosteroids lead to rapid symptomatic improvement in moderate-to-severe cases. A transient hypothyroid phase is common and often resolves without long-term thyroxine therapy.

High-dose intravenous methylprednisolone is effective in subacute thyroiditis unresponsive to oral steroids.

OBJECTIVE: Subacute thyroiditis (SAT) relapsing or refractory to standard of care oral prednisone (OP) therapy is rare, but affected patients experience significant discomfort due to prolonged and uncontrolled neck pain. Here, we report the efficacy of high-dose intravenous methylprednisolone (IVMP) therapy in the treatment of such cases. METHODS: In this pilot study, we included 14 patients diagnosed with SAT in the period 2014-2024, in a single center. Therapy consisted of 500 mg of IVMP, divided into two weekly infusions and titrated down based on clinical response. Thyroid function tests,

Long-Term Thyroid Complications Post-COVID-19: A Systematic Review.

Coronavirus disease 2019 (COVID-19) is increasingly shown to be a multisystem disorder with long-term complications, including endocrine system complications. The thyroid gland is also susceptible, as it contains ACE2 receptors, making it exposed to both direct viral damage and autoimmune-mediated dysfunction. Recent reports document the various thyroid complications that persist well after the acute infection phase. This systematic review investigates the long-term thyroid complications in individuals with a history of SARS-CoV-2 infection. A comprehensive literature search across several dat

Preemptive EBV-specific T-cell therapy: An emerging prophylactic approach to PTLD reduction and survival benefit in SOT recipients.

Hemithyroiditis is a rare form of thyroid inflammation characterized by involvement of only one lobe of thyroid gland. The exact pathophysiological mechanism remains unclear; however, it is thought to represent a localized manifestation of chronic thyroiditis that may be triggered by upper respiratory tract infections, trauma and radiation exposure. It has a preponderance for middle aged women. Normally, most thyroiditis conditions (like Hashimoto's or subacute thyroiditis) involve both lobes, but occasionally inflammation is confined to one side. We report a case of hemithyroiditis identified

Value of the Inflammatory Burden Index in the Diagnosis and Prognosis of Subacute Thyroiditis.

BACKGROUND: This study sought to assess the utility of the inflammatory burden index (IBI) in differentiating subacute thyroiditis (SAT) from Graves' disease (GD) and to investigate its association with recovery time, hepatic function impairment, recurrence, and permanent hypothyroidism in patients with SAT. METHODS: Clinical and laboratory data from 357 adult patients with SAT, 412 patients with GD, and 633 healthy controls were retrospectively analyzed. Determinants influencing recovery time, hepatic function impairment, recurrence, and permanent hypothyroidism in patients with SAT were sys

Predictive factors of permanent hypothyroidism after subacute thyroiditis: a systematic review and meta-analysis.

BACKGROUND: Subacute thyroiditis (SAT) is a self-limiting thyroid inflammatory disorder, but some patients develop permanent hypothyroidism, impacting long-term health. We aimed to systematically synthesize evidence on factors associated with permanent hypothyroidism after SAT. METHODS: A systematic search of PubMed, Embase, the Cochrane Library and Web of Science was conducted up to August 2025. Randomized trials, cohort, and case-control studies evaluating predictors of permanent hypothyroidism after SAT were included. Data were extracted following PRISMA guidelines, and study quality was a

COVID-19 pandemic and thyroid diseases.

BACKGROUND: The COVID-19 pandemic has been associated with various autoimmune manifestations. Several studies have suggested a potential association between COVID-19 and thyroid diseases (TDs); however, findings remain inconclusive and are primarily based on relatively small studies. Population-level data examining the differential impact of the pandemic on specific thyroid conditions are scarce. OBJECTIVE: To examine the incidence patterns of Hashimoto's Thyroiditis (HT), Graves' Disease (GD), and Subacute Thyroiditis (SAT) during the COVID-19 pandemic compared to the pre-pandemic period. M

Pyrexia of unknown origin due to subacute thyroiditis.

Subacute thyroiditis is an inflammatory thyroid disorder that typically presents with anterior neck pain, thyroid tenderness and transient thyrotoxicosis. Presentation as pyrexia of unknown origin (PUO) without classical thyroid symptoms is uncommon and may lead to diagnostic delay. We report a case of a patient in their late 50s who presented with persistent evening febrile spikes, weight loss and markedly elevated inflammatory markers, fulfilling the criteria for PUO. Extensive evaluation for infectious, malignant and autoimmune causes was unrevealing. Careful repeat clinical examination reve
