Diagnostic accuracy of thyroid/background ratio in distinguishing Graves' disease from subacute thyroiditis: A comparative study.

BACKGROUND AND OBJECTIVE: Thyrotoxicosis is a common clinical condition in endocrinology, with Graves' disease (GD) and subacute thyroiditis (SAT) as its predominant causes. These disorders often share overlapping clinical and biochemical features, making differential diagnosis challenging. This study evaluates the diagnostic efficacy of the thyroid/background ratio (TBR) as a semiquantitative method for differentiating GD from SAT and compares multiple diagnostic parameters. MATERIALS AND METHODS: This was a retrospective analysis of 106 consecutive patients newly diagnosed with thyrotoxicos

Qualitative and quantitative CEUS assessment for differential diagnosis in thyrotoxicosis: destruction vs. hyperfunction.

PURPOSE: Thyrotoxicosis results from either thyroid hyperfunction, as seen in Graves' disease, or tissue destruction, as in subacute thyroiditis (SAT). Differential diagnosis is essential but can be challenging, with thyroid scintigraphy traditionally used to distinguish between these types. This study investigates the potential of Contrast-Enhanced Ultrasound (CEUS) to provide a non-invasive alternative for differentiating hyperfunction and destructive thyroid disease. METHODS: In a prospective study, 42 patients with untreated thyrotoxicosis were evaluated using CEUS to analyze both qualita

Thyrotoxicosis: Unraveling the Mystery of Fever.

Subacute thyroiditis (SAT) is a transient inflammatory thyroid disorder, typically following an upper respiratory tract viral infection, and can rarely present as fever of unknown origin (FUO). Considering endocrine diseases in the differential diagnosis of fever is essential to prevent delayed diagnosis and unnecessary investigations. We report a case of a 65-year-old male who presented with a prolonged fever of two to three weeks, unresponsive to antibiotics, starting with mild upper respiratory symptoms. Extensive infectious and other workups were negative. Thyroid function tests revealed s

Subacute thyroiditis in pregnancy: a narrative review.

Thyroid dysfunction can adversely affect pregnancy outcomes. Apart from gestational thyrotoxicosis, thyroid dysfunction during pregnancy shares similar etiologies with the non-gravid state. Graves' disease is the most common cause of spontaneous hyperthyroidism in pregnancy, followed by thyroid autonomy. Although subacute thyroiditis is a less common cause of thyrotoxicosis in pregnancy, its associated pain, systemic symptoms, and thyroid dysfunction can present diagnostic and therapeutic challenges. In its painful form, subacute thyroiditis may lead to severe disability, with systemic glucoco

RASGEF1C methylation for the distinguishment and classification of benign and malignant thyroid tumors.

BACKGROUND: The incidence of thyroid cancer (TC) has significantly increased, highlighting the need for effective and objective approaches for the early diagnosis of TC. This study aimed to explore RASGEF1C methylation as a biomarker for papillary thyroid cancer (PTC). METHODS: Formalin-fixed paraffin-embedded samples from a total of 363 PTC and 409 benign thyroid nodules from multiple centers were analyzed. RASGEF1C methylation profiles were examined via MALDI-TOFF mass spectrometry. Statistical analysis was performed via logistic regression adjusted for covariates, nonparametric tests, and

[Acute suppurative thyroiditis: A case report].

Acute suppurative thyroiditis (AST) is an extremely rare and potentially life-threatening condition, which can at first be difficult to differentiate from the far more common subacute thyroiditis (SAT). AST is most often described in patients with immunosuppression, or in patients with a pre-existing thyroid disease or anatomical structural defect, such as a pyriform sinus fistula. Here, we describe a case where a 37-year-old immunocompromised woman presented with intermittent fever, fatigue, erythema and tenderness of the anterior neck. TSH was suppressed and free T3 and T4 were elevated. She

99mTc-pertechnetate cut-off to distinguish Graves' disease from subacute thyroiditis.

BACKGROUND: The cause of thyrotoxicosis can be determined through the thyroid uptake of 99mTc-pertechnetate. The goal of the study was to calculate the cut-off value of 99mTc-pertechnetate of the thyroid gland for the differential diagnosis of Graves' disease from subacute thyroiditis. METHODS: A total of 60 patients were included in the study. Based on thyroid scan findings, 30 patients were in the Graves group and 30 of them had been diagnosed as subacute thyroiditis. Biochemical parameters, thyroid scintigraphy, and the percentage thyroidal 99mTc-pertechnetate uptake results were thoroughl

Prolonged and Recurrent De Quervain's Thyroiditis After Thyroid Fine Needle Aspiration.

De Quervain's thyroiditis (subacute thyroiditis) is a self-limited inflammation of the thyroid gland. It is diagnosed based on clinical history, physical examination, labs, and thyroid imaging. Thyroid fine needle aspiration (FNA) is typically not recommended unless the diagnosis is unclear or there are clinical signs concerning for malignancy. We present the case of a 75-year-old male who presented with symptoms of De Quervain's thyroiditis and an inflammatory thyroid nodule. He had a thyroid FNA for further evaluation of the thyroid nodule. Ultimately, his clinical course was prolonged after

Thyroiditis and human blood metabolites: A mendelian randomization study.

BACKGROUND: The risk factors for thyroiditis, an inflammatory disease with a complex etiology, remain poorly understood. Blood metabolites are known to change during thyroiditis development, suggesting a close relationship between blood metabolites and thyroiditis progression. However, evidence for a causal link is lacking. We employed Mendelian randomization (MR) methodology to systematically investigate the putative causal relationships between blood metabolite profiles and two clinically distinct thyroiditis phenotypes-subacute and autoimmune thyroiditis-providing insights into their metabo

Case Report: Pediatric acute suppurative thyroiditis caused by Eikenella corrodens: a rare presentation revealing pyriform sinus fistula.

Acute suppurative thyroiditis (AST) is a rare, potentially life-threatening bacterial infection of the thyroid gland, characterized by rapid progression. We report a rare pediatric case of AST caused by Eikenella corrodens, secondary to a pyriform sinus fistula (PSF), initially misdiagnosed as subacute thyroiditis (SAT). A retrospective analysis highlights the diagnostic challenges and emphasizes the need for early suspicion of PSF in recurrent AST. Timely imaging (CT/laryngoscopy) and pathogen-directed antibiotics are critical to prevent life-threatening complications. This case underscores t

IgG4-Mediated Sclerosing Riedel Thyroiditis: A Multidisciplinary Case Study and Literature Review.

Riedel thyroiditis (RT) is a rare immune-mediated inflammatory disease that destroys the thyroid parenchyma, replacing it with storiform fibrosis extending to the extrathyroidal tissue. Secondary fibrotic lesions can be associated as parts of the systemic IgG4-related disease. We present the case of a 52-year-old female patient who presented initially with subacute thyroiditis when corticosteroid treatment was initiated. After a year, compressive respiratory symptoms and dysphagia appear, and fine-needle aspiration cytology is performed to rule out malignancy, but without results. Thyroidectom

Unilateral pharyngalgia as the dominant symptom in subacute thyroiditis: Case series and analysis of diagnostic pitfalls in clinical practice.

Subacute thyroiditis is a condition with varied and often misleading symptoms, which can lead to diagnostic delays and inappropriate treatment. This case series focuses on 32 patients who presented with unilateral pharyngalgia as the primary symptom and were ultimately diagnosed with subacute thyroiditis. The patients, aged 28-52 years with a female predominance (23 females and nine males), reported pharyngeal pain that lasted from 7 days to 3 months, with 19 having left-sided and 13 having right-sided initial pain. Of these patients, 43.7% (14/32) were initially misdiagnosed, primarily as pha

Case Report: SARS-CoV-2-associated immune dysfunction manifesting as concurrent fulminant type 1 diabetes mellitus and subacute thyroiditis.

OBJECTIVES: The association between SARS-CoV-2 infection and endocrine emergencies (such as fulminant type 1 diabetes mellitus and subacute thyroiditis) has received increasing attention. However, concurrent manifestations of these two conditions within a short period of time after infection are exceedingly rare, and the underlying mechanisms and clinical management strategies remain unclear. CASE PRESENTATION: A 45-year-old Chinese man developed sudden polydipsia, polyuria, and cervical pain on day 7, within 2 weeks of SARS-CoV-2 infection. The diagnosis of fulminant type 1 diabetes mellitus

Concurrent Thyroiditis and Hypophysitis in a Patient Receiving Dual Immune Checkpoint Inhibitor Therapy for Metastatic Melanoma.

Immune checkpoint inhibitors (ICIs), particularly when used in combination regimens, are associated with a broad range of immune-related adverse events (irAEs), including endocrinopathies. We present a case of a 53-year-old male patient with metastatic melanoma treated with a combination of ipilimumab and nivolumab who developed concurrent thyroiditis and hypophysitis. Although this co-occurrence is uncommon, it is clinically significant. A key learning point from this case is the importance of accurate interpretation of laboratory findings. Our patient was transitioning from the hyperthyroid

Recurrent acute suppurative thyroiditis in pediatrics: a case report.

Acute suppurative thyroiditis (AST) is a rare endocrinological emergency in pediatrics that may require immediate infectious disease and surgical intervention. It is generally associated with bacterial infections and, in some cases, congenital malformations of the branchial arch. We present the case of a 7-year-old girl with recurrent AST, in both episodes with cervical abscess formation, requiring intravenous antibiotics and surgical drainage. From the outset, a piriform sinus fistula was suspected, which was confirmed by direct laryngoscopy in the operating room during the hospitalization fo

Right thyroid hemiagenesis presenting with a contralateral simple diffuse goiter: case report.

Thyroid hemiagenesis (THA) is a rare congenital disorder in which one thyroid lobe fails to develop. A variety of pathological conditions can occur in the remaining thyroid tissue in association with this rare anomaly such as subacute thyroiditis, colloid nodules, Graves' disease, simple goiter, Hashimoto thyroiditis, adenoma and thyroid carcinoma. The association of thyroid hemiagenesis with simple diffuse goiter is rare, and very few cases have been reported in the literature. A 30-year-old female patient presented at the thyroid clinic because of anterior neck swelling for approximately 2 y
