The Role of Vitamin D in Autoimmune Thyroid Diseases: From Immunomodulation to Clinical Implications.

Vitamin D is involved in immune regulation through effects on innate and adaptive immune responses mediated by vitamin D receptor activation within immune cells. Experimental and translational studies support its role in promoting regulatory T-cell activity, modulating Th1/Th17 responses, and influencing autoantibody production. At the population level, low serum 25-hydroxyvitamin D concentrations are consistently associated with an increased risk of autoimmune diseases, including autoimmune thyroid disorders such as Hashimoto's thyroiditis (HT) and Graves' disease (GD), suggesting a potential

DYRK1A Expression and Thyroid Dysfunction in Subjects With Down Syndrome.

OBJECTIVE: Thyroid dysfunction is common in Down Syndrome (DS). The age of onset and etiology is largely unexplored. Murine models suggest role of DYRK1A gene in thyroid dysgenesis in DS. DYRK1A overexpression is associated with immune dysfunction and autoimmune diseases in DS. In this study, we aimed to determine the expression of the DYRK1A gene in relation to age of onset and autoimmune status in individuals with DS who have hypothyroidism. METHODS: This cross-sectional study was conducted in a tertiary care setting and included five groups: individuals with DS and hypothyroidism, DS with

Hashimoto's encephalopathy presenting with manic and psychotic features in a 13-year-old girl: a case report.

Hashimoto's encephalopathy (HE), also known as steroid-responsive encephalopathy associated with autoimmune thyroiditis (SREAT), is a rare autoimmune neuropsychiatric syndrome with highly variable clinical manifestations. Pediatric cases are particularly underrecognized, contributing to diagnostic delays. We describe a 13-year-old female patient who developed subacute behavioral changes, including withdrawal, emotional lability, and progressive manic and psychotic symptoms, over 1 month. Laboratory evaluation showed markedly elevated antithyroid antibodies with normal thyroid hormone levels, a

Associated Autoimmunity in Myasthenia Gravis in Denmark: A Nationwide Case-Control Study.

BACKGROUND: Myasthenia gravis (MG) is an autoantibody-mediated neuromuscular disease often co-occurring with other autoimmune diseases (ADs). We aimed to determine the temporal relationship between other ADs and MG. METHODS: We conducted a nationwide, population-based case-control study of MG patients and 10 age-, sex-, and index date-matched controls from 1985 to 2020. We used conditional logistic regression to estimate odds ratios (ORs) for AD diagnoses preceding MG, and Cox regression to calculate hazard ratios (HRs) for ADs succeeding MG. Analyses were stratified by sex, age group (≤ 50 a

Administration of Hormone Replacement Therapy Containing Estrogen and Development of Autoimmune Associated Vocal Fold Lesions in Women.

BACKGROUND: Autoimmune associated vocal fold lesions (AaVFL) are a result of underlying autoimmune conditions that include laryngeal rheumatoid nodules, bamboo nodules, and other nonspecific disease lesions. Women being more prone to autoimmune disease are therefore more likely to develop these vocal fold lesions. This relationship showed a greater association in women in their 4th and 5th decades when treating these lesions. This greater association is theorized to be due to introduction of hormone replacement therapy (HRT) containing estrogen in their 4th and 5th decades of life. The proinfl

Case Report: IL2RA (CD25) deficiency: first reported cases in Morocco.

CD25, the α-chain of the interleukin-2 receptor (IL2RA), is a key component of the IL-2 pathway and is essential for the development and stability of regulatory T cells. Loss-of-function variants in IL2RA cause a very rare autosomal recessive disorder marked by early-onset autoimmunity and recurrent infections with an IPEX-like presentation. We report the first two molecularly confirmed cases of IL2RA (CD25) deficiency in Morocco, each carrying a distinct homozygous mutation. Both patients were born to first-cousin parents and presented in early childhood with recurrent respiratory and gastroi

Hashimoto's thyroiditis: from pathogenesis to clinical management.

Hashimoto's thyroiditis (HT) is a chronic autoimmune thyroiditis characterized by thyroid-specific autoantibodies (TPOAb, TGAb) positivity and lymphocytic infiltration, and is a major cause of hypothyroidism in iodine-sufficient regions. Epidemiological data show a significant increase in the prevalence of HT, which is about four times more common in adult women than in men. The pathogenesis of HT involves a complex interaction of genetic susceptibility, environmental factors, and immune regulation. Its clinical diagnosis is mainly based on serological tests (TPOAb/TGAb) and thyroid ultrasound

Hypoxia-Inducible Factor 1-α in Autoimmune Diseases-Insights from the Paradigm of Hashimoto's Thyroiditis: A Narrative Review.

Background/Objectives: Given the rising prevalence of autoimmune diseases and the need for new insights into the pathology of these disorders, it is important to summarize current knowledge, with an emphasis on Hashimoto's thyroiditis (HT), since it is especially on the rise. Hypoxia is part of various pathophysiological conditions, and hypoxia-inducible factor (HIF) is a key factor in these processes. Hypoxia is involved in the regulation of hormones and the development of endocrine disorders. With this in mind, this narrative review summarizes the current state of knowledge on the relationsh

Inflammatory Bowel Diseases Are Not Associated with an Increased Risk of Autoimmune Thyroiditis.

Background/Objectives: The incidence of inflammatory bowel disease (IBD) is rising worldwide, particularly in Asia, while the highest prevalence remains in North America and Europe. Evidence on the relationship between IBD and the development of autoimmune thyroiditis is limited. This study investigated the association between IBD and a subsequent autoimmune thyroiditis in a large German primary care cohort over a 10-year period. Methods: Patients with IBD were propensity score matched to non-IBD individuals in a 1:5 ratio based on age, sex, index year, and average annual number of physician v
