Thyroid peroxidase gene variants and susceptibility to congenital hypothyroidism and autoimmune thyroid disease among Egyptian pediatric cohort.

BACKGROUND: Pediatric thyroid disorders arise from complex interactions among genetic susceptibility, immune dysregulation, and metabolic factors that influence their onset and severity. PURPOSE: This study aimed to evaluate the clinical, biochemical, micronutrient, and genetic determinants of thyroid dysfunction in children with particular emphasis on the thyroid peroxidase (TPO) Arg386His and Thr725Pro polymorphisms. METHODS: This case-control study enrolled 150 Egyptian children aged 2-15 years, including 50 with congenital hypothyroidism (CH), 50 with autoimmune thyroiditis (AIT), and 50

Primary adrenal insufficiency and autoimmune thyroiditis in an adolescent with Castleman-TAFRO syndrome.

The idiopathic multicentric Castleman disease (CD)-TAFRO variant (iMCD-TAFRO) is a rare, clinically aggressive lymphoproliferative disorder characterized by thrombocytopenia, anasarca, fever, reticulin fibrosis/renal failure, and organomegaly. Associated with elevated inflammatory cytokines, iMCD-TAFRO can present severe systemic complications. Here, we report a previously healthy adolescent male presenting with severe systemic inflammation, diffuse lymphadenopathy, anasarca, renal impairment, and hepatosplenomegaly. His clinical course was complicated by suspected bilateral adrenal infarction

Botulinum toxin injection induced autoimmune thyroiditis and myxedema coma: A case report.

This case describes a 28-year-old female with amyotrophic lateral sclerosis and recurrent urinary tract infections who developed myxedema coma following a botulinum toxin type-A (Btx) injection for muscle spasticity. On intensive care unit admission, she presented with hypothermia, bradycardia, and reduced consciousness. Laboratory evaluation revealed markedly elevated thyroid-stimulating hormone levels, confirming myxedema coma. Prompt treatment with intravenous levothyroxine and hydrocortisone resulted in rapid improvement in temperature and mental status. Further investigation showed elevat

Risk of Safety Events in Vitiligo Patients: A Retrospective Real-World Data Study in the US.

Vitiligo is a chronic autoimmune depigmenting disease characterized by loss of pigment in the skin, hair, or both. As treatment options evolve, particularly with the emergence of oral Janus kinase inhibitors and dual Janus kinase 3/tyrosine kinase expressed in hepatocellular carcinoma family kinase inhibitor, it is essential to assess population-level safety events in patients with vitiligo. This retrospective cohort study examined patients aged ≥ 12 years with vitiligo in the United States, comparing them with age-, sex-, and race-matched controls to evaluate patient demographics, baseline co

PD-1 inhibitor-induced systemic sclerosis with early interstitial lung disease and autoimmune thyroiditis in advanced gastric cancer: a case report and literature review.

Immune checkpoint inhibitors (ICIs) have revolutionized anti-tumor therapy, yet their use is increasingly complicated by immune-related adverse events (irAEs). We present a rare case in which sequential administration of two PD-1 inhibitors (ICIs), Camrelizumab and Sintilimab, in combination with conventional chemotherapy led to ICI-related systemic sclerosis (ICI-SSc) with early-stage interstitial lung disease (ILD) and autoimmune thyroiditis (AIT). The clinical presentation also suggested a possible scleroderma overlap syndrome. The underlying pathology is likely linked to widespread immune

Knuckle, Knuckle, Dimple, Dimple: Do Not Miss A Diagnostic Opportunity.

Turner syndrome (TS) often presents with subtle or overlooked clinical signs, contributing to frequent diagnostic delays. We describe the case of a 13-year-old girl referred for short stature whose only remarkable physical findings were the absence of the fourth and fifth knuckles on both hands-known as Archibald's metacarpal sign-and a short fourth metatarsal. Growth had decelerated after spontaneous menarche, and previous examinations had not recognized these skeletal clues. Radiographs confirmed short fourth and fifth metacarpals. Karyotype analysis identified a complete deletion of the sho

Photobiomodulation Therapy in Chronic Autoimmune Thyroiditis: A Systematic Review of Molecular Mechanisms and Clinical Applications.

Chronic autoimmune thyroiditis (CAT), a common autoimmune thyroid disorder, is the leading cause of hypothyroidism in iodine-sufficient regions and is characterized by thyroid autoimmunity, chronic inflammation, and progressive structural thyroid changes. Although levothyroxine (LT4) restores biochemical euthyroidism, it does not directly address the underlying autoimmune process, highlighting the need for adjunctive therapeutic strategies. Photobiomodulation (PBM), also known as low-level laser therapy (LLLT), has been proposed as a non-invasive intervention with potential immunomodulatory an

Association between autoimmune thyroiditis and rheumatoid arthritis: a cross-sectional risk stratification study.

BACKGROUND: Autoimmune thyroiditis (AIT) frequently coexists with other autoimmune diseases, suggesting shared pathophysiological pathways. However, large-scale evidence supporting the relationship between AIT and rheumatoid arthritis (RA), as well as the potential metabolic and endocrine mediators linking the two, remains limited. METHODS: A cross-sectional analysis was conducted using nationally representative data from NHANES 2007-2012 (n = 5,715). Multivariable logistic regression models were used to evaluate associations between AIT and RA. Mediation analyses assessed the potential indir

Withania somnifera Root Extract Ameliorates PTU-Induced Hypothyroidism by Regulating Hormone Levels and Gene Expression in Rats.

Hypothyroidism is a prevalent endocrine disorder characterized by the inadequate production of thyroid hormones (T3 and T4), leading to various physiological dysfunctions. Current treatments often involve synthetic hormone replacement, which is limited by some adverse effects. This study investigated the therapeutic efficacy of a methanolic extract of Withania somnifera root (MEWS) in propylthiouracil (PTU)-induced hypothyroid rats. Forty rats were divided into five groups (n = 8): (1) negative control (NC), receiving no treatment; (2) positive control (PC), administered 0.05% w/v PTU to induc

Association Between Autoimmune Thyroiditis and Cervical Artery Dissection: A Retrospective Cohort Study.

BACKGROUND AND AIMS: Limited evidence suggests autoimmune thyroiditis (AT) could represent a risk factor for cervical artery dissection (CeAD). We tested the hypothesis of a positive association between AT and CeAD within 3 years following diagnosis compared to matched euthyroid controls. METHODS: We searched a de-identified United States' data resource (TriNetX LLC) for adults age ≥ 18 with newly-diagnosed AT, and euthyroid controls, from 2014 to 2024. Propensity matching controlled for variables associated with CeAD. Our primary outcome was the risk ratio (RR) for CeAD within 3 years' follo

Claustrum Sign in Hashimoto Encephalopathy Presenting With New Onset Refractory Status Epilepticus (NORSE): First Report of a Rare Phenotype.

The presence of the bilateral claustrum sign in new-onset refractory status epilepticus (NORSE) should prompt consideration of Hashimoto's encephalopathy, even without thyroid dysfunction. Early recognition and corticosteroid therapy can be lifesaving, emphasizing an autoimmune-inflammatory mechanism underlying this rare presentation.

Evaluation of thyroglobulin autoantibodies in dogs at the time of diagnosis of hypoadrenocorticism and during treatment.

BACKGROUND: Autoimmune thyroiditis (AIT) may occur more frequently in dogs with hypoadrenocorticism (HA) than previously recognised. The objective of this study was to determine the presence of thyroglobulin autoantibodies (TgAAs) in dogs with HA. METHODS: Dogs were included if HA was confirmed by an adrenocorticotropic hormone stimulation test conducted between 2019 and 2023. Serum total thyroxine, thyrotropin and TgAA were analysed from samples taken before and during HA treatment. RESULTS: Thirty-seven dogs were enrolled, with nine dogs showing positive TgAA titres at enrolment. Following

A systematic review, meta-analysis, and trial sequential analysis of the effect of acupuncture and moxibustion cake on thyroid function in patients with Hashimoto thyroiditis.

BACKGROUND: Hashimoto thyroiditis (HT) is a prevalent autoimmune thyroid disorder that currently lacks a specific treatment. While oral levothyroxine sodium tablets have been found to significantly enhance thyroid function, they do not contribute to a decrease in concentrations of thyroid-related antibodies. We aimed to design a systematic review, meta-analysis, and trial sequential analysis of the effect of acupuncture and moxibustion cake on thyroid function in patients with HT. METHODS: We conducted an unrestricted search up until March 29, 2024, across 7 databases. These included 4 intern
