Levothyroxine adherence in primary hypothyroidism: clinical impact, measurement challenges, and opportunities for personalized care.

Primary hypothyroidism, most commonly caused by Hashimoto's thyroiditis, is a highly prevalent chronic condition typically managed with lifelong levothyroxine therapy. Although treatment is generally straightforward, a substantial proportion of patients do not achieve stable biochemical control or continue to report persistent symptoms. Suboptimal adherence may contribute to these challenges, but should be distinguished from other causes of treatment instability, including administration-related factors, drug and food interactions, and gastrointestinal malabsorption. This narrative review exam

Senotherapeutic potential against xeroderma pigmentosum.

BACKGROUND: Xeroderma pigmentosum (XP) is an inherited photoaging syndrome caused by mutations in genes involved in the nucleotide excision repair (NER) pathway. XP patients exhibit hypersensitivity to ultraviolet (UV) radiation, leading to accelerated skin aging and requiring lifelong sun avoidance. In this study, we aimed to investigate UV-induced cellular senescence in XP melanocytes and explore potential senotherapeutic strategies. METHODS: Melanocytes differentiated from iPSCs derived from an XP-A patient and a healthy individual, respectively, were used to induce senescence. Senescence

Epigallocatechin gallate suppresses adipogenesis by targeting tetranectin and inhibiting its endocytosis via modulation of ERK and AKT signaling pathways.

BACKGROUND AND AIMS: With the rapid increase in the elderly population, the number of people requiring care is also increasing and the capacity of the family to provide care is decreasing. Because institutionalization costs are high, more research is needed to investigate predictors of preventing institutionalization. The aim of this study was to examine the impact of social engagement and other predictive factors, including disability, household composition, and formal services, on continuity in home care of the elderly. METHODS: The study was retrospective longitudinal in design. Data were

Assessing clinical relevance of off-target effects observed at suprapharmacological concentrations of SGLT2 inhibitors. Reply.

In this study, using single-cell RNA-seq, cell mass spectrometry, flow cytometry, and functional analysis, we characterized the heterogeneity of polymorphonuclear neutrophils (PMNs) in cancer. We describe three populations of PMNs in tumor-bearing mice: classical PMNs, polymorphonuclear myeloid-derived suppressor cells (PMN-MDSCs), and activated PMN-MDSCs with potent immune suppressive activity. In spleens of mice, PMN-MDSCs gradually replaced PMNs during tumor progression. Activated PMN-MDSCs were found only in tumors, where they were present at the very early stages of the disease. These pop

Beyond glycaemic control: autoimmune polyglandular syndrome type 3 as an emerging distinct entity rather than a mere comorbidity in paediatric type 1 diabetes.

AIM: Autoimmune polyglandular syndrome type 3 (APS-3) is the coexistence of autoimmune thyroid disease (AITD) with type 1 diabetes (T1D) or other autoimmune diseases, after exclusion of adrenal insufficiency. We aimed to evaluate the prevalence, timeline and features of APS-3 in children and adolescents with T1D. METHODS: We studied 439 T1D paediatric patients who were diagnosed and followed up over 16 years using annual biochemical and autoantibody profile evaluations. RESULTS: The prevalence of APS-3a (T1D+AITD) was 23.5%, with significant female predominance (54.4%, P = 0.007). The vast m

Early Recurrence of Non-neuroendocrine Primary Cutaneous Mucinous Carcinoma of the Eyelid After Surgical Excision: A Case Report.

Primary cutaneous mucinous carcinoma (PCMC) is a rare malignant cutaneous adnexal neoplasm prone to local recurrence. Neuroendocrine differentiation has been recognized in a subset of these tumors, and emerging evidence suggests that its presence may be associated with differences in clinical behavior and prognosis. Reports characterizing neuroendocrine differentiation in eyelid PCMC remain limited. A 47-year-old man underwent excision of a chalazion-like mass of the right upper eyelid at another clinic, and histopathologic examination revealed mucinous carcinoma. Systemic evaluation found no

Seroprevalence of Celiac Disease in Mexican Mestizo Patients With Autoimmune Thyroid Disease.

GOALS: This study aimed to assess the seroprevalence of celiac disease in Mexican patients with autoimmune thyroid diseases, hypothesizing that prevalence would align with rates observed in other populations. BACKGROUND: The association between celiac disease and autoimmune thyroid diseases has been documented globally, with varying seroprevalence rates. Historically, Mexican Mestizos were considered at low risk for celiac disease, yet recent findings suggest similar prevalence to other regions. However, data regarding Hispanic patients with autoimmune thyroid diseases are limited. STUDY: Th

Role of protein kinase C subtype-specific signalling in regulating adhesion in human keratinocytes and skin in pemphigus.

BACKGROUND: Autoantibodies in the blistering skin disease pemphigus primarily target desmosomal cadherins and cause loss of keratinocyte adhesion and epidermal blistering via signalling events. Pemphigus vulgaris (PV) is associated with autoantibodies (PV IgG) against desmoglein (Dsg)1 and Dsg3 and pemphigus foliaceus (PF) with antibodies against Dsg1 only. In previous studies, protein kinase C (PKC) inhibition was protective in murine but not in human epidermis. OBJECTIVES: To investigate the roles of PKC subtypes in in PV IgG-induced epidermal blistering in human skin. METHODS: We applied

Are infertile men at a higher risk of morbidity and early mortality?

Increasing evidence supports the role of male infertility as a harbinger of broader health issues, given that infertile men often exhibit higher rates of chronic conditions later in life. Infertile men have double the risk of developing testicular and breast cancer and an increased risk of high-grade prostate cancer, are more likely to develop ischemic heart disease, hypertension, heart disease, and hyperlipidemia, and have higher cardiovascular mortality compared to fertile men. The risk of developing Type 2 diabetes seems to increase with the severity of the spermatogenic dysfunction, and me

Prospective monitoring of plasma abemaciclib in breast cancer patients: associations with age and adverse events.

PURPOSE: Plasma abemaciclib concentrations vary among individuals, and their association with hematologic toxicity has been reported in real-world settings. The incidence of interstitial lung disease (ILD), a serious adverse event of abemaciclib, is higher in clinical practice than in clinical trials. Here, to assess the potential usefulness of abemaciclib concentration monitoring, we conducted a prospective exploratory study of the association between plasma abemaciclib concentration, age, and the occurrence of adverse events in breast cancer patients. METHODS: Patients with hormone receptor

Another pleiotropic effect of SGLT2 inhibitors: Is it a new frontier in thyroid function regulation?

In our review, we present possible hypotheses that might explain how sodium glucose cotransporter 2 (SGLT2) inhibitors affect thyroid function. We describe mutual interactions between thyroid hormones and the development of diabetes and obesity. We show the effects of other antihyperglycemic drugs on thyroid hormone changes. We demonstrate how endocrine-disrupting chemicals (EDCs) may act as potential triggers for the development of thyroid disease, obesity and diabetes, and how SGLT2 inhibitors may constitute a potential protective barrier against their negative effects. We describe mechanism

CTLA4, PTNP22, and FOXO3A gene variants as genetic biomarkers for co-occurrence of type 1 diabetes and autoimmune thyroid diseases in the Polish population.

INTRODUCTION: Genetic predisposition is a common determinant of autoimmune endocrinopathies, particularly type 1 diabetes (T1D) and autoimmune thyroid disease (AITD). Although their coexistence is frequent, the underlying genetic background is not fully understood. OBJECTIVES: We aimed to identify genetic polymorphisms predictive of an increased risk for co‑occurring T1D and AITD, and to assess their potential impact on glycemic control. PATIENTS AND METHODS: We genotyped 5 selected polymorphisms (rs3087243 and rs231775 in the CTLA4 gene; rs12730735 in the PTPN22 gene; and rs2802292 and rs94

Repetitive mild traumatic brain injury with the closed-head impact model of engineered rotational acceleration (CHIMERA) promotes tau pathology in tau transgenic mice and its propagation in brains injected with tau fibrils.

Chronic traumatic encephalopathy (CTE) is a progressive neurodegenerative disease characterized by the presence of abnormally phosphorylated tau aggregates. Tau is a microtubule-associated protein expressed mainly in axons of neurons with a role in the regulation of microtubule dynamics and axonal transport. It is totally unknown when and how tau is abnormally hyperphosphorylated in CTE brains. Unlike other tauopathies such as Alzheimer's disease, in which diseases start several decades before clinical symptoms and the time point of onset is not clear, in the case of CTE it is evident when and

Clinical and Pathological Significance of GLUT-1 Expression in Angiosarcoma and Other Vascular Tumors.

Vascular anomalies are classified according to the ISSVA classification into vascular tumors with endothelial proliferation and vascular malformations without proliferation. Infantile hemangioma is a vascular tumor that is diagnosed by GLUT-1 immunoreactivity. GLUT-1 positivity is reportedly sometimes observed in angiosarcoma, but the clinical significance remains unclear. Here, immunohistochemistry was performed on tissue samples from patients with angiosarcoma (n = 10), pyogenic granuloma (n = 9), and senile hemangioma (n = 10), with infantile hemangioma serving as a positive control.
