[Thyroiditis : diagnostic and therapeutic features (excluding Graves disease and myxedema) : 13 case-reports (author's transl)].

The incidence of De Quervain's thyroiditis is probably underestimated, since a radioiodine uptake study is not performed in all cases. An isolated cold nodule can reveal Hashimoto's thyroiditis. In other cases, the diagnosis is suggested by otalgia and positive tests for antimicrosomial antibodies. Surgery is often necessary because of the possible association with cancer ; the procedure should be extensive since spontaneously the outcome is often thyroid insufficiency.

Fine-needle aspiration cytology in thyroid disease.

Fine-needle aspiration biopsy of palpable lesions of the thyroid is a diagnostic method routinely used in several medical centres, but still relatively new in others. Experience has shown that the technical steps involved in this biopsy procedure, that is, palpation, aspiration, smear making, fixation and staining, are critical for good results, and they are therefore reviewed and illustrated. Another prerequisite for the use of aspiration biopsy cytology is a specially interested cytopathologist who is familiar with the methodological aspects, as well as the interpretation of cells in aspirat

Association of pernicious anemia and intrinsic factor antibody with HLA-D.

One-hundred-and-six patients with pernicious anemia were HLA-A, B, C typed by serological technique and HLA-D typed by mixed lymphocyte culture technique for the specificities HLA-Dw1 - 8 and the locally defined D "H". In 13 cases, the D-typing was unsuccessful due to technical difficulties. HLA-A, B, C antigen frequencies did not show any significant deviation from expected values, while D typing showed increased frequencies of Dw2 and Dw5 and a possibly decreased frequency of Dw3. The typing was compared with clinical data such as the presence of organ specific autoimmune disease in first de

Suppressive therapy for postirradiation thyroid nodules.

From the thyroid screening program at the Roswell Park Memorial Institute in Buffalo, New York, 85 patients with thyroid nodules who had received previous irradiation to the head and neck were identified. The diagnosis was confirmed microscopically by needle aspiration cytology and the patients were treated with thyroid replacement therapy. In 24 patients, the nodules disappeared completely; a partial response (more than 50% reduction in size) was seen in 26; 11 patients were improved (20% to 50% reduction in size); 14 had stable disease (less than 20% reduction in nodule size) and in 10 the c

Radiation-associated thyroid carcinoma.

Since February, 1977, 735 patients having a history of receiving radiation therapy for benign conditions of the head and neck areas during infancy and childhood were examined in a thyroid screening program, and 159 patients were found to have palpable thyroid nodules. These patients had thyroid function tests and indirect laryngoscopy and were followed closely on suppression therapy consisting of either Cytomel or thyroid extract. Thyroidectomy was advised in those in whom the nodules persisted or increased in size. This study documents the incidence of carcinoma and other benign pathological

Fc receptor-bearing blood mononuclear cells in thyroid disorders: increased levels in patients with subacute thyroiditis.

Blood mononuclear cells bearing Fc receptors for immunoglobulin G were measured in patients with thyroid disorders as the percentage of EA rosette-forming cells (% EA-RFC). Levels were normal in patients with untreated Graves' hyperthyroidism, Graves' ophthalmopathy, and Hashimoto's thyroiditis. On the other hand, the % EA-RFC was increased in eight of nine patients with subacute thyroiditis (SAT) tested during the acute phase, returning to normal during recovery. Levels were normal in all five patients with "silent" thyroiditis tested. The majority of the Fc receptor-bearing cells in SAT pati

Bleeding disorder due to an isolated platelet factor 3 deficiency.

To determine the clinical and functional characteristics of isolated platelet factor 3 (PF-3) deficiency, platelet function studies were carried out in 316 patients who were suspected of having a bleeding tendency. Although PF-3 abnormality was observed in 69 patients (22%), an isolated PF-3 abnormality was noted in only three patients. The abnormality was congenital in one patient and secondary to Hashimoto's thyroiditis, Ehlers-Danlos syndrome, and chronic myelocytic leukemia in the other two. Further investigations in the patient with congenital abnormality disclosed that PF-3 activity fail

Postpartum lymphocytic thyroiditis in American women: a spectrum of thyroid dysfunction.

Six episodes of primary hypothyroidism followed pregnancy in five women. Within 6 months post partum, all were clinically hypothyroid, with a diffuse goiter of two or three times normal size, altered thyroid function tests (average thyroxine [T4] = 2.6 micrograms/dl, thyroid-stimulating hormone [TSH] = 58.2 microunits/ml), and elevated antithyroid antibodies. (All had antimicrosomal antibody titers of 1:6,400 or more.) After 5 more months, all patients had a decrease in antibody titer, resolution of symptoms, and recovery of thyroid hormone levels (average T4 = 7.3 micrograms/dl), although two

Generalized cytomegalic inclusion disease in neonates and infants.

Cytomegalic inclusion disease (C.M.I.) is caused by the salivary gland virus which is species-specific, and characterized by formation of inclusion-bearing cells in various organs. From the view point of spreading, it is divided into the localized and generalized type. We studied 18 generalized cases of infants in whom inclusion-bearing cells were detected in more than three organs. As to the portal of entry and the time of infection, cytomegalic inclusion disease can be classified into three subgroups in infants; congenital form (2 cases), early acquired form (8 cases) and acquired form with

Antiglycolipid autoantibody detected in the sera from systemic lupus erythematosus patients.

A high incidence of autoantibody against the neutral glycolipid "asialo GM1" was observed in sera from patients with systemic lupus erythematosus (SLE) with neurological disorders, using an immunoflocculation test. The sera from 14 out of 17 cases of SLE with neurological disorders showed antibody activity against asialo GM1 but not against the following glycolipids: asialo GM2 GM1, and galactocerebroside. In another 87 cases of SLE without any history of seizures, as well as 61 cases of other autoimmune diseases (rheumatoid arthritis, progressive systemic sclerosis, mixed connective tissue di

Yellow nail syndrome: complete triad with pleural protein turnover studies.

Lymphedema, pleural effusions, and yellow nails constitute the yellow nail syndrome, which commonly is associated with chronic lower respiratory infections and sinusitis. Of 50 cases reported, only 13 have had the complete triad; we report another. The patient has a history of bronchiectasis, bilateral leg lymphedema, and yellow nails since 1952, with recurrent effusions since 1953. She has other conditions which are reported in high frequency with the syndrome, including sinusitis, thyroiditis, and malignancy. An albumin turnover rate was determined on the pleural effusion by a radioactive tr

Immunological and genetic markers in a family with Hashimoto's disease.

A family with Hashimoto's disease in three generations is described. Seven persons (all female) had Hashimoto's disease was the only thyroid disorder occurring in this family; no other autoimmune disease was observed. Circulating thyroid antibodies were detected in all seven subjects with overt thyroiditis in this family. Thyroid antibodies were also detected in low titres in about half of the healthy relatives. Evidence of thyroid antigen-directed cell-mediated immunity was demonstrated using the leucocyte migration inhibition test in four out of seven subejcts with thyroiditis and also in ab

Ophthalmopathy after neck irradiation therapy for Hodgkin's disease.

Ophthalmopathy associated with autoimmune thyroid disease (Graves' ophthalmopathy) may occur years after irradiation of the neck for nonthyroid malignant disease. Two patients developed this complication 2 and 18 years after irradiation treatment of Hodgkin's disease: one with hypothyroidism associated with Hashimoto's thyroiditis and the other with hyperthyroidism. Careful long-term observation of thyroid function following neck irradiation is recommended in view of this unusual complication and the frequent occurrence of hypothyroidism.

Macrophage-lymphocyte interaction in Graves' disease and Hashimoto's thyroiditis.

The formation of macrophage-lymphocyte rosettes was studied in lymphocyte cultures from patients with Graves' disease; Hashimoto's thyroiditis, other thyroid diseases and control subjects; the cultures were incubated with normal human thyroid and other non-specific antigens. At the end of incubation, the cell pellets were smeared on slides, stained with Wright's stain and the number of rosettes determined under the microscope. The membrane immunofluorescence technique was employed to identify whether the surrounding lymphocytes were T- or B-lymphocytes. In Graves' disease and Hashimoto's thyro

Correlation between thyrotropin-displacing activity and human thyroid-stimulating activity by immunoglobulins from patients with Graves' disease and other thyroid disorders.

Several reports have been published on the anti-TSH receptor antibody in putative autoimmune thyroid disorders using a radioreceptor assay. We have carried out correlative studies between the ability of serum immunoglobulins to displace radiolabeled TSH from the thyroid plasma membrane receptor [TSH-displacing activity (TDA)] and that of actual stimulation of the human thyroid gland [human thyroid-stimulating activity (hTSA)] in Graves' and other thyroid diseases and in control subjects. TDA was assayed by the use of a radioligand technique, while the activation of adenylate cyclase in human t

Uptake of thallium-201 in enlarged thyroid glands: concise communication.

We have investigated the thyroid uptake of Tl-201 in 37 patients with various types of goiter, and in six with normal thyroids. Significant thallium uptake was found in all cases in which there was thyroid enlargement, including Graves' disease, toxic thyroid nodule, primary hypothyroidism, simple goiter, Hashimoto's disease, thyroid carcinoma, and thyroid adenoma. If goiter was absent, however, there was no demonstrable uptake--e.g., in secondary hypothyroidism, subacute thyroiditis, and the normal controls. Thallium uptake did not correlate with thyroid function tests such as BMR, T3-RU, T3,

Hypothyroidism as a late sequela in patient with Graves' disease treated with antithyroid agents.

In 1971, thyroid function was evaluated in 15 unselected patients whose only therapy for diffuse toxic goiter was a course of thionamide drug treatment completed 20-27 yr earlier. One patient was frankly hypothyroid by clinical and laboratory criteria. The remaining 14 patients appeared clinically euthyroid and had a normal serum thyroxine (T(4)) concentration and thyroid radioiodine uptake (RAIU). Nevertheless, only 6 of 14 appeared to be entirely normal according to more refined criteria. The serum thyrotropin (TSH) concentration was markedly elevated in one patient and above the normal rang

[Identification of thyroglobulin-binding cells in the blood of Hashimoto patients (author's transl)].

We developed a new method for demonstration and enumeration of antigen-binding cells, the principle and application of this method is demonstrated using peripheral blood lymphoid cells from Hashimoto patients, non-Hashimoto, non Graves' disease goiter patients and in patients without thyroid disease. A significantly higher number of thyroglobulin-binding cells was observed in the peripheral blood of Hashimoto patients as compared to members of the two other groups. The results of the FICA-tests were compared to conventional serological data of the individual patients.

A study was made of diabetic retinopathy in acromegaly. 10 of 15 patients with acromegaly had diabetes mellitus, and 3 of the 10 showed diabetic retinopathy. 2 of them had a diabetic family history. 1 patient with a diabetic family history had retinopathy of state IIIa in Scott's classification, and the other 2 showed a few microaneurysms and/or punctate hemorrhages in the macula. Diabetes mellitus and diabetic retinopathy in acromegaly showed no correlation with the duration of acromegaly and diabetes mellitus, age, or growth hormone level. No diabetic cataract was found in the present series
