BACKGROUND: Autoimmune disorders have been linked to increased cardiovascular disease (CVD) risk, but it is unclear whether the association is uniform across conditions or is concentrated in specific diseases, and whether inflammatory markers add value in a Middle Eastern primary-care setting. We examined disease-specific associations between autoimmune disorders and recorded CVD, rather than building a prognostic prediction tool.
METHODS: We conducted a matched cross-sectional analysis of 28,374 adults (9517 with a documented autoimmune disorder - rheumatoid arthritis [RA], systemic lupus er

BACKGROUND: Central lymph node metastasis in clinically node-negative papillary thyroid carcinoma is regulated by the tumor immune microenvironment, yet existing studies have addressed molecular, systemic immune, and local imaging signals in isolation without examining their cross-scale associations.
METHODS: A retrospective cohort of 1,000 cN0 PTC patients was enrolled. BRAF V600E mutation status, six peripheral blood immune-inflammatory indices, and an ultrasound radiomics deep learning score were integrated into four progressive LightGBM models. Cross-scale Spearman correlations, BRAF-stra

Spontaneous hemorrhage within a thyroid nodule is an uncommon but potentially alarming condition presenting with acute neck pain, swelling, and compressive symptoms that may threaten the airway and pose diagnostic and management challenges. We describe a 35-year-old Asian woman who presented with sudden onset of severe left-sided neck pain and swelling, accompanied by dysphagia and exertional dyspnea. There was no history of trauma, anticoagulant use, or prior thyroid intervention. Physical examination revealed marked unilateral thyroid tenderness and swelling without overt airway compromise.

A previously healthy 24-year-old woman presented with jaundice, diarrhea, tachycardia, fever, and altered mental status. Laboratory evaluation revealed severe hyperbilirubinemia, mixed hepatocellular-cholestatic injury, international normalized ratio 4.99, hypofibrinogenemia, and encephalopathy, fulfilling criteria for acute liver failure (ALF). Thyroid studies demonstrated suppressed thyroid-stimulating hormone, elevated free thyroxine and triiodothyronine, and strongly positive anti-thyroid peroxidase and anti-thyroid‑stimulating hormone receptor antibodies, confirming thyroid storm. Subacute thyroiditis (SAT) is a self-limited inflammatory thyroid disorder that typically produces a transient thyrotoxic phase. Intracranial hypertension (IH) is a rare but potentially sight-threatening complication of thyrotoxicosis, reported almost exclusively in hyperthyroidism, with only a single previous report linking it to SAT. We report the case of a 29-year-old woman who developed IH during the thyrotoxic phase of subacute thyroiditis, temporally related to dental manipulation. She presented with headache, nausea, vomiting, and fever, followed by a syncopal episode; clinical, laborat

Thyroid diseases have become the most common endocrine disorders globally, including hyperthyroidism, hypothyroidism, thyroid nodules (TN), and thyroid cancer (TC). Traditionally, the pathogenesis was attributed to autoimmune dysfunction, iodine metabolism imbalance, and genetic-environmental interactions. Recently, research suggests that vitamin D deficiency and gut microbiota dysbiosis are prevalent in patients with thyroid disorders, particularly in autoimmune thyroid disease (AITD). Vitamin D, as well as gut microbiota and its metabolites, such as short-chain fatty acids and bile acids, jo

BACKGROUND: Vitamin D, an important immunomodulator, has been implicated in the pathogenesis of thyroid disease. Although several studies have reported lower serum vitamin D levels in patients with autoimmune thyroid diseases (AITDS) and thyroid cancer, others have yielded shown inconsistent findings, leaving the clinical significance of vitamin D status in thyroid diseases unclear. This review aimed to clarify the association between serum vitamin D levels and thyroid diseases.
METHODS: Observational studies, investigating the association between serum vitamin D levels and thyroid diseases,

To predict the polypharmacological mechanism of Yueju pill (YJP) against Hashimoto's thyroiditis (HT), this study integrated network pharmacology and molecular docking to develop a computationally derived "dual-axis core" hypothesis involving the TNF-NF-κB inflammatory axis and the PI3K-Akt survival/apoptosis axis. Candidate active compounds and putative targets were identified, followed by PPI network construction and GO and KEGG enrichment analyses. Representative compounds were further evaluated by molecular docking, with redocking and reference-ligand comparisons used to assess docking rel

BACKGROUND: Differentiating papillary thyroid carcinoma (PTC) from benign nodules in patients with Hashimoto's thyroiditis (HT) remains a significant clinical challenge due to the complex sonographic background and overlapping inflammatory features. This study aimed to develop and validate an interpretable machine learning (ML) model integrating ultrasound features and immune-inflammatory biomarkers for PTC identification in HT patients.
METHODS: A multicenter retrospective study was conducted, enrolling 1, 200 HT patients with thyroid nodules from Wuxi People's Hospital (January 2020 to Dece

AIM: Autoimmune polyglandular syndrome type 3 (APS-3) is the coexistence of autoimmune thyroid disease (AITD) with type 1 diabetes (T1D) or other autoimmune diseases, after exclusion of adrenal insufficiency. We aimed to evaluate the prevalence, timeline and features of APS-3 in children and adolescents with T1D.
METHODS: We studied 439 T1D paediatric patients who were diagnosed and followed up over 16 years using annual biochemical and autoantibody profile evaluations.
RESULTS: The prevalence of APS-3a (T1D+AITD) was 23.5%, with significant female predominance (54.4%, P = 0.007). The vast m

Autoimmune encephalitis (AIE) is a relatively new group within the spectrum of encephalitic diseases. Awareness and the pathophysiological understanding of these conditions are increasing. They are also gaining importance in the field of intensive care medicine as severe cases are frequently accompanied by unconsciousness, epileptic seizures up to treatment-refractory status epilepticus, autonomic dysregulation and the need for mechanical ventilation. A major challenge is the early differentiation between autoimmune and infectious encephalitides as symptoms, cerebrospinal fluid findings and ch

West Nile virus (WNV) is an uncommon but serious case of encephalitis and meningitis, often presenting with nonspecific symptoms that can mimic a wide range of infectious, metabolic, and endocrine disorders. We present the case of a 61-year-old male with multiple comorbidities who arrived with 5 days of fever, chills, profuse diarrhea, black stools, and worsening confusion. Initial evaluation in the emergency department revealed hyponatremia, transaminitis, and concern for tick-borne illness, prompting empiric doxycycline. However, the patient denied any tick bites. BACKGROUND: Lead encephalopathy is an uncommon but potentially fatal condition that can present with nonspecific neurological symptoms. In older adults, it rarely mimics autoimmune diseases, leading to misdiagnosis. This case underscores the need to recognize hidden sources of lead exposure, particularly the prolonged use of herbal medicines, which is prevalent in South Asia, and to critically assess potentially inappropriate medications in geriatric care.

Primary thyroid lymphoma (PTL), a rare malignancy originating from thyroid lymphoid tissue, is strongly associated with Hashimoto's thyroiditis where accelerated lymphocyte proliferation and chronic antigen stimulation constitute key pathogenic mechanisms. The predominant pathological subtypes of PTL include diffuse large B-cell lymphoma and mucosa-associated lymphoid tissue lymphoma (MALTL). Current research on molecular alterations primarily focuses on MALTL, particularly pathogenic mutations in CD274 and TNFRSF14 . Diagnostic imaging modalities such as ultrasonography and computed tomograph

OBJECTIVE: Postoperative hypoparathyroidism (hypoPT) remains a significant complication after total thyroidectomy. Elevated thyroid autoantibodies are commonly considered surrogate markers of autoimmune thyroiditis and are presumed to increase surgical difficulty, potentially leading to parathyroid injury; however, their independent impact on postoperative parathyroid outcomes remain uncertain. This study aimed to evaluate the association between elevated preoperative thyroid antibody levels and postoperative parathyroid dysfunction in patients undergoing total thyroidectomy.
METHODS: This re

Observational studies have linked autoimmune diseases (ADs) with rhinosinusitis (RS) manifestations. To establish a causal relationship between ADs and RS, and to explore the potential mediating role of inflammatory mediators between ADs and RS, we utilized Mendelian randomization (MR) analysis. Using a two-sample MR methodology, we examined the causality between multiple sclerosis (MS), rheumatoid arthritis (RA), ankylosing spondylitis (AS), psoriasis (PsO), type 1 diabetes (T1D), Sjogren's syndrome (SS), celiac disease (CeD), Crohn's disease (CD), hypothyroidism (HT), Graves' disease (GD), a
