Parathyroid hormone-based algorithm reduces complications after total thyroidectomy.

BACKGROUND: Hypocalcemia after total thyroidectomy is a frequent complication that can be predicted and best managed by the postoperative measurement of parathyroid hormone levels. METHODS: This study included consecutive patients who underwent total thyroidectomy between 2017 and 2022. Hypocalcemia was defined as serum calcium <8.0 mg/dL and hypoparathyroidism as parathyroid hormone <15 pg/mL. After comparing serum calcium levels on postoperative day 1 and intraoperative parathyroid hormone levels 20 minutes after total thyroidectomy in the first period, an algorithm involving routine postop

Prophylactic central lymph node dissection for low-risk papillary thyroid cancer-Impact on subsequent therapy.

BACKGROUND: For small papillary thyroid cancers (PTCs) with no lateral nodal involvement, American Thyroid Association guidelines recommend performing prophylactic central lymph node dissection (pCLND) if it influences further management. Our cohort study explored to what extent performing pCLND for small PTCs can de-escalate subsequent therapy including completion thyroidectomy and adjuvant radioactive iodine (RAI) ablation. METHODS: Adults with T1, T2, and cN0 PTCs were identified from 42 centers across the prospectively maintained Australian and New Zealand Thyroid Cancer Registry (ANZTCR)

Feasibility of Lobectomy in Selected Patients with Unilateral N1b Papillary Thyroid Cancer.

BACKGROUND: Total thyroidectomy (TT) is usually recommended for unilateral papillary thyroid cancer (PTC) with lateral lymph node metastasis (LLNM), but no significant improvement in recurrence-free survival (RFS) is seen upon treatment. As an initial surgery, lobectomy may have advantages in appropriately selected unilateral PTC with ipsilateral LLNM. OBJECTIVE: This study aimed to explore the feasibility of lobectomy for selected unilateral PTC with ipsilateral LLNM. METHODS: From January 2014 to December 2021, we retrospectively reviewed patients with PTC and LLNM who were treated at our

Minimally invasive techniques for the management of benign thyroid nodules: Efficacy and impact on quality of life of percutaneous ethanol injection. Results from a high-resolution thyroid nodule unit.

BACKGROUND AND OBJECTIVE: The high incidence of thyroid nodular disease has raised concerns on the therapeutic management of benign thyroid nodules. The development of minimally invasive techniques is an alternative to surgery and has a low rate of complications. Our objective was to evaluate the safety and efficacy profile of percutaneous ethanol injections (PEI) with ethanol aspiration of symptomatic cystic and predominantly cystic thyroid nodules (cystic component > 50%) treated in the thyroid nodule high-resolution unit (TNHRU) of Hospital de Tortosa Verge de la Cinta Hospital, Tarragona,

Changes in the Volume and Diameter of Benign Thyroid Nodules: A 10-Year Follow-Up Study.

Background: The incidence of asymptomatic thyroid nodules has risen enormously, with > 90% being benign. Despite limited long-term data, significant nodule growth (SNG) is common. Guidelines recommend cytology reevaluation if SNG occurs. Our study aimed to identify the rate and factors associated with SNG, compare diameter and volume-based assessments, and examine the association between SNG and malignancy risk over the long term (10 years). Methods: The retrospective cohort study, conducted at Ankara University, School of Medicine, Department of Endocrinology and Metabolism, included 732 nodu

Skin Sceneries of Thyroid Disorders and Impact of Thyroid on Different Skin Diseases: A Scoping Review Focused on Pediatric Patients.

Thyroid function plays a seminal role in the growth and development of children, and alterations in signaling pathways which interfere with the biosynthesis, transport, and metabolism of thyroid hormones might impact on the skin of such patients: this review explores the relationship between different thyroid disorders and dermatological manifestations with a particular focus on the pediatric population. Common cutaneous conditions associated with thyroid dysfunction may include chronic urticaria, vitiligo, and alopecia, which can be early harbingers of an underlying endocrine disruption. This

Unilateral Graves' Orbitopathy in a Patient with Marine-Lenhart Syndrome: A case report.

Thyroid eye disease (TED) is the most common symptoms of Graves' disease. This condition commonly manifests bilaterally and symmetrically. The most prominent symptoms are lid retraction, exophthalmos and diplopia. Rarely, individuals with Graves' disease show asymmetrical or unilateral eye symptoms. Marine-Lenhart syndrome is a variant of Graves' disease with occasional hyperactive nodules. A 36-year-old male patient presented to the endocrinology outpatient department at a tertiary care hospital in Muscat, Oman, in 2022 with unilateral eye proptosis and was subsequently found to have Graves'

A novel approach to explore metabolic diseases: Neddylation.

Protein post translational modification (PTM) is the main regulatory mechanism for eukaryotic cell function, among which ubiquitination is based on the reversible degradation of proteins by the ubiquitin proteasome system to regulate cell homeostasis. The neural precursor cell expressed developmental downregulated gene 8 (NEDD8) is a kind of ubiquitin like protein that shares 80 % homology and 60 % identity with ubiquitin. The PTM process by covalently binding NEDD8 to lysine residues in proteins is called neddylation. The neddylation reaction could be regulated by NEDD8, its precursors, subst

Post-thyroidectomy hypoparathyroidism: A clinical surgical dilemma.

Thyroid dysfunction directly affects human health and overall well-being; various approaches are available for the treatment of thyroid diseases, including conservative measures and surgical interventions. Despite advancements in conservative treatment, surgery remains the preferred option. Hypoparathyroidism is the primary cause of hospitalization after thyroidectomy, leading to cost-related concerns and a detrimental impact on patients' overall quality of life. Postoperative hypoparathyroidism can result in hypocalcemia, and these are challenging conditions that must be identified promptly t

Modeling Clinical Radioiodine Uptake By Using Organoids Derived From Differentiated Thyroid Cancer.

Radioiodine-refractory differentiated thyroid cancer (RAI-R DTC) accounts for the vast majority of thyroid-related mortality and, until recently, there were limited preclinical models for iodine uptake prediction. In the current study, we aim to establish a primary tumor-derived organoid model of DTC and predict radioiodine (RAI) uptake of tumor residue. The genotypic and phenotypic features between organoid and parental tissue were compared. The RAI uptake assay was used to evaluate the organoid's RAI uptake capacity, and related patients' RAI whole-body scans were used to verify the assay's
