BACKGROUND: This cross-sectional study examined the relevance and impact of fear of disease progression (FoP) to parents of youth with two common childhood chronic conditions—juvenile idiopathic arthritis (JIA) and type 1 diabetes mellitus (T1DM).
METHODS: 124 parents (57 JIA, 67 T1DM; 107 mothers, 17 fathers) of youth aged 2–18 years (M=11.36, SD=3.90) were recruited through social media and patient organisation groups. Parents completed online questionnaires assessing fear of their child’s disease progression (FoP), responses to their children's symptoms and their own psychopathology. In ad

BACKGROUND: Continuous glucose monitoring (CGM) has improved glycemic management in Type 1 Diabetes Mellitus (T1DM). However, interpretation of CGM data remains challenging. CGM data clustering may support individualized management and identify insulin resistance.
AIM: This study aimed to cluster CGM data based on interstitial glucose levels (2.2-27.8 mmol/L) and glucose variability, and to compare the identified clusters with biochemical and anthropometric markers of insulin sensitivity.
METHODS: CGM data from 75 T1DM patients and 20 subjects without diabetes were analyzed using Uniform Man

BACKGROUND: Diabetic ketoacidosis (DKA) is a life-threatening complication of type 1 diabetes mellitus (T1D), potentially leading to severe outcomes, including cerebral edema (CE). The case describes an eight-year-old child with previously undiagnosed T1D complicated by DKA, and subsequent development of CE. It is followed by a short literature review including other pediatric cases of clinically overt CE.
CASE PRESENTATION: A boy was admitted to the outpatient clinic with a several-day history of vomiting, abdominal pain and headache, leading to a presumptive diagnosis of gastrointestinal in

This study aims to investigate the genetic correlation and potential causal relationship between diabetes mellitus (DM) and Hashimoto thyroiditis (HT), and to assess the effects of type 1 diabetes mellitus (T1DM) and type 2 diabetes mellitus (T2DM) on HT risk, with the goal of informing thyroid-function monitoring in patients with DM and improving HT management. We used linkage disequilibrium score regression (LDSC) to estimate the genetic correlation between DM and HT. We then performed two-sample Mendelian randomization (TSMR) using data from genome-wide association studies (GWAS) to evaluat

In response to Huang et al., we clarify the take-home message of our study on autoimmune polyendocrine syndrome (APS) in type 1 diabetes mellitus (T1DM). Existing guidelines recommend screening T1DM patients for thyroid and celiac diseases. When positive, there is a lack of guidance on how to proceed. Nevertheless, if T1DM patients do have AITD or celiac disease, they qualify for an APS diagnosis, which signals a high pretest probability for additional autoimmunity. Our data show that among 198 patients meeting APS criteria at baseline, only 17/198 (8.6%) had a formal APS diagnosis documented

Hyperglycemic hyperosmolar state (HHS) is a rare but life-threatening hyperglycemic emergency in children and adolescents. It is characterized by severe hyperglycemia, hyperosmolality, and dehydration, with absent or limited ketoacidosis. Although more commonly associated with type 2 diabetes mellitus, HHS may also occur as the initial presentation of type 1 diabetes mellitus in pediatric patients. We report the case of a previously healthy 13-year-old boy presenting with a three-week history of polydipsia and nocturnal polyuria. Initial evaluation revealed profound hyperglycemia (>1,000 mg/dL

Necrobiosis lipoidica is uncommon in childhood, and a multifocal presentation early in the course of type 1 diabetes mellitus appears to be infrequently reported. In our patient, multiple lesions developed within 3 years of diabetes diagnosis, despite the absence of overt microvascular complications. The persistence of lesions alongside recurrent episodes of ketoacidosis may suggest a potential association between suboptimal metabolic control and cutaneous microangiopathic changes; however, causality cannot be established on the basis of a single case. Early recognition of such lesions may be

Cardiovascular disease (CVD) is among the leading causes of morbidity and mortality worldwide. Advances in the development of novel diagnostic and therapeutic strategies are required to improve diagnostic accuracy and provide personalized treatments in CVD. Emerging research substantiates the role of genetic composition in CVD, prompting more comprehensive understanding via multimodal approaches that integrate clinical, genomic, and transcriptomic data. Traditional bioinformatics and statistical modelling have long been defaults for the analysis and interpretation of single-omic and clinical d

INTRODUCTION: Neglected pathological femoral neck fractures in young patients are rare and present significant surgical challenges, particularly in the presence of systemic metabolic disorders such as autoimmune polyglandular syndrome (APS). Poor bone quality, impaired healing potential, and endocrine instability complicate management, often limiting the success of head-preserving procedures.
PURPOSE: The purpose of the study was to describe the surgical decision-making, technical considerations, and early outcomes of total hip arthroplasty (THA) in a young patient with a neglected pathologic

PURPOSE: Differentiating diabetes subtypes is essential for long-term management; however, overlapping features among type 1 diabetes (T1D), type 2 diabetes (T2D), and maturity-onset diabetes of the young (MODY) complicate diagnosis. Clinical characteristics and temporal trends according to childhood-onset diabetes subtype were investigated over a 20-year period.
METHODS: We retrospectively reviewed 662 patients diagnosed with T1D, T2D, or MODY at Seoul National University Children's Hospital between 2003 and 2022 at ages ≥6 months and <19 years, excluding secondary and other specific forms.

The study, a cross sectional survey, was carried out at the Changing Diabetes in Children (CDiC) Outpatient Department of CDiC, BIRDEM-2 Hospital over a 1-year period from July 2018 to June 2019. Diabetes mellitus was diagnosed according to WHO, ISPAD and Mohan's criteria. Height, weight and waist circumference were recorded and body mass index (BMI) scores were calculated as weight (kg)/height (m²). BMI was classified using the cutoff points (<5th percentile underweight, 5th - 85th percentile normal, 85th - <95th percentile overweight and ≥95th percentile as obese) using the Center for Diseas

Euglycemic diabetic ketoacidosis can mimic acute asthma, including in previously undiagnosed diabetes and without SGLT-2 inhibitor use. Carbohydrate restriction may precipitate life-threatening ketoacidosis, particularly with insulin deficiency. Early arterial blood gas analysis and urine ketone testing facilitate prompt diagnosis, enabling timely treatment and preventing complications.

BACKGROUND: Type 1 diabetes mellitus (T1D) is increasingly recognised to affect neurodevelopmental, cognitive and psychological functioning in children and young adults. Existing studies have largely evaluated biological, cognitive and psychological domains separately, and neuropsychological approaches suitable for multilingual and resource-constrained settings remain limited. This paper describes the MIND-T1D protocol, a culturally sensitive, clinically feasible, family-centred assessment framework for Indian children and young adults with T1D. The protocol addresses language-related barriers
