Assessment of P1NP and β-CTX in Type 2 Diabetes Mellitus: a Case-Control Study from North India.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is associated with increased skeletal fragility despite normal or elevated bone mineral density (BMD), suggesting impaired bone quality and remodeling. Bone turnover markers (BTMs), including procollagen type I N-terminal propeptide (P1NP) and β-C-terminal telopeptide of type I collagen (β-CTX), reflect bone formation and resorption, respectively. This study evaluated serum P1NP and β-CTX concentrations in patients with T2DM from North India and their associations with clinical and biochemical parameters. METHODS: This hospital-based case-control st

Association of Dietary Vitamin B12 Intake, Nutrition Knowledge and Peripheral Neuropathy in Patients with Type 2 Diabetes Mellitus: a Cross-Sectional Study.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is rapidly increasing in India, with rural communities facing greater challenges due to limited nutrition literacy and micronutrient deficiencies. Dietary vitamin B12 inadequacy and poor vitamin B12 status have been implicated in neuropathy, yet evidence linking nutrition knowledge, B12 intake and neuropathy risk in rural Indian populations in patients with diabetes is limited. This study assessed nutrition knowledge, vitamin B12 intake, glycemic control and neuropathy prevalence among adults with T2DM in rural Telangana. METHODS: A cross-sectional

Red Cell and Platelet Indices as Biomarkers of Poor Glycemic Control in Patients With Type 2 Diabetes: A Case-Control Study.

BACKGROUND AND AIM: Type 2 diabetes mellitus (T2DM) is associated with systemic inflammation and altered hematological function, including changes in red cell indices and platelet hyperreactivity. While glycosylated hemoglobin (HbA1c) is the gold standard for monitoring glycemic control, its cost and susceptibility to conditions like anemia highlight the need for accessible, reliable alternative biomarkers, particularly for identifying patients with poor glycemic control (HbA1c ≥ 7.0%). This case-control study, therefore, investigated the utility of red cell and platelet indices as potential b

Combined empagliflozin and liraglutide therapy protects against tacrolimus-induced pancreatic and renal injury associated with oxidative stress reduction.

BACKGROUND: Sodium-glucose cotransporter-2 inhibitors (SGLT2is) and glucagon-like peptide-1 receptor agonists (GLP-1RAs) are widely used in type 2 diabetes mellitus for glycemic control and organ protection. This study investigated whether the combined use of empagliflozin (EMPA) and liraglutide (LIRA) could protect against pancreatic and renal damage caused by tacrolimus (TAC). METHODS: Male Sprague-Dawley rats received TAC for 4 weeks, either alone, or in combination with EMPA, LIRA, or both. Metabolic, histologic, and molecular parameters were assessed in vivo and in vitro. TAC caused hype

Keap1 Deficiency Promotes Macrophage M2 Polarization and Protects β Cells.

BACKGROUND: Progressive pancreatic β-cell injury is a core feature of Type 2 diabetes mellitus (T2DM). Imbalance between macrophage M1 and M2 polarization, driven by chronic inflammation, is a key contributor to this injury. The Keap1-Nrf2-ARE pathway regulates macrophage polarization and inflammatory responses, but whether it affects pancreatic β-cell survival through macrophages is unknown. METHODS: We generated a Keap1-knockout THP-1 line using CRISPR-Cas9, differentiated the cells into M0 macrophages with PMA, polarized them toward M2 with IL-4/IL-13, and cocultured them with MIN6 cells i

SGLT2 Inhibitors Reduce Heart Failure Hospitalization, Cardiovascular Death and Kidney Outcomes in Type 2 Diabetes: A Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Cardiovascular disease and chronic kidney disease are the principal drivers of morbidity and mortality in type 2 diabetes mellitus (T2DM). Sodium-glucose cotransporter-2 inhibitors (SGLT2i) have demonstrated cardiorenal benefits in landmark trials, yet cross-trial inconsistencies in cardiovascular mortality, renal endpoints and drug-specific effects leave the class-level estimates incompletely synthesized. METHODS: PubMed, Web of Science, Scopus and CENTRAL were searched from inception through 15 May 2026. Large placebo-controlled RCTs (≥ 1000 participants) and prespecified subgro

Type 2 diabetes remission following a comprehensive metabolic program including surgery in adults with BMI ≥50 kg/m2 versus BMI <50 kg/m2.

AIMS: Previous reports have suggested that higher baseline BMI (bBMI) or %excess weight loss (EWL) are independent predictors of type 2 diabetes mellitus (T2DM) remission after metabolic bariatric surgery (MBS). However, %EWL appears lower in individuals with bBMI ≥50 kg/m2 than those with lower bBMI. Thus, this cohort study aimed to examine T2DM remission after MBS in adults with bBMI <50 kg/m2 versus bBMI ≥50 kg/m2. METHODS: This cohort study included adults with obesity and T2DM who underwent MBS between 01/06/2018 and 31/05/2024 at an Australian tertiary hospital. The primary outcome was

Serum Orexin A and Anthropometric, Hemodynamic and Lipid Profiles in Newly Diagnosed Treatment-Naïve Type 2 Diabetes: a Case-Control Study.

BACKGROUND: Orexin A is a neuropeptide from the lateral hypothalamus and is involved in the regulation of energy balance and peripheral metabolic processes. There is extensive mechanistic evidence in animal models, but its circulating behavior during the initial phases of type 2 diabetes mellitus (T2DM) in South Asian populations in general and resource-limited Indian settings in particular is poorly characterized. OBJECTIVES: The purpose of this study was to compare serum Orexin A levels in newly diagnosed, treatment-naïve T2DM patients with healthy controls and to investigate possible assoc

Circulating FTO Protein Levels and β-Cell Function in Individuals with Different Metabolic Profiles: A Cross- Sectional Study.

BACKGROUND: Obesity and type 2 diabetes mellitus are characterized by insulin resistance and progressive impairment of β-cell function. Fat mass and obesity-associated (FTO) protein, an m6A demethylase involved in metabolic regulation, has been linked to obesity. However, the relationship between circulating FTO protein levels and β-cell function among individuals with different metabolic profiles remains unclear. This study aimed to investigate circulating FTO levels and their relationship with β-cell function, assessed by (HOMA-β), in individuals with newly diagnosed T2D, obese individuals w

Vitamin D Deficiency Associated Dyslipidemia in Type 2 Diabetes and Changes Following Vitamin D2 Supplementation.

Vitamin D deficiency (VDD) is common in type 2 diabetes mellitus (T2DM) and has been linked to dyslipidemia, but the specific lipids associated with low vitamin D status are not well defined. To address this, we sought to characterize lipidomic alterations associated with VDD in T2DM and describe the changes following vitamin D₂ (VD2) supplementation. Specifically, untargeted serum lipidomics with anthropometric and clinical measurements were performed in 219 adults with T2DM, classified as VDD (25(OH)D < 20ng/mL; n = 138) or non-VDD (25(OH)D ≥ 20ng/mL; n = 81). A subset of VDD participants (n

Five-Year Outcomes of Sleeve Gastrectomy with Transit Bipartition in Women with Severe Obesity and Insulin-Treated Type 2 Diabetes: A Prospective Single-Center Cohort Study of 35 Women.

INTRODUCTION: Five-year data on metabolic durability, nutritional outcomes, and late morbidity after sleeve gastrectomy with transit bipartition (SG + TB) remain limited. We evaluated glycemic, weight, nutritional, and safety outcomes in women with severe obesity and insulin-treated type 2 diabetes mellitus (T2DM). METHODS: This prospective single-center follow-up included 35 women who underwent laparoscopic SG + TB in 2016-2017. At 5 years, 30/35 (85.7%) retained SG + TB anatomy and had complete efficacy data; weight and HbA1c were available in one additional revised patient (31/35, 88.6%).

Serum melatonin levels and their relationship with inflammation and oxidative stress in patients with diabetic nephropathy: a cross-sectional case-control study.

BACKGROUND: Diabetic nephropathy (DN), a major microvascular complication of type 2 diabetes mellitus (T2DM), is driven by oxidative stress and chronic inflammation. Melatonin, an endogenous indoleamine with potent antioxidant and anti-inflammatory properties, has been implicated in diabetic complications but its relationship with systemic inflammation and oxidative balance in DN remains incompletely characterised. This study aimed to evaluate serum melatonin levels in T2DM patients with and without nephropathy compared with healthy controls, and to assess associations with inflammatory indice

Hidden metabolic obesity phenotypes and risk of incident type 2 diabetes among BMI-defined non-obese adults: a secondary analysis of the NAGALA cohort.

BACKGROUND: Body mass index (BMI) may fail to capture clinically relevant heterogeneity in adiposity-related diabetes risk, particularly among Asian adults who are not classified as obese. We investigated whether abdominal obesity and fatty liver identify distinct risks of incident type 2 diabetes mellitus (T2DM) among adults with BMI <25 kg/m2. METHODS: This secondary analysis used data from the Japanese NAGALA cohort. A total of 12,940 adults without diabetes at baseline and with BMI <25 kg/m2 were classified into four phenotypes: neither abdominal obesity nor fatty liver, isolated abdomina

A Proposed Oral-Liver-Pancreas Axis in Type 2 Diabetes: Linking Oral Dysbiosis, Hepatic Insulin Resistance, and Pancreatic Beta-Cell Stress.

Type 2 diabetes mellitus develops through the interaction of metabolic, inflammatory, and microbial processes that extend beyond the pancreas. Although the gut microbiota has received substantial attention, the oral microbiota may offer an additional, modifiable source of microbial and inflammatory signals. Periodontitis repeatedly exposes the circulation and gastrointestinal tract to bacteria, lipopolysaccharide, outer membrane vesicles, and host mediators. This narrative perspective asks whether oral dysbiosis may amplify pancreatic beta-cell stress through hepatic immunometabolic dysfunctio
