Skeletal muscle alterations in type 2 diabetes mellitus with and without dyslipidemia.

BACKGROUND: Diabetic dyslipidemia, a common comorbidity in people with type 2 diabetes, has been reported to be associated with adverse metabolic outcomes. Skeletal muscle is a key organ impacted by metabolic disorders; however, the specific effects of diabetic dyslipidemia on skeletal muscle mass, strength, and performance remain unclear. The aim of this study was to assess the skeletal muscle alterations of people with type 2 diabetes with and without dyslipidemia. METHODS: This retrospective study included 144 participants (mean age 51.3 ± 7.1 years; 103 female, 41 male) aged 40–65 years. B

GLP-1 receptor agonists and sarcopenia: Weight loss at a cost? A brief narrative review.

Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) and dual agonists targeting both GLP-1 and glucose-dependent insulinotropic polypeptide receptors (GLP-1/GIP RAs) are established therapies for type 2 diabetes mellitus (T2DM) and obesity. However, there is evidence that treatment with these agents may lead to significant loss of muscle mass, potentially resulting in sarcopenia or sarcopenic obesity. This brief narrative review explores the complex relationship between GLP-1 based therapies and muscle health. Some studies have linked GLP-1 RAs and dual GLP-1/GIP RAs with significant reducti

Association Between Leptin and Adiponectin Levels and Sarcopenia in Non-Geriatric Type 2 Diabetes Mellitus Patients.

BACKGROUND: Type 2 Diabetes Mellitus (T2DM) in young adults is associated with an increased risk of early sarcopenia due to insulin resistance and inflammation. This insulin resistance and inflammation can be influenced by leptin and adiponectin, which are key adipocytokines produced by adipose cells. However, no studies have examined the relationship between leptin, adiponectin levels, and sarcopenia in T2DM patients under 60 years old. This study aimed to investigate the relationship between leptin, adiponectin levels, and Leptin-to-Adiponectin ratio (LAR) with sarcopenia in non-geriatric T2

Coffee and health outcomes: a systematic review of Mendelian randomisation studies.

Coffee is a widely consumed beverage, which has been extensively studied for its potential effects on health. We aimed to map genetic evidence for the effect of habitual coffee consumption on health. We searched PubMed, Embase, Cochrane Database of Systematic Reviews, Cumulative Index to Nursing and Allied Health Literature and two preprint repositories from inception to 30 September 2022, and included fifty-nine studies, spanning 160 disease or biomarker associations. We evaluated the articles for certainty of evidence using a modified GRADE tool and robustness of the associations by comparin

The association between abnormal glucose metabolism and rheumatoid arthritis.

The research has found that the incidence of rheumatoid arthritis (RA) combined with Type 2 Diabetes Mellitus (T2DM) and insulin resistance (IR) is higher than that in the general population. This indicates that there is an association mechanism between RA and abnormal glucose metabolism. IR is one of the main causes of T2DM, and high blood glucose levels are one of the main symptoms of T2DM. Therefore, revealing the intrinsic mechanism between RA and the abnormal glucose metabolism in T2DM becomes the key to treating RA complicated with T2DM. This article summarizes the mechanism of RA combin

Comparative risk of rheumatoid arthritis between glucagon-like peptide-1 receptor agonists and sodium-glucose cotransporter-2 inhibitors in type 2 diabetes.

INTRODUCTION: Rheumatoid arthritis (RA) causes disability, chronic pain, and reduced quality of life. Preclinical studies suggest glucagon-like peptide-1 receptor agonists (GLP-1 RAs) may reduce inflammation and protect joint tissues. This study compared GLP-1 RAs with non-GLP-1 RAs on the risk of RA in patients with type 2 diabetes mellitus (T2DM). METHODS: Using the TriNetX network (2016-2023), we identified 2,688,327 patients with T2DM. Propensity score matching yielded 89,938, 111,074, and 88,054 pairs of GLP-1 RA users versus dipeptidyl peptidase-4 inhibitors (DPP-4i), basal insulin, and

Osteo-sarcopenic obesity in midlife and older women: a current worldwide public health challenge.

Sarcopenic obesity, defined as the coexistence of excess adiposity and sarcopenia, represents a high-risk clinical condition that amplifies the adverse effects of each disorder. When accompanied by bone loss, the entity progresses to osteo-sarcopenic obesity (OSO), which further compromises physical function, metabolic health and overall prognosis. Affected individuals face an increased risk of falls, fractures, functional disability, hospitalization, cardiometabolic complications and premature mortality. In women transitioning through menopause, declining estrogen levels accelerate fat accumu

Glucagon-like peptide-1 receptor agonists and muscle health: potential role in sarcopenia prevention and treatment.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have emerged as a cornerstone therapy for weight loss and glycemic control in type 2 diabetes mellitus and obesity. Moreover, robust cardiometabolic benefits and favorable safety profiles have positioned GLP-1RAs at the forefront of modern obesity pharmacotherapy. However, findings from large-scale trials (eg, SUSTAIN, STEP, SURPASS) have raised concerns that a significant proportion of the weight loss achieved with GLP-1RA treatment may derive from lean body mass-particularly skeletal muscle-which could be detrimental in populations already

Glucagon-Like Peptide-1 Receptor Agonists for Arthritis and Osteoarthritis.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) play a pivotal role in managing type 2 diabetes mellitus (T2DM), obesity, and other conditions, with proven benefits including, but not limited to, weight loss, glycemic control, and cardiovascular protection. However, their implications for arthritis are still debatable. GLP-1RAs may reduce systemic inflammation, alleviate joint stress through overall weight loss, and exhibit chondroprotective effects. Nevertheless, concerns such as augmented gout flares, joint pain, and indefinite long-term outcomes temper their promise. This editorial hig

Sarcopenia and sarcopenic obesity in type 2 diabetes mellitus and cardiovascular disease: current perspective and narrative review.

Sarcopenia, initially described as a disease of the elderly and its coexistence with obesity, now termed as 'sarcopenic obesity' (SO), are now emerging to be important health problems worldwide. Their relatively recent recognition as distinct clinical entities with growing research has shown an increasing prevalence of both sarcopenia and SO. There is a strong independent association, which exists between sarcopenia/SO, type 2 diabetes mellitus (T2DM), and cardiovascular disease (CVD). Studies investigating prevalence of sarcopenia/SO indicate considerable heterogeneity, likely due to the lack

Predictors of Development of Hepatocellular Carcinoma in Non-Cirrhotic Patients With Metabolic Dysfunction-Associated Steatotic Liver Disease/Metabolic Dysfunction-Associated Steatohepatitis - A Retrospective Analysis of the National Inpatient Sample Database.

BACKGROUND: One-quarter of the world population is thought to have metabolic dysfunction-associated steatotic liver disease (MASLD). The incidence of metabolic dysfunction-associated steatohepatitis (MASH) and MASLD is rapidly increasing due to the ongoing global epidemic of type 2 diabetes mellitus and obesity. Hepatitis B and C have declined in incidence due to advances in prevention and treatment, yet the overall burden of hepatocellular carcinoma (HCC) continues to rise, largely driven by the growing prevalence of MASLD/MASH. MASLD/MASH is now the fastest-growing etiology of HCC in the USA

[Type 2 diabetes mellitus in rheumatoid arthritis and systemic lupus erythematosus: A review].

Immunoinflammatory rheumatic diseases (IIRD), such as rheumatoid arthritis and systemic lupus erythematosus, are characterized by common mechanisms of development and similar comorbid pathology, including diabetes mellitus (DM) type 2. Patients with IIRD with concomitant DM represent one of the most difficult groups to manage due to serious complications and forced polypharmacy. The article considers risk factors for carbohydrate metabolism disorders, both traditional and directly associated with rheumatoid arthritis and systemic lupus erythematosus, including the effect of anti-inflammatory t

The muscle-brain axis in type 2 diabetes: Molecular pathways linking sarcopenia and cognitive decline.

Type 2 diabetes mellitus (T2DM) is increasingly recognized as a shared pathological substrate for both sarcopenia and cognitive decline, particularly Alzheimer's disease (AD). This review synthesizes current evidence on the converging molecular pathways linking insulin resistance, hyperglycaemia, mitochondrial dysfunction, oxidative stress, and chronic inflammation to muscle wasting and neurodegeneration. Central to this interplay is the muscle-brain axis, a bidirectional communication network mediated by myokines, exercise-induced cytokines that influence metabolic and neural homeostasis. Key

Rapidly Progressive Cardiac Tamponade in Uncontrolled Rheumatoid Arthritis Complicated by Diabetic Ketoacidosis.

Rheumatoid arthritis (RA) can involve extra-articular organs, including the heart, with pericardial effusions being common but rarely progressing to cardiac tamponade. We report a 42-year-old female with uncontrolled RA, markedly elevated anti-cyclic citrullinated peptide (anti-CCP) and rheumatoid factor (RF), and type 2 diabetes mellitus, who presented with severe chest pain, shortness of breath, and diabetic ketoacidosis. Rapid accumulation of a large pericardial effusion led to cardiac tamponade, requiring emergency pericardial drainage during a peri-arrest state. Despite aggressive critica
