[Clinical characteristics and investigation for the first death case of group Y epidemic cerebrospinal meningitis in Henan Province, 2024].

On November 22, 2024, Henan Province reported the first fatal case of serogroup Y meningococcal disease. The case was a 14-year-old male student from a boarding school in Dengfeng City, Zhengzhou. He developed symptoms on November 20, 2024, primarily presenting with disturbed consciousness, pallor, and other signs of circulatory failure, but without meningeal irritation. He was diagnosed with meningococcal disease (fulminant type) on November 22 and died on the same day. The patient had no history of vaccination against Neisseria meningitidis serogroup Y. He had enrolled in the school on Augus

[Molecular Detection of Neisseria meningitidis Serogroup C in an Adolescent with Atypical Presentation: A Silent Threat].

Neisseria meningitidis is an important pathogen that can cause invasive meningococcal diseases, especially with serogroups A, B, C, W and Y, which can be seen endemic or sporadic. Early diagnosis and treatment of invasive meningococcal disease is life-saving. Delay in diagnosis of meningitis infection, especially in the adolescent age group, which initially presents with atypical symptoms, can further increase the risk of morbidity and mortality. In this case report the clinical evaluation and diagnostic process of a 15-year-old female patient who admitted to the emergency department with abdo

Emergence of Neisseria meningitidis ST-3587 harbouring bla ROB-1 and exhibiting dual resistance to penicillin and ciprofloxacin, Spain, 2024.

BACKGROUNDDual penicillin- and ciprofloxacin-resistant Neisseria meningitidis causing invasive meningococcal disease (IMD) have recently emerged in association with sequence type (ST) 3587, harbouring ROB-1 β-lactamase (bla ROB-1) and a mutated DNA gyrase (gyrA). These strains pose a threat to current antimicrobial treatment and prophylaxis.AIMWe aimed to characterise the first dual-resistant N. meningitidis ST-3587 isolates harbouring bla ROB-1 and a mutated gyrA identified in Spain.METHODSThree N. meningitidis isolates encoding bla ROB-1 were identified in 2024. They were characterised by wh

Application of Causal Inference to Establish Assay Effect in the Absence of a Bridging Study: A Case Study of MenACWY-CRM Conjugate Vaccine Data.

During a vaccine development program, if the assay used to measure immunological endpoints is changed, ideally, a bridging study is performed to establish the relationship between results obtained with the new and previous assay. However, this is not always feasible, and when bridging study data are absent, this can limit the ability to use historical study information to strengthen evidence generated in the clinical program. We present a case study on GSK's quadrivalent meningococcal vaccine (MenACWY-CRM), where the immunogenicity assay was changed over time. A large amount of study data was

Implementation of a Novel Dormant State to Address Denominator Inflation in the Wisconsin Immunization Registry.

CONTEXT: Denominator inflation (DI) can burden long-standing immunization information systems (IIS), evident through a system having more client records than population estimates. DI underestimates IIS vaccination coverages compared to National Immunization Surveys (NIS) data. To mitigate DI, the Wisconsin Immunization Registry (WIR) implemented a dormant indicator in June 2020. Clients are presumed to have left the jurisdiction and are placed in the dormant state if they meet the following criteria: client age is at least 11 years old, no updates to the client record in at least 10 years, and

Live-attenuated chikungunya vaccine in children: a randomized phase 2 trial.

Currently, no licensed vaccine is available against chikungunya virus (CHIKV) in children aged less than 12 years. In this phase 2, observer-blind, randomized, dose-response trial we evaluated the tolerability (solicited adverse events (AEs)), safety (unsolicited AEs) and immunogenicity of a live-attenuated CHIKV vaccine (VLA1553) in healthy children aged 1-11 years in endemic countries. Here we provide a prespecified interim analysis to 28 days after vaccination. Participants (n = 304) received either a half dose (n = 119) or a full dose (n = 124) of VLA1553, or active control meningococcal v

Determinants of vaccine hesitancy among primary healthcare workers in Türkiye.

Healthcare workers (HCWs) are pivotal to sustaining public confidence in vaccination. This study assessed attitudes toward vaccine hesitancy, vaccine-related knowledge, and professional factors among primary healthcare workers (PHCWs) in Türkiye. We conducted a cross-sectional study among PHCWs in Erzurum (Eastern Anatolia), Türkiye. A structured, interviewer-administered questionnaire captured sociodemographics, vaccine-related knowledge, practices and recommendations, and the 21‑item Vaccine Hesitancy Scale (higher scores=greater hesitancy). Analyses included descriptive statistics, correlat

Investigation of invasive Neisseria meningitidis serogroup Y ST1466 case increases in New York State.

Bacterial meningitis and septicemia caused by Neisseria meningitidis is a serious infection that requires immediate medical attention and prompt treatment. In the United States, cases of N. meningitidis serogroup Y increased sharply in 2023, leading to a CDC health advisory issued in March 2024, alerting public health agencies and healthcare providers of this surge. N. meningitidis serogroup Y is of particular concern because these strains demonstrate higher levels of resistance to the antimicrobials ciprofloxacin and penicillin. At the Wadsworth Center Bacteriology Laboratory (WCBL), all N. m

Development of a rapid, simple multiplex PCR-dipstick assay for the detection of Neisseria meningitidis serogroups in clinical isolates.

The rapid identification and serogrouping of Neisseria meningitidis are crucial for effectively managing invasive meningococcal disease (IMD) patients and their close contacts, particularly in developing countries with limited laboratory resources. We developed a simple multiplex polymerase chain reaction (PCR)-dipstick DNA chromatography (mPCR-dipstick) assay for detecting six major serogroups (A, B, C, W, Y, and X) prevalent worldwide. The assay performance, sensitivity, and specificity were evaluated using 116 unencapsulated and encapsulated N. meningitidis, 29 other Neisseria spp., and 11

Neisseria meningitidis serogroup W ST-4821 clonal complex shares a highly conserved capsular switching pattern with ST-11 clonal complex strains.

OBJECTIVES: Neisseria meningitidis ST-4821 clonal complex (cc4821) has become the most prevalent in China, which can be divided into four sublineages globally, with the L44.4 sublineage incorporating all serogroup W (MenW) cc4821 isolates. We aimed to illustrate the capsular switching that generated MenW cc4821 isolates, focused on the origin and the universal capsular switching pattern. METHODS: The nucleotide sequences of capsular polysaccharide synthesis (cps) gene cluster were extracted from the genomes of cc4821 L44.4 sublineage in the Neisseria PubMLST Database and analyzed using MEGA s

Meningococcal Serogroup Y Meningitis Reveals Inborn Factor B Deficiency.

Complement factor B (FB) deficiency is an extremely rare alternative pathway (AP) defect predisposing to invasive infections, with only three families reported to date. We investigated a 14-year-old girl who developed Neisseria meningitidis serogroup Y meningitis complicated by bacteremia and cerebral venous sinus thrombosis. Complement function was assessed by TCA and AP50 hemolytic assay, FB quantification, and functional reconstitution with purified proteins. Genetic analysis of CFB was performed by next-generation sequencing. TCA, C3, and C4 were normal, whereas AP50 was undetectable. Plas

Trends in bacterial meningitis in Guinea in the post-MenAfriVac era: a retrospective cross-sectional study (2021-2024).

BACKGROUND: Since the introduction of the A conjugate vaccine (MenAfriVac) in 2010, epidemics of Neisseria meningitidis serogroup A have declined sharply in Africa, giving rise to other serogroups and pathogens, including Streptococcus pneumoniae and Haemophilus influenzae. In Guinea, located in the meningitis belt, post-vaccination surveillance remains essential to adapt prevention and diagnostic strategies. This study aimed to analyze the epidemiological evolution in order to assess the impact of vaccination and guide policies to combat emerging pathogens. METHODS : We conducted a retrospect

Clinical impact and cost-effectiveness of vaccinating infants and adolescents against invasive meningococcal B disease in the Netherlands.

BACKGROUND: Invasive meningococcal B disease (IMD-B) causes morbidity and mortality among infants and adolescents in the Netherlands. While multiple vaccines against IMD-B are licensed by the European Medicines Agency, none of them is currently part of the Dutch National Immunization Program (NIP). We evaluated the clinical impact and cost-effectiveness of different IMD-B vaccination strategies for Dutch infants and adolescents. METHODS: We developed a static, single-cohort Markov model to estimate the lifetime number of IMD-B cases and deaths prevented, as well as the incremental cost-effect

An observational, cohort, multi-centre, open label phase IV extension study comparing IPV immune responses to preschool dTaP-IPV booster vaccines in children whose mothers received or did not receive an IPV-containing pertussis vaccine during pregnancy in England.

A diphtheria-tetanus-acellular pertussis-inactivated polio combination vaccine (dTaP-IPV) was offered as part of the UK antenatal vaccination programme from 2012 to July 2024. Prior research established that infants of mothers who received a dTaP-IPV vaccine in pregnancy have significantly reduced poliovirus-specific neutralising antibodies after their primary immunisation series compared with infants of non-dTaP-IPV vaccinated mothers. We investigated whether sufficient poliovirus-specific neutralising antibody titres are achieved in these children following the pre-school dTaP-IPV booster va

Pulmonary Abscess Caused by Neisseria meningitidis: A Case Report.

Neisseria meningitidis is a Gram-negative diplococcus commonly associated with meningitis or septicemia. However, it may rarely cause pulmonary abscesses, particularly in adults with risk factors such as smoking, chronic alcoholism, or underlying lung disease. The differential diagnosis should include other bacterial etiologies, and laboratory confirmation is essential. Recognition of atypical pulmonary presentations is crucial for appropriate management and mandatory reporting, given the potential for severe progression and the need for chemoprophylaxis among contacts. We report the case of a
