Type 5 diabetes mellitus: nutritional-imprinted β-cell insufficiency, diagnostic gaps, and emerging therapeutic strategies.

Historical descriptions of malnutrition-related diabetes mellitus (MRDM) have regained attention in contemporary discourse, with the term type 5 diabetes mellitus (T5DM) increasingly used as a harmonized research construct to describe insulin-deficient diabetes associated with early-life undernutrition. Although the International Classification of Diseases, 11th Revision (ICD-11) includes categories related to malnutrition-associated diabetes, the pathophysiological interpretation, diagnostic boundaries, and therapeutic implications of T5DM remain incompletely defined, resulting in variable cl

Malnutrition-related diabetes mellitus: Rushing toward "type 5" amid unresolved questions and limited evidence.

AIM: To critically evaluate the historical context, diagnostic ambiguity, epidemiological relevance, and recent proposals to reclassify malnutrition-related diabetes mellitus (MRDM) amid changing nutritional landscapes and existing metabolic data. METHODS: This review synthesizes data from early clinical reports, WHO documents, epidemiological studies, and recent literature on malnutrition and diabetes. Emphasis is placed on diagnostic inconsistencies, the overuse of low BMI as a surrogate for malnutrition, and evolving nutrition trends in India and low- and middle-income countries (LMICs).

Type 5 diabetes: A comprehensive review to understand the basis of diabetes of poverty.

Malnutrition-Related Diabetes Mellitus (MRDM) is a subtype of diabetes associated with chronic malnutrition, primarily observed in low- and middle-income countries (LMICs) and impoverished populations. The disease is characterized by onset in young, lean patients with a history of chronic undernutrition, insulin deficiency, persistent insulin sensitivity, and resistance to ketosis. MRDM was recognized by the World Health Organization (WHO) in 1985, then removed from classification in 1999 due to a lack of evidence. Emerging evidence has revealed a unique pathophysiology, prompting the Internat

Diabetes with low body mass index in India: A secondary analysis of NFHS-5 data.

BACKGROUND: The International Diabetes Federation (IDF) recently recognized "Type 5 Diabetes" ("T5DM") as a distinct entity. Proponents estimate a burden of 6 million cases in India. However, the applicability of this classification in the Indian context, which is dominated by Lean Type 2 Diabetes (T2D), remains debated. METHODS: We conducted a secondary analysis of the National Family Health Survey-5 (2019-2021), comprising 788,974 respondents, to estimate the prevalence of young and middle-aged people (15-49 years) with low BMI Diabetes in India. The proxy phenotype was defined as: BMI <18.

Could DTI Unlock the Mystery of Subjective Tinnitus: It's Time for Parameters That Go A Little Out of the Routine.

OBJECTIVES: Immune complexes (ICs) from sera of juvenile idiopathic arthritis (JIA) patients show increased complement opsonisation; however, a definitive role for involvement of the classical or alternative pathway is not entirely clear. To delineate the role of these pathways, we measured activated complement products bound to circulating IC (CICs) in the sera of JIA patients. METHODS: Sera from 100 JIA patients and 22 healthy children were collected. C1q, C4, C3, C3d, and membrane attack complex (MAC) bound to CICs were measured by enzyme-linked immunosorbent assay. Data was compared to Ig

Severe malnutrition as a cause of transient carbohydrate metabolism disorders which evolved into hyperosmolar hyperglycaemic state.

The hyperosmolar hyperglycaemic state (HHS) is a very severe condition characterised by hyperosmolality, hyperglycaemia and dehydration without significant ketosis. The article presents the case of a 14.5-year-old cachectic patient with diagnosed HHS. Appropriate treatment per the ISPAD Guidelines was implemented. After metabolic stabilisation was achieved, the patient was transferred for further treatment to the Pediatric Gastroenterology Department due to her life-threatening cachexia. Normal glucose levels were observed during hospitalisation and the patient required no further insulin supp

An Atypical Form of Diabetes Among Individuals With Low BMI.

OBJECTIVE: Diabetes among individuals with low BMI (<19 kg/m2) has been recognized for >60 years as a prevalent entity in low- and middle-income countries (LMICs) and was formally classified as "malnutrition-related diabetes mellitus" by the World Health Organization (WHO) in 1985. Since the WHO withdrew this category in 1999, our objective was to define the metabolic characteristics of these individuals to establish that this is a distinct form of diabetes. RESEARCH DESIGN AND METHODS: State-of-the-art metabolic studies were used to characterize Indian individuals with "low BMI diabetes" (LD

Atypical forms of diabetes mellitus in Africans and other non-European ethnic populations in low- and middle-income countries: a systematic literature review.

BACKGROUND: Atypical presentations of diabetes mellitus (DM) have been reported in non-European ethnic populations under various names. It is unclear whether those names are used for the same or different clinical phenotypes. Unclear terminology may lead to inappropriate treatment and an underestimation of the burden caused by atypical diabetes phenotypes overlapping with classic types of diabetes. This review aimed to describe the terms used for atypical forms of diabetes and to investigate whether the terms are used for similar or different phenotypes. METHODS: PubMed and Scopus were search

Ketosis-resistant diabetes: a rare case in unlikely territory.

BACKGROUND: Ketosis-resistant diabetes is a syndrome that has undergone numerous classification schemes in the past. In 1979, the National Diabetes Data Group (NDDG) introduced an association of malnutrition and diabetes. In 1985, the World Health Organization (WHO) created a new diabetes category called malnutrition-related diabetes mellitus (MRDM). MRDM consisted of two subclasses: fibrocalculous pancreatic diabetes (FCPD) and protein-deficient pancreatic diabetes (PDPD). Ketosis-resistant diabetes of the young (KRDY) was included in the subclass of PDPD. We report a rare case of a 37-year-o

A sub-Saharan African perspective of diabetes.

Diabetes mellitus is an important and increasing cause of morbidity and mortality in sub-Saharan Africa. Accurate epidemiological studies are often logistically and financially difficult, but processes of rural-urban migration and epidemiological transition are certainly increasing the prevalence of type 2 diabetes. Type 1 disease is relatively rare, although this may be related to high mortality. This diabetic subgroup appears to present at a later age (by about a decade) than in Western countries. Variant forms of diabetes are also described in the continent; notably 'atypical, ketosis-prone

No association of SUMO4 M55V with autoimmune diabetes in Asian-Indian patients.

Autoimmune diabetes [type 1 diabetes mellitus (T1DM), latent autoimmune diabetes in adults (LADA) and part of malnutrition-related diabetes] has been shown to have genetic predisposition. Studies in IDDM 5 have lead to the discovery of a novel polymorphism 163 A-->G, of SUMO4 (small ubiquitin-related modifier) gene, associated with risk to T1DM in Asians, but not in Caucasians. We studied patients with T1DM (n = 134), patients with LADA (n = 101), patients with malnutrition-modulated diabetes mellitus (n = 66) and patients with fibrocalculous pancreatic diabetes (n = 43) and healthy controls s

Ketosis resistance in under thirty diabetic subjects.

Young onset diabetic subjects in tropical developing countries include a group of subjects who exhibits a characteristic ketosis resistance termed as Malnutrition Related Diabetes Mellitus (MRDM) by the WHO Study Group. The mechanism for this resistance to ketosis is still uncertain. To understand this mechanism we have studied the serum responses of glucose, non-esterified fatty acid (NEFA) and triglyceride (TG) to intravenous fat emulsion in newly diagnosed 8 fibrocalculous pancreatic diabetes (FCPD) and 11 low insulin secretory (LIS) subjects under 30 years of age along with 27 age-matched

A pancreaticographic study of malnutrition-related diabetes mellitus.

Pancreatic involvement is considered to be the hallmark of malnutrition-related diabetes mellitus (MRDM). Of the 2 subgroups of the disease, fibrocalculous pancreatic diabetes (FCPD) is characterized by pancreatic calcification. The nature of pancreatic abnormalities in MRDM have not been studied extensively in Indian patients. The present study was designed to compare pancreatic abnormalities (exocrine and endocrine) including endoscopic retrograde pancreaticography in patients with FCPD and protein deficient pancreatic diabetes (PDPD), in relation to controls. Ten patients each of FCPD and P
