AA in Obesity and Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD) Nonalcoholic Fatty Liver Disease (NAFLD).

Obesity and MASLD (NAFLD) are chronic, low-grade systemic inflammatory diseases resulting from complex interactions among diet, adipose tissue, gut microbiota, liver, muscle, pancreas, and the brain. Central to this integrative framework are AA and other polyunsaturated fatty acids (PUFAs), which regulate inflammation, insulin sensitivity, neuroendocrine signaling, and energy homeostasis.Excess consumption of energy-dense foods rich in saturated and trans fats leads to adipocyte hypertrophy, altered membrane fluidity, increased antigenicity, and enhanced recruitment of macrophages and T cells

Anti-N-Methyl-D-Aspartate Receptor Encephalitis With Autonomic Instability and Focal Seizures Without Detectable Ovarian Teratoma.

Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is a severe but potentially reversible form of autoimmune encephalitis, and it typically affects young women. It can be challenging to differentiate from other conditions that cause symptoms such as seizures and psychiatric and autonomic manifestations. We present the case of a previously healthy young woman without any significant past medical history who developed acute-onset, progressive neuropsychiatric disturbance, focal impaired awareness seizures, cognitive impairment, and intermittent autonomic disturbance manifesting as sinu

Particle-size-associated differences in follicular accessibility, granulosa cell oxidative injury, and oocyte developmental competence following polystyrene exposure.

Polystyrene particles are emerging environmental contaminants with potential reproductive hazards, but whether particle size influences access to the follicular microenvironment and subsequent oocyte dysfunction remains unclear. Here, fluorescent and non-fluorescent PS particles with nominal diameters of 0.1, 0.5, and 1.0 μm were compared using mouse cumulus-oocyte complexes, ovarian tissue, and KGN granulosa-like cells under equal-mass and theoretically matched particle-number exposure conditions. Smaller PS particles showed greater follicular-associated fluorescence, with particle-associated

Extensive Parenchymal Brain Calcification in a Patient With Autosomal Dominant Polycystic Kidney Disease and Stage 5 Chronic Kidney Disease.

Extensive parenchymal brain calcification is uncommon in chronic kidney disease (CKD): intracranial vascular calcification affects up to 10% of uremic patients, but widespread Fahr-type parenchymal deposits are rare and, where reported, have occurred in patients with a coexisting hypoparathyroid or parathyroid-hormone (PTH)-resistant state. We describe a man in his 50s with autosomal dominant polycystic kidney disease (ADPKD) and Stage 5 CKD who presented with word-finding difficulty, gait imbalance, and myoclonus and was found to have extensive symmetric calcification of the basal ganglia, th

Primary Amenorrhea Revealing Adamantinomatous Craniopharyngioma Associated With Central Hypogonadotropic Hypogonadism in an Adolescent Girl: A Case Report.

Adamantinomatous craniopharyngioma is an uncommon benign epithelial tumor arising from remnants of Rathke's pouch within the sellar-suprasellar region. Although patients typically present with headache, visual impairment, growth retardation, or multiple endocrine abnormalities, primary amenorrhea as the initial clinical presentation is unusual, particularly in the absence of neurological symptoms. We report the case of a 15-year-old adolescent girl who presented with primary amenorrhea and delayed pubertal development without headache, visual complaints, or other focal neurological deficits. Recurring and isolated intronic FGFR1 variants as an underdiagnosed cause of congenital hypogonadotropic hypogonadism.

BACKGROUND: Congenital hypogonadotropic hypogonadism (CHH) is a genetically heterogeneous disorder characterized by absent or delayed puberty and infertility due to gonadotropin-releasing hormone deficiency. While loss-of-function variants in FGFR1 are the most commonly known genetic cause of CHH, diagnostic screening typically focuses on coding exons, potentially overlooking pathogenic variants in noncoding regions. METHODS: We investigated 4 unrelated families with CHH. Family 1 included a mother and 3 daughters presenting with variable reproductive phenotypes and incomplete penetrance of a

NNMT enhances hyperandrogenism in polyendocrine metabolic ovarian syndrome through epigenetic remodelling.

Polyendocrine metabolic ovarian syndrome (PMOS) features intertwined reproductive, endocrine and metabolic defects, yet its initiating mechanisms remain incompletely understood. Here we show that an epigenetic mechanism driven by nicotinamide N-methyltransferase (NNMT) in granulosa cells contributes to disease progression in PMOS. We observe that Nnmt overexpression in mouse granulosa cells reduces histone H3 lysine 27 trimethylation (H3K27me3), leading to increased retinoic acid receptor beta (RARB) expression. RARB, in turn, amplifies ovarian androgen synthesis. The resulting androgen excess

Microglia as candidate links between hormonal signals and neuronal circuits in polycystic ovary syndrome: evidence gaps and a research agenda.

Polycystic ovary syndrome (PCOS) combines reproductive endocrine dysfunction with metabolic, autonomic, and neurobehavioral phenotypes, yet the contribution of brain microglia remains uncertain. We conducted a structured narrative review that separates human PCOS evidence, PCOS-like animal findings, PCOS-related cellular observations, and non-PCOS mechanistic bridges. Only three heterogeneous PCOS-like studies directly assessed microglial outcomes, and none established a conserved microglial state, microglial necessity, a complete hormone-microglia-neuron causal chain, or human relevance. Nasopharyngeal diffuse large b-cell lymphoma in a teenage presenting with cavernous sinus syndrome: A case report.

Primary lymphoma of the nasopharynx is uncommon and is more commonly seen as an association with other primary pathologies. Nasopharyngeal lymphoma is an aggressive tumor and can affect all age groups but is more commonly seen in middle-aged and older individuals. Majority of these tumors belong to the non-Hodgkin's group. Cavernous sinus syndrome (CSS) is a rare but severe complication indicating advanced aggressive stage of disease with direct extension of tumor into the cavernous sinus. Our patient is a young teenage 18-years old presented to emergency department with the symptoms of left j

Delusion of Pregnancy: An Updated Systematic Review.

Delusion of pregnancy (DoP) is a rare psychopathological symptom characterised by the fixed, delusional conviction of being pregnant in the absence of objective evidence. The systematic review by Bera and Sarkar remains, to this day, the principal quantitative reference on the phenomenon, based on 84 cases published up to August 2014. The aim of the present work is to update this case series by including literature published between September 2014 and August 2026, and to analyse any clinical and aetiological differences that emerge relative to the original sample. Thirty-two new cases were ide

Pituitary Apoplexy Presenting as a Transient Ischemic Attack: An Endocrine Emergency.

Pituitary apoplexy (PA) is an endocrine emergency caused by hemorrhage or infarction of a pituitary adenoma and occurs more frequently in males and patients with macroadenomas. It typically presents acutely with headache, visual disturbances, or impaired consciousness. Focal neurological deficits are uncommon and may be mistaken for a primary cerebrovascular event. Acute secondary adrenal insufficiency is a major contributor to hemodynamic instability and mortality. Delayed Ovarian Teratoma Resection in Anti-N-Methyl-D-Aspartate (NMDA) Receptor Encephalitis Despite Immunotherapy: Implications for Early Source Control.

Anti-N-methyl-D-aspartate (NMDA) receptor encephalitis is a severe autoimmune neurologic disorder frequently associated with ovarian teratomas in adult women. Tumor removal is a central component of management and is associated with improved neurologic outcomes. We report a 33-year-old woman with confirmed anti-NMDA receptor encephalitis complicated by refractory seizures, movement disorder, and prolonged ventilator dependence in whom ovarian teratoma resection was delayed after the family initially declined surgical intervention. Prior to surgical intervention, the patient received high-dose

Functional hypothalamic amenorrhea: A call for global awareness and research.

Functional hypothalamic amenorrhea (FHA) is a common yet under-recognized cause of secondary amenorrhea resulting from reversible suppression of the hypothalamic-pituitary-ovarian axis. Triggered by chronic stress, low energy availability, disordered eating, and/or excessive exercise, FHA leads not only to anovulation and hypoestrogenism but also to broader neuroendocrine disruption involving the hypothalamic-pituitary-adrenal and thyroid axes. Although historically associated with extreme trauma and the "female athlete triad," FHA occurs worldwide and remains significantly underdiagnosed acro

Fertility preservation in girls and adolescents with Turner syndrome: lessons for reproductive care in differences of sex development - a mini-review.

Current knowledge regarding fertility preservation (FP) in girls is derived predominantly from pediatric oncology cohorts; however, FP is increasingly being applied to non-oncological conditions associated with impaired fertility, including differences of sex development (DSD). Turner syndrome (TS), characterized by accelerated germ cell depletion beginning during fetal life, represents the DSD condition with the greatest clinical experience in FP. This mini-review summarizes current evidence on ovarian tissue cryopreservation and oocyte cryopreservation in TS. Available data support the techn

Long-term cyclic ectopic adrenocorticotropic hormone syndrome with extreme fluctuations and adrenal insufficiency.

A 77-year-old woman presented with worsening glycemic control and hypokalemia and was found to have adrenocorticotropic hormone (ACTH)-dependent hypercortisolism with loss of diurnal cortisol variation and failure of suppression on low- and high-dose dexamethasone suppression tests. Computed tomography (CT) demonstrated bilateral adrenal enlargement, whereas pituitary magnetic resonance imaging and chest CT failed to identify a source of ACTH secretion, leading to a diagnosis of occult ectopic ACTH syndrome. During 15 years of follow-up, she exhibited cyclic fluctuations in ACTH and cortisol c

Hydrocephalus-related amenorrhoea: A scoping review.

STUDY OBJECTIVE: Hydrocephalus is a rare, potentially reversible cause of gonadotropic dysfunction, amenorrhoea, and delayed puberty; fewer than 60 cases have been reported. This scoping review aimed to characterize hydrocephalus-associated amenorrhoea, assess time to postoperative menstrual (re)occurrence, and identify factors influencing recovery. METHODS: This scoping review followed Joanna Briggs Institute methodology, with a systematic search of PubMed, Embase, Google Scholar, and Web of Science for cases of amenorrhoea associated with hydrocephalus. Two reviewers independently screened

The Many Faces of Acromegaly: Understanding Its Clinical and Biological Heterogeneity.

BACKGROUND: Acromegaly, in 95% of cases resulting from a PitNET that hypersecretes growth hormone (GH), is constantly revisited in the literature due to the repercussions on morbidity and mortality and difficulties that persist in early diagnosis. SUMMARY: To shorten the interval between the onset of acromegaly manifestations and the diagnosis, our goal was to summarize the main situations that deviate from the typical presentation of the disease, highlighting the variability in phenotypic features, biochemical findings, and GH tumor expression. These non-classic findings contribute to the di

Palatable-Food-Driven Top-Down Circuit Inhibits PVNCRF Activity to Mitigate Stress Via Peri-PVNCRFR1 Neurons.

Stress is a major precipitating factor for emotional disorders, including anxiety. To cope with stress, individuals frequently engage in hedonic behaviors, such as eating palatable food, which provide transient relief from psychological distress and may protect against the development of pathology. However, the neural mechanisms by which hedonic experience counteracts stress-induced anxiety remain poorly understood. Here, we identify a neural circuit functionally connecting the prefrontal cortex (PFC) to the paraventricular nucleus (PVN) of the hypothalamus that mediates stress mitigation thro

Ovarian endometrioid adenofibroma in a perimenopausal woman with intermittent vaginal bleeding: a case report.

Ovarian endometrioid adenofibroma (EA) is an exceptionally rare benign epithelial tumor of the ovary with nonspecific clinical and imaging manifestations, and its diagnosis relies mainly on pathological examination. This case describes a 55-year-old perimenopausal woman with a 1-year history of intermittent vaginal bleeding and a progressively enlarging right adnexal mass. Imaging revealed a solid mass in the right adnexal region with minimal blood flow signal. Serum tumor markers were within normal ranges, and the sex hormone profile showed no evidence of estrogen excess. Serial ultrasonograp
