Long-term cyclic ectopic adrenocorticotropic hormone syndrome with extreme fluctuations and adrenal insufficiency.

A 77-year-old woman presented with worsening glycemic control and hypokalemia and was found to have adrenocorticotropic hormone (ACTH)-dependent hypercortisolism with loss of diurnal cortisol variation and failure of suppression on low- and high-dose dexamethasone suppression tests. Computed tomography (CT) demonstrated bilateral adrenal enlargement, whereas pituitary magnetic resonance imaging and chest CT failed to identify a source of ACTH secretion, leading to a diagnosis of occult ectopic ACTH syndrome. During 15 years of follow-up, she exhibited cyclic fluctuations in ACTH and cortisol c

Autoimmunity induced by epigenome alteration affecting thymic Treg cell development.

Methyl-CpG-binding domain protein 3 (MBD3), an essential component of the nucleosome remodeling and deacetylase (NuRD) complex, controls chromatin accessibility at lineage-specific transcriptional enhancers and promoters during hematopoietic cell differentiation. Here, we have shown that MBD3 deficiency in the T cell compartment resulted in spontaneous development of T cell-mediated multiorgan autoimmune diseases including fatal autoimmune myocarditis. The MBD3-deficient mice notably showed a significant reduction of CD25+Foxp3+ regulatory T (Treg) cells in both the thymus and periphery withou

Underutilization of parathyroidectomy among dialysis patients with tertiary hyperparathyroidism.

INTRODUCTION: Tertiary hyperparathyroidism (THPT) is often diagnosed post-kidney transplant (KT); Parathyroidectomy is curative. Pre-KT THPT increases post-KT THPT risk; however, epidemiology among dialysis patients remains unknown. We examined THPT prevalence, risk factors, and treatment among dialysis patients. METHODS: We identified THPT patients using the United States Dialysis Outcomes and Practice Patterns Study phases 4-7, defining THPT as multiple calcium levels>10 mg/dL with PTH≥70 pg/mL. We identified THPT and calcimimetic treatment risk factors using adjusted multivariable logistic

Patient-specific zebrafish models reveal a complex phenotypic spectrum in NBAS-associated atypical osteogenesis imperfecta.

Neuroblastoma amplified sequence gene (NBAS) variants are associated with short stature, optic atrophy, and Pelger-Huët anomaly (SOPH) syndrome. We previously identified compound heterozygous variants in NBAS to cause atypical Osteogenesis Imperfecta (OI), with these patients presenting with short stature, developmental delay, and recurrent long-bone fractures. However, skeletal disease progression due to these variants and the disease mechanisms underlying NBAS-associated OI remain poorly understood. Here, we provide a clinical update on previously identified patients and investigate the role

Assessment of health literacy across types of hyperparathyroidism.

BACKGROUND: Health literacy is the ability to obtain, process, and understand basic health information necessary to make informed decisions. Low health literacy has been associated with delays in diagnosis and treatment, reduced adherence, and worse clinical outcomes. This study aimed to assess health literacy among individuals with hyperparathyroidism. METHODS: The National Institutes of Health All of Us Research database was used to evaluate health literacy in adults with primary, renal secondary, nonrenal secondary, and tertiary hyperparathyroidism. Health literacy was assessed using 3 que

Reversible Thyroid Nodule in Van Wyk-Grumbach Syndrome: An Unusual Case Report.

Van Wyk-Grumbach syndrome (VWGS) is a rare complication of longstanding, untreated primary hypothyroidism in children. It is characterized by isosexual precocious puberty, delayed bone age, and multicystic ovarian enlargement despite the presence of severe hypothyroidism. Although thyroid enlargement is common, a discrete thyroid nodule has not previously been documented in this syndrome. This study presents a 6-year-old Saudi girl who exhibited a 2-day history of vaginal hemorrhage and a history of breast development for 6 months. The physical examination demonstrated a palpable right thyroid

Elemental analysis in menstrual effluent of women with PCOS: association with oxidative stress mechanisms: a case-control study.

Polycystic ovarian syndrome (PCOS) is a common endocrine disorder in women of reproductive age. Metal ions can disrupt antioxidant defenses, alter the endometrial environment, and contribute to its pathophysiology. To compare element levels in menstrual effluent (ME) between women with PCOS and healthy controls, and to assess their association with antioxidant defence mechanisms, a case-control study was conducted among women diagnosed with PCOS according to the Rotterdam criteria. ME was collected and analysed by Inductively Coupled Plasma Mass Spectrometry (ICP-MS) for elements. Antioxidant

Discontinuation of palopegteriparatide in chronic hypoparathyroidism without rebound hypocalcemia.

A woman with chronic hypoparathyroidism following total thyroidectomy was treated with palopegteriparatide, a long-acting parathyroid hormone replacement therapy, for more than 4 years. Previous reports have suggested that stopping parathyroid hormone therapy may lead to severe rebound hypocalcemia and increased calcium requirements due to a "hungry bone syndrome." After discontinuation of palopegteriparatide, the patient resumed conventional treatment with calcium supplements and active vitamin D at doses similar to those used before starting hormone therapy; the alfacalcidol dose was subsequ

Venous thromboembolism in Cushing syndrome: current evidence and a practical thromboprophylaxis algorithm.

Cushing syndrome (CS) is a rare endocrine disorder associated with a markedly increased risk of venous thromboembolism (VTE). Chronic hypercortisolism induces a prothrombotic state through activation of coagulation pathways, impaired fibrinolysis, and endothelial dysfunction. VTE represents a major source of morbidity and mortality in this population, yet optimal strategies for prevention and treatment remain insufficiently standardized. To summarize current evidence on the epidemiology, pathophysiology, risk factors, prevention, and treatment of VTE in patients with endogenous CS and to propo

Severe growth retardation in Dent disease: a case report of a novel OCRL variant.

INTRODUCTION: Dent disease type 2 (DD-2) is a rare X-linked recessive renal tubulopathy caused by mutations in the OCRL gene. Clinically it is characterised by low molecular weight proteinuria (LMWP), hypercalciuria, and nephrocalcinosis. The phenotypic spectrum of DD-2 is expanding with the identification of new pathogenic variants. CASE REPORT: A 12-year-old male presenting with marked short stature and poor weight gain was found to have LMWP, haematuria, hypercalciuria, and bilateral nephrocalcinosis on initial clinical evaluation. Renal function was normal. There was no relevant family hi

Reframing traumatic brain injury and spinal cord injury as chronic multisystem disorders: toward integrated, long-term care.

Traumatic brain injury (TBI) and spinal cord injury (SCI) affect more than 220 million people worldwide and are associated with cardiometabolic, endocrine, neurological, and psychiatric sequelae for years to decades after an acute injury. However, most health systems still treat neurotrauma (TBI and SCI, collectively) as a time-limited event, leaving long-term surveillance of these patients fragmented or absent, which leads to preventable mortality and avoidable readmissions. TBI and SCI in adults should be reclassified as acute-onset multisystem chronic-risk conditions. Existing cardiovascula

Case Report: Veno-Arteriovenous Extracorporeal Membrane Oxygenation for Management of Pheochromocytoma-Induced Cardiac Arrest in a Post-Partum Patient.

A 34-year-old woman, 2 days post-partum, was admitted to the intensive care unit (ICU) for management of hypertensive crisis and respiratory failure. An 18-cm left adrenal mass was found on computed tomography (CT), suspicious for a pheochromocytoma. After cardiac arrest, the patient was placed on veno-arteriovenous extracorporeal membrane oxygenation (V-AV ECMO) given recurrent destabilizing arrhythmias. After successful ECMO decannulation, the patient underwent an uncomplicated resection of her adrenal mass. Although VA-ECMO has been used to support patients with pheochromocytoma, this appea

Low Anterior Access Approach for Single-Port Retroperitoneal Robot-Assisted Adrenalectomy: Surgical Technique and Early Outcomes.

INTRODUCTION: The da Vinci single-port (SP) system has been utilized across various surgical procedures. However, a standardized technique for SP robot-assisted adrenalectomy (SP-RAA) remains unestablished. Herein, we report our technique for retroperitoneal SP-RAA via the low anterior access (LAA) approach. PATIENTS AND SURGICAL TECHNIQUE: Nine patients underwent SP-RAA using our standardized procedure. Patients were placed in a modified supine position. The primary incision was typically located two fingerbreadths cranial to a point two-thirds laterally along the spino-umbilical line. The o

Vitamin D status and inflammatory Interleukins in women with and without polyendocrine metabolic ovarian syndrome: a cross-sectional exploratory study.

BACKGROUND: Polyendocrine Metabolic Ovarian Syndrome (PMOS), formerly referred to as polycystic ovary syndrome (PCOS), is an intricate endocrine disorder, defined by a constellation of hormonal, metabolic, and reproductive irregularities. Vitamin D deficiency and increased levels of interleukin-6 (IL-6) and interleukin-8 (IL-8) have been implicated in the pathogenesis of PMOS. Although vitamin D deficiency and low-grade inflammation have each been associated with PMOS, the relationship between vitamin D status and circulating inflammatory cytokines remains incompletely characterized. Thus, thi

Investigating the effect of diet-induced prediabetes on the HPA-axis, sympathetic nervous system and endogenous opioids in male Sprague-Dawley rats.

BACKGROUND: Prediabetes is associated with metabolic dysfunction that may disrupt neuroendocrine regulation before the onset of overt type 2 diabetes mellitus. Although previous studies have independently examined alterations in the hypothalamic-pituitary-adrenal (HPA) axis, sympathetic nervous system (SNS), and endogenous opioid system during prediabetes, the temporal interactions among these systems during disease progression remain poorly understood. This study aimed to investigate changes in these interconnected neuroendocrine pathways in a diet-induced prediabetic rats. METHODS: Male Spr
