Androgen driven organoids for the male urogenital system.

Androgen signalling plays a central role in the development and function of the male urogenital system, including the adrenal gland, prostate, and testis. Disruption of the androgen axis underlies diseases ranging from infertility to castration-resistant prostate cancer, yet current models fail to fully display its multi-organ and hormone-driven complexity. Organoid systems provide a promising platform to model androgen-responsive tissues while preserving three-dimensional structure and cellular heterogeneity, with the ability for long-term culture and cryopreservation. Recent advances have en

Hypomagnesemia in Hypoparathyroidism, Sensorineural Deafness, and Renal Dysplasia Syndrome Caused by Pathogenic Variants of GATA3.

BACKGROUND: Heterozygous pathogenic variants in the transcription factor GATA binding protein 3 (GATA3) cause hypoparathyroidism, sensorineural deafness and renal dysplasia (HDR) syndrome, also known as Barakat syndrome. Although not typically associated with the disease, hypomagnesemia has been reported. We aimed to determine its prevalence in HDR syndrome and explore how GATA3 impacts magnesium balance. METHODS: Retrospective clinical and laboratory data from 66 patients with GATA3 variants were collected via European Rare Kidney Diseases Reference Network (ERKNet) and European Reference Ne

Diagnostic challenges of masked arginine vasopressin deficiency in neurosarcoidosis complicated by sodium-glucose cotransporter 2 inhibitor use.

A wide variety of endocrine disorders caused by sarcoidosis have been reported to date. We report the case of an 82-year-old male patient who presented with hypercalcemia and anterior pituitary hormone deficiency at the time of his sarcoidosis diagnosis and was subsequently diagnosed with arginine vasopressin deficiency (AVP-D) 6 months after the initiation of glucocorticoid therapy. Magnetic resonance imaging revealed an enlarged pituitary gland, thickened pituitary stalk, and loss of the posterior pituitary bright spot, raising the suspicion of hypothalamic-pituitary sarcoidosis. Despite imp

Anti-N-Methyl-D-Aspartate Receptor Encephalitis With Autonomic Instability and Focal Seizures Without Detectable Ovarian Teratoma.

Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is a severe but potentially reversible form of autoimmune encephalitis, and it typically affects young women. It can be challenging to differentiate from other conditions that cause symptoms such as seizures and psychiatric and autonomic manifestations. We present the case of a previously healthy young woman without any significant past medical history who developed acute-onset, progressive neuropsychiatric disturbance, focal impaired awareness seizures, cognitive impairment, and intermittent autonomic disturbance manifesting as sinu

Thyroid and Parathyroid Dysfunction Following Larynx-Trachea-Thyroid-Parathyroid Allotransplantation.

Larynx-trachea-thyroid-parathyroid allotransplantation can restore essential laryngeal functions, but postoperative endocrine changes remain poorly understood. This study assessed thyroid and parathyroid function in four recipients. All developed early thyroid hormone alterations consistent with euthyroid sick syndrome, followed by variable endocrine outcomes. Two experienced thyrotoxicosis associated temporally with graft rejection, while another developed hypothyroidism and elevated parathyroid hormone during late rejection. Transient parathyroid hormone suppression was also observed. In con

From genotype to phenotype: reverse phenotyping of a novel INSR variant in a large family with hyperinsulinemic hypoglycemia.

The insulin receptor gene (INSR) encodes a ligand-activated tyrosine kinase that plays a central role in glucose homeostasis. Pathogenic variants in INSR are associated with a spectrum of insulin resistance syndromes. Here we report a novel INSR variant in a large multigenerational family. A 53-year-old woman presented to the endocrinology clinic with recurrent hypoglycemic episodes. Exome sequencing was performed for the index case, and variant segregation analysis was conducted in family members by Sanger sequencing. Genetic testing revealed a likely pathogenic variant in exon 19 of the INSR

Particle-size-associated differences in follicular accessibility, granulosa cell oxidative injury, and oocyte developmental competence following polystyrene exposure.

Polystyrene particles are emerging environmental contaminants with potential reproductive hazards, but whether particle size influences access to the follicular microenvironment and subsequent oocyte dysfunction remains unclear. Here, fluorescent and non-fluorescent PS particles with nominal diameters of 0.1, 0.5, and 1.0 μm were compared using mouse cumulus-oocyte complexes, ovarian tissue, and KGN granulosa-like cells under equal-mass and theoretically matched particle-number exposure conditions. Smaller PS particles showed greater follicular-associated fluorescence, with particle-associated

Endocrine therapies and hormonal dysfunction after traumatic brain injury: A scoping review.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of morbidity and mortality worldwide, with current management strategies primarily focused on preventing secondary injury rather than modifying disease progression. Post-traumatic endocrine dysfunction has emerged as a significant and potentially treatable contributor to outcomes after TBI. METHODS: A scoping review was conducted following PRISMA-ScR guidelines. PubMed, Embase, Scopus, and Web of Science were searched from inception to September 2025 for clinical studies evaluating endocrine dysfunction and endocrine-targeted therapi

Psychosocial stress and autoimmune thyroid disease: a systematic review of clinical phenotypes and vulnerability factors.

Psychological stress has been proposed as a potential trigger or modulator of autoimmune thyroid diseases, but evidence remains fragmented. This systematic review examined the relationship between stress and thyroid autoimmunity, with particular attention to Hashimoto's thyroiditis and Graves' disease, highlighting the critical methodological distinction between stressor exposure (objective events) and the subjective stress response (psychological burden). PubMed, Embase, Scopus, and PsycINFO were searched for controlled human studies; 25 studies involving a total of N = 117,034 participants m

Expanding the Clinical and Molecular Spectrum of Cartilage-Hair Hypoplasia in the Mexican Population.

Cartilage-hair hypoplasia (CHH) is a rare autosomal recessive skeletal dysplasia caused by variants in RMRP (RNA component of mitochondrial RNA processing endoribonuclease), which encodes a non-coding RNA. Despite its well-established clinical and molecular spectrum in some populations, data from Mexico remain limited. We report eight patients from seven unrelated families with CHH. Clinical and radiographic findings were assessed, and RMRP was analyzed by polymerase chain reaction and Sanger sequencing. All patients exhibited disproportionate short stature; prenatal long-bone shortening and s

Extensive Parenchymal Brain Calcification in a Patient With Autosomal Dominant Polycystic Kidney Disease and Stage 5 Chronic Kidney Disease.

Extensive parenchymal brain calcification is uncommon in chronic kidney disease (CKD): intracranial vascular calcification affects up to 10% of uremic patients, but widespread Fahr-type parenchymal deposits are rare and, where reported, have occurred in patients with a coexisting hypoparathyroid or parathyroid-hormone (PTH)-resistant state. We describe a man in his 50s with autosomal dominant polycystic kidney disease (ADPKD) and Stage 5 CKD who presented with word-finding difficulty, gait imbalance, and myoclonus and was found to have extensive symmetric calcification of the basal ganglia, th

Primary Amenorrhea Revealing Adamantinomatous Craniopharyngioma Associated With Central Hypogonadotropic Hypogonadism in an Adolescent Girl: A Case Report.

Adamantinomatous craniopharyngioma is an uncommon benign epithelial tumor arising from remnants of Rathke's pouch within the sellar-suprasellar region. Although patients typically present with headache, visual impairment, growth retardation, or multiple endocrine abnormalities, primary amenorrhea as the initial clinical presentation is unusual, particularly in the absence of neurological symptoms. We report the case of a 15-year-old adolescent girl who presented with primary amenorrhea and delayed pubertal development without headache, visual complaints, or other focal neurological deficits. Recurring and isolated intronic FGFR1 variants as an underdiagnosed cause of congenital hypogonadotropic hypogonadism.

BACKGROUND: Congenital hypogonadotropic hypogonadism (CHH) is a genetically heterogeneous disorder characterized by absent or delayed puberty and infertility due to gonadotropin-releasing hormone deficiency. While loss-of-function variants in FGFR1 are the most commonly known genetic cause of CHH, diagnostic screening typically focuses on coding exons, potentially overlooking pathogenic variants in noncoding regions. METHODS: We investigated 4 unrelated families with CHH. Family 1 included a mother and 3 daughters presenting with variable reproductive phenotypes and incomplete penetrance of a

NNMT enhances hyperandrogenism in polyendocrine metabolic ovarian syndrome through epigenetic remodelling.

Polyendocrine metabolic ovarian syndrome (PMOS) features intertwined reproductive, endocrine and metabolic defects, yet its initiating mechanisms remain incompletely understood. Here we show that an epigenetic mechanism driven by nicotinamide N-methyltransferase (NNMT) in granulosa cells contributes to disease progression in PMOS. We observe that Nnmt overexpression in mouse granulosa cells reduces histone H3 lysine 27 trimethylation (H3K27me3), leading to increased retinoic acid receptor beta (RARB) expression. RARB, in turn, amplifies ovarian androgen synthesis. The resulting androgen excess

Microglia as candidate links between hormonal signals and neuronal circuits in polycystic ovary syndrome: evidence gaps and a research agenda.

Polycystic ovary syndrome (PCOS) combines reproductive endocrine dysfunction with metabolic, autonomic, and neurobehavioral phenotypes, yet the contribution of brain microglia remains uncertain. We conducted a structured narrative review that separates human PCOS evidence, PCOS-like animal findings, PCOS-related cellular observations, and non-PCOS mechanistic bridges. Only three heterogeneous PCOS-like studies directly assessed microglial outcomes, and none established a conserved microglial state, microglial necessity, a complete hormone-microglia-neuron causal chain, or human relevance. Clinical and genetic heterogeneity of Triple A syndrome: a multicenter study.

Triple A syndrome (AAAS) is a rare autosomal recessive neuroendocrine disorder caused by pathogenic variants in the AAAS gene and characterized by marked clinical heterogeneity. In this multicenter retrospective study, we analyzed 23 patients from 18 families diagnosed across nine centers in Türkiye to characterize the molecular and clinical features of AAAS. Six distinct vari-ants in the AAAS gene were identified in this cohort, with truncating variants predominating. The most frequent variant was c.1066_1067delCT (p.Leu356Valfs*8), detected in 7 of 18 families, followed by the nonsense varia
