The hypothalamic orexin-medial prefrontal cortex pathway modulates susceptibility to depression in the learned helplessness paradigm in male mice.

Depression is a prevalent psychiatric disorder primarily characterized by behavioral despair and helplessness. The orexinergic system plays a crucial role in stress-induced depression; however, the precise mechanisms remain unclear. The medial prefrontal cortex (mPFC), a major target of orexinergic projections, is essential for emotional regulation. This study suggests that impaired orexinergic input to the mPFC underlies depression-like behaviors in murine models. In the learned helplessness (LH) paradigm, the majority of male mice subjected to inescapable foot shocks developed depression-lik

Eating Behavior in Menopausal Women: From Molecular to Clinical Aspects-What Changes?

The menopausal transition represents a critical period in women's lives characterized by profound hormonal, metabolic, and behavioral changes. Estrogen deficiency during menopause triggers complex alterations in eating behavior through multiple molecular mechanisms involving hypothalamic neuropeptide systems, neurotransmitter pathways, and peripheral hormonal signals. This narrative review examines the molecular basis of eating behavior changes in menopausal women, integrating evidence from basic science and clinical research. At the molecular level, declining estradiol disrupts the hypothalam

Functional hypothalamic amenorrhea: A call for global awareness and research.

Functional hypothalamic amenorrhea (FHA) is a common yet under-recognized cause of secondary amenorrhea resulting from reversible suppression of the hypothalamic-pituitary-ovarian axis. Triggered by chronic stress, low energy availability, disordered eating, and/or excessive exercise, FHA leads not only to anovulation and hypoestrogenism but also to broader neuroendocrine disruption involving the hypothalamic-pituitary-adrenal and thyroid axes. Although historically associated with extreme trauma and the "female athlete triad," FHA occurs worldwide and remains significantly underdiagnosed acro

Impact of BMI on perioperative outcomes following total thyroidectomy: a systematic review and meta-analysis.

BACKGROUND: The influence of body mass index (BMI) on perioperative outcomes following total thyroidectomy (TT) remains controversial. We performed a systematic review and meta-analysis to evaluate the association between BMI and perioperative complications after TT. METHODS: Twelve observational studies including 22,145 patients undergoing elective TT were analyzed. BMI was categorized per WHO criteria. Primary outcomes were recurrent laryngeal nerve (RLN) palsy and hypoparathyroidism. Secondary outcomes included operative time, length of stay (LOS), postoperative bleeding, and surgical site

Prediction of early postoperative hypocalcemia and hypoparathyroidism after thyroidectomy for papillary thyroid carcinoma: dual-outcome prediction models.

BACKGROUND: Postoperative hypocalcemia and hypoparathyroidism are two common complications after total or subtotal/near-total for papillary thyroid carcinoma (PTC). This study aimed to identify significant predictors of early postoperative hypocalcemia and hypoparathyroidism and develop prediction nomograms for risk stratification in PTC patients. METHODS: A total of 1036 PTC patients who underwent total or subtotal/near-total thyroidectomy were retrospectively enrolled. Multivariable logistic regression with prespecified variables was performed to identify independent predictors for two outc

[Outcomes of adult patients with mucopolysaccharidoses type Ⅰ treated by enzyme replacement therapy].

Mucopolysaccharidosis type Ⅰ was a rare inherited metabolic disorder caused by a deficiency of alpha-L-iduronidase, resulting in the incomplete degradation of glycosaminoglycans that accumulate in lysosomes, leading to tissue damage and organ dysfunction. The main treatment options for mucopolysaccharidosis type Ⅰ were hematopoietic stem cell transplantation and enzyme replacement therapy. Patients who did not receive early replacement therapy often struggle to survive into adulthood, and cases of enzyme replacement therapy initiated in adulthood were extremely rare. This article summarized an

Diabetes in Cushing's disease: age, hypercortisolism, central adiposity, and β-cell stress as determinants of glycaemia.

OBJECTIVE: Diabetes is a frequent complication of Cushing's disease, but metabolic differences between normal glucose tolerance, prediabetes, and diabetes remain incompletely characterised. We aimed to characterise glucose metabolism across these groups, assess abnormalities in patients with normal glucose tolerance, and identify clinical, hormonal, and body-composition factors associated with glycaemia. DESIGN: Cross-sectional analysis of a prospective cohort with matched healthy controls. MATERIAL AND METHODS: The study included 50 patients with Cushing's disease and 52 age- and sex-matche

Molecular mechanisms underlying the effectiveness of triiodothyroacetic acid (Triac) in patients with resistance to thyroid hormone β (RTHβ): a multicentre, international, retrospective cohort study.

BACKGROUND: Disease features in resistance to thyroid hormone beta (RTHβ), caused by THRB mutations, arise from insensitivity to thyroid hormone in tissues expressing thyroid hormone receptor (TR)β and from lifelong thyrotoxic effects in TRα1-expressing tissues, posing substantial cardiovascular morbidity/mortality. The T3-analogue triiodothyroacetic acid (Triac) holds potential as an effective treatment. Systematic data on Triac therapy in RTHβ are lacking, and the mechanisms through which Triac acts are unclear. This study aims to elucidate these knowledge gaps. METHODS: Clinical and bioche

Emerging NPR-B agonists for cartilage and bone: from CNP biology to translational compound design.

Disorders affecting cartilage and bone development frequently originate from disrupted endochondral ossification within growth-plate cartilage. Nonetheless, few therapeutic interventions directly target the local signaling pathways governing this process. This review explores the C-type natriuretic peptide (CNP)/natriuretic peptide receptor B (NPR-B) axis as a mechanism-based therapeutic approach addressing issues in cartilage and bone, including skeletal dysplasias, genetic short-stature conditions, and acquired ailments such as bone fractures. It summarizes the production and processing of C

Fertility preservation in girls and adolescents with Turner syndrome: lessons for reproductive care in differences of sex development - a mini-review.

Current knowledge regarding fertility preservation (FP) in girls is derived predominantly from pediatric oncology cohorts; however, FP is increasingly being applied to non-oncological conditions associated with impaired fertility, including differences of sex development (DSD). Turner syndrome (TS), characterized by accelerated germ cell depletion beginning during fetal life, represents the DSD condition with the greatest clinical experience in FP. This mini-review summarizes current evidence on ovarian tissue cryopreservation and oocyte cryopreservation in TS. Available data support the techn

Near-Infrared Autofluorescence for Parathyroid Identification in Pediatric Thyroid and Parathyroid Surgery.

INTRODUCTION: Identification and preservation of parathyroid glands (PGs) in children is particularly challenging due to their small size and frequent ectopic location. Near-infrared autofluorescence (NIRAF) enables real-time intraoperative recognition of PGs; however, its application in pediatric patients remains poorly documented. Accurate PG identification is essential to reduce postoperative hypoparathyroidism. METHODS: We conducted a prospective observational case series including patients <18 years undergoing thyroidectomy or parathyroidectomy with the assistance of a NIRAF probe. Detec

Revisiting Cinacalcet HCl Hepatic Disposition: Insights from Isolated Perfused Rat Liver.

INTRODUCTION: Hepatic drug handling determines systemic exposure, yet the balance between tissue uptake, intracellular binding, and metabolic clearance is difficult to dissect in vivo. Cinacalcet hydrochloride (CINA), a calcimimetic used to manage secondary hyperparathyroidism, has low oral bioavailability and is widely described as a high-extraction drug subject to extensive hepatic metabolism. Whether tissue sequestration rather than biotransformation underlies its rapid hepatic removal has not been tested directly in an intact ex vivo liver model. METHODS: We investigated the hepatic clear

Thyroid disease-related false-negative ultrasonography and 99mTc-MIBI scintigraphy in primary hyperparathyroidism: a case report and literature review.

BACKGROUND: Multiple gland disease associated with primary hyperparathyroidism (PHPT) are relatively rare, and those combined with underlying thyroid diseases are even rarer. This study aimed to investigate the approaches to preoperative localization and enumeration of lesions, as well as the determination of surgical endpoints in such patients. CASE DESCRIPTION: A female patient was admitted due to "elevated serum calcium detected for more than 20 days", with a past medical history of hyperthyroidism and Hashimoto's thyroiditis. Neck ultrasound only suggested a single-gland lesion, and 99mTc

Beyond Primary Epilepsy: Fahr's Syndrome Secondary to Congenital Parathyroid Agenesis Presenting With Generalised Seizures in a Young Adult.

Symmetrical, bilateral intracranial calcifications, commonly involving the basal ganglia, constitute the characteristic features of Fahr's syndrome, which is an uncommon neurometabolic condition. Its incidence in conjunction with congenital parathyroid agenesis, however, has rarely been reported. The present case emphasises how crucial it is to identify the metabolic origins of intracranial calcification and differentiate Fahr's syndrome from primary familial brain calcification to prevent repeated neurological consequences through adequate therapy. A 26-year-old man with congenital absence of
