A novel mutation in exon 18 of FGFR1 causing hypoparathyroidism: A case report.

Hypoparathyroidism is a rare endocrine condition characterized by insufficient secretion of parathyroid hormone (PTH), resulting in abnormally low calcium levels (hypocalcemia) and elevated phosphate levels (hyperphosphatemia) in the blood. This report describes a man in his late 30s with a chronic skin condition marked by dryness and desquamation. He occasionally experienced mild perioral numbness. Over the past year, he developed recurrent neuromuscular irritability, including worsening perioral numbness, tingling or numbness in the hands and feet, and muscle spasms consistent with tetany. H

Recurrent hypocalcaemic seizures: diagnosing 22q11.2 deletion syndrome in early adulthood.

We report the case of a male in his late teens who presented to our emergency department with acute generalised tonic-clonic seizures. Neurological examination, neuroimaging and laboratory tests confirmed severe hypocalcaemia, secondary to hypoparathyroidism, as the cause of his seizures. A detailed history was significant for recurrent urinary tract infections and epilepsy, accompanied by impaired academic performance and an intracardiac repair 8 years prior to presentation for a congenital cyanotic heart disease. Dysmorphic features on clinical examination, accompanied by the chronicity of c

Quality of life assessment in patients undergoing partial and total thyroidectomy.

INTRODUCTION: Thyroidectomy, which consists of partial or total removal of the thyroid gland, is a commonly performed surgery to treat various thyroid diseases. In recent years, the trend has been toward partial thyroidectomies, due to their association with lower complication rates and the fact that they may be sufficient for adequate management. OBJECTIVE: To evaluate the impact of partial and total thyroidectomy on quality of life. METHODS: This study used the ThyPro questionnaire, which assesses the quality of life of patients with thyroid disorders, to investigate the impact of differen

Evolving Practices in Low-Risk Papillary Thyroid Cancer: Impact of the 2015 ATA Guidelines.

Background/Objectives: The extent of initial surgical resection for low-risk papillary thyroid carcinoma (PTC) remains debated. Traditionally, total thyroidectomy (TT) has been the standard approach, although the 2015 American Thyroid Association (ATA) guidelines endorsed a more conservative strategy. The real-world adoption of these recommendations, however, is unclear. This study evaluated changes in the surgical management of low-risk PTC in a high-volume center following the implementation of the ATA guidelines and analyzed the impact on postoperative outcomes. Methods: We conducted a retr

Basal Ganglia Calcification Secondary to Postsurgical Hypoparathyroidism Presenting With Seizures and Acute Confusion: A Case Report (Fahr Syndrome).

Fahr syndrome is a rare neurologic condition characterized by bilateral intracranial calcifications, most commonly associated with metabolic or endocrine disorders, particularly hypoparathyroidism. We report a middle-aged woman with post-surgical hypoparathyroidism who presented with acute confusion and a generalized tonic-clonic seizure. Laboratory evaluation revealed severe hypocalcemia with undetectable parathyroid hormone levels. Non-contrast computed tomography (CT) of the head demonstrated extensive, symmetrical calcifications predominantly involving the basal ganglia, with additional th

Outcomes of Surgery Versus Radioactive Iodine as Definitive Therapy in Pediatric Graves' Disease: A Systematic Review and Meta-Analysis of Cohort Studies.

INTRODUCTION: Definitive second-line treatment for pediatric Graves' disease (GD) includes radioiodine ablation (RAI) or thyroidectomy. The current treatment practice in pediatric GD patients is a contentious issue as the decision to consider either of the treatment options depends on preferences of patients, physicians, and access to surgical care and radioactive iodine treatment. This systematic review and meta-analysis were performed to compare the cure and relapse rates of RAI versus surgery as definitive therapy in children with Graves' Disease. METHODS: A comprehensive search on Cochran

Advancing hypoparathyroidism treatment: FDA approval of Palopegteriparatide as a promising orphan drug.

Hypoparathyroidism (HypoPT) is a rare endocrine disorder characterized by insufficient parathyroid hormone (PTH) levels, leading to hypocalcemia and hyperphosphatemia. Affecting approximately 70 000-90 000 individuals in the United States, HypoPT can arise from genetic mutations or, most commonly, as a result of surgical removal of the parathyroid glands. Traditional treatments involve calcium and vitamin D supplementation, which can pose long-term risks such as renal complications. On 9 August 2024, the U.S. Food and Drug Administration (FDA) approved Palopegteriparatide (Yorvipath, Ascendis,

When One Gland Speaks First: Autoimmune Polyendocrinopathy Syndrome Type 1 (APS-1) Unmasked by Isolated Hypoparathyroidism.

Autoimmune polyendocrinopathy syndrome type 1 (APS-1) is a rare autosomal recessive disorder caused by pathogenic variants in the AIRE gene and classically characterized by the triad of hypoparathyroidism, chronic mucocutaneous candidiasis, and adrenal insufficiency. Although hypoparathyroidism is often the earliest manifestation, isolated and prolonged monosymptomatic presentations remain uncommon and may delay recognition of the syndrome. We report the case of a child who was first presented at four years of age with severe hypocalcemia in the setting of acute gastroenteritis and was diagnos

Prospective Observational Study of the Role of Unilateral Prophylactic Central Compartmental Lymph Node Dissection in Early Papillary Thyroid Cancer.

BACKGROUND: In clinically node-negative (cN0) early papillary thyroid carcinoma (PTC), prophylactic central compartment lymph node dissection (PCND) remains debated because occult nodal upstaging may not translate into improved recurrence outcomes and may increase hypocalcaemia and recurrent laryngeal nerve injury. Unilateral dissection may provide a risk-limiting compromise. METHODS: This prospective single-centre study included 50 consecutive adults with T1 or T2 cN0 PTC who underwent total thyroidectomy with unilateral central compartment lymph node dissection on the tumour side. The prima

Surgical outcomes of modified versus conventional transoral endoscopic thyroidectomy vestibular approach for papillary thyroid carcinoma.

BACKGROUND: Transoral endoscopic thyroidectomy via the vestibular approach (TOETVA) offers superior cosmetic outcomes but is associated with prolonged operative time and mental nerve injury. We have implemented a modified TOETVA technique and compared its surgical outcomes with the conventional approach in early-stage papillary thyroid carcinoma (PTC). METHODS: This retrospective study included 505 consecutive patients with T1aN0M0 PTC who underwent TOETVA hemithyroidectomy with ipsilateral central neck dissection between January 2022 and March 2025. Patients were divided into conventional (n

Use of intraoperative neuromonitoring in robotic thyroidectomy: a systematic review of recurrent laryngeal nerve outcomes.

BACKGROUND: Intraoperative neuromonitoring (IONM) is an emerging alternative to visual identification of the recurrent laryngeal nerve (RLN) during surgery. Its goal is to reduce RLN injury and vocal cord paralysis (VCP). However, evidence has been inconsistent concerning its benefits. This study aims to assess IONM's efficacy in preventing RLN injury during robotic thyroidectomy (RoT). METHODS: This systematic review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. A comprehensive literature search was conducted across four electronic datab
